Objective The aim of this study was to evaluate the optimal time for decompression in a 24-hour lasting porcine model.Methods After baseline data were recorded,24 pigs were randomly allocated into three groups as follows:one SAP-alone group,and two SAP + ACS groups (decompression at 6 and 12 hours,respectively).We used a N2 pneumoperitoneum to increase the intra-abdominal pressure to 25 mmHg and retrograde intra-ductal infusion of sodium taurocholate to induce severe acute pancreatitis (SAP).Systemic hemodynamic profiles,urine output,systemic oxygenation,and serum biochemical parameters of the animals were obtained.Results After induction of ACS,the hemodynamics and oxygenation of the study animals deteriorated significantly.The survival time of the 12-hour group was significantly shortened (P =0.008 vs.6 hours).Early decompression (6 h) restored systemic hemodynamics,oxygenation,organ function,and inflammatory intensity to a level comparable to that of the SAP-alone group.In contrast,animals in 12-hour group developed more severe hemodynamic suppression,oxygenation and organ dysfunction and inflammatory process.For instance,the cardiac output levels in the three groups were 2.70 ±0.50 for the SAP group,2.75 ±0.48 for the 6 hour-group and 2.19 ±0.43 for the 12 hour-group.Conclusion Early decompression could significantly reduce the mortality in a porcine model of SAP incorporating ACS,and also improve systemic hemodynamics,organ function and inflammatory intensity.
4.Depressor Substances Test and Histamine Test for the Quality Control of Deproteinized Culf Blood Extrac-tives Injection

Objective:To explore the applicability of depressor substances test and histamine test for the quality control of depro-teinized culf blood extractives injection. Methods: Depressor substances test and histamine test were carried out on 10 batches of deproteinized culf blood extractives injection samples. Results:Some samples showed positive reactions in the experiments. The correl-ative analysis showed positive correlation existed between the results of depressor substances test and histamine test. However, part of samples causing blood pressure decrease in anesthetized cats showed negative or relatively reduced results in histamine test. Conclu-sion:Deproteinized culf blood extractives injection may be contaminated by depressor substances. Part of these substances may not be histamine analogue, therefore, depressor substances test may be considered in the quality control of the product.
5.Effect of Zuoguiyin on expression of ovarian VEGF and SPARC in rats during peri-menopausal period.

OBJECTIVETo investigate the effect of one kind of regulating-qi and Nourishing-yin Chinese herbs Zuoguiyin on the expression of rats ovarian vascular endothelial growth factor (VEGF) and secreted protein acidic rich in cysteine (SPARC) during the period of peri-menopause.
METHODThe animal models of perimenopause rats were established by natural aging. Perimenopause rats were treated by intragastric administration (ig) with low (13.78 g x kg(-1)), middle (20.67 g x kg(-1)) and high (31 g x kg(-1)) dose of Zuoguiyin for 8 weeks. Expression of VEGF and SPARC mRNA were detected by RT-PCR. Immunohistochemistry was used to evaluate expression levels of VEGF protein.
RESULTCompared with that in the control group, ovarian expression levels of VEGF mRNA and its protein in rats during peri-menopause increased significantly (P < 0.01). Middle and high dose of Zuoguiyin could both down-regulate the expression of VEGF in ovaries of praesenilis rats, and the difference has statistical significance (P < 0.01). Expression levels of SPARC mRNA in rat ovaries during peri-menopausal period decreased obviously compared to that in the control group (P < 0.01). Middle and high dose of Zuoguiyin could greatly promote ovarian SPARC mRNA expression of praesenilis rats (P < 0.01).
CONCLUSIONThe abnormal changes of VEGF and SPARC may play an important role in the aging process of ovary. Zuoguiyin, one kind of regulating-qi and Nourishing-yin Chinese herbs, can improve ovarian vascular formation through down-regulating the expression of VEGF meanwhile up-regulating the expression of SPARC. And it may delay aging by this way.

Piwi-interacting RNA (piRNA) is a new class of small non-coding RNAs that function by specifically binding to the Piwi subfamily members of the Argonaute protein family. piRNA regulates the expression of target genes at epigenetic and post-transcriptional levels, and participates in the development and progression of cerebral ischemia by mediating pathophysiological processes such as inflammation, angiogenesis, and neuronal synaptic plasticity.

8.Investigation of SLC26A4 mutations associated with inner ear malformations.

OBJECTIVE:
To study the molecular pathogenesis of SLC26A4 mutations associated with inner ear malformations including large vestibular aqueduct syndrome (LVAS), Mondini dysplasia and inner ear malformations but not accompanied with LVAS.
METHOD:
DNA sample and clinical material were obtained from 14 sporadic LVAS probands, six Mondini dysplasia probands and seven inner ear malformations excluding IVAS probands. SLC26A4 gene mutation was analyzed by direct sequencing for its 20 coding exons. GJB2 gene and also mt12SrRNA were analyzed by direct sequencing.
RESULT:
In 14 cases of LVAS, two mutations were detected in 12 patients (85.7%, either homozygous or compound heterozygous mutations), and one mutation was found in two patients (14.3%). In six cases of Mondini dysplasia, two mutations were detected in all of patients (100%). No mutation could be found in the seven cases of other inner ear abnormalities not accompanied with LVAS. No pathogenic mutation was detected in all of these 27 probands in GJB2 gene and mt12SrRNA 1555/1494T.
CONCLUSION
We have shown that LVAS and Mondini dysplasia closely correlate with SLC26A4 gene. No mutation was detected in seven probands of inner ear malformations not accompanied with LVAS. We should study the molecular pathogenesis of this disease in depth.

9.Clinical and genetic analysis of a child with Focal segmental glomerulosclerosis due to a novel variant of PLCE1 gene

Objective:To explore the genetic basis and clinical phenotype of a Chinese pedigree affected with Focal segmental glomerulosclerosis (FSGS).Methods:A male patient who was admitted to the First Affiliated Hospital of Zhengzhou University on July 26, 2018 was selected as the study subject. Clinical data of the patient was collected. Next generation sequencing and Sanger sequencing were carried out to detect the variant sites. Bioinformatic software was used to simulate the effect of candidate variants on the protein functions.Results:Ultrasound exam of the patient showed enhanced echo for the renal parenchyma. Kidney biopsy had confirmed the pathological diagnosis of FSGS (non-specific). Electronic microscopy displayed segmental sclerosis of the glomeruli, mild hyperplasia of mesangial cells and matrix. The proband was found to harbor two novel variants of the PLCE1 gene, namely c. 3199delA (p.N1067Mfs*15) and c.4441_4443delATC (p.1481_1481del), which were respectively inherited from his mother and father. Based on the guidelines from the American College of Medical Genetics and Genomics (ACMG), both variants were rated as pathogenic (PVS1+ PM2_Supporting+ PP3; PM2_Supporting+ PM3+ PP3). Bioinformatic simulation suggested that both variants could significantly affect the tertiary structure of the PLCE1 protein. Conclusion:The c. 4441_4443delATC and c. 3199delA variants of the PLCE1 gene probably underlay the pathogenesis of the FSGS in this pedigree.
10.Association between symptom burden and depression after hepatectomy in hepatocellular carcinoma patients:a moderated mediation model

Objective To explore the mechanisms associated with symptom burden and depres-sion in hepatocellular carcinoma patients after hepatectomy.Methods A total of 230 hospitalized pa-tients with hepatocellular carcinoma who were underwent hepatectomy for the first time were selected as study subjects.A self-administered General Information Questionnaire,MD Anderson Symptom In-ventory,Patient Health Questionnaire,Connor-Davidson Resilience Scale,and Pittsburgh Sleep Quality Index were used to investigate sociodemographic,sociodemographic data,symptom burden,depression,psychological resilience,and sleep quality of postoperative hepatocellular carcinoma pa-tients.Pearson correlation analysis was used to identify specific associations among variables.The mediating and moderating effects of psychological resilience and sleep quality between symptom bur-den and depression were analyzed using the Process.Results Psychological resilience partially me-diated the association between symptom burden and depression(β=-0.137,SE=0.084,95%CI,-0.243 to-0.082),with the mediating effect accounting for 44.08%of the total effect,and the interaction term between symptom burden and sleep quality had predictive value for psychological re-silience(β=-0.014,t=-6.209,P<0.001).Conclusion Postoperative symptom burden in hepatocellular carcinoma patients can play a role in depression through psychological resilience,where the effect of symptom burden on psychological resilience is moderated by sleep quality.Clinical staff should take reducing patients'postoperative symptom burden and improving psychological elastic-ity with sleep quality as important goals and develop appropriate nursing interventions to reduce the risk of depression in patients.
