1.Clinical Efficacy of Guizhi Shaoyao Zhimutang Combined with Fire Needling in Treatment of Periarthritis of Shoulder with Wind-cold-dampness Impediment Syndrome by Stimulating Pain Points and "Shoulder Three Acupoints"

ObjectiveTo investigate the clinical efficacy of Guizhi Shaoyao Zhimutang (GZSYZM) combined with fire needling in the treatment of periarthritis of shoulder with wind-cold-dampness impediment syndrome by stimulating pain points and "shoulder three acupoints". MethodA total of 120 patients with periarthritis of shoulder with wind-cold-dampness impediment syndrome, admitted to Hainan General Hospital from August 2020 to August 2022, were randomly divided into two groups using a random number table. The control group (60 cases) received treatment with GZSYZM for two weeks, while the observation group (60 cases) received treatment with GZSYZM combined with fire needling at pain points and "shoulder three acupoints" for two weeks. The clinical efficacy, adverse reactions, shoulder pain intensity, shoulder joint function, and levels of calcitonin gene-related peptide (CGRP), cyclooxygenase-2 (COX-2), interleukin (IL)-2, and IL-17 before and after treatment were compared between the two groups. ResultThe total effective rate in the observation group was 88.33% (53/60), significantly higher than 68.33% (41/60) of the control group (χ²=7.070, P<0.01). Compared with the results before treatment, both groups showed significant reductions in pain rating index (PRI), visual analog scale (VAS) scores, present pain intensity (PPI), and Simplified McGill Pain Questionnaire (SF-MPQ) total scores, as well as serum levels of CGRP, COX-2, IL-2, and IL-17 after treatment (P<0.01), and improved pain intensity, daily life abilities, joint mobility, muscle strength, and Constant-Murley score (P<0.01). Compared with the control group after treatment, the observation group showed significantly reduced PRI, VAS score, PPI, SF-MPQ total score, as well as serum levels of CGRP, COX-2, IL-2, and IL-17 (P<0.01), and increased pain intensity, daily life abilities, joint mobility, muscle strength, and Constant-Murley score (P<0.01). There was no significant difference in the incidence of adverse reactions between the two groups. ConclusionGZSYZM combined with fire needling at pain points and "shoulder three acupoints" can effectively reduce the levels of serum inflammatory factors and pain mediators, alleviate pain, and improve shoulder joint function in patients with periarthritis of shoulder with wind-cold-dampness impediment syndrome.
2.Cross-sectional analysis of gonadal hormone expression and relevant factors in female centenarians in Hainan, China.

OBJECTIVES:
Gonadal hormone is essential for the health of postmenopausal women, however, few studies have focused on the epidemiological distribution of gonadal hormones in postmenopausal women in very late postmenopausal women. This study aims to investigate and analyze the differences of serum gonadal hormone content and its influential factors among female centenarians in Hainan, China.
METHODS:
The questionnaire and physical examination data of 741 female centenarians and 401 elderly females in Hainan Province were collected, and venous blood samples were taken to detect the indexes of lipid metabolism, bone metabolism, and gonadal hormone. The differences of gonadal hormones and relavant factors in female centenarians were analyzed and compared.
RESULTS:
The serum levels of estradiol and progesterone of female centenarians were significantly higher than those of the elderly females (both P<0.001). The serum levels of estradiol and testosterone of ethnic minority centenarians were higher than those in Han nationality (P<0.001), and the serum estradiol and testosterone concentrations were relatively higher when the daily activities were more than 10 min (both P<0.05). Serum estradiol concentration was negatively correlated with apolipoprotein A-I, high density lipoprotein, triglyceride and bone formation markers such as calcium, inorganic phosphorus and vitamin D3, and was positively correlated with the special sequence of β-collagen (markers of bone resorption) (all P<0.01).
CONCLUSIONS
For the extremely late postmenopausal women (such as centenarians), there may be characteristic expressions of gonadal hormones, especially estradiol. There is an unprotective correlation of serum estradiol with lipid metabolism index and bone metabolism index in female centenarians, so it is necessary to evaluate the estrogen content and the use of estrogen therapy in postmenopausal women.

3.Whole-process individualized pharmaceutical care for a case of melioidosis sepsis

OBJECTIVE To provide reference for the adjustment of antibiotic treatment regimens, identification of adverse reactions, and individualized pharmaceutical care for melioidosis sepsis (MS). METHODS Clinical pharmacists participated in the intensive and eradicating therapeutic processes for an MS patient by using blood concentration and gene detection. Based on the literature, antibiotic treatment regimens of MS were adjusted by determining the blood concentrations of β-lactam and trimethoprim/ sulfamethoxazole (TMP/SMZ) and calculating PK/PD parameters. The causes of adverse drug reactions were analyzed and addressed by detecting drug-related gene polymorphisms through high-throughput sequencing. RESULTS Clinical pharmacists used blood concentration and genetic testing methods to propose adjustments to imipenem-cilastatin sodium dosage and analyze the causes of various adverse drug reactions. PK/PD targets were calculated by measuring the blood concentrations of β-lactam and TMP/SMZ. Clinical pharmacists explained to clinical doctors the compliance status of patients with melioidosis in sepsis and non- sepsis stages through reviewing guidelines and literature; the results of blood concentration and genetic test were used to analyze the correlation of neurotoxicity of MS patients with B14) IMP cmin, and it was found that nephrotoxicity was not related to the cmax of TMP/SMZ, but to the patient’s water intake. After whole-process antibiotic treatment, the patient’s condition improved and was discharged, and the adverse reactions were effectively treated. CONCLUSIONS Clinical pharmacists use blood concentration and genetic tests to assist clinicians in formulating MS treatment regimens, and provide whole-course pharmaceutical care for a MS patient. This method has improved the safety and effectiveness of clinical drug therapy.
4.Improvement effect and mechanism of N-butylphthalide on inflammatory injury of bone marrow mesenchymal stem cells in rats

OBJECTIVE To study the improvement effect and possible mechanism of N-butylphthalide on inflammatory injury of bone marrow mesenchymal stem cells (BMSCs) in rats. METHODS BMSCs of rats were divided into control group, model group, N-butylphthalide low-concentration, medium-concentration and high-concentration groups (10, 20, 50 μmol/L). BMSCs were cultured in vitro and lipopolysaccharide (the final concentration of 10 mg/L) was used to establish the inflammatory injury model. After the intervention of N-butylphthalide, the survival rate, apoptotic rate, the contents of tumor necrosis factor α (TNF- α), interleukin 1β (IL-1β) and IL-6 in cell culture medium, the mRNA expression of nuclear factor-κB(NF-κB) p65, and the protein expressions of caspase-3, B-cell lymphoma 2 (Bcl-2), Bcl-2 related X protein (Bax) and NF-κB p65 in cells were detected. RESULTS Compared with control group, the survival rate and protein expression of Bcl-2 were decreased significantly in model group (P<0.05); the apoptotic rate, contents of TNF-α, IL-1β and IL-6, the mRNA expression of NF-κB p65, and the protein expressions of caspase-3, Bax and NF-κB p65 were increased significantly (P<0.05). Compared with model group, above indexes were significantly reversed in all concentration groups of N-butylphthalide (P<0.05), in concentration-dependent manner. CONCLUSIONS N-butylphthalide can ameliorate the inflammatory injury of BMSCs induced by lipopolysaccharide, and its mechanism may be related to the inhibition of NF-κB signaling pathway.
5.Survival of engineered myocardium constructed by bone marrow mesenchymal stem cells in vitro

BACKGROUND:So far, engineered myocardium is stil facing many problems. Research has demonstrated that bone marrow mesenchymal stem cells can be differentiated into myocardial cells. Polyglycolide and polycaprolactone are commonly used artificial polymers, which have good biocompatibility.
OBJECTIVE:To observe the growth of the poly(glycolic acid)/poly(e-caprolactone) copolymer patch in vitro in normal myocardium and infarcted myocardium.
METHODS:Bone marrow mesenchymal stem cells in Sprague-Dawley rats were separated using adherent separation and selection method, cultured in vitro. The third passage was labeled with DAPI. The bone marrow mesenchymal stem cellsuspensions (2×106/cm2) were produced and planted on poly(glycolic acid)/poly(e-caprolactone) copolymer scaffolds to form poly(glycolic acid)/poly(e-caprolactone) copolymer patch. After culturing for 48 hours, the specimens were observed under electron microscope, stained with hematoxylin and eosin, and then observed under light microscope. Rat models of myocardial infarction were established by ligating left anterior descending coronary artery. Poly(glycolic acid)/poly(e-caprolactone) copolymer patch was implanted into the normal and infarcted myocardium for 5 weeks. The survival of bone marrow mesenchymal stem cells was determined by the detection of pathology.
RESULTS AND CONCLUSION:Results of light microscope and electron microscope demonstrated that bone marrow mesenchymal stem cells grew three-dimensional y on poly(glycolic acid)/poly(e-caprolactone) copolymer patch. cells and patch were adhesive wel . Under laser confocal microscopy, compared with the first week, bone marrow mesenchymal stem cells were marked by DAPI in the myocardium at the fifth week. There were bone marrow mesenchymal stem cells marked by DAPI in the infracted area. Results of hematoxylin-eosin staining exhibited that bone marrow mesenchymal stem cells were detected in the infarct area. These results suggested that bone marrow mesenchymal stem cells adhered to the poly(glycolic acid)/poly(e-caprolactone) copolymer stent wel . The complexes of poly(glycolic acid)/poly(e-caprolactone) copolymer and bone marrow mesenchymal stem cells can be used for reparation of myocardium.
6.Progress of laboratory diagnostic methods of Clostridium difficile infection

Clostridium difficile is a gram-positive,anaerobic spore-forming bacillus commonly found in the environment and human gut.Excessive use of antibiotics,immunosuppressive agents and chemotherapy drugs may lead to overgrowth of the toxic strain of Clostridium difficile with high resistance,which are supposed to be the major contributing factors leading to Clostridium difficile associated diarrhea(CDAD).The globally increasing incidence of CDAD,especially the outbreak of nosocomial infection caused by the hypervirulent strain in North America,arouse worldwide attention.For providing new ideas to the early diagnosis and treatment of CDAD,this review summarize the latest development on pathogenesis and rapid laboratory diagnosis of Clostridium difficile.
7.Expression Level of TGFβ1 and VEGF Gene in Acute Myeloid Patients and Its Clinical Prognostic Value.

OBJECTIVE:
To study the expression level of TGFβ1 and VEGF gene in patients with acute myeloid leukemia (AML) and its clinical prognostic value.
METHODS:
Seventy-eight AML patients treated in our hospital from July 2016 to September 2018 were selected. After isolation of bone marrow mononuclear cells from the patients, the levels of TGFβ1 and VEGF genes were detected by RT-PCR, and the correlation of TGFβ1 with VEGF genes and clinical characteristics of AML patients was analyzed. OS and EFS of the patients were evaluated by Kaplan-Meier, and Cox risk ratio model was used to analyze the prognostic risk factors of AML patients.
RESULTS:
The relative expression level of TGFβ1 gene in AML patients was 0.32±0.04, which was significantly lower than that in control group (P<005). The relative expression level of vascular endothelial growth factor(VEGF) gene in the patients was 2.65±0.15, which was significantly higher than that in the control group (P<0.05). The levels of TGFβ1 and VEGF genes significantly correlated with leukocyte count, hemoglobin, platelet and peripheral blast levels in AML patients (P<0.05). The level of TGFβ1 in AML patients with complete remission was higher than that in patients with partial remission or non-remission (P<0.05). The level of TGFβ1 in AML patients with partial remission was significantly higher than that in patients with non-remission (P<0.05). The level of VEGF in AML patients with complete remission was lower than at in patients with partial remission or non-remission (P<0.05). The level of VEGF in AML patients with partial remission was significantly lower than that in patients with non-remission (P<0.05). Kaplan-Meier survival analysis showed that OS and DFS in AML patients with high expression of TGFβ1 were better than those in patients with low expression of TGFβ1 (P<0.05), OS and DFS in AML patients with low expression of VEGF were better than those in patients with high expression of VEGF (P<0.05). Multivariate Cox regression analysis showed that platelet, TGFβ1 and VEGF gene were independent influencing factors of OS (P<0.05). Leukocyte, TGFβ1 and VEGF gene were independent influencing factors of DFS (P<0.05).
CONCLUSION
Decreased expression of TGFβ1 and increased expression of VEGF gene in AML patients closely relate to the poor prognosis of AML patients, which can provide reference for improving clinical efficacy of AML patients.
8.Strategies against blood collection and supply during the COVID-19 epidemic, Hainan Province

【Objective】 To investigate the strategies against blood collection and supply in Hainan Province during the COVID-19 epidemic, so as to provide theoretical and technical support for ensuring blood balance and safety under epidemic situation. 【Methods】 Countermeasures of blood collection and supply against the COVID-19 epidemic, taken by blood centers in Hainan, were reviewed, and data from 2016 to 2020 were collected and analyzed to evaluate the effectiveness of countermeasures by comparison. 【Results】 During the COVID-19 epidemic, the total units of blood collected and issued continued to grow and the annual growth rate reached 10.3% in 2020. The proportion of blood donation by groups increased by 0.3%. The number of blood donations by soldiers and medical personnel has increased significantly, reaching 91.2% and 24.7%, respectively. 【Conclusion】 The countermeasures in Hainan during the COVID-19 epidemic achieved remarkable effects and ensured blood safety in the whole province, which provided references and experience for blood collection and supply under public emergencies.
9. The protective effect and underlying mechanism of Hainan papaya water extract against neuronal apoptosis induced by Aβ40

Objective To investigate whether Hainan papayas has protective effects in an Aβ40-induced primary neuron injury model and elucidate the underlying molecular mechanism. Methods Cultured primary neurons from the dorsal root ganglia (DRG) of Sprague–Dawley (SD) rats were treated with 20 μM Aβ40 peptide, 100 μg/L Hainan papaya water extract, peptide plus extract, or culture medium for 24 h. Cell viability was measured by MTT assay, and neuronal apoptosis was evaluated by DAPI staining. ERK signaling pathway-associated molecule activation and changes in Bax expression were analyzed by Western blotting and immunofluorescence. Results A cell viability rate of (44.11 ± 6.59)% in the Aβ40 group was rescued to (79.13 ± 6.64)% by adding different concentrations of the extract. DAPI showed pyknotic nuclei in 39.5% of Aβ40-treated cells; the fraction dropped to 17.4% in the 100 μg/L extract group. ERK phosphorylation was observed in the Aβ40 group but was ameliorated by pretreatment with 100 μg/L extract. Hainan papaya water extract also prevented Aβ40-induced phosphorylation of MEK, RSK1 and CREB associated with ERK signaling and downregulated Bax expression in the neurons. Conclusion The results suggest that Hainan papaya water extract has protective effects on neurons; the mechanism may be related to suppression of ERK signaling activation.
10.Identification of the new allele D18S1364-10 by sequence analysis.

OBJECTIVETo identify a new allele by analyzing the polymorphism of D18S1364 in Power Plex(TM)16.
METHODSAn abnormal band was found in paternity test by short tandem repeat-PCR, and collected by gel extraction. Then the DNA was amplified, cloned and sequenced.
RESULTSA fragment containing eight bases less than the minimal allele in the D18S1364 ladder, was found with the core sequence of (ATCT)₆(ATGT)₁(ATCT)₃. The allele was D18S1364-10.
CONCLUSIONThe D18S1364-10 allele was found and reported for the first time in China.
