1.Anterior rotation in occiput transverse and occiput posterior positions

Objective To investigate the natural rotation of the fetal head during labor when it was engaged by occiput transverse (OT) or occiput posterior(OP) positions.Methods A total of 1 013 parturients was included who had a singleton term gestation, vertex presentation, OT or OP position with cervical dilatation of ≤ 3 cm, and vaginal delivery.All parturients were delivered in Xiamen Hospital of Traditional Chinese Medicine during April 1 to December 31,2014.Cervical dilation, station, and fetal position of every delivery were systematically recorded.Main outcome was the cervical dilation and station of anterior rotation from OT or OP position.They were Stratified by cervical dilation every 2 cm since 4 cm into 4 ~ 5 cm, 6 ~7 cm, 8 ~9 cm, 10 cm;or were stratified by fetal head station every 1 cm since-2 (S-2) into S-1 , S0, S+1 , S+2 , S+3.The differences in anterior rotation rate, dilatation, and head station between OT and OP were analyzed.Results There were 850 OT and 407 OP with cervical dilatation of ≤≤ 3 cm.The rate of anterior rotation was 82.5 % (701/850) and 76.7 % (312/407), respectively.The constituent ratio of OT rotating into occiput anterior position (OA) at different dilatation was different with that of OP (x2 =260.93, P < 0.01).The constituent ratio of OT rotating into OA at different fetal head station was different with that of OP (x2 =133.18, P <0.01).The cervical dilatation and fetal head station that make it easy for OT 's anterior rotation were 6 ~7 cm (OR =5.81 , 95% CI :3.81 ~8.87), 8 ~9 cm(OR =4.99 ,95% CI:3.19 ~7.83), and S+1(OR =3.01, 95% CI: 1.99 ~4.54), S+2(OR =5.43, 95% CI:3.53 ~ 8.35).The cervical dilatation and fetal head station that make it easy for OP 's anterior rotation were 6~7 cm(OR =4.11,95% CI :2.24 ~7.89), 8 ~9 cm(OR =27.31, 95% CI :15.12 ~49.34), 10 cm(OR =27.76 , 95% CI :14.87 ~51.82), S+2(OR =4.84 , 95% CI :3.03 ~7.73).Conclusions The anterior rotation time of OT is different from OP.It's easy for OT to rotate into OA when cervix dilate into 6 ~9 cm and fetal head station is at S+1 ~ S+2.When cervix dilate into 6 ~ 10 cm and fetal head station is at S+2, it's easy for OP to rotate into OA.
2.Effect of Fu Qingzhu's Shenghua decoction on uterine involution after repeat cesarean section

Objective To observe the effect of modified Fu Qingzhu's Shenghua decoction on uterine involution after repeat cesarean section.Methods Randomized and parallel design was adopted in the study.A total of 112 cases were recruited into an observation group and a control group.56 cases in the observation group were treated with oral Fu Qingzhu's Shenghua decoction for 5 days,while 56 cases in control group were treated without any herbs.The follow-up was 42 days.Duration of lochia,hemoglobin concentration,and abnormalities of uterine ultrasound were observed.Results Duration of lochia rubra (5.1 ± 1.2 d vs.5.8 ± 1.3 d;t=-2.734,P=0.007) and the duration of lochia rubra and lochia serosa in the observation group 25.0 (21.0,29.8) d vs.29.0 (26.0,33.8) d;Z=3.873,P<0.001) were all lower than the control group.The lochia complete ration in 42 days (94.9% vs.82.1%,x2=4.264),the average hemoglobin concentration (115.43 ± 10.68 g/L vs.104.29 ± 9.90 g/L,F=4.159) and sum of the three uterine diameters (14.2 ± 1.0 cm vs.14.8 ± 1.3 cm,t=-2.686,P=0.008) in the observation group were all higher than the control group.The incidence of uterine cavity hydrops (3.57% vs.17.85%,P=0.015) in the observation group was lower than the control group.Conclusion Fu Qingzhu's Shenghua decoction has promotive effects on uterine involution after repeat cesarean section.
3.Spiral CT Observation of Normal Lung Lobes in Respiration on Different Position

Objective The study the formative causes of low dense area in lung on CT images during expiration.Methods This group included 15 young volunteers,mean age was 22 years.Thin sections of spiral CT at right middle hilum were scanned at suspended deep inspiration and deep expiration on both supine and prone positions.The density and area of lobes on matching structural images in the middle hilum at deep inhalation and exhalation were measured on the supine and prone.The increasing rate in area and absolute increasing value in attenuation as full inhalation comparing to full exhalation were calculated.Results Changes of distention and density in the same section of the right lower lobe on dependent position were greater than that on nondependent one.Correlation between distention and density of the lobes was high(P
4.Identification and Drug Resistance of Mycoplasma from Semen of infertility Men

OBJECTIVE To study infection and drug resistance of mycoplasma from semen of infertility men. METHODS Mycoplasma from semen of infertility men was identified by cultivation,and the sensitivities to drugs were also performed. RESULTS In 267 cases the positive rate of mycoplasma was 46.07%.Simple infection of Ureaplasma urealyticum(Uu) accounted for 41.57%(11),and Mycoplasma hominis(Mh) 1.50%(4),and the mixed 3.00%(8).The result of drug sensitive test showed that sensitivities of mycoplasma to minocycline,doxycycline and josamycin were the highest,and then were roxithromycin and azithromycin.The drug resistance of mycoplasma to ofloxacin and clindamycin was the highest. CONCLUSIONS The infectious rate of mycoplasma from semen of infertility men is on big rise.It is important to culture and test the drug sensitivities of mycoplasma to use drugs rationally.
5.Identification and Antimicrobial Resistance of Mycoplasma from Female Cervical Secretions:Analysis of 2263 Cases

OBJECTIVE To study infection status and drug resistance of Ureaplasma urealyticum(Uu) and Mycoplasma hominis(Mh) in the female genital tract.METHODS The retrospective analysis of identification and the antimicrobial susceptibility testing of mycoplasma in 2263 female cervical secretions from Jan 2007 to Jun 2008 were conducted.RESULTS In 2263 cases,the positive rate of mycoplasma was 66.46%.The infection only by Uu accounted for 45.03%,by Mh for 1.46%,and the mixed infection for 19.97%.The results of drug sensibility test showed that drug resistances of mycoplasma were diffirent among three types of mycoplasma infections.CONCLUSIONS The infectious rate of mycoplasma from female cervical secretions is on big rise.It is important to culture and test the drug sensitivities of mycoplasma for choosing drugs rationally and control the resistant strains.

Objective To evaluate the efficacy and side effects of arsenic trioxide (ATO) combined with thalidomide in the treatment of relapsed/refractory multiple myeloma (MM). Methods 35 patients with relapsed/refractory MM were treated with ATO (10 mg/d) and Vit C (2 g/d) for 14 days,28 days per cycle.Meanwhile they were orally administered thalidomide 50 mg/d,and then one week later orally thalidomide was added to 100-150 mg/d. After 3 cycles of treatment, response and the side effects were evaluated. Patients responsed to the regimen were continued the treatment of thalidomide and were followed up to estimate the PFS. Responses were assessed according to the criteria of the EBMT.Adverse events were graded according to the criteria of the WHO.Results The overall response Was 71.43 % (25/35),CR 5.71% (2/35),PR 34.29 % (12/35),MR 31.43 % (11/35).After a median follow-up of 11 months,the median PFS was 9 months.The major adverse reactions were digestive tract reaction,leukocytopenia,liver function damage,peripheral neuropathy.They were mild,and could be tolerated.Conclusions The combination of ATO and thalidomide is a feasible and active regimen in treatment of relapsed/refractory MM with better compliance in general.

10.Region Ⅵ lymph nodes dissection in treatment of 38 cases with cN0 papillary thyroid carcinoma

Objective To discuss the significance of region Ⅵ lymph nodes dissection in treatment of patients with clinical cervical nodes negative(cN0)papillary thyroid carcinoma(PTC).Methods Clinical data of 38 cases of cN0 PTC treated with region Ⅵ lymph nodes dissection were retrospectively analyzed.The related literature was reviewed.The cervical nodes dissection scope and the key operation points in treatment of cN0 PTC were discussed.Results No permanent recurrent laryngeal nerve injury or parathyroid damage happened.14 cases(36.84%)had occult lymph node metastasis to region Ⅵ lymph nodes.After more than 5 years of follow-up,all the cases had excellent life quality.3 cases(7.89%)were found lymph node metastasis to lateral cervical region and they were given functional neck dissection.No recurrence or metastasis occurred to the 3 cases during more than 2 years of follow-up.Conclusion Region Ⅵ lymph node dissection in treatment of cN0 PTC is safe,reliable,and with less complications.
