1.Effect of social interaction model on the teaching of clinical internal medicine

Objective To evaluate the effect of social interaction model on the internal clinical practice and to explore the teaching model which can improve the quality of medical students in clinical medical practice.Methods Totally 261 students of practiced in department of internal medicine from July 2012 to June 2013 were enrolled and were randomly divided two different teaching teams:traditional teaching group (n=130) and social interaction group (n=131).Results of theory examination,skills assessment,medical record writing test and medical humanities appraisal in two groups were compared.Questionnaire was employed to investigate the effect of social interaction model on the internal clinical practice.SPSS 17.0 software was used to do statistical analysis and measurement data was expressed x ± s.Two-sample mean was analyzed by t test andx2 test was used to compare the ratio.P<0.05 stands for the statistically significant difference.Results There was no statistically significant difference in theory resuhs between traditional teaching group and social interaction model group(P=0.108).Results of medical record writing test and medical humanities appraisal in social interaction group were obviously higher than those of traditional teaching group(P=0.013,0.009).Number of class A medical record and qualified students in clinical skill examination was obviously increased in social interaction group than in traditional teaching group(P<0.05).Most students held a positive attitude to the social interactive teaching model.Conclusions Social interaction teaching helps to improve the quality of clinical internal medical practice,especially the improvement of clinical practice skills and the cultivation of medical humanities knowledge of medical students.
2.Effect of zinc supplement on bone-contacting implant surface ratio

Objective: To study the effect of zinc supplement on bone-contacting implant surface ratio (BCSR).Methods: 40 rabbits with titanium implants placed into the proximal tibial heads were divided randomly into zinc supplement group(n=20) and control group(n=20). 1% zinc sulfate in 0.9% saline was administered intramuscularly at the dose of 4 mg/(kg?d) for each tested animal, and the same amount of 0.9% saline was used for the controls. The animals were sacrificed 1, 2, 4, 8 and 12 weeks after treatment respectively.BCSR,bone zinc content and serum Cu/Zn values were measured with morphometry, atomic absorption photometry and atomic nebulizntion absorption method respectively.Results: Zinc supplement could significantly increase the BCSR value (P0.05). Conclusion: Moderate zinc supplement may increase the bone-contacting implant surface ratio, but not alter the zinc metabolism.
3.Peripheral nerve injuries during cardiac surgery

Neuropathies are complications of cardiac surgery. The reported frequency of nerve injuries varies from 1. 5% to 2. 4% for the brachial plexus and 10% to 60% for the phrenic nerve. But the recurrent laryngea! nerve,lower limb (saphenous) nerves, and the sympathetic chain are less frequently injured. The reasons of neuropathy are mainly positioning and topical hypothermia for myocardial protection. The overall prognosis of neuropathies after cardiac surgery is good, and they need no special treatment. However, prolonged recovery of phrenic neuropathy did occurr.
4.Determination of Diclofenac Sodium in Huamn Plasma by HPLC

OBJECTIVE:To establish a HPLC procedure to determine diclofenac sodium in human plasma.METHODS:The plasma sample was deproteinized by adding acetonitrile,A Hypersil-BDS(250?4.6mm,5?m)column with a mobile phase consisting of methanol-water-10%phosphate acid(60∶40∶1)was used,the detecting wavelength was276nm,column tem?perature was35℃.RESULTS:The linear range was20.50~2050.0ng/ml(r=0.9998,n=5),the detective limit was10ng/ml.The recovery was99.47%~105.4%with the within-day RSD from3.77%to5.81%(n=5),and the between-day RSD from4.93to8.96%(n=5)at three different concentrations.CONCLUSION:The method is simple,rapid and suitable for the pharmacokinetic study of diclofenac sodium.

6.Determination of magnolol and honokiol in Huoxiangzhengqi Capsule by SFE-CGC

Objective: To determine magnolol and honokiol in Huoxiangzhengqi Capsule(oil of Herba Agastachis, oil of Perillae, Pericarpium Citri Reticulatae, Cortex Magnoliae Officinalis, etc.).Methods: A supercritical Fluid Extraction (SFE) has been used to extract magnolol and honokiol in Huoxiangzhengqi Capsule, and CGC was used to analyse the extracts.Results: The average rccovery was 96.81% ( RSD =2.81%, n =5) for magnolol, and 97.75% ( RSD =2.36%, n =5) for honokiol, respectively. Conclusion: The SFE CGC offline method for the analysis of Huoxiangzhengqi capsule is simple, quick and accurate.
7.The risk factors analysis of complications of 364 cases in treatment of radical esophagectomy

Objective To study the risk factors of complications of 364 cases in treatment of rad-ical esophagectomy.Methods 364 patients underwent two or three-incision radical esophagectomy and the clinic data were retrospectively analyzed.After review of the reference,ten factors were regarded as the potential risk factors of postoperative complications,then univariate and multivariate analyses were per-formed.Results In 364 patients,66 patients had complications of this research field within postoperative 6 months.By univariate analysis,preoperative nutritional risk score、T-staging、preoperative complications、operation time、operation experience、anastomotic position、anastomotic extra manual suturing were deter-mined as influence factors.By multivariate analysis,preoperative nutritional risk score≥3、T-staging score≥3、preoperative complications、operation time≥240 min、cervical anastomosis were determined as risk factors,anastomotic extra manual suturing was determined as a protective factor.Conclusion The risk factors of the complications after radical esophagectomy are preoperative nutritional risk score≥3、T-stag-ing score≥3、preoperative complications、operation time≥240 min、cervical anastomosis;and the protective factor is anastomotic extra manual suturing.Paying attention and controling these risk factors may reduce the occurrence of postoperative complications.

Purpose:To study the expression of survivin a nd its relationship with the expression of bcl-2,p53,PCNA in the dysplasia of gastric mucous epithelium and gastric carcinoma.Methods:The expression of survivin,bcl-2,p53,PCNA was detected in 60 cases of gastric carcinoma, 30 cases of dysplasia of gastric mucous epithelim and 8 cases of normal gastric mucous tissue by using immunohistochemical SP method. Results:The positive rates of survivin were 10%,13%,80% and 82%, respectively, in mild, moderate and svere dysplasia and gastric carcinoma. The positive rates of survivin in mild and moderate dy splasia were significantly lower than in severe dysplasia and carcinoma (P0 05). Objective:To investigate the specific immune act ivated effects in the mouse immunized by MHC I-restriced tumor antigen peptide pulsed dendritic cells(DC) with IL-2 gene modified.Methods:DC were transfected with IL-2 gene via adenovirus vector(DC-IL-2) the mRNA of mIL-2 was examined by RT-PCR.Syngeneic mice were immunized by DC-IL-2 pul sed with MHC-I-restricted Mutl tumor antigen peptiede of 3LL Lewis lung carcin oma (DC-IL-2-Mutl),the effects of lymphocyte subsets in the draining lymphoid node from the tumor antigen pulsed DCs with gene modified immunized mice by flo w-cytometric analysis(FACS) analysis.Results:The mRNA of mIL-2 was expressed in the DCs.The ratio of CD8+T cells was hosted in the dra ining lymphoid node from Mutl pulsed DCs(DC-Mutl) immunized mice.The ratio of C D8+T cells and NK cells were all hosted obviously in the draining lymphoid nod e from DC-IL-2-Mutl immunized mice .Conclusion:Immunized mice with DC-IL-2-Mutl can induce the mouse specific antitumor immunized affect in vivo and can activate a variety of immunized effect in the mouse.
10.Protective effect of isoflurane preconditioning on the brain against ischemia-reperftision injury in gerbils

Objective To investigate the protective effect of isoflurane preconditioning on the brain against ischemia-reperfusion (I/R) injury and the underlying mechanism.Methods Seventy-five male gerbils weighing 60-80 g were randomly divided into 5 groups ( n = 15 each): group A sham operation (sham) ; group B I/R; group C isoflurane + I/R (ISO); group D 5-HD (mito-KATP channel blocker) + I/R (5-HD) and group E 5-HD + isoflurane + I/R (5-HD+ISO). The animals were anesthetized with intraperitoneal (i.p.) 10% chloral hydrate 0.35 g?kg-1 . Global cerebral I/R was produced by occlusion of bilateral common carotid arteries for 5 min and confirmed by isoelectric potential on EEG. In ISO group the animals inhaled 1.2%-1.5% isoflurane for 30 min followed by 30 min wash-out period before I/R. In 5-HD group 5-HD 10 mg?kg-1 was injected i.p. 30 min before I/R. In 5-HD+ ISO group 30 min after i.p. 5-HD 10 mg?kg-1 , isoflurane was inhaled as in ISO group. The animals were killed 24 h after global cerebral ischemia and their forebrains were excised for (1) electron microscopic examination of mitochondria and (2) measurement of mitochondrial permeability transition pore (MPTP) (ultraviolet spectrophotometer) and (3) calcium content in mitochondria (spectrophoto-fluorometer) . Results Marked swelling of mitochondria with disrupted cristae and damaged matrix were observed in group I/R (B) , 5-HD (D) and 5-HD + ISO (E) while in sham (A) and ISO (C) groups relative intact mitochondria were seen.
