Objective:To evaluate the masking ability of the Rely XTM Veneer try-in pastes. Methods: 100 discolored teeth, including 50 nonvital ones and 50 vital ones, has been collected and restored respectively with all-ceramic crowns and veneers. Five try-in pastes including A1, A3, Translucent (Trt), White opaque(WOT) and B0.5 Opaque(B0.5),which were different in shade, were used to try in before adhesion. The color was measured by a digital colorimeter, and expressed in terms of 3 coordinate values(L*、a*、b*) of the CIE1976- L*a*b* system.Then the color differences of crowns and veneers ?E=[(?L*)2+(?a*)2+(?b*)2]1/2 as well as the value of Light (L= L*)、Chroma[C=〔(a*)2+(b*)2〕1/2] and Hue [H=arctan(b*/a*)] were calculated. The thickness of the measurement point was also measured and recorded. A one-way ANOVA was used to analyze ?E as well as L, C and H. 2.Adverse reactions of chemoradiotherapy with docetaxel after modified radical mastectomy in locally advanced breast cancer

Objective:To compare the adverse reactions of concurrent chemoradiotherapy with docetaxel versus those of sequential and scheduled adjuvant therapy after a modified radical mastectomy in locally advanced breast cancer. Additionally, this work aims to evaluate the safety and feasibility of a synchronous therapy schedule. Methods:A total of 155 female breast cancer patients in the Fourth Affiliated Hospital of Guangxi Medical University were enrol ed from January 2009 to December 2014. Al the patients were diagnosed with infiltrating ductal carcinoma and stage pT3-4, pN1-3c M0, or pAnyTpN2-3cM0 after modified radical mastectomy. After completing the fluorouracil+epirubicin+cyclophosphamide adjuvant chemotherapy, all the patients were randomly divided into two groups by using the method of sealed envelopes. The synchronous group, which received synchronous chemoradiotherapy with docetaxel, comprised 78 cases. The sequential group, which received radiotherapy fol owing docetaxel chemotherapy, comprised 77 cases. The clinical toxic reactions and effects in both groups were assessed after all schedules. Results:A median follow-up period of 39 (16-62) months showed that the radiation side effects of the synchronous and sequential groups were mild. No patients with 3-4 grade radiation-induced skin reactions or symptoms of heart and lung radiation side effects were reported. The rate of 1-2 grade radiation-induced skin reactions was 89.7%(70/78) in the synchronous group and 88.3%(68/77) in the sequential group, but the difference was not statistically significant (P>0.05). The three-year recurrence-free survival rate was 92.3%(72/78) in the synchronous group and 81.8%(63/77) in the sequential group, and the difference was statistically significant (P=0.046). Conclusion:Synchronous chemoradiotherapy with docetaxel as adjuvant therapy exhibited mild and tolerable adverse reactions fol owing modified radical mastectomy in local y advanced breast cancer. Compared with the sequential schedule, the synchronous schedule showed a significantly increased three-year recurrence-free survival rate. Therefore, a synchronous chemo-radiotherapy schedule is safe and feasible and can be used as a treatment option for locally advanced breast cancer.
3.Effects of high-fat diet on the formation of colorectal adenomas in rats

Objective To investigate the effects of high-fat diet on the formation of colorectal adenomas in rats.Methods Twenty Wistar rats were evenly divided into two groups,one group were fed with common food and the other group were fed with high fat-diet.The rats of two groups were intraperitoneally injected with 1,2-dimethyl-hydrazine (DMH) (40 mg/kg) once a week for 10 consecutive weeks.Rats with common diet and DMH were assigned to SDT group,and those with high-fat diet and DMH were assigned to HFDT group.The expression of Ki-67 was detected by immunohistochemistry and the expression of proliferating cell nuclear antigen (PCNA) and cyclooxy-genase 2 (COX2) was detected by immunofluorescence.The levels of serum triglyceride,cholesterol,insulin like growth factor-1 (IGF-1),insulin,leptin and tumor necrosis factor-α were determined by enzyme linked immunosorbent assay.Student's t test and Mann-Whitney test were performed for statistical analysis.Results At the 18th week,the numbers of Ki-67 positive cells of SDT group and HFDT group were 24.00± 1.84 and 75.17 ± 3.17,respectively,and the difference was statistically significant (t=13.960,P<0.01).The fluorescence intensity of PCNA of SDT group and HFDT group was 213.70±21.61 and 333.30±30.44,respectively,and the difference was statistically significant (t =4.987,P<0.01).The levels of serum triglyceride of SDT group and HFDT group were (0.43 ± 0.05) mmol/L and (1.14±0.07) mmol/L,respectively,and the difference was statistically significant (t=8.366,P<0.01).The levels of cholesterol were (3.75±0.'26) pmol/L and (7.77±0.79) pmol/L,respectively,and the difference was statistically significant (t=4.851,P<0.01).Conclusion High-fat diet can promote the genesis and development of adnomas through influencing the body's endocrine metabolism,increasing cell proliferation cycle and regulating inflammatory reaction.
4.Laparoscopic right hemihepatectomy following a standardized technique

Objective To determine the clinical value of laparoscopic right hepatectomy (LRH) carried out following a standardized technique.Methods The medical records of 15 patients who underwent LRH at the First Affiliated Hospital of Soochow University were retrospectively reviewed.The perioperative indicators which included the operation duration,blood loss,tumor diameter,hospitalization duration and postoperative complications were analyzed.Results There was no conversion to open surgery.The operation duration was (251.1 ± 73.3) min.The resected tumor diameter was (8.5 ± 4.2) cm,the blood loss was (550.8 ± 343.6) ml,and the hospitalization duration was (10.2 ± 3.7) days.There was one patient who developed postoperative bile leakage.There was no other complication and there was no perioperative death.Conclusions The standardized technique of LRH was easy and effective.This technique shortened the operation duration and improved patient safety.
5.Relation between serum high sensitive C-reactive protein and acute cerebral infarction and its degree of nervous function defect

Objective To investigate the changes of serum high sensitive C-reactive protein(hs-CRP) level in patients with acute cerebral infarction(ACI),and the relationship between serum hs-CRP level and ACI severity as well as subtypes according to Chinese Ischemic Stroke Subclassification(CISS)criteria. Methods The serum hs-CRP level in 256 patients with ACI and 196 normal controls were measured. The degree of nervous function defect in patients with ACI was assessed by the United States National Institutes of Health Stroke Scale ( NIHSS ) score. All patients were classified into five major ischemic stroke subtypes based on CISS criteria. Logistic regression analysis was applied to analyze the risk factors of ACI. Results The serum hs-CRP level in patients with ACI and control group were(4. 69 ± 2. 58)mmol/ L and(2. 13 ± 1. 79)mmol/ L,and the difference between groups was significant(t = 12. 439,P = 0. 000). The hs-CRP in patients with severity ACI (147 cases)were(5. 89 ± 4. 15)mmol/ L,significantly higher than that in patients with mild ACI,and the difference between groups was significant((2. 11 ± 1. 45)mmol/ L,t = 10. 230,P = 0. 000)). As for subtype ACI,the case of the large artery atherosclerosis subtypes was 106( 41. 57% ),highest than any other subtypes. The hs-CRP level of large artery atherosclerosis was(7. 01 ± 3. 12)mmol/ L,higher than that of control group( P = 0. 000). The logistic regression analysis showed that many factors were related to ACI including total cholesterol,homocysteine and high sensitive C-reactive protein( OR = 0. 324,0. 749,0. 809;P< 0. 05). Conclusion The serum hs-CRP level in patients with ACI increase significantly,and relate to the degree of neural function defect. The level of hs-CRP of large artery atherosclerotic stroke is the highest. The change of serum hs-CRP is very valuable to estimate the severity of ACI.
6.Relationship between high sensitive C-reactive protein and stroke subtype according to Chinese Ischemic Stroke Subclassification criteria in patients with acute cerebral infarction

By analyzing the clinical data of 216 cases of acute cerebral infarction (ACI) from 2012 January to 2013 June retrospectively,we found that the serum levels of high sensitive C-reactive protein (hs-CRP) in patients were significantly higher than those in 186 controls (P <0.01).The degree of neural function defect in ACI patients was assessed by the National Institutes of Health Stroke Scale (NIHSS) score.The hs-CRP level of the patients with NIHSS score > 8 were higher than that in those with NIHSS score ≤8 (P < 0.05).The hs-CRP level of patients of large artery atherosclerosis were (6.32 ± 4.12) mg/L and the positive rate of hs-CRP was 85.7% (84/98).All were respectively higher than those in patients of penetrating artery disease [(1.97 ±0.86) mg/L,7/71],cardiogenic stroke [(3.70 ± 2.76) mg/L,14/24],undetermined etiology [(3.43 ± 3.52) mg/L,5/11] and other etiologies [(3.41 ± 3.25) mg/L,5/12] (all P < 0.05).Logistic regression analysis was performed for the risk factors of ACI.The correlative factors of ACI included hypertension,diabetes mellitus,atrial fibrillation,smoking,total cholesterol,homocysteine and high sensitive C-reactive protein (OR =1.56,1.19,1.23,1.17,3.08,1.34,1.25,all P < 0.01).The serum levels of hs-CRP increased significantly in ACI patients and were correlated with the degree of neural function defect.
7.Expression of C3a, C5a, TXA2, LT in pulmonary injury of infant open heart surgery improved with the modified ultrafiltration technique

Objective To study the mechanism of pulmonary injury and protective effect of modified ultrafiltration on lung function in infant open heart surgery.Methods According to the wishes of parents,40 cases of congenital heart disease were divided into without modified ultrafiltration control group (C) and modified ultrafiltration group (M),and parents signed informed consent.The cardiopulmonary bypass (CPB) was used without ultrafiltration in Group C,while with modified ultrafiltration in group M.The pneumodynamic parameters and C3a,C5a,TXA2,LT were measured at specific time points.Results The static pulmonary compliance (Cstat) and oxygen index (OI) were lower,and alveolar-arteria oxygen difference (AaDO2) was higher after CPB in the two groups(P < 0.05).At T3,T4 and T5 time points,the Cstat and OI in Group M was higher than that in Group C; AaDO2 in Group M was lower than that in Group C (P <0.05).The levels of C3a and C5a were lower after CPB in the two groups; levels of TXA2,LT were higher after CPB in the C groups.At T2,T3,T4 and T5 time points,the TXA2 and LT in Group M were lower than that in Group C(P <0.05).Conclusions The pulmonary injury in pediatric open heart surgery may be concerned with the the alexin(C3a,C5a) activation and I/R.The level of C3a and C5a was considered earlier index of inflammatory reaction and pulmonary injury.Modified ultrafiltration improves pulmonary function due to elevating coloid osmotic pressure and degrading the plasma level of TXA2,LT.
8.Analysis of genetic mutation of UGT1A1 * 28 and UGT1A1 * 6 in renal transplant patients with unconjugated hyperbilirubinemia

Objective To explore the clinical significance and gene mutation profiles of renal transplant patients with unconjugated hyperbilirubinemia (Gilbert's syndrome).Methods Genomic DNA was extracted from peripheral blood samples of 8 renal transplant patients with Gilbert'S syndrome.UGT1A1 * 6 and UGT1A1 * 28 genotypes were identified through digital fluorescence molecular hybridization and DNA sequencing.Results There are 2 cases of UGT1A1 * 28 heterozygous mutant,3 cases of UGT1A1 * 6 homozygous mutant,2 case of UGT1A1 * 6 heterozygous mutant,1 case of UGT1A1 * 28 heterozygous mutant combined with UGT1A1 * 6 heterozygous mutant.Conclusion There is a higher heterozygous or homozygous gene mutation rate of UGT1A1 * 6 and UGT1A1 * 28 in renal transplant patients with Gilbert's syndrome.Genetic mutation of UGT1A1 * 6 and UGT1A1 * 28 may be the reason of Gilbert's syndrome after renal transplant.
9.Application of capillary electrophoresis by dried filter blood paper for screening of α-thalassemia in neonates

Objective To investigate the application of capillary electrophoresis by dried filter blood paper for screening of α-thalassemia in neonates .Methods The hemoglobin (Hb) of 46 718 cases of neonatal dried heel blood spots were analyzed by the capillary electrophoresis and the content of HbA ,HbF ,HbA2 and abnormal Hb were detected ,the phenotype cases which was screened positive were recalled for genetic analysis .Results A total of 2 598 cases of Bart hemoglobin (Hb Bart′s) positive were detected in 46 718 cases of neonatal heel blood dried blood spots .The screening positive rate was 5 .56% (2 598/46 718) .A total of 477 cases of α-thalassemia gene carriers were confirmed by genetic analysis in the 544 cases which were recalled .The coincidence rate of Hb Bart′s screening and genetic diagnosis was 87 .68% (477/544) .By analyzing the relationship between the clinical pheno-types and the content of Hb Bart′s ,we found the Hb Bart′s content gradually increased with the severity of clinical phenotype ,and the difference was statistically significant (P= 0 .000) .Conclusion There is a good consistency between the capillary electrophore-sis of dried filter blood paper and the genetic analysis .It could be determined α-thalassemia clinical type according to the Hb Bart′s content .
10.A standardized laparoscopic left lateral sectionectomy in liver resection

Objective A retrospective analysis was conducted on standardized laparoscopic left lateral sectionectomy in liver resection (LLLR) using the "Two Step Two Endo-GIA" procedure.The aim of the study was to improve safety and efficacy of the operation.Methods All patients who underwent LLLR in Department of General Surgery,the First Affiliated Hospital of Soochow University from May 2014 to July 2018 were included in the study.All patients were divided into laparoscopic group (n=56) and open group (n=44).The operative plan followed the standardized procedure used in our department.Results Of 56 patients,there were 28 males and 28 females.No hepatic hilar occlusion was required and no case was converted to laparotomy.The average age was (55.7± 13.0),tumor diameter (6.3±3.7) cm,liver dissection time (30.0± 10.9) min,intraoperative blood loss (142.3±22.8) ml,and postoperative length of hospital stay (6.1±2.4) d.The average follow-up was (36.6± 10.1) months.One patient developed mild bile leakage and recovered after drainage.The other patients had no serious postoperative complications.The laparoscopic group was superior to the open group in operation time (90.0±17.0 vs.129.3±38.8) min,fasting time (1.5±1.0 vs.2.1±1.1) d,TBil (13.0±2.6 vs.19.0±3.1) μmol/L and ALT (80.0±19.3 vs.200.0±32.1) U/L.Conclusion A standardized LLLR has the advantages of short operation time,good reproducibility and short learning curve.It can be used as a standard procedure at all hospital levels.
