1.Research advances on stem cell therapy for diabetic foot wounds.

Diabetic foot wound repair is a challenging issue in clinical practice. Due to the influence of multiple factors including the damage and regeneration failure of local tissue, the impaired pathways of wound repairing through blood vessels and nerve nutrition, and disorders of a variety of cellular factors, traditional treatment methods are often difficult to achieve good therapeutic effects. Stem cells are a type of cells with potentials of multidirectional differentiation, which also possess functions such as regulating immunity and paracrine to facilitate the comprehensive wound repair, so they have promising application prospect at present for the treatment of diabetic foot wounds. Because the relevant parameters of stem cell treatment are in the exploratory phase, there were no standardized data. This paper reviews the application of stem cells in the research of diabetic foot wound treatment over the past 6 years, analyzing and summarizing the contents in focused aspects including the types and sources of stem cells, effects of donor age and gender on stem cells, mode of administration, transplantation survival rate and safety, which may provide a reference for further application of stem cells in the clinical treatment of diabetic foot wound.

2. Effects of long-acting antimicrobial material on pathogenic distribution and serum inflammatory factor concentration in patients with indwelling catheter

BACKGROUND: Long-acting antimicrobial material Is a new polymer activator of organoslllcon quaternary ammonium salt and has obvious advantages In anti-Infection. OBJECTIVE: To investigate the effects of Jieyoushen, a long-acting antimicrobial material, on pathogenic distribution and serum inflammatory factors in patients with indwelling catheters. METHODS: A total of 200 male patients with indwelling catheter who received treatment during January 2014 to January 2018 in Hainan West Central Hospital, China were Included In this study. These patients were aged 35-55 years and were randomly divided Into two groups: A control group (n=100) and an observation group (n=100). Patients In the control group were given routine urethral orifice nursing. Patients in the observation group were sprayed with long-acting antimicrobial material “Jieyoushen” on the urethral orifice, catheter and catheter bag interface. Infection was detected at 1, 3, 5 and 7 days after catheterization. The changes of pathogenic bacteria and serum inflammatory factors in two groups were analyzed at 7 days after catheterization. This study was approved by the Medical Ethics Committee, Hainan West Central Hospital, China (approval No. 2013120602). RESULTS AND CONCLUSION: Compared with the control group, the Infection rates of the patients In the observation group decreased significantly at 1, 3, 5 and 7 days after catheterization (P < 0.05). Twenty pathogenic bacteria were Isolated In patients of the control group, Including 13 Gram-positive bacteria (65.0%) and 7 Gram-negative bacteria (35.0%). Seven pathogenic bacteria were Isolated In patients of the observation group, including 5 Gram-positive bacteria (71.4%) and 2 Gram-negative bacteria (28.6%). Compared with before indwelling catheter, serum levels of interleukin-1ß, interleukln-2, homocysteine, and procalcltonin Increased significantly in each group after catheterization (P < 0.05). After catheterization, serum levels of above-mentioned factors in the control group were significantly higher than those In the observation group (P < 0.05). These results suggest that long-acting antimicrobial material “Jieyoushen” can effectively reduce the Infection rate of pathogenic bacteria and lower serum levels of Inflammatory factors In patients with Indwelling catheter.

4.Change and clinical significance of T helper 9 cells in previously untreated patients with chronic hepatitis C

ObjectiveTo investigate the changes of T helper 9 (Th9) cells, interleukin-9 (IL-9), and related transcription factors in previously untreated patients with chronic hepatitis C, as well as their association with clinical indices. MethodsA total of 29 previously untreated patients with chronic hepatitis C who attended Hainan Provincial People’s Hospital from December 2018 to July 2019 were enrolled, and 15 healthy individuals were enrolled as healthy controls. The patients with chronic hepatitis C received sofosbuvir/velpatasvir antiviral therapy for 12 weeks, and then plasma and peripheral mononuclear cells (PBMCs) were isolated. Flow cytometry was used to measure the percentage of CD3+CD4+IL-9+ Th9 cells in PBMCs; ELISA was used to measure the plasma level of IL-9; quantitative real-time PCR was used to measure the relative mRNA expression of IL-9 and the transcription factors PU.1 and Foxo1 in PBMCs. The t-test or the paired t-test was used for comparison between two groups, and a Pearson correlation analysis was used to investigate correlation. ResultsCompared with the healthy controls, the previously untreated chronic hepatitis C patients had significantly lower percentage of peripheral Th9 cells (0.92%±0.14% vs 1.14%±0.21%, t=4.31, P<0.001) and plasma IL-9 level (248.2±66.97 pg/ml vs 309.02±88.48 pg/ml, t=2.63, P=0.012). The previously untreated chronic hepatitis C patients had significantly lower relative mRNA expression of IL-9 and PU.1 than the healthy controls (t=20.67 and 23.21, both P<0.001), while there was no significant difference in the relative mRNA expression of Foxo1 between the previously untreated chronic hepatitis C patients and the healthy controls (P>0.05). In the previously untreated chronic hepatitis C patients, the percentage of peripheral Th9 cells, IL-9 level, and mRNA expression of IL-9 and PU.1 were negatively correlated with HCV RNA (r=-0.46, -0.38, -0.52, and -0.41, all P<0.05), but they were not correlated with the level of alanine aminotransferase (all P>0.05). Sofosbuvir/velpatasvir antiviral therapy achieved virologic response in 29 chronic hepatitis C patients, and the percentage of peripheral Th9 cells and the mRNA expression of PU.1 after antiviral therapy were significantly higher than those at baseline (t=2.20 and 6.52, both P<0.05), while there were no significant changes in the plasma level of IL-9 and the relative mRNA expression of IL-9 from baseline to after treatment (both P>0.05). ConclusionChronic hepatitis C virus infection may suppress the activation of Th9 cells, suggesting that Th9 cells might be involved in the chronicity of hepatitis C virus infection.

Objective To investigate residents' psychological stress factors and research the correlation between post-traumatic stress disorder (PTSD) and platelet 5-HT concentrations so as to provide scientific bases for diagnosis and treatment of PTSD and psychological intervention for people in the disaster area. Methods A questionnaire survey of 5 500 residents who have accepted psychological help was conducted by the emphatic investigation method. 6.Protective Effects of Danmu Extract Syrup on Acute Lung Injury Induced by Lipopolysaccharide in Mice through Endothelial Barrier Repair.

OBJECTIVE:
To investigate the effects of Danmu Extract Syrup (DMS) on lipopolysaccharide (LPS)-induced acute lung injury (ALI) in mice and explore the mechanism.
METHODS:
Seventy-two male Balb/C mice were randomly divided into 6 groups according to a random number table (n=12), including control (normal saline), LPS (5 mg/kg), LPS+DMS 2.5 mL/kg, LPS+DMS 5 mL/kg, LPS+DMS 10 mL/kg, and LPS+Dexamethasone (DXM, 5 mg/kg) groups. After pretreatment with DMS and DXM, the ALI mice model was induced by LPS, and the bronchoalveolar lavage fluid (BALF) were collected to determine protein concentration, cell counts and inflammatory cytokines. The lung tissues of mice were stained with hematoxylin-eosin, and the wet/dry weight ratio (W/D) of lung tissue was calculated. The levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-6 and IL-1 β in BALF of mice were detected by enzyme linked immunosorbent assay. The expression levels of Claudin-5, vascular endothelial (VE)-cadherin, vascular endothelial growth factor (VEGF), phospho-protein kinase B (p-Akt) and Akt were detected by Western blot analysis.
RESULTS:
DMS pre-treatment significantly ameliorated lung histopathological changes. Compared with the LPS group, the W/D ratio and protein contents in BALF were obviously reduced after DMS pretreatment (P<0.05 or P<0.01). The number of cells in BALF and myeloperoxidase (MPO) activity decreased significantly after DMS pretreatment (P<0.05 or P<0.01). DMS pre-treatment decreased the levels of TNF-α, IL-6 and IL-1 β (P<0.01). Meanwhile, DMS activated the phosphoinositide 3-kinase/protein kinase B (PI3K/Akt) pathway and reversed the expressions of Claudin-5, VE-cadherin and VEGF (P<0.01).
CONCLUSIONS
DMS attenuated LPS-induced ALI in mice through repairing endothelial barrier. It might be a potential therapeutic drug for LPS-induced lung injury.

8.Patchy echogenicity of the liver in patients with chronic hepatitis B does not indicate poorer elasticity

PURPOSE: This study was conducted to investigate whether the presence of patchy echogenicity in the liver of patients with chronic hepatitis B (CHB) is predictive of liver stiffness. METHODS: A total of 200 CHB patients with and without patchy echogenicity of the liver were assigned to two groups, with 100 patients in each group, and 32 of them underwent liver biopsy. Additionally, 80 healthy subjects, 100 inactive HBV carriers, and 100 patients with decompensated hepatic cirrhosis were assigned to the control groups. Laboratory tests and clinical data were collected, and shear wave velocity (SWV) of the liver was measured for all 480 subjects. RESULTS: The median SWV in patients with a normal liver, inactive hepatitis B virus carriers, CHB patients with and without patchy echogenicity, and decompensated hepatic cirrhosis were 1.07 m/sec, 1.08 m/sec, 1.16 m/sec, 1.16 m/sec, and 2.02 m/sec, respectively; there was no significant difference in SWV values between CHB patients with patchy echogenicity and those without patchy echogenicity. Furthermore, among CHB patients with and without patchy echogenicity, no significant difference in SWV was found according to fibrosis stage. CONCLUSION: The presence of patchy echogenicity of the liver does not indicate a higher degree of liver stiffness.

9.Efficacy of sorafenib combined with capecitabine in Hui versus Han residents with advanced hepatocellular carcinoma in Sanya, Hainan, China: a comparative analysis

ObjectiveTo compare the clinical efficacy of sorafenib combined with capecitabine in Hui versus Han residents with advanced hepatocellular carcinoma (HCC) in Sanya, Hainan, China. MethodsA total of 96 Hui and Han residents with advanced HCC took oral capecitabine 1500 mg/m2 twice daily for 14 days followed by a 7-day withdrawal, which was repeated at least twice; besides, sorafenib was given orally at a dose of 400 mg twice daily until tumor progression occurred. Comparison of continuous data between the two groups was made by t test, while comparison of categorical data was made by chi-square test. ResultsIn the two groups of Hui and Han patients, the rates of alpha-fetoprotein reduction were 60.9% and 40.0%, respectively (χ2=4.173, P=0.041); the rates of serum ferritin reduction were 50.0% and 30.0%, respectively (χ2=4.007, P=0.027); the rates of tumor regression were 54.3% and 34.0% as shown by CT (χ2=4.030, P=0.045); the response rates were 32.6% and 14.0%, respectively (χ2=4.697, P=0.030). Survival analysis suggested the combination of sorafenib and capecitabine had provided a significantly higher overall survival rate in Hui patients than in Han patients (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). ConclusionIn Sanya, a combination of sorafenib and capecitabine has better efficacy in Hui patients with advanced HCC than in Han patients, and the former have a higher overall survival rate.
10. Chemical constituents from Alpinia coriacea

Objective: To study the chemical constituents of Alpinia coriacea. Methods: The chemical constituents were separated and purified by silica gel, ODS, Sephadex LH-20 column chromatographies, and preparative HPLC. Their structures were determined by physicochemical properties and spectral data analyses. Results: Fifteen compounds were isolated from A. coriacea, and identified as 4-hydroxybenzaldehyde (1), anisic acid (2), pinocembrin (3), apigenin (4), izalpinin (5), kaempferol (6), 5,7,3',4'-tetrahydroxy-flavanone (7), 3,5-dihydroxy-7,4'-dimethoxyflavone (8), α-tocospiro A (9), stigmast-4-ene-6α-ol-3-one (10), stigmast-4-en-3-one (11), 7-keto-β-sitosterol (12), (22E,24R)-5α, 8α-epidioxyergosta-6,22-dien-3β-ol (13), stigmasterol (14), and β-sitosterol (15). Conclusion: All compounds are isolated from A. coriacea for the first time, among which compounds 2,7, and 9-13 are isolated from the plants of Alpinia L. for the first time.
