1.The relation of change of intestinal permeability with early operation in acute pancreatitis

Objective To study the change of intestinal permeability in patients with acute pancreatitis (AP) to investigate the indication of early operation in AP patients. Methods Serum concentrations of glutamine, endotoxin and urine lactulose/mannito (L/M) ratio were measured and compared at different time in 18 AP patients received non-operative treatment and 7 underwent operation . Results Serum concentrations of glutamine, endotoxin and urine lactulose mannito (L/M) ratio were significantly higher in operation treatment AP patients than those in non-operation treatment AP patients. Conclusion In the early course of AP, continued increased intestinal permeability and serum concentration of endotoxin is the indications of operation.
2.Application of nucleic acid test in voluntary blood donors in Hefei area

Objective To discuss the role of nucleic acid test(NAT)in blood screening to provide the scientific basis for the se-lection of blood screening strategies.Methods The serological detection and NAT were simultaneously performed on 68 662 speci-mens of voluntary blood donation from January 2012 to April 2013;29 samples of HBV DNA positive with serological HBsAg nega-tive were performed the serum HBV markers detection.Partial samples of anti-HIV antibody positive were sent to CDC for conduc-ting the confirmation test.Results Among 68 662 samples,120 cases of single NAT positive were detected out,the residual risk of blood transfusion was 0.175%.In the serum HBV markers detection,the mode of HBcAb was predominant.11 cases of HIV posi-tive were confirmed and all were the NAT positive samples.Conclusion NAT can reduce the residual risk of blood transfusion,en-sure the safety of blood transfusion.NAT and the serological detection are mutual complementation and not replaced by each other. The suitable screening strategy shold be selected.
3.Effect of miRNA interference to annexinA3 gene on growth of human gallbladder cancer cells

Objective To construct a recombinant interfering RNA(miRNA)plasmid vector targeting annexinA3 and observe its impact on the growth and apoptosis of human gallbladder cancer cell line SGC-996.Methods Four small fragments of miRNA and one negative control were sequenced and cloned into vector pcDNATM 6.2-GW/EmGFPmiR.The recombinant plasmids were then transfected into gallbladder cancer cell line SGC-996 by positive liposomal transfection.The transcription and translation level of annexinA3 expression were detected by RT-PCR and Western Blot.Meanwhile, the proliferation and apoptosis of transfected tumor cells were determined by MTT and flow cytometry.Results The recombinant plasmids containing annexinA3 miRNA were successfully constructed.AnnexinA3 expression was significantly knocked down after miRNA plasmid transfection.Transfection of annexinA3 gene miRNA could effectively inhibit the proliferation of gallbladder cancer cell and promote apoptosis.Conclusion AnnexinA3 gene silenced by miRNA can induce the apoptosis of human gallbladder cancer cell line SGC-996.The process might serve as a potential approach for cancer gene therapy.
4.Combined rapid palatal expansion (RPE), edgewise technique and protraction headgear in correction of skeletal Class III malocclusion.

OBJECTIVEThe skeletal Class III malocclusion often occurs in ethnic Asia population, and it is difficult to obtain an ideal results if only treated with protraction headgear (PH). However, the combined rapid palatal expansion (RPE), Edgewise Technique and PH can reach it, so the authors investigated the methods and characteristics of combined RPE, Edgewise Technique and PH in the treatment of skeletal Class III malocclusion.
METHODSFifteen patients, aged 8.5-12 years, including six males and 9 females, with skeletal Class III malocclusion and maxillary retrusion were selected for this study. Radiographs of left hand and wrist showed that all patients were at the pre-pubertal stage. The duration of treatment was nine months on average.
RESULTSThe SNA and A-ptm increased with a mean of cephalometric analysis, which showed significant difference (P < 0.05).
CONCLUSIONObvious skeletal and profile changes can be achieved with combined RPE, Edgewise Technique and PH in the treatment of early skeletal Class III malocclusion.

5.Establishment of a mouse model of cutaneous squamous cell carcinoma induced by 7,12-DMBA, croton oil and narrow-band ultraviolet B irradiation

Objective To establish a mouse model of cutaneous squamous cell carcinoma induced by 7,12-dimethylbenz(a)anthracene (7,12-DMBA)/croton oil and narrow-band ultraviolet B (NB-UVB) irradiation.Methods A total of fifty 6-8-week old BALB/c mice (male:female 1:1) were randomly divided into three experimental groups.The group A was treated with chemical carcinogens alone, group B was treated with NB-UVB alone, and group C was treated with chemical carcinogens plus NB-UVB.The general status and skin appearance of mice were observed during the experiment.The survival rate and tumor formation rate of each group was calculated at weeks 5, 10, 15, and 20. Pathological examination was carried out to observe the histological changes of skin lesions.Results Papules measuring≥l mm in diameter began to develop in some mice of the group C at 5 weeks after the first treatment with chemical carcinogens.The tumor formation rates at 20 weeks after treatment were 86.67%, 7.14%, 94.12%in the groups A, B, C, respectively.Pathological examination revealed characteristic changes of squamous cell carcinoma in 13.34%, 0%, 70.59%of the mice in the group A, B, C, respectively.Conclusions Establishment of a mouse model of cutaneous squamous cell carcinoma induced by 7,12-DMBA/croton oil and NB-UVB is a better method than treated with chemical carcinogens alone or NB-UVB alone.This method can increase the tumor formation rate and incidence rate of SCC, and within a shorter period.
6.An EMD based epileptic spike detection method.

The automatic spike detection in EEG is significant in both diagnosing illness and alleviating the heavy labour force of the doctor. This paper proposes a new EMD based method to complete spike detection. It decomposes a signal into a few intrinsic mode functions (IMF), and then applies the nonlinear energy operator (NEO) to the first IMF to complete the automatic detection. Sufficient results are obtained by applying this method to the spike detection of the simulation signal and the real epileptic EEG signal.

7.An EMD based time-frequency distribution and its application in EEG analysis.

Hilbert-Huang transform (HHT) is a new time-frequency analytic method to analyze the nonlinear and the non-stationary signals. The key step of this method is the empirical mode decomposition (EMD), with which any complicated signal can be decomposed into a finite and small number of intrinsic mode functions (IMF). In this paper, a new EMD based method for suppressing the cross-term of Wigner-Ville distribution (WVD) is developed and is applied to analyze the epileptic EEG signals. The simulation data and analysis results show that the new method suppresses the cross-term of the WVD effectively with an excellent resolution.

8.Anterior reconstruction plate fixation for unstable posterior pelvic ring injuries

Objective To discuss the surgical indications,relative merits and clinical outcomes of anterior reconstruction plate fixation for treating unstable posterior pelvic ring injuries.MethodsA retrospective study was done on clinical data of 36 patients with unstable posterior pelvic ring injuries treated with anterior reconstruction plate fixation from August 2004 to July 2010 and followed up for minimum one year.There were 25 males and 11 females,at mean age of 35.8 years ( range,13-62 years).There were two patients with anterior dislocation of the sacroiliac joint and 34 with posterior dislocation.According to Tile classification,there were four patients with type B fractures ( B1:3,B2:1 ) and 32 with type C fractures (C1-1:17,C1-2:12,C2:2,C3:1 ).Results All patients were followed up for average 2.3 years ( range,1-6 years).Fat liquefaction and superficial infection were founded in two patients who were cured by dressing change and frequent but low-dose blood transfusion.Eight patients were subjected to iatrogenic lateral femoral cutaneous nerve injury.Meanwhile,five of them was recovered after treatment with neurotrophic drugs,but there were still three patients leaving lateral thigh numbness.One patient had L5 nerve root injury due to pull force,and was recovered after three months of neurotrophic drug therapy.According to Matta and Tometta criteria,the reduction results were excellent in 23 patients,good in 11 and fair in four,with excellence rate of 94%.No reduction loss or implant failure occurred.According to Majeed scoring system,the clinical outcomes were excellent in 17 patients,good in 14,fair in four and poor in one,with excellence rate of 86%.Of the seven patients pre-operatively associated with sacral plexusinjury,three obtained full recovery,two got partial recovery and two were free from recovery.Conclusion The anterior reconstruction plate fixation takes advantages of wide surgical indications,simple exposure,low infection rate,satisfactory reduction and solid fixation and may be a main treatment method for unstable posterior pelvic ring injuries.
9.Development of a cell-based diagnostic system for vitamin K-dependent coagulation factor deficiency 1.

OBJECTIVE:
To develop a cell-based system for the diagnosis of vitamin K-dependent coagulation factor deficiency 1 (VKCFD1).
METHODS:
In HEK293 cells stably expressing the reporter gene FIX-Gla-PC, the gamma-glutamyl carboxylase (GGCX) gene was knocked out by using CRISPR/Cas9 technology. Enzyme-linked immunosorbent assay (ELISA), DNA sequencing and Western blotting were used to identify the GGCX gene knockout cells. A quickchange point variant method was used to construct the GGCX variant. ELISA was used to assess the influence of GGCX variant on the activity of reporter gene.
RESULTS:
Two monoclonal cell lines with no reporter activity by ELISA was identified. Edition and knockout of the GGCX gene was confirmed by DNA sequencing and Western blotting. The activity of the reporter gene was recovered by transfection of the wild-type GGCX gene. Thereby two monoclonal cells with GGCX knockout were obtained. By comparing the wild-type and pathogenic GGCX variants, the reporter activity was decreased in the pathogenic variants significantly.
CONCLUSION
A cell-based system for the detection of GGCX activity was successfully developed, which can be used for the diagnosis of VKCFD1 caused by GGCX variants.
10.Policy analysis on the function orientation and development of infectious disease hospitals in China

Objective:To analyze the policy tools designed for function orientation and development of China′s infectious disease hospitals, in order to provide references for formulating and optimizing the functional implementation and sustainable development policies of these hospitals.Methods:Consulting the website of Peking University Law and official websites on health, and using such keywords as " medical institutions, infectious diseases, hospitals for infectious diseases, public health emergencies, specialized hospitals", and searching the national policy texts on the functions and development of infectious disease hospitals issued from December 1991 to January 2023. By means of Rothwell and Zegveld′s policy tool classification method, Nvivo 11 Pro software was used to analyze the text content from the dimensions of demand-type, supply-type, and environmental-type policy tools.Results:A total of 41 policy texts were included and 204 codes were obtained through text analysis. Of all the tools, environmental and supply-oriented policy tools were used the most, being 104 (50.99%) and 95 (46.56%) respectively, while demand-oriented policy tools were used the least, only 5 (2.45%). Of all the environmental-oriented policy tools, " goal planning" was the most used, being 34 (16.67%), while " publicity and guidance" was less used, only 4 (1.96%). Among the supply-oriented policy tools, " clarifying the reporting and handling service functions of public health emergencies" was the most widely used, being 34 (16.67%), while " Informationization" and " infrastructure construction" was less used, being 8 (3.92%) and 6 (2.94%) respectively. Few of the demand-oriented policy tools were in use, as " government purchase" and " medical insurance support" were both 2 (0.98% each), and " social medical care" was only 1 (0.49%).Conclusions:The distribution of three types of policy tools for function and development of infectious disease hospitals is unbalanced, the use of environmental and supply-oriented policy tools is excessive and internal structure is unbalanced. The proportion of environmental-oriented policy tools needs to be adjusted, and the frequency of use of supporting tools such as relevant standards, norms, incentives, supervision and publicity should be increased. Supply-oriented policies need to be further optimized, especially in terms of personnel training, funding, information technology and infrastructure construction. Demand-oriented policy tools are seriously underused and need to be further developed.
