Objective To explore the clinical value of preparatory treatment using estradiol valerate and mifepristone before the curettage of the late small missed abortion.Methods One hundred and twenty patients with late small missed abortion were randomly divided into two groups:pretreated group(n =660) and control group(n =60).Women in the pretreated group were given 5 mg of Estradiol Valerate 3 times/day and 25 mg of Mifepristone twice/day for 3 days each before conducted curettage on the fourth day.While for the control group,curettage was done directly.The condition of operation and the occurrence of short-and long-term complications were compared between the two groups.Results Compared with the control group,the pretreated group had a significant shorter operation duration[(8.6±2.7) rnin vs.(10.5±3.4) min,t =3.390,P =0.001]and less blood lose during operation[(47.5±18.3) ml vs.(61.8±20.5) ml,t =4.031,P ＜ 0.001].In terms of complications,the rates of incomplete curettage[(0 vs 13.3％,x2 =6.563,P =0.010],artificial abortion syndrome(3.3 ％ vs 15.0％,x2 =4.904,P =0.027),the cervical canal adhesion/Asherman' s syndrome(0 vs 11.7％,x2 =5.461,P =0.019),and the vaginal bleeding duration[(7.5±2.7) days vs(8.7±3.4) days,t =2.141,P =0.034]in the pretreated group were significantly lower than those in the control group.The average recovery time of menstruation after operation in the pretreated patients was significantly shorter than that in the control group[(30.5±9.3) days vs.(35.5±7.4) days,t =3.259,P =0.002].Contusion Estradiol valerate and Mifepristone as a preparatory treatment before the operation can help reduce the occurrence of complications and side effects in the late small missed abortion curettage and is helpful for the patients' recovery,thus worth wide clinical application.

The notational DSA was first proposed for the application as a managing tool of neuronradiology in the early 70's of 20th centery.Later on,three dimensional image reconstruction(3D DSA) was aquired along with the progressing development of technique and equipment devices.As a rather practical instrument,it has been utilized successfully in the diagnosis and management of head and neck vascular disorders,and abdominal parenchymal diseases during recent years.We comprehensively intoduce the application of rotational DSA for the diagnosis and management in hepatic diseases.

Purpose:To study the expression of survivin a nd its relationship with the expression of bcl-2,p53,PCNA in the dysplasia of gastric mucous epithelium and gastric carcinoma.Methods:The expression of survivin,bcl-2,p53,PCNA was detected in 60 cases of gastric carcinoma, 30 cases of dysplasia of gastric mucous epithelim and 8 cases of normal gastric mucous tissue by using immunohistochemical SP method. Results:The positive rates of survivin were 10%,13%,80% and 82%, respectively, in mild, moderate and svere dysplasia and gastric carcinoma. The positive rates of survivin in mild and moderate dy splasia were significantly lower than in severe dysplasia and carcinoma (P0 05). BACKGROUND:Tissue and cellimplantation entails high-quality seed cells. In order to satisfy this requirement, it is crucial to produce adequate wel-conditioned, high-purity and strong proliferation ability bone marrow-derived mesenchymal stem cells.
OBJECTIVE:To establish a simple, rapid and effective in vitro isolation and culture method of bone marrow-derived mesenchymal stem cells, and to define the biological features of bone marrow mesenchymal stem cells.
METHODS:Rat bone marrow mesenchymal stem cells were isolated from the bilateral tibial and femoral bones by the method of whole bone marrow, then purified and passaged by attachment method. The morphology and features of bone marrow mesenchymal stem cells were observed, the growth curve was drawn and the cellsurface antigen was detected by flow cytometry. The bone marrow mesenchymal stem cells were induced to differentiate along the osteogenic, chondrogenic and adipogenic lineages.
RESULTS AND CONCLUSION:Bone marrow mesenchymal stem cells isolated by the whole bone marrow adherence method grew vigorously and were highly purified. The cultured cells were spindle-shaped. The growth curve was S-shaped and the population doubling time was 29 hours. The cells stil maintained a strong proliferative capacity after they were passaged for 10 generations. The surface markers such as CD44, CD29, CD90 were positive, while CD45, CD34, CD11b were negative. At the third passage, bone marrow mesenchymal stem cells were induced to differentiate along the osteogenic, chondrogenic and adipogenic lineages, respectively. Fol owing induction, Alizarin red staining, alkaline phosphatase staining, von-kossa mineralized nodules staining, toluidine blue staining, and oil red O staining were al positive. This shows that the whole bone marrow adherence method is a simple and reliable method for the in vitro isolation, culture and proliferation of bone marrow mesenchymal stem cells. Moreover, they have multi-lineage differentiation capacity under different inducers. The third passage bone marrow mesenchymal stem cells have the highest biological activity and can act as the ideal seed cells for subsequent experiments.

OBJECTIVE:To investigate the effects and safety of Compound musk injection combined with edaravone on coag-ulation and neurological function of patients with traumatic brain injury. METHODS:A total of 84 patients with traumatic brain in-jury in our hospital during Apr. 2015 to Apr. 2016 were divided into observation group and control group,with 42 cases in each group. Control group was given Edaravone injection 30 mg+0.9% sodium chloride injection 100 mL,bid,ivgtt;observation group was additionally given Compound musk injection 20 mL+0.9% sodium chloride injection 250 mL,qd,ivgtt. Both groups were treated for 4 weeks. NIHSS score,GCS score,the levels of NSE,CRP,PT,APTT and D-dimer,GOS score,the occurrence of ADR were compared between 2 groups before and after treatment. RESULTS:After treatment,NIHSS score of 2 groups decreased significantly,compared to before treatment;the observation group was significantly lower than the control group,with statistical significance (P<0.05). GCS score of 2 groups increased significantly,compared to before treatment;and the observation group was significantly higher than the control group,with statistical significance(P<0.05). The serum levels of NSE,CRP,PT,APTT and D-dimer in 2 groups decreased significantly,compared to before treatment;and the observation group was significantly lower than the control group,with statistical significance(P<0.05). GOS score of observation group was significantly higher than that of control group,with statistical significance(P<0.05). No obvious adverse reaction was found in 2 groups during treatment. CON-CLUSIONS:Compound musk injection combined with edaravone can improve coagulation and neurological function of patients with traumatic brain injury,relieve traumatic brain injury and improve prognosis.

Objective To investigate the related factors for Alzheimer's disease(AD) in Han versus Uygur populations in Uygur Autonomous Region.Methods A case-control study was conducted.Firstly,people over 50 years old were involved in epidemiology inquisition in Xinjiang Uygur Autonomous Region.Then we screened for dementia by MMSE and scrutinized the person who got the score of MMSE for illiteracy(≤17),elementary school(＜20),middle school and above it(＜ 24)by Hachinski Ischemic Score(HIS) and activity of daily living scale(ADL).Finally,AD cases were diagnosed by standard of the national institute of neurologic,communicative disorders and stroke and AD-related disorders association (NINCDS-ADRDA),and controls were selected based on age roughly.Univariate and multivariate analysis of related factors for AD in the two populations were conducted by using logistic regression mode.Results There were 109 AD cases and 218 controls in Han population and 127 AD cases and 254 controls in Uygur population.Logistic regression analysis showed that the risk for AD in Han population was increased by 3.078,1.369 and 2.794 folds respectively in women,people with high level of serum total cholesterol and people with low-density lipoprotein cholesterol as compared with men,people with normal level of serum total cholesterol and people with low-density lipoprotein cholesterol(OR =3.078,1.369 and 2.794,respectively,all P＜ 0.05); and the risk for AD in Uygur population was increased by 3.279,3.447,1.407 and 1.746 folds respectively in women,people with brain trauma history,and people with high level of serum triglyceride and total cholesterol as compared with men,people without brain trauma history and with normal level of serum triglyceride and total cholesterol(OR=3.279,3.447,1.407 and 1.746,all P＜ 0.05).Conclusions The study demonstrates that female and high total cholesterol are the risk factors for AD in both of Han and Uygur populations.High level of low-density lipoprotein cholesterol is one of the risk factors for AD in Han population,and brain trauma history and high level of serum triglyceride are risk factors for AD in Uygur population.

Objective To investigate and analyze the postoperative progression of ossification of the patients with ossification of the posterior longitudinal ligament (OPLL) on cervical spine. Methods From Jaunary 2001 to December 2007, 95 postoperative patients with cervical OPLL were followed and analyzed retrospectively. There are 72 males, 23 females, with the average age of 56.3 years (range, 40-73years). The follow-up time was from 1 to 6 years, average 3.1 years. Among them 36 patients were performed with anterior cervical corpectomy, fusion with titanium mesh and fixed with cervical plates, others treated with posterior cervical laminectomy and fixation. 2 of the 95 cases were performed anterior and posterior combined operation. Clinical data, X-rays, CT and MR images and progression of ossification, were measured and analyzed in details .The relationships between the progression of ossification and relative factors, as gender, age, C3 ossified involved, T-OPLL, OPLL-type, time of follow-up, surgical approach, Japanese Orthopaedic Association (JOA) scores and improvement rate of JOA scores, were analyzed. Results Progression of ossification in 39 cases among the 95 followed postoperative OPLL patients, 28 men and 11 women, average age 55.9 years, range 41-71 years. The age of progressed patients included 12 cases of ≤49 years, 12of 50-59 years, 12 of 60-69 years and 3 of ≥70 years. 35 patients were operated by posterior approach and only 4 treated with anterior operation. According to the standard of the progression of ossification that 2 mm in the length or/and thickness, there are 4 cases progressed only in length, 2 only in thickness, other 33 patients both the length and thickness. Progression of length is from 2mm to 20mm (average 7.74±4.71). But thickness is progressed from 2 mm to 6 mm (average 2.67±1.51). From 1 to 3 years follow-up time it appeard as a downtrend about the progression of ossification. But it may appear an uptrend from the 4th year. JOA score and improvement rate of the JOA score were almost improved to the high-point in three years. And according to the statistic data there are obvious relationship between progression of ossification with age, surgical approach and C3 ossified involved. Conclusion There is a high rate of postoperative ossification progression in cervical OPLL patients. Cervical OPLL patients with C3 ossification involved, performed with posterior laminectomy and those young at surgery may have higher rate of progression of the ossification. The JOA score and improvement rate of the JOA score were little influenced by the progression of the OPLL during the short and intermediate-term follow-up.

Objective To detect the selective inhibitory effects of irradiation plus adenovirusmediated horseradish peroxidase ( HRP)/indole-3-acetic acid (IAA) suicide gene system using tumorspecific and radio-inducible chimeric promoter on human hepatocellular carcinoma subcutaneously xenografted in nude mouse.MethodsRecombinant replicated-deficient adenovirus vector containing HRP gene and chimeric human telomerase reverse transcriptase (hTERT) promoter carrying 6 radioinducible CArG elements was constructed.A human subcutaneous transplanting hepatocellular carcinoma (MHCC97 cell line) model was treated with -γ-ray irradiation plus intra-tumor injections of adenoviral vector and intra-peritoneal injections of prodrug IAA.The change of tumor volume and tumor growth inhibiting rate,the survival time of nude mice,as well as histopathology of xenograft tumor and normal tissues were evaluated.Results Thirty one days after the treatment,the relative tumor volumes in the negative,adenovirus therapy,irradiation,and combination groups were 49.23 ± 4.55,27.71 :± 7.74,28.53 + 10.48 and 11.58 ± 3.23,respectively.There was a significantly statistical difference among them (F = 16.288,P ＜0.01 ).The inhibition effect in the combination group was strongest as compared with that in other groups,and its inhibition ratio was 76.5%.The survival period extended to 43 d in the combination group,which showed a significantly difference with that in the control group(x2 = 18.307 ,P ＜0.01 ).The area of tumors necrosis in the combination group was larger than that in the other groups,and the normal tissues showed no treatment-related toxic effect in all groups.However,multiple hepatocellular carcinoma metastases were observed in the liver in the control group,there were a few metastases in the monotherapy groups and no metastasis in the combination group.Conclusions Adenovirus-mediated suicide gene therapy plus radiotherapy dramatically could inhibit tumor growth and prolong median survival time.It might provide a promising therapeutic modality for hepatocellular carcinoma therapy.

This paper proposed a rehabilitation training system with electromyography (sEMG) feedback for stroke patients based on ARM embedded system and LabVIEW. The system can achieve real-time acquisition, processing and dualview of multi-channel sEMGs and compute related sEMG parameters including iEMG, RMS, MPF and co-contraction ratio. The system was detected by clinical experiments and related inspection department. The result showed that the system is functional, interactive and in accordance with the relevant standards for medical devices so that it can fully satisfy the clinical demands. In addition, the system can help doctors to master the training state of the patient more effectively in a real-time and quantitative way that is direct to improve the training programs of stroke patients.

Objective To investigate the therapeutic effects and toxic effects of radiochemotherapy versus chemotherapy alone in patients with advanced gastric cancer after gastrectomy.Methods A total of 183 patients with stage Ⅱ-Ⅳ (M0) gastric cancer,who received treatment after gastrectomy from 2004 to 2010,were included in the study.Of the 183 patients,64 received radiochemotherapy after gastrectomy,and 119 received chemotherapy alone after gastrectomy.The survival rates and hematological and gastrointestinal toxic effects were compared between the two groups;survival difference was also analyzed after the patients were stratified by TNM stage,number of metastatic lymph nodes,and extent of lymph node dissection (D0,D1,and D2).The Kaplan-Meier method was used for calculating survival rates,and the log-rank test was used for survival difference analysis and univariate prognostic analysis;the chi-square test was used for comparing toxic effects between the two groups.Results The follow-up rate was 87.8％ ;72 patients were followed up for at least 3 years,and 29 patients for at least 5 years.The 1-,3-,and 5-year survival rates for the radiochemotherapy group were 86％,62％,and 55％,respectively,as compared with 77％,53％,and 43％ for the chemotherapy group (P =0.079).There were no significant differences in grade 3-4 hematological and gastrointestinal toxic effects between the two groups (P =0.363 and 0.617).The univariate analysis showed that radiochemotherapy had a significantly better survival benefit than chemotherapy alone in patients with stage ⅢB-Ⅳ (M0) gastric cancer,patients who underwent D0 lymph node dissection,and patients with more than 6 metastatic lymph nodes (P =0.022,0.025,and 0.021).Conclusions Compared with chemotherapy alone,radiochemotherapy tends to improve survival in patients with gastric cancer after gastrectomy,and its toxic effects are tolerable.The patients with stage ⅢB-Ⅳ (M0) gastric cancer,patients who undergo D0 lymph node dissection,and patients with more than 6 metastatic lymph nodes can benefit from radiochemotherapy.
