1.The changes of cranial suture induced by the distraction forces in growing goats

Objective:To observe the response and changes of cranial suture to the distraction forces in growing goats.Methods:A custom-made distractor was used for expanding coronal suture of 11 growing goats at the rate of 0.4 mm/day for 8 days.The animals were killed 0,2 and 4 weeks respectively after completion of suture distraction application. X-ray examination was taken and the distracted suture samples were harvested and processed for scanning electron microscopic observation and immunohistochemistry examination of BMP and TGF-? expression. The coronal sutures taken from other undistracted animals were used as the controls. Results:The coronal sutures were separated successfully in distracted goats. 0 and 2 weeks after application of suture distraction, the collagen fiber bundles were strengthened and aligned in the direction of the distraction. Strong expression of BMP and TGF-? were detected in the fibroblast-like cells and the active osteoblasts. 4 weeks after suture distraction, signs of intramembranous ossification were found in the edge areas of the distracted suture, and the positive staining of BMP and TGF-? was still noted in the osteoblasts around the newly formed bone trabeculae. Conclusion:BMP and TGF-? may play important roles in the process of bone formation and remodeling during suture distraction osteogenesis.
2.Humeral head replacement for treatment of complex proximal humerus fractures

Objective To discuss the indication, surgical technique and curative effect of humeral head replacement in treatment of complex proximal humerus fracture in old patients. Methods From 2000 to 2006, 23 patients with complex proximal humerus fractures were treated with humeral head replacement and fixated with bone cement in old patients. Rehabilitation training was performed after operation.Results Of all, 19 patients were followed up for average 1.8 years, which showed no infection, nerve injury or periprosthetic fracture except for one patient with semiluxation. NEER score system was used to evaluate the function joint prosthesis. Mean NEER score was 82.1 points, including 80-89 points ( satisfactory) in 17 patients and 70-79 points (unsatisfactory) in 2, with satisfaction rate of 89%. No patient needed revision. Conclusion With strict indication, active reconstruction of rotating cuff and stable structure of the joint, planned postoperative rehabilitation training, the humeral head replacement is a good method for treating complex proximal humerus fractures in the old patients.
3.Qualitative research about faculty experiences of patients in ICU during the course of orotracheal intubation

Objective To know the faculty experiences of patients in ICU during the course of oretracheal intutation, analyze the influence of related stressor in ICU on patients, and then carry out certaincounter nursing measures according to the results.Methods Phenomenological approach was adopted in this research, 8 patients with orotracheal intubtion received interviews, the data collected from the interviews were analyzed thematically. Results Three themes were showed: a series of mental and psychiatric symptom, obviously emotional change and the aggravation of local irritation. Conclusions Scientific ICU should be build by related nursing managers, the individualization and humanization nursing cares should be used for patients in ICU to promote nursing quality.
4.Establishment and application of human tissue biobank

Objective To explore issues of the human tissue biobank, ranging from its establishment, collection and preservation of samples, quality control, management and application. Methods Development of standardized operational procedures, collection of such samples as fresh frozen tissues from the surgery, paraffin-embedded tissues, whole blood, serum, plasma, and cerebrospinal fluid. Specimens were classified and made aliquots according to different requirements, and then stored at room temperature, -80℃ refrigerator or in liquid nitrogen. Microsoft Access database system was used in the management of these specimens. Results From September 2004 to September 2008, a total of 11 872 samples from patients with benign and malignant diseases were collected and preserved. Among them, 4360 tubes of fresh frozen tissue samples from 2500 cases were provided within and beyond the province. These samples were also applied to DNA, RNA, protein extraction and tissue microarray, and immunohistochemistry-related research. Conclusion Human tissue biobank is highly useful in sharing human tissue resources effectively, as it can provide high quality specimens from benign and malignant diseases and normal control. In addition, it plays a very important role in exploring pathogenesis, developing new technologies for early detection of disease and new therapeutic strategies.
5.Immediate implantation of coralline granules for repair of osseous defect

BACKGROUND: The direct ratio is formed between the size of osseous defect around implant and the required time of complete repair. It is viewed that bone implantation is suggested if osseous defect is larger than 1 mm, which benefits the bone regeneration and early fixation of implant.OBJECTIVE: To compare the effects between coralline granules and hydroxyapatite (HA) during union after immediate implantation.DESIGN: Group observation and controlled experiment was designed.SETTING: Department of Stomatology Renmin Hospital of Wuhan University.MATERIALS: HA coated implant, HA granule, coralline granule and 3adult mongrel dogs.METHODS: The experiment was performed in Department of Stomatology, Renmin Hospital of Wuhan University from August 2002 to April 2003.Under anesthesia, 6 foramens were drilled on femurs of 3 dogs (3 foramens on each side) to result in osseous defect. Coralline granules were embedded in the osseous defect around the implant in all of proximal ends (coralline granule group, CG group), HA granules were embedded in the osseous defect around the implant in all of distal ends (HA group) and nothing was embedded in the defect around the implant in the center (the control). One dog was sacrificed under anesthesia on the 2nd, 3rd and 4th months after operation successively and the samples were collected from the implanted section in each group for X-ray examination and scanning electronic microscopic observation.MAIN OUTCOME MEASURES: X-ray examination on implanted sections and scanning electronic microscopic observation on samples in each the 4th month, it was observed that implants and osseous tissue were closely integrated in CG group and HA group and there was still partial osseous on samples in each group: on the 4th month, it was discovered that the regenerated osseous tissue was matured completely in CG group and few coralline granule residue was left. In HA group, the regenerated osseous tissue was matured completely, but there was still a large amount of HA granules left and the granules had not been absorbed obviously. In the control group, the space appeared partially in the implanted cervical region.CONCLUSION: The implantation of artificial bone is suggested if the osseous defect around immediate implant is larger than 1 mm. As the implanting materials, coralline granules and HA granules act on the most advantageous guide activity. Coralline granules can be degraded and substituted with osseous tissue,but HA granules cannot be absorbed, which affects osseous reconstruction.
6.Progress in application of propeller flap in reconstruction of soft tissue defect of oral and maxillofacial-head and neck

Oral and maxillofacial-head and neck soft tissue defects affect the appearance of patients, as well as pronunciation, swallowing and other functions. Introduction of the propeller flap in 1991 has improved reconstruction procedures for oral and maxillofacial-head and neck soft tissue defects. A propeller flap has several advantages over traditional local flaps. It improves mobility, colour and texture matching for maxillofacial defect, surgical procedure, and individual satisfaction. Therefore, it can be used as a complement to the traditional flap by providing surgeons with more options. This paper reviews the classification, surgical procedures, and recent clinical applicatiosn and indications of the propeller flap.
7.Influence of induced expression nursing method on anxiety in gastroscopy patients

Objective To research the effect of the induced expression nursing method on anxiety and pain experience in gastroscopy patients. Methods A total of 121 patients from September to November 2012 were selected and randomly divided into the control group ( 60 patients ) and the intervention group (61 patients). The physiological indicators, anxiety score, and pain tolerance were measured and compared between the two groups. Results The systolic blood pressure, diastolic blood pressure and heart rate in the intervention group before gastroscopy were (121.64 ±11.49), (82.72 ±15.33)mmHg and (73.53 ±13.68) times/min,which were significantly better than those of the control group (t=4. 32, 3. 25, 5. 13, respectively;P<0. 05). There was no significant difference in SAS scores when making the test reservation between the two groups (P>0. 05). The SAS scores in the intervention group before gastroscopy was (52. 14 ± 2. 79), which was significantly lower than that of the control group (t=2. 96,P<0. 05). After gastroscopy, the pain tolerance grading was significantly better than that of the control group (P<0. 05). Conclusions The induced expression nursing method can effectively reduce the patients′anxiety and pain experience to gastroscopy.
8.Association of polymorphism in the promoter region of PCA3 gene with risk of prosate cancer.

OBJECTIVETo investigate the polymorphism in the promoter region of PCA3 gene and its relationship with risk of prostate cancer (PCa).
METHODSThe promoter region of PCA3 gene of the DNA of peripheral blood mononuclear cells was detected by sequence analysis in the 186 PCa and 141 BPH patients and 135 healthy control individuals. If the samples were detected with polymorphism of insection/deletion, clone sequence analysis was used with pBS-T carrier to verify it.
RESULTSThere were 5 polymorphisms. TAAA repeat times: 4, 5, 6, 7, 8, and 8 genotypes (TAAA 4/5, TAAA 4/6, TAAA 5/5, TAAA 5/6, TAAA 5/7, TAAA 5/8, TAAA 6/6, and TAAA 6/7) were detected in the promoter region of PCA3 gene. The eight genotypes were divided into three groups: ≤10TAAA, 11TAAA, ≥12TAAA. Unconditional logistic regression analysis models were used to analyze the relationship between different genotypes and cancer risks adjusted by sex and age. The type 11TAAA and ≥12TAAA was associated with higher relative risk for prostate cancer than the group ≤10TAAA [OR=1.74, 95% CI=1.06-2.87 (for type 11TAAA); OR=5.63, 95% CI=1.85-17.19 (for type ≥12TAAA)]. In the 186 PCa patients, there was 62.4% allele of PCA3 gene with AG/CA mutation found in the promoter 18-19 bp region of PCA3 gene and it had a close relation with the development of prostate cancer.
CONCLUSIONSShort tandem repeats are found in the promoter region of the PCA3 gene in PCa patients, and the increase of TAAA repeat sequences highly enhance the relative risk of prostate cancer development. The occurrence of such STR might be related to the mutations in their upstream loci.

9.Clinical characteristics and genetic analysis of a combined inherited antithrombin and factor Ⅶ deficiency pedigree

Objective:To study the clinical characteristics and gene mutations in a family with combined inherited antithrombin (AT) and factor Ⅶ (FⅦ) deficiency, and explore the relationship between AT gene, F7 gene mutations and diseases. Methods:Pedigree investigation. Blood samplesand clinical dataswere collected fromthe proband and her family members (a total of 16 people in 3 generations) who admitted to the First Affiliated Hospital of Wenzhou Medical University in November 2018. The prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB), antithrombin activity (AT: A), antithrombin antigen (AT: Ag), protein C activity (PC: A), protein S activity (PS: A), FⅦ activity (FⅦ: C), FⅦ antigen (FⅦ: Ag) and other indicators were detectedto confirm the diagnosis. DNA direct sequencing analysis of all exons, flanking sequences, 5′ and 3′ untranslated regions of AT genes and F7 genes, and the mutation sites were confirmed by clone sequencingor reverse sequencing. Results:The AT: A and AT: Ag of the proband were 46% and 135 mg/L, respectively (reference range: 250-360 mg/L), some of her family members′ s (father, aunt, two cousins, younger brother and nephew) AT: A and AT: Ag were reduced to 50% of normal range. Her father (Ⅰ 2), aunt (Ⅰ 4), elder brother (Ⅱ 7), younger brother (Ⅱ 8), and nephew (Ⅲ 3)′s FⅦ: C were 45%, 50%, 48%, 47% and 48%, respectively; and their FⅦ:Ag was within the normal range. Genetic analysis revealed that the proband(Ⅱ 6) and some of her family members (father, aunt, two cousins, younger brother and nephew) took rs3138521 polymorphism in the 5′ untranslated region of AT gene. Her father (Ⅰ 2), aunt (Ⅰ 4), elder brother (Ⅱ 7), younger brother (Ⅱ 8), nephew (Ⅲ 3) took c.1091G>A heterozygous missense mutationin exon 8 of F7 gene, resulting in p.Arg304Gln. Conclusion:The rs3138521 in AT gene and c.1091G>A in F7 gene, which may be the molecular mechanism leading to combined inherited AT and FⅦ deficiency in this family.
