1.Malignant peripheral nerve sheath tumor in the maxillary sinus: a case report.

We report a case of 46-year-old xanthoderm woman who was diagnosed as malignant peripheral nerve sheath tumors of right maxillary sinus, and have a literature review. Histology confirmed a diagnosis of malignant peripheral nerve sheath tumor. The woman had the right total maxillectomy and postoperative adjuvant radiotherapy. There is no local recurrence or metastasis of one year following up. Literature review revealed MPNST in the nasal cavity and para-nasal sinuses were not common with poor prognosis. The main cause of death is local recurrence and metastasis. Surgical resection showed more advantage than adjuvant radiotherapy and chemotherapy.

3.Study on the Preparation Technique and Quality of Xuelian Freeze-dry Sterile Injection powder

OBJECTIVE:To prepare xuelian freeze-dry sterile injection powder and to establish a quality control for this drug.METHODS:The best formulation and preparation technique of the powder for injection was optimized by screening the variety and quantity of vehicle;the contents,which were determined by UV,together with the finger print were taken as the main index to study the quality.The stability was observed through long test.RESULTS:Take the40%mannitol as excipient;The mean recovery results was99.4%(RSD=0.90%);The freeze-dry sterile injection powder could be reflection the finger print character of extract fluid better than injection.The preparation was stable after12months storage at room tempera-ture.CONCLUSION:The formulation is reasonable,the preparation technique is feasible and the quality is controllable.
4.Association of the C-reactive protein gene polymorphisms and plasma hs-CRP level in ischemic stroke

Objective To investigate the association of C-reactive protein (CRP) gene polymorphisms and plasma hs-CRP level,and effect on the genetic susceptibility of ischemic stroke (IS).Methods A case-control study was conducted and 548 patients with acute ischemic stroke and 993 agematched controls from community-based population were included in this study.Epidemiological questionnaires were managed to collect for demographic information.Blood pressure was measured and blood glucose,triacylglycerol,cholesterol,and high sensitivity C-reaction protein (hs-CRP) were detected.Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was used for genotyping of CRP gene in all participants.Results The levels of plasma hs-CRP and the proportion of elevated plasma hs-CRP (≥3.0 mg/L) in the ischemic stroke patients (3.534 ± 3.484) mg/L (43.1%) were significantly higher than those of controls (1.957 ±2.344) mg/L (16.6%),t =9.475,P < 0.01,x2 =128.326,P < 0.01.The results of association analysis indicated that rs3093059 and rs3091244 of CPR gene presented statistical associations with ischemic stroke.After correction for confounding factors,ORs (95% CI) of additive model and dominant model of rs3093059 were 0.697 (0.528-0.921),0.671 (0.487-0.923) respectively.ORs 95% CI) of dominant model of rs3091244 was 0.728 (0.536-0.988).Further analysis indicated the polymorphism of rs876537,rs3093059,rs3091244 of CPR genotyping were significantly associated with plasma hs-CRP elevation (≥ 3.0 mg/L) both in ischemic stroke patients and in controls (P <0.05).Conclusion The CRP genetic polymorphisms were negatively associated with ischemic stroke,and positively corrleted with plasma hs-CRP elevation.However,plasma hs-CPP was positively correlated with ischemic stroke.These results suggested that the plasma hs-CRP levels might be accompanied by ischemic stroke.
5.Automatic evaluation of left ventricular systolic function in rats with myocardial infarction by myocardial contrast echocardiography based on mathematical morphology algorithm

Objective To evaluate the feasibility and value of quantitatively assessing left ventricular systolic function in rats with myocardial infarction (MI) by myocardial contrast echocardiography (MCE) based on mathematical morphology algorithm.Methods SD rats model of MI were mede,then MCE were performed before MI and at 1,3 weeks after MI.In vitro study:based on the principle of binary morphology,segmentation of endocardium was acquired using dilation,erosion,closed operations and connected domain object tag-based.The automatic segmentation of endocardial contour was compared with the manual segmentation boundary.In vivo study:the left ventricular area was calculated using the software,and the area variation fraction (AVF),a new index of left ventricular systolic function,were acquired.The correlation between AVF and left ventricular ejection fraction (LVEF) was analyzed.The value of AVF for diagnosis for left ventricular dysfunction was evaluated by ROC curve.Results ① The cross-correlation coefficient (CCC) and percent error (PE) between automatic contours and manual boundary were more than 0.90 and less than 9%,respectively.② AVF correlated positively with LVEF (r =0.934,P < 0.001).③ ROC analyses showed the area under curves for AVF diagnosing left ventricular dysfunction was 0.834.The best cutoff value was 27.5 % for diagnosing left ventricular dysfunction with the sensitivity of 85 % and specificity of 60 %.Conclusions Left ventricular endocardium can be identificated automatically and rapidly by MCE based on mathematical morphology algorithm.AVF can assessed quantitatively left ventricular systolic function in rats with myocardial infarction.
6.Experience on surgical treatment to 78 cases of acetabular fracture

Objective To discuss the treatment and clinical results of acetabular fracture. Method Clinical data of 78 cases of acetabular fracture was analyzed retrospectively on treatment methods and clinical effects from August 1988 to December 2007, 41 cases were performed operation and 37 cases were performed non-operative management. Result The treatment effects were excellent in 43 cases, good in 22 cases, fair in 10 cases, poor in 2 cases and dead in 1 cases, with a good rate of 83.3%, and 20 cases had complications: 1 posterior dislocations of hip, 13 posttraumatic arthritis, 1 incisional infection, 5 ectopic ossi-fication. Conclusions It is very important to choose appropriate treatment methods by judging the real state of acetabular fractureexactly. Acetabular reconstruction plate is convenient, safe, and reliable in treat-ment of posterior wall and superior wall fractures.
7.Effect of 3-N-butylphthalide pretreatment on the score of neurological deficit, oxidative stress and pathomorphology in rats with cerebral ischemia reperfusion injury

Objective To investigate the effects of 3-N-butylphthalide ( NBP ) pretreatment on the score of neurological deficit , oxidative stress and pathomorphology in rats with cerebral ischemia reperfusion injury ( CIRI ) . Methods Ninety male SD rats were randomly divided into sham operation group ( Sham group ) , model group ( IR group), NBP pretreatment low dose group (NBPⅠgroup), NBP pretreatment middle dose group(NBPⅡgroup) and NBP pretreatment high dose group(NBPⅢgroup), 18 rats per group.Pretreatment was given once a day within 1 week before establishing the model of cerebral ischemia reperfusion injury .The model of middle cerebral artery occlusion ( MCAO) was subjected by suture method .The score of neurological deficit was executed after ischemia for 2h and reperfusion for 24h in all the rats.The cerebral infarction was observed by TTC staining .The pathologic change of brain was observed by HE staining under the microscope .Hydroxylamine method was used to detect activity of SOD , chemical colorimetry method was used to measure activity of GSH-PX, and TBA method was used to detect content of MDA .Results (1) In Sham group, the score of neurological deficit and the percentage of infarction volume were zero , the morphology of nerve cell was regular , and activity of SOD, GSH-PX and content of MDA of brain tissue were normal .(2) Compared with IR group , the score of neurological deficit was significantly reduced in NBP pretreatment groups (all P<0.01); the score of neurological deficit was decreased progressively in turn in NBP Ⅰ,Ⅱ,Ⅲgroup (all P<0.05).(3) Compared with IR group, the percentage of infarction volume was cut down progressively in turn in NBPⅠ,Ⅱ,Ⅲgroup (all P<0.05), and neuron injury was also induced obviously in NBP pretreatment groups .(4) Activity of SOD, GSH-PX was largely increased , and content of MDA was greatly decreased in NBP pretreatment groups ( P<0.01 ) .Activity of SOD , GSH-PX went up progressively in turn , and contents of MDA were cut down progressively in turn in NBP Ⅰ,Ⅱ,Ⅲgroup ( all P<0.05 ) .Conclusion 3-N-butylphthalide can significantly up-regulate the activity of SOD and GSH-PX, decrease the content of MDA , reduce the percentage of infarction volume , and relieve the damage of nerve cell to preventively protect the rats with cerebral ischemia reperfusion injury .
8.Effects of 3-n-butylphthalide pretreatment on microvascular architecture and free radical metabolism in hippocampus induced by cerebral ischemia/reperfusion

Objective To observe microvascular architecture and free radical metabolism in hippocampus after focal cerebral ischemia/reperfusion and to explore the effect of NBP (3-n-butylphthalide). Methods Fifty-four SD rats were ran?domly divided into NBP pretreatment group, ischemia/reperfusion group and sham operation group (n=18 in each group). The model of middle cerebral artery occlusion(MCAO)was established by suture method. The neurological scores were counted and the volume of infarction was measured;TA-Fe method was applied to observe the microvascular architecture of hippo?campus, Mivnt image analysis system was used to analyze the microvessel density(MVD)and the microvessel area density (MVA)of hippocampus quantitatively;The activity of SOD and content of MDA were measured by colorimetric method. Re?sults Compared to the ischemia reperfusion(IR)group, the neurological scores and the volume of infarction were decreased sharply in NBP group. What′s more, the activity of SOD, MVD and MVA were all enhanced but the content of MDA and the count of closed microvessels were both reduced(P < 0.01). Conclusion NBP can improve microvascular architecture of hippocampus and reduce the free radical injury. There is a protective effect on hippocampus of rats who suffered focal cere?bral ischemia reperfusion.
9.Association between the polymorphism of C-reactive protein gene and plasma hs-CRP level in ischemic stroke patients

Objective To investigate the association between the polymorphism of C-reactive protein (CRP) gene and plasma level of hs-CRP in patients with ischemic stroke.Methods 548 patients with acute ischemic stroke were considered to be the case group and 993 age-matched healthy controls were randomly selected from community-based population.Three tagging SNPs of the CRP gene (rs876537,rs3093059,and rs3091244) were genotyped by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP).The plasma hs CRP levels were measured by routine method and multiple logistic regression was applied to evaluate genetic effect on plasma hs-CRP level.Results (1) The hs-CRP concentration and elevation (≥3.0 mg/L) proportion were significantly higher in case group than in control group [(3.534 ± 3.484) mg/L vs.(1.957 ± 2.344) mg/L,43.1% vs.16.6%,t=10.74 and 23.45,both P<0.05].(2) Compared with controls,the case group had higher conventional vascular risks,including higher blood pressure [SBP:(145.33 ± 22.11) vs.(134.92±19.48) mmHg,t=14.30,P<0.05; DBP:(88.14±12.72) vs.(81.42± 12.44) mmHg,t=12.44,P<0.05],higher low-density lipoprotein cholesterol [(2.714±0.904)vs.(2.286 ± 0.704) mmol/L,t =9.72,P< 0.05],and lower concentration of high-density lipoproteincholesterol [(1.213±0.317) vs.(1.438±0.262) /L,t=-12.89,P<0.05].(3) Both rs876537 and rs3093059 polymorphism of CRP gene showed statistical associations with elevation of plasma hs-CRP level in case group.After adjusted for covariates including age,sex,hypertension,type 2 diabetes and other factors,additive model and dominant model of rs3093059 showed that the adjusted ORs (95 % CI) were 1.548 (1.103-2.171),1.562 (1.070-2.281) respectively (both P< 0.05).And ORs (95% CI) of additive model and dominant model of rs876537 were 1.368 (1.067-1.753),1.719 (I.190-2.482) respectively after adjusted for covariates (both P < 0.05).Conclusions Plasma hs-CRP concentration is significantly higher in ischemic stroke patients than in healthy population.The polymorphism of rs876537 and rs3093059 of CRP gene has association with elevation of plasma hs-CRP level in ischemic stroke patients.
10.Evaluation of pulmonary arterial hypertension with simple congenital heart disease using tissue Doppler imaging

Objective To evaluate the value of tissue Doppler imaging (TDI) in diagnosing pulmonary arterial hypertension(PAH) with common simple congenital heart disease(CHD).Methods 104 patients of CHD approved by operation or intervention were retrospectively analyzed.Both TDI and right cardiac catheterization were performed in all patients,whose were divided into PAH and non-PAH groups according to mean pulmonary arterial pressure (mPAP) on diagnostic right cardiac catheterization:60 PAH patients[mPAP ≥25 mmHg(1 mmHg =0.133 kPa) and pulmonary arterial capillary wedge pressure ≤15 mmHg],44 non-PAH patients (mPAP <25 mmHg).The PAH group was divided into mild (25 mmHg≤ mPAP≤35 mmHg),moderate (36 mmHg≤mPAP≤50 mmHg) and severe (mPAP>50 mmHg) subgroups.The tricuspid annular systolic peak velocity (TS'),early diastolic peak velocity (TE'),late diastolic peak velocity (TA') were measured by TDI and TE'/TA' and Tei index were calculated.The correlation between TDI parameters and the cardiac catheterization findings (sPAP,mPAP and dPAP) were assessed.Results ①TS' and TE' in PAH group were significantly lower than those in non-PAH(P < 0.05),but Tei in PAH group were significantly higher than that in non-PAH (P <0.001),however,there were no significant difference in TA' and TE'/TA' between two groups (P > 0.05).②Tei correlated positively with pulmonary arterial pressure (P <0.001),TS' was found to be negatively correlated with pulmonary arterial pressure(P <0.05),but no correlation between TE',TA',TE'/TA' and pulmonary arterial pressure(P >0.05).③ROC analysis showed the area under curves for Tei index and TS' diagnosis for PAH was 0.893 and 0.699,respectively.At the cutoff of Tei>0.49 and TS'<16 cm/s for indicating PAH,the sensitivity were 86.67 % and 80.00%,respectively and the specificity were 79.55 % and 45.45 %,respectively.④Tei in the moderate and severe PAH were significantly higher than that in mild PAH (P < 0.05),TS' and TE' in severe PAH were significantly lower than those in mild and moderate PAH (P < 0.05),At the cutoff of Tei>0.55 for indicating moderate or severe PAH,the sensitivity and specificity were 95.24% and 72.22%.Conclusions Tei index and TS' have high application value in assessing PAH and classification of PAH.Tei>0.49 indicates PAH,while Tei>0.55 suggests moderate or severe PAH.
