1.Clinical research of Gegensu injection combined with levodopa on parkinson's disease

Objective To observe the clinical efficacy of puerarin injection combined with levodopa in the treatment of Parkinson. Methods 126 Parkinson patients were grouped into two groups randomly. Control group was treated with levodopa and observation group was added to Gegensu injection. Results 3 months later, UPDRS scale score, ADL and HAMD score of observation group were lower than that of control group(P<0.05).Serum Hcy and plasma hs-CRP level of observation group were lower than that of control group( P<0.05).Adverse reactions of observation group was 6.35% during treatment, compared with the control group 9.52% had no significant difference. Conclusion Gegensu injection can help improve cognitive ability and improve self-care ability in patients with Parkinson, alleviate symptoms and medication safety.
2.Clinical analysis of endoscopic ultrasonography in preoperative TNM staging of 126 gastric cancer cases

Objective To evaluate the accuracy of endoscopic ultrasonography (EUS) in preoperative TNM staging of gastric cancer.Methods EUS and abdominal helical computed tomography (HCT) were performed one week before the surgery in 126 gastric cancer patients who would receive surgery to determine the depth of tumor invasion (T),lymph node metastasis (N) and distant metastasis (M) stage and which were also compared with pathologic TNM stage after surgery to evaluate the accuracy of EUS in TNM staging.Pairing x2 test was performed for data analysis.Results Compared with pathologic TNM stage after surgery,the accuracy of EUS in T1,T2,T3 and T4 staging of gastric cancer were 84.6%,14/18,82.0 % and 85.7% respectively.The accuracy of EUSin N0,N1,N2 and N3 staging of gastric cancer were 74.2%,75.0%,57.9% and 5/17 respectively.The accuracy of HCT in N0,N1,N2 and N3 staging of gastric cancer were 80.6%,75.0%,73.7% and 12/17 respectively.The accuracy of EUS was similar to HCT in N0 and N1 stage.For N2 and N3 stage,HCT was obviously better than EUS (x2 =4.89,P=0.027; x2 =13.88,P<0.01).The accuracy of EUS and HCT in M1 stage of gastric cancer were 36.4% and 95.5%respectively,HCT was better than EUS in M1 determination (x2 =7.90,P=0.001).Conclusions The clinical application value of EUS in the preoperative gastric cancer T staging was high,however the accuracy in determining lymph node metastasis N2 and N3 staging and distant metastasis M staging should be improved.In order to acquire more accurate preoperative TNM stage to guide the treatment selection,the combination with HCT examination is necessary.
3.The study on dual-energy lung perfusion imaging in the diagnosis of pulmonary embolism using dualsource CT

Objective To explore the diagnostic values of dual energy lung perfusion in the diagnosis of pulmonary embolism by using dual-source CT (DSCT). Methods Thirty patients with clinically suspected pulmonary embolism underwent dual-energy scanning with dual-source CT. The scanned data were integrated into three groups including 140, 80 kV and coefficient of 0.3. According to the CT pulmonary angiography (CTPA) of the fusion data, the patients were divided into pulmonary embolism group and normal group. The thin-slice reconstruction of data was analyzed using dual-energy perfusion imaging analysis software. The lung field was divided into upper, middle and lower part to make quantitative analysis of lung tissue perfusion. Paired t-tests were used in the normal patients to compare bilateral lungs, and independent samples t-tests were applied to compare the embolism group and normal group, while minimum intensity projection images (MinIP) were utilized in the assessment of lung ventilation. Results Dual energy CT showed symmetrical homogeneous perfusion in 16 normal cases, without significant perfusion defects. Quantitative analysis showed that left and right lung perfusion were (27 ± 7) and (28 ± 8 ) HU respectively, and no significant difference was found between the two sides ( t=-1.73, P >0.05 ).Perfusion of the left upper, middle and lower lung was ( 23 ± 6), (24 ± 6), and (28 ± 8) HU respectively, while the perfusion of right upper, middle and lower lung was (26 ±8), (27 ±8), and (28 ±9) HU respectively, showing no statistical significant difference between the two sides (t=-1.91, -1.96,-1.73 ,P>0.05 ). Angiography of pulmonary embolism group(14 cases)showed filling defects in the pulmonary trunk, segments and sub-segments. Pulmonary perfusion imaging showed low perfusion or defectsin lung field that dominated by embolic vessels. Quantitative analysis showed that the perfusion of the whole lung and the middle and lower lung were (22 ±5), (22 ±8), and (21 ±8) HU in the embolism group,which were significantly different from the normal group (t=-2. 10, -2.32, -2.63, P<0.05).Minimum intensity projection images showed a good consistency of abnormal ventilation zone area and perfusion abnormalities. Conclusions Pulmonary perfusion status, especially pulmonary embolism, can be analyzed by dual energy CT scanning. It helps to early discover and precisely locate the embolism.
4.Factor analysis of traditional Chinese medicine syndromes in 815 patients with unstable angina

OBJECTIVE: To explore the laws of traditional Chinese medicine (TCM) syndromes in unstable angina, and to establish the preliminary diagnostic criteria for TCM syndromes. METHODS: Multi-center prospective research on TCM syndromes in 815 cases of unstable angina was done with the nonlinear dimension reduction by factor analysis. RESULTS: There were five extracted factors in factor analysis: F1, F2, F3, F4 and F5. F1 was yin deficiency of heart and kidney, F2 was deficiency of both heart and spleen, F3 was intermingled phlegm and blood stasis, F4 was qi deficiency and blood stasis, and F5 was yang deficiency and coagulated cold. Qi deficiency and blood stasis (F4) syndrome accounted for the maximum proportion. The diagnostic criteria for TCM syndromes were preliminarily and respectively established. CONCLUSION: Qi deficiency and blood stasis is the key factor of pathogenesis. The factor analysis can help us classify traditional Chinese medicine syndromes and establish the preliminary diagnostic criteria.
5.Nosocomial Infecion of Inpatients in 2007

OBJECTIVE To study the clinical feature of nosocomial infection and analyze the correlative reasons,to offer the scientific theory basis for preventing and controlling the nosocomial infection. METHODS A retrospective survey was undertaken in the data of hospitalized cases during 2007. RESULTS Totally 2325 nosocomial infections in 54 505 patients were analyzed in 2007.The infection rate of nosocomial infection was 4.27%;the highest infection rate was in ICU(33.57%);the infection sites were different in distinct departments and the most common infection site was lower respiratory tract(27.19%);among pathogens isolated from nosocomial infection cases,48.05% of them were Gram-negatives,23.25% were virus,14.45% were fungi,and 14.15% were Gram-positives;the infection rate was diverse in different months. CONCLUSIONS Nosocomial infection rate is related to different underlying diseases;there are many effective actions to reduce the nosocomial infection,such as strengthening the nosocomial infection management,using the antibacterial drugs reasonably and preventing communicable diseases prevalence.
6.Effectiveness of distance electrocardiographic consultation network for regional primary medical institutions

Objective To evaluate the effectiveness of regional electrocardiographic consultation network for primary medical institutions.Methods A distance electrocardiographic ( ECG ) consultation center was established in Yuyao People′s Hospital in September 2012, which was connected to 56 regional primary medical through internet.The online consultation was provided by ECG specialists in the center for general practitioners in regional primary health care institutions.Additionally, the ECG training course was also conducted monthly for general practitioners.The 2-year data of ECG consultation were retrospectively reviewed and the effectiveness of the consultation network was evaluated.Results During the past two years, 86 593 electrocardiograms were uploaded by primary medical institutions, in which 3 976 were required for consultation. The monthly number of electrocardiograms sent by general practitioners was increased from 3 269 ±290 in the first year to 3 947 ±376 in the second year ( P<0.01 ) .However, the requirements of ECG consultation were not significantly different between two years.Compared to the first year, the rates of consultation requirements, misdiagnosis and missed diagnosis for ECGs of atrial and ventricular premature beats, atrial fibrillation, supraventricular tachycardia, left and right bundle branch blocks,ⅠandⅡdegree atrioventricular blocks and myocardial infarction were significantly decreased ( all P<0.05) in the second year;meanwhile there was a tendency towards the reduction for ECGs of atrial flutter and Ⅲ degree atrioventricular block in the second year.Conclusion The regional electrocardiographic consultation network is effective to improve the capability of primary medical institutions in electrocardiographic diagnosis.
7.Optimal time of decompression for acute pancreatitis combined with abdominal compartment syndrome

Objective The aim of this study was to evaluate the optimal time for decompression in a 24-hour lasting porcine model.Methods After baseline data were recorded,24 pigs were randomly allocated into three groups as follows:one SAP-alone group,and two SAP + ACS groups (decompression at 6 and 12 hours,respectively).We used a N2 pneumoperitoneum to increase the intra-abdominal pressure to 25 mmHg and retrograde intra-ductal infusion of sodium taurocholate to induce severe acute pancreatitis (SAP).Systemic hemodynamic profiles,urine output,systemic oxygenation,and serum biochemical parameters of the animals were obtained.Results After induction of ACS,the hemodynamics and oxygenation of the study animals deteriorated significantly.The survival time of the 12-hour group was significantly shortened (P =0.008 vs.6 hours).Early decompression (6 h) restored systemic hemodynamics,oxygenation,organ function,and inflammatory intensity to a level comparable to that of the SAP-alone group.In contrast,animals in 12-hour group developed more severe hemodynamic suppression,oxygenation and organ dysfunction and inflammatory process.For instance,the cardiac output levels in the three groups were 2.70 ±0.50 for the SAP group,2.75 ±0.48 for the 6 hour-group and 2.19 ±0.43 for the 12 hour-group.Conclusion Early decompression could significantly reduce the mortality in a porcine model of SAP incorporating ACS,and also improve systemic hemodynamics,organ function and inflammatory intensity.
8.Hypoxic preconditioning up-regulates the activity and gene expressions of glucose transporters (GLUT1 and GLUT3) in cultured rat hippocampal neurons and astrocytes under anoxic condition

Objective To explore the effect of hypoxic preconditioning on the activity and gene expressions of glu-cose transporters in the cultured rat hippocampal neurons and astrocytes under anoxic condition. Methods The cultured rat hippocampal neurons and astrocytes were treated for 6 days by intermittently exposing to hypoxic gas mixture (1% O_2, 10% CO_2, 89% N_2) for20 min each day. 24 h after the last hypoxic exposure, the cells were exposed to anoxic gas mixture (10% CO_2, 90% N_2) for 6 h, and the uptake rate of [~3H]-2-deoxyglucose (2-DG), the levels of glucose transporter GLUT1 and GLUT3 mRNAs and the cell survival rate were examined im-mediately after anoxic exposure. Results Neurons and astrocytes preconditioned with hypoxia showed higher 2-DG uptake rates and increased expressions of GLUT 1 mRNA in the astrocytes and GLUT 1 and GLUT 3 mRNA in the neurons. The preconditioned neurons also showed an increased tolerance to anoxia. Conclusion Hypoxic precon-ditioning up-regulates the activity and gene expressions of glucose transporters of hippocampal neurons and astro-cytes under anoxic condition.
9.Study on LDL adsorbent modified by lauric acid.

A hydrophobic low-density lipoprotein cholesterol (LDL-C) adsorbent was synthesized with lauric acid and chitosan. The condition for adsorption was obtained by investigating the influence of adsorbent amount and adsorption time. The results of adsorption in vitro showed that the average adsorption rates for total cholesterol (TC), LDL-C, high-density lipoprotein cholesterol (HDL-C) and total protein (TP) were 47.7%, 84.7%, 18.1% and 5.9% respectively. The adsorbent possesses good selectivity in removing LDL-C.

10.Xaf1 regulates TNFR signal pathway and induces apoptosis

AIM:Xaf1-Saos inducible cell lines,which contain "gene switch" system were used to detect the effect of Xaf1 on tumor necrosis factor receptor(TNFR) signal pathway and to investigate the mechanism of cooperation between Xaf1 and TNF-? in inducing cell apoptosis.METHODS:Xaf1 on TNFR1 expression was measured by RT-PCR and Western blotting.The effect of NF-?B on Xaf1 induced apoptosis was detected by DNA content flow cytometry after co-transfection.DNA binding activity of NF-?B was identified by gel mobility shift assay and transcription activity of NF-?B was analyzed by luciferase assay and RT-PCR.SAPK/JNK activity was checked by SAPK/JNK assay.RESULTS:Xaf1 did not modulate TNFR1 at protein and mRNA levels.Increased NF-?B activity in cells inhibited Xaf1 induced apoptosis.Expression of Xaf1 impaired modestly TNF-? induced NF-?B DNA binding activation and transcription activation,also modestly reduced SAPK/JNK activity.CONCLUSION:Xaf1 inhibits TNFR signal pathway,partly contributing to cooperation with TNF-? to induce apoptosis.
