Objective To summary the clinical characteristics of epileptic spasm(ES)that is the unique form of seizures in childhood. Methods A total of 149 patients with ES who visited our hospital from May 2009 to June 2015 were collected. The documents of clinical data, treatment and outcome were analyzed retrospectively. The statistical analysis of some clinical characteristics and prognosis of patients were performed. Results There were 97(65.1%) male and 52 (34.9%) female in the 149 patients. The onset age of ES was from 30 days to 42 months (median value was 6 months) including 17 patients more than 12 months.The peak age of onset was 4-7 months. One hundred and forty-seven patients manifested clustered spasm. One pair of twins showed single spasm.Other types of seizure were seen before ES onset in 13 (8.7%) patients, and after ES onset in 42 (28.2%) patients.Eighty-three patients (55.7%) were diagnosed as West syndrome (WS). There were 46 patients (30.9%) with cryptogenic cause, 103 patients (69.1%) diagnosed with symptomatic epilepsy, in which 69 patients were with a clear cause. The proportions of developmental delay in the cryptogenic and symptomatic patients were 74.5% and 92.2% after the onset of the ES. The degree of developmental delay was linearly related to the course of ES. The hyperarrhythmia of electroencephalogram (EEG) background was seen in 122 patients (81.9%), and there was no significant difference in hyperarrhythmia between different etiology groups or different onset age groups. The short-term efficiency of topiramate evaluated within one month was 30.5% in 59 newly diagnosed patients. The short-term effective rate of adrenocorticotropic hormone (ACTH) was 80.3% in total 76 patients, but single ACTH treatment was associated with an increased recurrent rate. Conclusion ES is a highly age dependent seizure type that mainly found in WS,which typically follows a EEG hyperarrhythmia and developmental delay. The treatment of ES is difficult. ACTH combined with effective antiepileptic drugs (AEDS) is currently the most reasonable drug treatment program.

Objective:To investigate the effects of IL-12p35 small interference RNA on immune functions of Dendritic cells in rats.Methods:DCs were generated by culturing bone marrow progenitor cells of rats with GM-CSF, IL-4 and LPS in vitro. The IL-12p35 siRNA was synthesized and transfected into DCs. The expressions of CD80 and MHCⅡwere examined by flow cytometry. Reverse transcriptase PCR analysis was used to detect IL-12p35 mRNA transcription and ELISA was used to detect IL-12 and IL-10 protein levels, respectively. The antigen presenting by DCs was evaluated by mixed lymphocyte responses.Results:IL-12p35 siRNA silenced DCs expressed lower IL-12, higher IL-10,and exhibited weak activity in stimulating the proliferation of allogenic T cells, but no changes on CD80 and MHCⅡexpressions.Conclusion:IL-12p35 siRNA interference exerts no effects on maturation of DC, but negative effect on immume function of DCs.

Objective To investigate the effects of a proliferation-inducing ligand(APRIL) on migration and invasion of colorectal cancer (CRC) and matrix metalloproteinases (MMPs) expression in order to observe the role of APRIL in CRC metastasis.Methods The siRNA plasmid vector targeting APRIL gene (siRNA-APRIL) was transfected into SW480 cells and recombinant human APRIL(rhAPRIL) was used to stimulate HCT-116 cells.Tumor cell migration and invasion were measured by Transwell chambers.RT-PCR and ELISA were applied to examine the expression level of MMPs.Results Metastatic and invasive capacities of siRNA-APRIL transfected SW480 were significantly inhibited,and these capacities of APRIL stimulated HCT-116 cells were significantly enhanced compared with their respective controls( all P＜0.05 ),accompanied with the alterations of MMPs mRNA and secreted protein expression( P＜0.05).The number of invading cells of SW480 control and rhAPRIL stimulated HCT-116 was significantly decreased by a MMP inhibitor GM6001 ( P＜0.05 ).Conclusion APRIL facilitates migration and invasion of CRC via regulation of MMPs,which suggests that APRIL might be used as a new target for the intervention and treatment of CRC metastasis.

ObjectiveTo determine the effect of multi-criteria decision analysis (MCDA) on the effect of bundle treatment for severe pneumonia.Methods A prospective historical control observation was conducted. Seventy-five patients with severe pneumonia having received MCDA (from January 2013 to August 2014) were assigned as intervention group. MCDA group was set up by the medical staff. Bundled treatment plan was composed of the MCDA evaluation results, anti-infection, phlegm and other conventional treatment measures which was adjust on time until the patient was transferred out of the respiratory intensive care unit (RICU) or died. Seventy patients with severe pneumonia before receiving MCDA (from August 2010 to December 2012) were set as historical control group. Comparison of general condition before treatment and the incidence of hospital infection, average hospitalization cost, duration of RICU stay and mortality between these two groups were performed.Results There were no statistically significant differences in gender, age, past history, and acute physiology and chronic health evaluationⅡ (APACHEⅡ) score at admission between two groups. Compared with control group, the incidence of hospital infection [1.33% (1/75) vs. 11.43% (8/70),χ2 = 4.723,P = 0.030], mean hospitalization cost in RICU (10 thousand Yuan: 3.44±0.79 vs. 3.76±0.91,t = 2.265, P = 0.025), length of RICU stay (days: 15.01±4.22 vs. 16.92±4.79,t = 2.552,P = 0.012) and mortality in RICU [8.0% (6/75) vs. 21.4% (15/70),χ2 = 5.272,P = 0.032] in intervention group was significantly decreased. Conclusions Application of MCDA in the bundle treatment of severe pneumonia could elevate the scientificalness of decision, and reduce the medical cost. Additionally, MCDA is worth to be generalized because the implementation of guidelines can improve the clinical outcome and prognosis of the patients.

Objective　To investigate the diagnosis and treatment of acute rejection of orthotopic liver transplantation (OLT). Methods　From July 1999 to April 2000, two piggyback liver transplantations were performed on two patients with Wilson's disease. Results　Cyclosporine A, azathioprine and methylprednisolone were the baseline immunosuppression management. Acute rejection occured 5 times in the 2 patients. The correct diagnosis was obtained through clinical inspection, liver function test and biopsy. The rejections were controlled by intensive steriod therapy plus OKT3 or FK506. Conclusions　Clinical inspection and liver function test can suggest the episode of acute rejection in time. Hepatic biopsy is the key point for diagnosis of acute rejecton, Reasonable use of immunosppression is critical for the treatment.

Objective.Spleen transplanlation has developed to be an effective strategy for hemophilia A.But it has not been reported up to date that which kind of established solutions is most suitable for perfusion and preservation of spleen.This study aimed to establish some experiences with the comparison among Hartmann's solution, Collins'solution and WMO-I solution,in order to instruct the clinical spleen transplantation.Methods.After the splenic artery and vein were dissociated clearly,three kinds of perfusion solutions began to perfuse the corresponding segments of spleen with a randomized sequence.When the efferent fluids from the splenic vein became clear, the perfused spleen segments were preserved for different durations with the same perfusing solution to calculate the survival rate of splencocyte(SRS) and were examined with light and electron microscopy.Results.Among the three solutions,SRS with WMO-I solution was significantly higher than those of the other two(P＜0.001).The perfused spleen with WMO-I solution showed the slightest morphological changes and a significant longer preservation duration than those with the other two(P＜0.05 or P＜0.01).Conclusion.Among the three solutions,WMO-I solution was most suitable for perfusion and preservation of spleen.

Abstract Obesity has become a global public health problem that affects human health and poses a huge threat to the health of children in China. More and more studies have shown that social environment including social culture, food tax, food advertising and marketing, community food environment and community built environment exerts impact on childhood obesity. With the aim of fully developing the Healthy China and building a moderately prosperous society in all respects, as well as achieving the goal of "Achieving socialist modernization by 2035", it is also necessary to strengthen the comprehensive social environment, develop a sustainable and health supported environment for childhood obesity control and healthy growth and development for all children.

Objective: To evaluate the impact of food advertising on childhood obesity by reviewing relevant literatures, and to provide evidence support for childhood obesity prevention and control strategies in China. Methods: A systematic review method was used to search relevant literatures published to November 30，2020 from 8 databases including CNKI, VIP, Wanfang Data, PubMed, Medline, SpringerLink, Web of Science, Science Direct. The high quality systematic reviews published since 2016 were included directly, and those published before 2016 were rereviewed after combined with other literatures. Results: A total of 13 articles were included for evaluation, including systematic reviews, cohort studies, randomized controlled trials and cross sectional studies. Children s exposure to food advertisements could increase energy intake. Television food advertising could influence children s food choices and increase the consumption of unhealthy foods such as sweets and sugary drinks. Children s exposure to unhealthy food advertisements could increase the risk of obesity. Conclusion Regulating the food advertisement can help reduce the risk of childhood obesity.

Objective: To evaluate the impact of sugar sweetened beverages (SSBs) tax on childhood obesity by literatures reveiew, and to provide evidence support for childhood obesity prevention and control strategies in China. Methods: A systematic review method was used to search relevant literatures published to November 30，2020 from 8 databases including CNKI, VIP, Wanfang Data, PubMed, Medline, SpringerLink, Web of Science, and Science Direct. The high quality system reviews published since 2016 were included directly, and those published before 2016 were rereviewed before combined with other literatures. Results: Twelve studies including 5 systematic reviews (or Meta analysis), 1 cohort study and 6 cross sectional studies were included. Comprehensive analysis showed that the prevention effect on childhood obesity was small or even ineffective if the tax on SSBs lower than 5%, a certain protective effect on childhood obesity could be shown if the tax on SSBs increased to 20%. From the perspective of cost benefit analysis, the implementation of SSBs tax showed certain net benefits. Conclusion SSBs tax can help reduce the risk of childhood obesity.
