1.Health Expenditure, Human Capital and Economic Growth:Based on Analysis of Simultaneous Equations

Objective: To investigate the function of the heath expenditure in the role of the economic growth through the research on the effect by the human capital. Methods: To establish endogenous growth models and human capital growth models of simultaneous equations, to analyze the mechanism by the method of a three-stage least squares. Results: Growth of human capital is positively related to economic growth, health expenditure growth is negatively related to economic growth, health expenditure growth and growth of human capital is positively correlated. Conclusion: Influences of health expenditure growth on economic growth depends on the comprehensive response.
2.Market Structure, Medical Arms Race and Health Expense Growth:An Empirical Analysis Based on the Cross-province Data

To analyze the relationship between hospital non-price competition behavior and market structure, analyze the relationship between hospital market construction and health care costs gtowth. Methods:Using cross-province data(SCP) theory to establish the regression function of health cost expenditure, hospital market concentration ratio and the occupancy of public hospital market, process the empirical analysis based on the two-factor effectiveness model of panel data. Results: There is no medical arms race in hospital business under the current market concentration. Conclusion: The reinforcement of market competition played a more important role in the improvement of operational efficiency rather than the differences in the nature of property rights and the profit of hospitals.
3.Analysis of the Role of Medical Service Quality in the Growth of the Total Expenditure on Health

Objective:To investigate the function of the quality factor in the growth of the total heath expenditure, in order to provide references for making the object of the health policy. Methods: Through the decomposition the identical equation of the growth of the total expenditure on health, to analyze the contribution of medical service quality and its factor. Results: The sustaining improvement of medical service quality in the important factor on the constant growth of the total expenditure on health. Conclusion: The primary objective of health policy is to improve the quality of medical services, and the secondary is to control the expense.
4.Hypoglycemic effect of chromium-enriched compound acidophilous milk in rats with hyperglycemia

Objective: To study the hypoglycemic effects of chromium-enriched compound acidophilous milk in rats with hyperglycemia. Methods: 120 adult Wister rats were randomly divided into Control group, DM group, therapy group and protective therapy group. Each group were fed a basic diet and Control group were intragastric administration with sodium chloride,DM group and therapy group were intragastric administration with ordinary acidophilous milk and protective therapy group were intragastric administration with chromium-enriched compound acidophilous milk. After 15 days, the rats of DM group, therapy group and protective therapy group were given a single shot of alloxan (50 mg per 1 kg), while Control group was given a single shot of sodium chloride (50 mg per 1 kg). After the diabetic model had been induced by alloxan,the rats of therapy group were intragastric administration with chromium-enriched compound acidophilous milk, and other groups had no change. The blood-fasting sugar, sugar tolerance, and glycosylated hemoglobin were examined. 5.Two dimensional finite element analysis of ceramic veneer bonding layer

Objective: To intimately assess the internal stress distribution in ceramic veneer bonding layer. Methods:Six buccolingual cross section models of the incisor restored with IPS-Empress ceramic veneer were built with the MSC/NASTRAN software. 2D-finite element method was used to analyze the stress distribution regularity of the bonding layer on the models under different functional load. Results: Stress concentration areas of ceramic veneer bonding layer were regularly located on the incisal margin and cervical margin. Under the same condition, incisal load caused highest stresse on the adhesive layer. Following the load shift from incisal to gingival area, the stress was gradually decreased.When the functional load acted on the palate margin of the restoration, the compressive stress on the bonding layer was significantly increased. Different load angle generated stress change in the bonding layer. The shear stress on adhesive layer increased following the horizontal load angle increasing close to the debonding level. Among the different incisal reduction designs there was no significant Von Mises stress variation under the same load condition. However, when the incisal edge was reduced by more than 4 mm, the tensile stresses in the bonding layer greatly increased. Conclusion: Restoration margin should be kept away from the tooth contact area, be exposed to minimal occlusion contact. The functional load should be induced to follow the tooth long axis. Edge to edge occlusion should be avoided. When incisal areas need to be altered, the incisal overlap or butt margin design will not cause any stress problem in the bonding layer. When restoring the incisal defect for more than 4 mm, the bonding agents must be selected carefully.
6.Fatigue test of retention force of magnetic attachment and stud-type attachment

Objective:To study retention force durability of root-type attachments.Methods:Five magnetic attachments(Magfit EX-600),semi-precise stud-type attachments(Bedent) and precise stud-type attachments(Dalbo-classic) were fixed on a thermal-cycling and mechanical-loading machine respectively.The retention force of each attachment was measured at the baseline and after every 1,000 cycles of seating-removing.Results:The average retention force of magnetic attachments was(3.85?0.107) N at the baseline which attenuated for 3.38% after 10,000 cycles;The retention force of semi-precise group was(14.10?0.144) N originally and decreased 90.06% of the original force after 10,000 cycles.The primary force of the precise group was 33.14 N and was adjusted to(11.95?0.082) N as baseline attenuating for 74.56%.Conclusion: The retention force of magnetic attachments is lower than the stud attachments but is stable for 10 years.The retention force of stud-type attachments is larger at the baseline but reduced rapidly.
7.Fatigue damage analysis of porcelain in all-ceramic crowns

Objective:To investigate the fatigue damage mechanism of porcelain,and its relation with the microscopic defects in clinically failed all-ceramic crowns.Methods: Collecting the bilayered all-ceramic crowns failed in vivo.The fractured surfaces and occlusial surfaces of failed crowns were examined by an optical microscope followed by detailed fractography investigations using a field emission scanning electron microscope.When chemical impurities were of concern,energy-dispersive X-ray spectroscopy analysis was performed to examine chemical composition.A standard practice for fractography failure analysis of advanced ceramics is applied to disclose the fracture mode,and damage character.Results: Three types of fracture features are defined as breakdown of the entire crown,and porcelain chipping-off/delamination.Alumina crowns were usually characterized by breakdown of the entire crown,while zirconia crowns by porcelain chipping-off and delamination.The fatigue damage of porcelain was classified into surface wear,cone crack,and porcelain delamination.The observed microscopic defects in this study included air bubbles and impurity particles.Conclusion: The multi-point occlusial contacts were recommended in all-ceramic restorations clinically.The thickness of porcelain is important for the anti-fatigue ability of porcelain.Cautions have to be taken to avoid contaminations during the veneering processes.
8.Effect of pelvic floor muscle training with biofeedback on stress urinary incontinence in postpartum and post-menopausal women

Objective To investigate the short-term effect of pelvic floor muscle training (PFM) with biofeedback on stress urinary incontinence (SUI) in postpartum and post-menopausal women.Methods According to the different period that the SUI occurs,107 women with SUI were divided into two groups:the group of SUI in postpartum with 60 women,and the group of SUI in post-menopausal with 47 women.PFM with biofeedback was performed on all patients for 8 weeks.One hour pad-weighing test,voiding diary,transperineal three-dimensional ultrasound and female pelvic floor muscle assessment were recorded before and after treatment.Results There was statistically significant difference in 1 hour padweighing test between pre-treatment and post-treatment for the group of SUI in postpartum (the negative,mild,moderate,and severe cases of post-treatment:21,24,14,1,of pre-treatment:0,30,28,2; P<0.05),and the group of SUI in post-menopausal (the negative,mild,moderate,and severe cases of post-treatment:7,22,11,7,of pre-treatment:0,14,25,8; P<0.05).The strength of the pelvic floor muscles of type Ⅰ and type Ⅱin two groups after treatment were significantly different from those in pre-treatment (P<0.01).The efficient rate of improvement in symptoms after treatment in the group of SUI in postpartum was 88% (53/60) and the cure rate was 38% (23/60).While the efficient rate in the group of SUI in post-menopausal women was 64%(30/47) and the cure rate was 15% (7/47).There was statistically significant difference in the development of symptoms in two groups after treatment (P=0.003).Conclusion PFM with biofeedback is an effective treatment for SUI in postpartum and post-menopausal women,especially for postpartum ones.
9.Assessment of the left ventricular early diastolic synchrony of cardiac resynchronization therapy by real time three-dimensional echocardiographic volume-time curves in patients with dilated cardiomyopathy

Objective To evaluate the diastolic function and relationship between diastolic function and early diastolic synchrony in patients with dilated cardiomyopathy (DCM) by real-time three-dimensional echocardiography (RT-3DE) volume-time curves (VTC) after cardiac resynchronization therapy (CRT).Methods Thirty-nine patients with DCM were enrolled by RT-3DE VTC before and 1 week,6 months,12 months after CRT,draw the left ventricular (LV) 16,12,6 segments LV diastolic early volume standard deviation of the time (Tedv-SD),and with the R-R interval normalized as early diastolic unsynchronized index (DDI) ;draw end-systolic volume (LVESV),LV end-diastolic volume (LVEDV),LV ejection fraction (LVEF) ;and calculate the diastolic peak filling rate(PFR),the ratio of early diastolic volume and enddiastolic volume(EDVearly/EDV).Results LVEDV,LVESV had no significant improvement.Compared with the before and 1 week after CRT,but the improvement was statistically significant after 6 months and 12 months (P <0.05,P <0.01); LVEF after 1 week,6 months and 12 months were statistically significant (P <0.05) ;Compared with before,PFR after 6 months was significantly increased (P <0.05),EDVearly/EDV at 12 months after CRT was significantly reduced (P < 0.01); There was a significantly shortened in each segment (Tedv-SD)/R-R 1 week after CRT (P <0.01),but the parameters had no obvious improvement later.Correlation analysis:△ DDI and △ EDVearly/EDV reduction was significant positive correlation (r =0.52,P <0.01),△DDI and PFR has negative correlation (r =-0.40,P < 0.05),△ EDVearly/EDV and △PFR also had a good relationship (r =-0.56,P <0.01).Conclusions The LV synchrony and diastolic function were improved after CRT in patients with DCM; PFR,EDVearly/EDV can be used as evaluation of left ventricular diastolic function effectively targets.
10.Manifestations of the connective tissue associated interstitial lung disease under high resolution computed tomography

Objective:To analyze the features of the connective tissue associated interstitial lung disease (CTD-ILD) by high resolution computed tomography (HRCT).Methods:A total of 127 patients with CTD-ILD,who were diagnosed by clinic laboratory examination and pathology in Xiangya Hospital of Central South University form September 2013 to September 2015,were enrolled for this study.Their lung features of HRCT imaging were retrospectively analyzed.Results:The classifications for 127 patients were as follows:36 cases of rheumatoid arthritis (28.3%),34 cases of dermatomyositis and polymyositis (26.8%),31 cases of systemic sclerosis (24.4%),18 cases of Sj(o)gren syndrome (14.2%),7 cases of mixed connective tissue disease (5.5%),and 1 cases of systemic lupus erythematosus (0.8%).According to the features of HRCT imaging,the patients were divided as follows:77 cases (60.6%) ofnonspecific interstitial pneumonia (NSIP),46 cases (36.2%) of usual interstitial pneumonia (UIP),2 cases (1.6%) of lymphocytic interstitial pneumonia (LIP),1 case (0.8%) of cryptogenic interstitial pneumonia (COP),and 1 case (0.8%) of acute interstitial pneumonia (AIP).The HRCT findings for 36 cases of rheumatoid arthritis associated interstitial lung disease were UIP (24 cases,66.7%) and NSIP (12 cases,33.3%);the HRCT findings for 34 cases of dermatomyositis and polymyositis associated interstitial lung disease were NSIP (32 cases,94.1%),UIP (1 case,2.9%) and COP (1 case,2.9%);the HRCT findings for 31 cases of systemic sclerosis associated interstitial lung disease were NSIP (21 cases,67.8%),UIP.(9 cases,29%),LIP(1 case,3.2%);the HRCT findings for 18 cases of Sj(o)gren syndrome associated interstitial lung disease were NSIP (9 cases,50.0%),UIP (8 cases,44.4%),LIP (1 case,5.6%);the HRCT findings for 7 cases of mixed connective tissue disease associated interstitial lung disease were UIP (4 cases,57.1%),NSIP (3 cases,42.9%).SLE-ILD was rare,with only 1 case of AIP.Conclusion:Different types of CTD-ILD patients display relatively unique manifestation of HRCT.
