1.In vitro antimicrobial effect of polymethylmethacrylate denture base with silver-containing antimicrobial agents of nanometer level

Objective:To study the antimicrobial effect of polymethylmethacrylate (PMMA) denture base with silver-containing antimicrobial agents of nanometer level against Streptococcus mutans and Candida albicans in vitro. Methods:Minimum inhibitory concentration (MIC) of the silver-containing antimicrobial agent against S. mutans and C. albicans was examined. According to the MIC, 4 concentrations(1,2,5 and 10 mg/ml) of the agent were selected for the preparation of antimicrobial PMMA resin base. Then, the antimicrobial effect of the resin base was examined by in vitro bacteria culture and the most probable number(MPN) counting. Results:The MICs of the agent against S. mutans and C. albicans were 0.1 mg/ml and 1 mg/ml respectively. With the agent at 1, 2, 5 and 10 mg/ml, the inhibition ratios of the base against S.mutans were 67.4%,71.3%,99.0 and 99.5% respectively, that against C. albicans were 25.8%,54.8%,90.3% and 93.0%, respectively. Conclusion:The polymethylmethacrylate resin base with silver-containing antimicrobial agent of nanometer level at 5 mg/ml has ideal antimicrobial activity against S. mutans and C. albicans.
2.Durability of in vitro antimicrobial effect of polymethylmethacrylate denture base containing silver-supported antimicrobial agent of nanometer level

Objective: To study the durability of the antimicrobial effect of polymethylmethacrylate (PMMA) denture base containing silver-supported antimicrobial agents of nanometer level against Streptococcus mutans and Candida albicans in vitro. Methods:The antimicrobial PMMA resin bases with the antimicrobial agent STR-1 at concentration of 5 mg/ml were prepared. Then the samples were divided into 4 groups: positive control group, the group immersed in distilled water at 57 ℃ for 14 days, the group irradiated by ultraviolet for 8 hours, and the group irradiated by ultraviolet for 8 hours after immersed in distilled water at 57 ℃ for 14 days. Then the inhibition ratios of the 4 groups against Streptococcus mutans and Candida albicans in vitro were tested. Results:The inhibition ratios of the 4 groups against Streptococcus mutans were 99%, 96%, 98%, and 90% in turn. The inhibition ratios of the 4 groups against Candida albicans were 91%, 82%, 90%, and 80% respectively. Conclusion: The antimicrobial effect of PMMA denture base containing silver-supported antimicrobial agents STR-1 of nanometer level at concentration of 5 mg/ml against Streptococcus mutans and Candida albicans in vitro is durable.
3.Short-term and long-term effects of occlusal rehabilitation on the co-contractration patterns of the electromyography of masticatory muscles

Objective:To investigate the short-term and long-term effects of occlusal rehabilitation on the co-contractration patterns of masticatory muscles of patients with extensive tooth wear.Methods: Twenty-nine patients with severe tooth wear were selected and conducted with occlusal rehabilitation.The surface electromyography(EMG) of masseter(MM),anterior temporalis(TA) and posterior temporalis(TP) at rest position,during clenching and chewing were separately recorded in the stage of pre-treatment,1 month after temporary restoration,1 month after permanent restoration and long-term observation(mean 3.5 years).The Asymmetry Indexes(AI) of bilateral muscles,Activity Indexes of MM/TA,MM/TP and TA/TP were compared in each stage.Results:(1) After occlusal rehabilitation,all the AIs of bilateral MM,TA and TP at rest position,during clenching and chewing showed obvious decrements.(2) In every stage of occlusal rehabilitation,all of the Activity Indexes of MM/TA at rest position,during clenching and chewing had no obvious changes.(3) The Activity Indexes of MM/TP and TA/TP during clenching and chewing significantly increased after occlusal rehabilitation.Conclusion: The treatment of occlusal rehabilitation can significantly improve the bilateral symmetry of masticatory muscles in patients with extensive tooth wear,and also change the coordinated EMG activity of different masticatory muscles.
4.Effect of pelvic floor muscle training with biofeedback on stress urinary incontinence in postpartum and post-menopausal women

Objective To investigate the short-term effect of pelvic floor muscle training (PFM) with biofeedback on stress urinary incontinence (SUI) in postpartum and post-menopausal women.Methods According to the different period that the SUI occurs,107 women with SUI were divided into two groups:the group of SUI in postpartum with 60 women,and the group of SUI in post-menopausal with 47 women.PFM with biofeedback was performed on all patients for 8 weeks.One hour pad-weighing test,voiding diary,transperineal three-dimensional ultrasound and female pelvic floor muscle assessment were recorded before and after treatment.Results There was statistically significant difference in 1 hour padweighing test between pre-treatment and post-treatment for the group of SUI in postpartum (the negative,mild,moderate,and severe cases of post-treatment:21,24,14,1,of pre-treatment:0,30,28,2; P<0.05),and the group of SUI in post-menopausal (the negative,mild,moderate,and severe cases of post-treatment:7,22,11,7,of pre-treatment:0,14,25,8; P<0.05).The strength of the pelvic floor muscles of type Ⅰ and type Ⅱin two groups after treatment were significantly different from those in pre-treatment (P<0.01).The efficient rate of improvement in symptoms after treatment in the group of SUI in postpartum was 88% (53/60) and the cure rate was 38% (23/60).While the efficient rate in the group of SUI in post-menopausal women was 64%(30/47) and the cure rate was 15% (7/47).There was statistically significant difference in the development of symptoms in two groups after treatment (P=0.003).Conclusion PFM with biofeedback is an effective treatment for SUI in postpartum and post-menopausal women,especially for postpartum ones.
5.Manifestations of the connective tissue associated interstitial lung disease under high resolution computed tomography

Objective:To analyze the features of the connective tissue associated interstitial lung disease (CTD-ILD) by high resolution computed tomography (HRCT).Methods:A total of 127 patients with CTD-ILD,who were diagnosed by clinic laboratory examination and pathology in Xiangya Hospital of Central South University form September 2013 to September 2015,were enrolled for this study.Their lung features of HRCT imaging were retrospectively analyzed.Results:The classifications for 127 patients were as follows:36 cases of rheumatoid arthritis (28.3%),34 cases of dermatomyositis and polymyositis (26.8%),31 cases of systemic sclerosis (24.4%),18 cases of Sj(o)gren syndrome (14.2%),7 cases of mixed connective tissue disease (5.5%),and 1 cases of systemic lupus erythematosus (0.8%).According to the features of HRCT imaging,the patients were divided as follows:77 cases (60.6%) ofnonspecific interstitial pneumonia (NSIP),46 cases (36.2%) of usual interstitial pneumonia (UIP),2 cases (1.6%) of lymphocytic interstitial pneumonia (LIP),1 case (0.8%) of cryptogenic interstitial pneumonia (COP),and 1 case (0.8%) of acute interstitial pneumonia (AIP).The HRCT findings for 36 cases of rheumatoid arthritis associated interstitial lung disease were UIP (24 cases,66.7%) and NSIP (12 cases,33.3%);the HRCT findings for 34 cases of dermatomyositis and polymyositis associated interstitial lung disease were NSIP (32 cases,94.1%),UIP (1 case,2.9%) and COP (1 case,2.9%);the HRCT findings for 31 cases of systemic sclerosis associated interstitial lung disease were NSIP (21 cases,67.8%),UIP.(9 cases,29%),LIP(1 case,3.2%);the HRCT findings for 18 cases of Sj(o)gren syndrome associated interstitial lung disease were NSIP (9 cases,50.0%),UIP (8 cases,44.4%),LIP (1 case,5.6%);the HRCT findings for 7 cases of mixed connective tissue disease associated interstitial lung disease were UIP (4 cases,57.1%),NSIP (3 cases,42.9%).SLE-ILD was rare,with only 1 case of AIP.Conclusion:Different types of CTD-ILD patients display relatively unique manifestation of HRCT.
6.Ultrastructural study of non-carious cervical sclerotic dentin bonded by a all-in-one self-etch adhesive

Objective:To examine the ultrastructure of resin-infiltrated sclerotic dentine following the application of an all-in-one self-etch dentin adhesive. Methods:Naturally-occurring, non-carious cervical sclerotic lesions on 10 extracted premolars were bonded using an all-in-one self-etch dentin adhesive(Adper Prompt L-Pop,3M ESPE, St Paul, USA).Artificially prepared wedge-shaped lesions were also made in 10 extracted sound premolars and restored using the same adhesive as the controls. The morphological change of dentin surface conditioned by the adhesive and the dentin-resin interface were studied by SEM.Results:Most dentinal tubules were obliterated by rod-like sclerotic casts in the non-carious cervical lesion, the sclerotic casts could not be totally dissolved by the self-etch adhesive. After restoration the resin tags were fewer and shorter in sclerotic dentin than those in sound dentin.Conclusions:Bonding to sclerotic dentin is different from that to sound dentin, and may be compromised by the fewer resin tags.
7.The conservative management of pneumoperitoneum in children

Objective Pneumoperitoneum is usually the result of hollow visceral perforation and requires emergent laparotomy.However,about 10% of patients lack a pathologic cause requiring surgical intervention and can be resolved with conservative management.The aim of this study is to discuss the indications for conservative management of pneumoperitoneum in children.Methods We retrospectively reviewed the data of 5 children with pneumoperitoneum without peritonitis treated in our pediatric surgery department between January 2009 and December 2010.Results There were 5 children with an average age 5.4 years.Radiologic examinations demonstrated pneumoperitoneum and visceral perforations were suspected in the local hospitals.The abdominal examination revealed no peritoneal signs in all patients.Among them,peritoneal aspirations were implemented in 4 patients and the results were negative.One of five patients underwent laparotomy that demonstrated no visceral perforation.The remaining four were successfully managed conservatively and the pneumoperitoneum was completely resolved between 3 and 9 days.All children were without abdominal symptoms and signs throughout at least 2 years follow-up.Conclusion The presence of free intraperitoneal air does not always indicate hollow visceral perforation and the need for emergent laparotomy.It appears that therapeutic effectiveness can be readily achieved with conservative management in children with pneumoperitoneum without peritonitis.
8.Diagnosis and treatment of hepatolithiasis complicated by bronchobiliary fistula: a study on 35 patients

Objective To investigate the diagnosis and treatment of hepatolithiasis complicated by bronchobiliary fistula.Method The data of 35 patients with hepatolithiasis and bronchobiliary fistula treated in our department in the last 10 years were retrospectively studied.Results The operations were:-repair of fistula in the diaphragm (n=35),hepatic segmentectomy (n=22) biliary stricturoplasty (n=13),T-tube drainage of common bile duct (n=19),hepaticojejunostomy (n=3) and bilateral hepatojejunostomy with a Roux-en-Y loop of jejunum (n=13).Residual stones were left in 4 patients.There was no recurrence of the bronchobiliary fistula on follow-up.Conclusions Expectoration of bitter and purulent yellow sputum was an important clinical feature of bronchobiliary fistula.The key steps in a successful operation were reliefing the obstructed bile duct and re-establishment of adequate biliary drainage.
9.Biocompatibility test of polymethylmethacrylate denture base resin containing silver-supported antimicrobial agent STR-1 at nanometer level

Objective: To evaluate the biocompatibility of polymethylmethacrylate(PMMA) denture base resin containing silver-supported antimicrobial agent STR-1 of nanometer level in vitro. Methods: According to the national standards for biological evaluation of dental materials, the cytotoxicity of denture base resin containing STR-1 at concentrations of 5 g/L and 10 g/L was examined by molecular filtrating method, and the hemolysis of STR-1, denture base resin containing STR-1 at concentrations of 5 g/L and 10 g/L was also surveyed. Results: The control denture base resin without containing STR-1 and the denture base resins containing STR-1 at concentrations of 5 g/L and 10 g/L were not cytotoxic to L929 cells. Two hours and 24 hours after cell culturing, the filter membranes of the control and experimental groups were stained evenly with blue color. The staining intensity was not decreased and the fading areas were 0 mm~2 during the culturing. The cytotoxicity grades were 0. The hemolysis rates of the antimicrobial agent STR-1 and the denture base resins containing STR-1 at concentrations of 5 g/L and 10 g/L were 1.7%, 3.5% and 3.7% respectively. They were less than the national guild standard 5% which represent no hemolysis. Conclusion: The PMMA denture base resins containing silver-supported antimicrobial agents STR-1 of nanometer level at concentrations of 5 g/L and 10 g/L exhibit good biocompatibility.
10.The significance of COX-2 and PTEN expression in hepatobiliary calculus with cholangiocarcinoma

Objective To explore the expressions of cyclooxygenase-2 (COX-2), phosphatase and tensin homolog deleted on chromosome ten (PTEN) in hepatobiliary calculus associated with cholangiocarcinoma (HCWC) and their clinical significance. The relationship between the expressions of COX-2, PTEN and the onset and progression of HCWC was investigated to form an experimental base for the prevention and treatment of HCWC. Methods Thirty seven patients with tumor tissues of HCWC (group C), thirty patients with tissues of bile duct surrounding intrahepatic calculus (group B), and ten patients with normal tissues of bile duct from operations of hemangiomas of liver or liver trauma as the control (group A) were sampled and collected. A two-step immunohistochemistry (SP method) was employed to detect and statistically analyze the expressions of COX-2 and PTEN in each of the 3 groups. Results In groups A, B, C, the positive rate of the expression of COX-2 was 10%,33.3%, and 70.3%, respectively. The positive rates of expression of COX-2 in the carcinoma tissues of HCWC was significantly higher compared with the control group (P<0. 01). In groups A, B, C the positive rates of the expression of PTEN was 90. 0%, 80. 0%, and 35.0%, respectively. The positive rate of expression of PTEN in the carcinoma tissues of HCWC was significantly lower than the control group (P<0. 01). The expression of COX-2 was followed by a low expression of PTEN in the tissues of HCWC. Kendall's related analysis showed a strong negative correlation between the expression of COX-2 and PTEN in HCWC (r=-0. 323, P<0. 05). Conclusions A high expression of COX-2 was related to HCWC. There was a negative correlation between the expressions of COX-2 and PTEN in HCWC. A high expression of COX-2 and a low expression of PTEN suggested a high chance of HCWC in extrahepatic or lymphatic metastasis.
