1.A application evaluation of sentinel lymph node biopsy and axillary lymph node dissection under mastoscopy for patients with early stage breast cancer in clinical diagnosis and treatment

Objective: To explore the advantages and evaluate feasibility of sentinel lymph node (SLN) biopsy and axillary lymph node dissection under mastoscopy for patients with early stage breast cancer in clinical diagnosis and treatment. Methods: 200 patients with breast cancer stage I and II diagnosed by breast surgery were divided into observation group (100 cases) and control group (100 cases) as different surgery method. The patients of observation group were treated by SLN biopsy and axillary lymph node dissection under mastoscopy, while those of control group were treated by traditional open surgery. To analyze the detection rate and accuracy rate, and compare the bleeding volume during operation, the operation time and hospitalization time between the two groups. And series of indexes, such as incidence of post-operative complication, recurrence rate and good appearance rate of breast were compared between the two groups. Results: The difference of accuracy rate between the two groups was no significant. The incidence of complication, good appearance rate of breast and recurrence rate of observation group were, respectively, 4%, 92% and 4%. And the differences between those results of observation group and those of control group were significant (x2=25.25, x2=16.61, x2=170.40; P<0.05). Other some indexes, such as the bleeding volume during operation, the operation time and hospitalization time, the differences between observation group and control group also were significant (t=9.73, t=13.86, t=11.66; P<0.05). Conclusion:Sentinel lymph node (SLN) biopsy and axillary lymph node dissection under mastoscopy are feasibility in clinical practice based on its series advantages, such as better detection rate, lower incidence of complication, better protection for breast appearance and so on.

The sonographic features of male breast lesions, which underwent ultrasound examination in our hospital for the past 10 years, were retrospectively analyzed. Sonographic features of these lesions were standardized as BI RADS image lexicon. The differences in ultrasonic malignant signs were assessed between the benign and the malig nant diseases. Between the two groups, incomplete boundary was statistically different. The specificity was above 95% within the two groups in terms of speculated margin, echogenic halo, calcification, axillary lymphadenopathy, thickening of skin and eccentric of mass to the nipple. High-frequency sonographic examination has a high level of differential diagnosis for male breast lesions.

3.The characteristic of enlarged vestibular aqueducts syndrome in pure tone audiometry: low frequency air-bone gap.

OBJECTIVE:
The purposes of this study was to analyse the significant of low frequency air-bone gap in enlarged vestibular aqueduct (EVA) patients according to the pure tone audiogram and the benefit for clinic diagnosis of the EVA.
METHOD:
The audiology testing include (1) play audiometry or pure tone audiometry, tympanometry, middle ear muscle reflex thresholds and stapedius muscle reflex; (2) High-resolution computed tomography (CT) scan of the temporal bone and magnetic resonance imaging was used for diagnoses the EVA.
RESULT:
All the 78 patients (154 ears) were diagnosed as the typical EVA by CT or MRI. Inner ear malformations were found in 3 ears otherwise the structures of middle ear in all the patients were absolutely normal. The audiology analysis showed 154 ears were type A tympanogram including 126 ears with typical A, 25 ears with As, 3 ears with Ad type. In the 250 Hz pure-tone test:A-B gap were observed in 126 ears(126/154,81. 8%) with the different hearing loss degree: 1 ear mild, 11 ears moderate, 19 ears moderate severe, 40 ears severe and 53 ears profound. In the 500 Hz pure-tone test: A-B gap were found in 102 ears(102/154, 66. 2%) with the different hearing loss degree: 9. ears moderate, 17 ears moderate severe, 35 ears severe and 41 ears profound.
CONCLUSION
Our study suggested a 66. 2% -81. 8% possibilities to find the EVA through the pure-tone audiometry firstly in the basis of the normal tympanograms with the significant A-B gap.

4.Effect of three-stage rehabilitation education mode on patients with maintenance hemodialysis

Objective To explore the scientific rehabilitation education mode for maintenance hemodialysis patients .Methods Eighty patients with uremia were selected as the research object and divided into the control group and the observation group , from March to July 2013 .The control group accepted routine treatment and nursing , while the observation group received three-stage rehabilitation education intervention on the basis of conventional therapy and nursing for twelve weeks .The disease related knowledge and measure biochemical indicators of two groups before and after the intervention were evaluated .Results After intervention, the number of patients of correct answer to the mastery of the disease knowledge ,including blood dialysis principle , the principle of medication knowledge , diet nursing , care of blood vessel passageway , dialysis adequacy was 32, 25, 37, 35 and 19, respectively, in the observation group.Those numbers in the control group were 26, 20, 30, 34 and 9.The differences were statistically significant (χ2 =2.56, 2.03, 4.50, 2.11, 5.50, respectively;P<0.01).The creatinine, urea nitrogen and hemoglobin were (204.18 ± 24.85)μmol/L,(14.46 ±5.13) mmol/L and (98.68 ±13.72) g/L in the observation group, which were significantly better than (426.42 ±71.37)μmol/L, (20.64 ±9.65)mmol/L and (78.64 ±12.71)g/L in the control group, and the differences were statistically significantly ( t=6.11, 6.04, 7.69, respectively;P <0.01).Conclusions Though the three-stage rehabilitation education mode , patients can improve the understanding of the knowledge related to hemodialysis and the quality of life .
5.Effects of Peiyuan Jieyu Formula on TRP-KYN metabolic pathway and its neuron protection mechanism in depressed rats induced by glucocorti-coid

Objective To study the effects of Peiyuan Jieyu Formula ( PYJYF) on rats treated with glu-cocorticoid, its influences on metabolism of the TRP-KYN pathway and the impacts on hippocampal and cortical neuronal plasticity. Methods 70 rats were randomly divided into the following 7 groups ( n=10 in each group): normal blank group, model group, fluoxetine group, mid- and high-dose Tiansi Yin ( TSY) groups, Sini San ( SNS) groups, and PYJYF groups. Rat model of depression was established by intraperitoneal injection of glucocorticoid for 28 days. Sucrose solution consumption and Open-field test were conducted. The content of TRP, KYN, KA and Q in serum was measuredby using HPLC-MS/MS technique. The mRNA expressions of NR and BDNF in brain tissue were measured by using RT-PCR. Results 28 days after the injection of glucocorticoid in model group, the consumption of sucrose solution and total score in open-field test was significantly decreased ( P <0 . 05 ) . The ratios of KYN/TRP and KYN/KA increased significantly ( P<0 . 05 ) . The mRNA expression level of BDNF in hippocampal de-creased significantly ( P<0 . 05 ) . The mRNA expression level of NR increased significantly ( P<0 . 05 ) . Peiyuan Jieyu Formula increased the consumption of sucrose solution (P<0. 05),the total score in open-field test,the ratios of KYN/Q and the mRNA expression level of BDNF in hippocampus(P<0. 05) when compared with the model group. The ratios of KYN/TRP and KYN/KA(P<0. 05) and the mRNA ex-pression level of NR in cortex ( P <0. 05 ) of Peiyuan Jieyu Formula group decreased. Conclusion Peiyuan Jieyu Formula can reduce the metabolism of TRP-KYN, inhibit neurotoxicity, improve neural protection, and promote the hippocampal and cortical neural plasticity.
6.Influence of continued-nursing on physical and mental health for elderly patients with spinal cord injury

Objective To investigate influence of continued-nursing on physical and mental health for elderly patients with spinal cord injury.Methods A total of 80 elderly spinal cord injury patients (ASIA grade A) were selected in our department and divided into observation and control groups based on random number table method.The control group received conventional nursing and discharge guidance, while observation group received continued-nursing for 6 months after discharge, including telephone follow-up, family visit, and theme education activities.Self-care ability and psychological states before and after intervention were evaluated.Results The scores of Functional Independence Measurement (FIM), HAMA, and HAMD showed no significant difference in both groups before intervention(P>0.05).However, the scores above in observation group had significant difference after 6 months of continued-nursing than that of control group (P<0.01).The incidence of complication in observation group was lower.Conclusion Continued-nursing can improve the rehabilitation adherence and self-management ability of elderly spinal cord injury patients after discharge, and reduce the incidence of complications.

8.A study of the home care model based on the Omaha system for elderly maintenance hemodialysis patients

Objective:To analyze the application effect of the home care model based on the Omaha system for elderly maintenance hemodialysis patients.Methods:A total of 73 elderly patients undergoing maintenance hemodialysis in the First People's Hospital of Longquanyi District, Chengdu city from December 2018 to December 2019 were enrolled.According to the randomization and double-blind principle, they were divided into the control group receiving the clinical routine care mode(n=32)and the intervention group undergoing the home care mode based on Omaha system(n=41). The Quality of Life Score, care satisfaction and awareness rate of health knowledge of patients were compared between the two groups before and after the care intervention.Results:Compared with before intervention, the quality of life scores was increased after caring in both two groups.And the scores in all dimensions were better in the intervention group than in the control group( t=3.173, 3.833, 3.514 and 4.593, P=0.001, 0.000, 0.000 and 0.000). The awareness rates of nutrition knowledge, water-intaking knowledge, iron supplementation knowledge, potassium and phosphorus limitation and dialysis mode were better in the intervention group than in the control group( χ2=7.592, 5.423, 8.494, 6.161 and 6.962, P=0.011, 0.011, 0.000, 0.010 and 0.011). The total satisfaction rate was 97.6%(40/41)in the intervention group, which was superior to that in the control group(75.0% or 24/32)( χ2 =8.460, P=0.000). Conclusions:The extended home care model based on the Omaha system can effectively improve patients' awareness of health knowledge, improve patients' quality of life and improve the satisfaction with care in elderly maintenance hemodialysis patients with a low education level, and it is worthy of clinical promotion.
9. Relationship between complete Freund ' s adjuvant-induced chronic pain-induced negative emotions and morphological changes of hippocampal microglia

Objective To investigate the alteration of mood and hippocampal microglia morphology in a mouse model of chronic inflammatory pain induced by complete Freund' s adjuvant (CFA). Methods Thirty-two male ICR mice were randomly divided into two groups, including normal saline control group (NS) and CFA model group (CFA). The pain model was established by right hindpaw intraplantar CFA injection. The change of mechanical pain threshold after CFA injection was measured by von Frey fiber needle, the locomotor activity and anxiety-like behavior were determined by open field test (OFT), the depression-like behavior was determined by sucrose preference test (SPT) and forced swimming test (FST). The expression of microglia marker ionized calcium binding adaptor molecule-1 (IBA-1) in the hippocampus was determined by immunohistochemistry and its morphological change was analyzed by Sholl analysis. Results Compared with the NS group, the mechanical pain threshold of CFA group decreased significantly (P<0.01). The behavior result showed that the CFA group showed remarkably reduced time in the inner area (P<0.01) compared with the NS group in the open field test; In the sucrose preference test, the percentage of sucrose preference (P<0.01) of CFA mice decreased significantly compared with the NS mice, while the immobility time of CFA mice (P<0.01) increased significantly in the forced swimming test compared with the NS mice. The immunohistochemistry showed that the number of microglia in the dentate gyrus (DG) of CFA mice increased significantly compared with the NS mice. The Sholl analysis result showed that compared with the NS mice, the number of intersections of microglia in hippocampal DG decreased significantly in CFA mice. Conclusion Our finding indicates that the negative emotions in CFA-induced chronic inflammatory pain may be related to the morphological changes of hippocampal microglia in the mice.
