1.Comparison of efficacy between procedure for prolapse and hemorrhoids and rubber band ligation in treatment of Ⅲ degree internal hemorrhoids

Objective To compare efficacy of procedure for prolapse and hemorrhoids (PPH) and rubber band ligation (RBL) in treatment of Ⅲ degree internal hemorrhoids.Methods One hundred patients with Ⅲ degree internal hemorrhoids were randomly divided into two groups and received PPH (42 cases) and RBL (58 cases) respectively.At once and after two weeks and two and six months the efficacy were compared.Results PPH and RBL were equally effective in controlling symptomatic prohpse (P>0.05),but RBL was associated with an increased incidence of recurrent bleeding (PPH 8/42 vs RBL 30/58,P<0.05);there was no difference in scores of patient satisfaction (P>0.05); PPH was associated with increased pain and analgesia usage at both at once and 2-week follow-up (P<0.05);the rate of complications in PPH was higber than in RBL (7/42 vs 0/58,P<0.05).Conclusion RBL can be used as treatment of first-line treatment of Ⅲ degree internal hemorrhoids.
2.A clinical research of endoscopic myringoplasty with modified sandwich technique.

OBJECTIVE:
To explore the curative effect of endoscopic myringoplasty with modified sandwich technique.
METHOD:
Endoscopic myringoplasty was performed with modified sandwich technique for traumatic perforation of tympanic membrane and chronic suppurative otitis media (simple type) of 43 patients.
RESULT:
All the perforating tympanic membranes were repaired successfully in one time. Six months after the operation, 1 case was out of follow-up and 2 cases were failed: one got a small perforation when the transplantation shifted and the transplantation of the other one was lost because of infection. The closure rate of tympanic membrane perforations was 95.2%. The air-bone gap of speech frequency of 28 ears increased by > 15-20 dB, 8 ears were enhanced by 10-15 dB, while 6 ears (including 2 failed cases) without improvement. The acoustic immittance test showed that "Type A", "Type As", "Type Ad", "Type B" and "Type C" tympanogram were in 30 ears,12, 4, 6 and 4, respectively.
CONCLUSION
The endoscopic myringoplasty with modified sandwich technique has the advantages of simple operation, better security, less trauma and better efficacy, and it is worth popularizing.

3.Diagnostic value of cardiac magnetic resonance in patients with acute viral myocarditis.

OBJECTIVETo assess the diagnostic value of cardiac magnetic resonance (CMR) in patients with acute viral myocarditis.
METHODSThirty patients with suspected acute viral myocarditis admitted in first people's hospital of Shunde from June 2011 to June 2013 were included in this prospective study. The diagnostic sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of acute viral myocarditis were evaluated by clinical diagnosis. Diagnostic value among different scan methods and Lake Louise criteria were compared.
RESULTSAcute viral myocarditis was diagnosed in 63.33% (19/30) patients.Values for sensitivity, specificity, PPV, NPV, and diagnostic accuracy within the overall cohort were 57.89%, 72.73%, 78.57%, 50.00%, 63.33%, respectively by edema imaging (ER).Values for sensitivity, specificity, PPV, NPV, and diagnostic accuracy within the overall cohort were 78.95%, 63.64%, 78.95%, 63.64%, 73.33%, respectively using global relative enhancement (gRE).Values for sensitivity, specificity, PPV, NPV, and diagnostic accuracy within the overall cohort were 78.95%, 54.55%, 75.00%, 60.00%, 70.00%, respectively using late gadolinium enhancement (LGE) criteria.Values for sensitivity, specificity, PPV, NPV, and diagnostic accuracy within the overall cohort were 84.21%, 81.82%, 88.89%, 75.00%, 83.33% using Lake Louise criteria. The sensitivity, specificity, PPV, NPV, and diagnostic accuracy using Lake Louise criteria were significantly higher than using ER, gRE, LGE alone(all P < 0.05).Specificity was higher using ER than using gRE and LGE (both P < 0.05). The sensitivity, NPV, and diagnostic accuracy were significantly higher using gRE than using ER (all P < 0.05) and was similar as using LGE (all P > 0.05).
CONCLUSIONCardiac magnetic resonance is an excellent imaging modality for the diagnosis of acute viral myocarditis.

4.Mechanism of suppressing astrocyte mitogen-activated protein kinase 14 to alleviate neuronal injury caused by glutamate excitatory toxicity

Objective:To explore the action mechanism of suppressing expression of mitogen- activated protein kinase 14(MAPK14)to alleviate glutamate excitatory toxicity and its neuronal protection effect.Methods:Lentivirus-mediated MAPK14 interference vector was synthetized by Shanghai Jikai Gene Chemical Technology Co.,Ltd. Astrocytes were obtained from SD rats 48 hours after birth,which were cultured in vitro and transfected by lentivirus-mediated transfection. According to the random number table,the cells were divided into three groups:(1)un-transfected group(normal group)with normal astrocytes and the cells were cultured in regular medium composed of Dulbecco's?modified Eagle's?medium(DMEM);(2)negative control group with astrocytes transfected by MAPK14 no-loaded interference vector;(3)lentivirus transfected group with astrocytes transfected by MAPK14 interference vector. Seventy-two hours after transfection,astrocytes were co-cultured with neurons for 48 hours,and then they were cultured in a medium containing glutamate for 2 hours. The detection indexes included the optimal multiplicity of infection(MOI)value for astrocytes transfected by lentivirus vector,mRNA levels of MAPK14 and glial glutamate transporter 1(GLT-1)detected by rPCR 72 hours after transfection,protein levels of MAPK14 and GLT-1 detected by Western blot 72 hours after transfection,level of lactate dehydrogenase(LDH)and mortality of neurons measured by spectrophotometry and flow cytometry 2 hours after culturing in the medium with glutamate. Results:(1)The optimal MOI value for lentivirus transfecting astrocytes was 30,and astrocytes grew well after transfection.(2)Seventy-two after transfection,the mRNA level of MAPK14 in lentivirus transfected group(0.005 7±0.000 6)was significantly decreased as compared with un-transfected group(0.013 1±0.001 1)and negative control group(0.013 9±0.001 0)( P<0.01),the mRNA level of GLT-1 in lentivirus transfected group(0.009 1±0.001 2)was not significantly changed as compared with un-transfected group(0.008 7±0.000 3)and negative control group(0.008 9±0.001 1)( P>0.05).(3)Seventy-two hours after transfection,the protein level of MAPK14 in lentivirus transfected group(0.29±0.04)was significantly decreased as compared with non-transfected group(0.61±0.05)and negative control group(0.63±0.01)( P<0.01),the protein level of GLT-1 in lentivirus transfected group(0.73±0.06)was significantly increased as compared with un-transfected group(0.20±0.03)and negative control group(0.23±0.09)( P<0.01).(4)After astrocytes were co-cultured with neurons and subsequently cultured in the medium containing glutamate for 2 hours,the level of LDH in lentivirus transfected group[(109.67±2.40)U/L]was significantly lower than that in un-transfected group[(141.52±3.88)U/L]and negative control group[(141.29±3.61)U/L]( P<0.01). The mortality of neurons in lentivirus transfected group[(38.72±0.26)%]was significantly lower than that in un-transfected group[(52.94±1.36)%]and negative control group[(54.30±1.23)%]( P<0.01). Conclusions:The transfection with lentivirus-mediated MAPK14 interference vector can increase expression of GLT-1 in astrocytes to increase glutamate re-uptake and relieve the glutamate excitatory toxicity in neurons,which may provide a new experimental basis for future use of astrocyte gene regulation to alleviate neuronal injury caused by glutamate excitatory toxicity after traumatic brain injury.
5.Evaluation of correlation between left ventricular mechanical contraction synchrony and left ventricular systolic function using a novel Cadmium-Zinc-Telluride SPECT

Objective:To evaluate correlation between left ventricular mechanical contraction synchrony and left ventricular systolic function by gated myocardial perfusion imaging(GMPI) using Cadmium-Zine-Telluride (CZT) SPECT.Methods:Three hundred and forty three consecutive patients( 232 males, 111 females, age (60.08±12.88) years) who underwent CZT SPECT GMPI in Shanxi Cardiovascular Hospital between January and August 2019 were retrospectively analyzed. The Emory cardiac toolbox was used to process the imaging data, and the left ventricular systolic synchrony parameters and systolic function parameters were acquired, including peak phase(PP), phase standard deviation (PSD), phase histogram bandwidth(PHB), histogram skewness(HS), histogram kurtosis(HK), and end-diastolic volume( EDV), end-systolic volume (ESV), left ventrieular ejection fraction (LVEF). All patients were divided into 4 groups: the normal group (147 cases), ischemic cardiomyopathy group (114 cases), nonischemic cardiomyopathy without left bundle branch block (LBBB) group(50 cases)and nonischemic cardiomyopathy with LBBB group(32 cases). The relationship between systolic synchrony parameters and systolic function parameters were analyzed with Pearson correlation analysis.Results:Statistic results for all patients showed that PSD and PHB were well correlated with LVEF( r values: -0.790, -0.799, both P<0.01), but PP was poorly correlated with LVEF( r=-0.194, P<0.01); HS, HK were positively correlated with LVEF( r values: 0.767, 0.676, both P<0.01); PSD, PHB were positively correlated with ESV( r values: 0.778, 0.795, both P<0.01) and EDV ( r values: 0.722, 0.732, both P<0.01); but PP was poorly correlated with ESV( r=0.145, P<0.01) and not correlated with EDV ( r=0.095, P>0.01). HS, HK were negatively correlated with EDV and ESV ( r values: -0.700 to -0.580, all P<0.01). PSD and PHB showed negatively correlation with LVEF ( r values: -0.834 to -0.492, all P<0.01), while HS, HK showed positive correlation with LVEF ( r values: 0.243-0.792, all P<0.01) in normal group, the ischemic cardiomyopathy group, the nonischemic cardiomyopathy without LBBB group and the nonischemic cardiomyopathy with LBBB group. Conclusions:The left ventricular systolic synchrony parameters provided by CZT SPECT GMPI correlate well with the left ventricular systolic function parameters, and the worse systolic function, the worse systolic synchrony. Both ischemic cardiomyopathy and non-ischemic cardiomyopathy can affect left ventricular mechanical contraction synchrony, and the effect on contraction synchrony in non-ischemic cardiomyopathy patients with LBBB is greater.
