1.Pathological characteristics of carcinoid tumors of the urinary bladder and its treatment (report of 4 cases)

Objective To study the clinicopathological features,diagnosis and treatment of carcinoid tumors of the urinary bladder. Methods The clinical,histopathologic data of 4 carcinoid tumors of the urinary bladder were retrospectively analysed.The bladder carcinoid was diagnosed in 3 male patients and 1 female patient with age from 54 to 79 years.All patients were presented with gross hematuria,but with no clinical manifestations of the carcinoid syndrome such as flush,tachycardia,diarrhea.Cystoscopies showed 5 mm?5 mm to 25 mm?30 mm sessile polypoid masses. Results One patient received radical cystectomy followed by radiotherapy,3 patients received partial cystectomy.Two patients died of the tumor metastasis at survival interval 5 years and 3 months,respectively;other 2 patients were still alive 19 years and 8 months following the surgical treatment.Two cases were stage Ⅱ,and other 2 cases were stage Ⅲ.Histologically,tumor cells exhibited strong argyrophilia and electron microscopy revealed characteristic dense-core granules. Immunohistochemical testing showed the cancer cell was positive for chromogranin A and keratin. Conclusions Carcinoid of the urinary bladder is uncommon,the diagnosis depends on histopathological and immunohistochemical tests.Based on the degree of infiltration,size,location,lymph node metastasis,different surgical intervention can be choosed.
2.Simultaneous Determination of Minoxidil and Tretinoin in Compound Minoxidil Gel by HPLC

Objective:To establish an HPLC method for the simultaneous determination of minoxidil and tretinoin in compound minoxidil gel. Methods:A Shim-Pack VP-ODS C18(250 mm × 4. 6 mm,5 μm)column was used with the mobile phase of methanol and 2% acetic acid solution by gradient eleution . The detection wavelength was respectively set at 280 nm and 350 nm,and the flow rate was 1. 0 ml·min -1 . The column temperature was 35℃ and the injection volume was 10 μl. Results:Minoxidil and tretinoin had a good linear relationship within the range of 4. 0- 240. 0 μg·ml-1 and 0. 05- 3. 00 μg·ml-1(r =0. 999 6 and 0. 999 3),respectively. The average recovery was 99. 56%(RSD = 0. 42% ,n = 9)and 99. 36%(RSD = 0. 50% , n = 9),respectively. Conclusion:The established method is simple,and suitable for the simultaneous determination of the two constituents in compound minoxidil gel.
3.Determination of astragloside Ⅳ in Danqi Injection by HPLC-ELSD

AIM: To establish a method for determination of astragloside Ⅳ in Danqi Injection (Radix Salviae Miltiorrhizae,Radix Astragali). METHODS:HPLC-ELSD with Hypersil C_(18) column was used.The mobile phase was composed of acetonitrile-water(38∶62).The flow rate was 0.85 mL?min~(-1).An evaporative light-scattering detector(ELSD) model 2000 was used as detector. RESULTS:For astragloside Ⅳ,the linear range was 1.074-(3.408 ?g),and the average recovery was 100.71%,RSD was 1.79%. CONCLUSION:The method is simple,accurate,reproducible and can be applied to the quality control of the preparation. anqi

There are three major ways of for the treatments of post-stroke cognitive impairment.Medicine,acupuncture and the cognitive training is the most popular.Both clinical reports and experimental studies proved positive effect of acupuncture to post-stroke cognitive function impairment.There are more clinical reports and comprehensive mechanism studies on the Brain-activating and Orifice-opening Acupuncture than the other two popular treatments of acupuncture for cognitive function impairment.Acupuncture show positive effect on post-stroke cognitive function impairment.Further studies are required to validate whether acupuncture has better curative effect than cognitive training.
5.Percutaneous transhepatic stent angioplasty for portal vein stenosis after liver transplant

Objective To evaluate the therapeutic results of percutaneous transhepatic stent angioplasty for portal vein stenosis following liver transplant.Methods From 2005 to 2007,7 patients developed portal vein stenosis following liver transplant.Percutaneous transhepatic stent angioplasty of the portal vein was performed in all patients.The therapeutic results were monitored by clinical follow-up and imaging examination.Results In seven patients,the percutaneous transhepatic stents were placed successfully.The follow up period ranged from 3 months to 34 months.Portal venous patency Was maintained in six patients(one patient died due to hepatic arterial thrombosis and ischemic insult to bile duct at three months following the stent placement).No complications due to stent angioplasty occurred.Conclusion Percutaneous transhepatic stent angioplasty is an effective and safe method for treatment of portal vein stenosis following liver transplant.
6.Study of the value of SPECT lung perfusion imaging in optimizing lung cancer radiotherapy plan for lung function protection

Objective:To evaluate the application value of SPECT lung perfusion imaging in guiding radiotherapy path, optimizing the radiotherapy plan for lung cancer and protecting lung function during radiotherapy for locally advanced non-small cell lung cancer.Methods:In this study, 84 patients with stage Ⅲ non-resectable non-small cell lung cancer were randomly divided into the control group ( n=44) and observation group ( n=40). In the control group, radiotherapy plan based on conventional CT images was delivered, and two plans based on the lung function information suggested by conventional CT and SPECT lung perfusion imaging: P1 and P2 were given in the observation group. All patients in the observation group were finally treated according to the P2 plan. The incidence of radiation pneumonitis, and changes in lung function before and after radiotherapy were statistically compared between two groups. The dose-volume parameters of P1 and P2 were statistically compared. Results:After the plan was optimized, the incidence of radiation pneumonitis in the observation group was significantly reduced and the decline of lung function was significantly improved (both P≤0.001). The functional dose parameters were significantly improved in the P2 plans (both P<0.05), whereas the irradiation dose of organs at risk did not significantly change ( P>0.05). Conclusion:SPECT lung perfusion imaging optimizes the intensity-modulated radiotherapy plan, which can reduce the functional lung dose and increase the tumor radiotherapy dose without increasing the irradiation dose of other organs at risk.
7.Prevalence of cataracts and the coverage rate of cataract surgery in Ningxia region

Objective:To investigate the prevalence of cataracts, the surgical coverage, and postoperative visual acuity of adults in Ningxia.Methods:A cross-sectional study using multistage cluster random sampling was conducted.Ten survey sites in Ningxia were selected and the population aged 18 years and over was surveyed with questionnaire, height and weight measurements, visual acuity, intraocular pressure, fundus photography and slit-lamp examinations.Cataract prevalence and its influencing factors were analyzed.Cataract prevalence, surgical coverage and presenting visual acuity (PVA) and best corrected visual acuity (BCVA) after surgery were investigated in different age groups of the examined population.The study adhered to the Declaration of Helsinki and was approved by the Ethics Committee of the People's Hospital of Ningxia Hui Autonomous Region (No.[2023]-LL-010).Participants signed informed consent prior to the examination.Results:A total of 6 145 people should be examined, and 5 721 people were actually examined, with an examination rate of 93.10%.The study population consisted of 2 558 males, accounting for 44.71%, and 3 163 females, accounting for 55.28%, with ages ranging from 18 to 93 years old and an average age of (64.27±13.48) years.Among them, 1 180 patients diagnosed with cataract, with a cataract prevalence of 20.62%.The prevalence of cataract increased with age and decreased with education level, showing statistically significant differences ( χ2=1 091.32, 581.92; both at P<0.01).The prevalence of cataract was significantly higher among people with hypertension, diabetes mellitus, hyperlipidemia, and coronary heart disease than those without these diseases ( χ2=274.65, 118.15, 78.05, 182.71; all at P<0.01).Cataract surgery was performed in 245 cases in the cataract patient population, with a surgical coverage rate of 20.76%.Of the 245 cases, 229 cases were implanted with IOLs, with an implantation rate of 93.40%.The social burden rate of cataract blindness was 2.29%, and increased with age.Of the 339 eyes that underwent cataract surgery, 241 had a PVA≥0.3, accounting for 71.09%, and 272 had a BCVA≥0.3, accounting for 80.24%. Conclusions:In Ningxia, cataracts are still the main cause of vision impairment and blindness in the elderly, and the social burden rate of cataract blindness is high.Moreover, the coverage rate of cataract surgery is low, so both the coverage and quality of surgery need improvement.
8.Association of maternal pre-pregnancy BMI, gestational weight gain, and gestational diabetes mellitus with BMI trajectory in early childhood: a prospective cohort study

Objective:To examine the associations of pre-pregnancy body mass index (BMI), gestational weight gain, and gestational diabetes mellitus (GDM) with early childhood BMI trajectories.Methods:A total of 1 227 mother-child pairs from the Peking University Birth Cohort in Tongzhou were included in this study. In the cohort, maternal pre-pregnancy weight, height, gestational weight gain, and GDM diagnosis were collected. The children were followed up at birth and at 1, 3, 6, 9, 12, 18, 24, 30, and 36 months of age to obtain height/length and weight data. The longitudinal data-based k-means clustering algorithm was used to identify early childhood BMI trajectory groups. The associations of maternal pre-pregnancy BMI, gestational weight gain, and GDM with early childhood BMI trajectories were analyzed using the logistic regression model. We further explored whether there is an interaction effect between pre-pregnancy overweight/obesity and excessive gestational weight gain on the risk of the high BMI trajectory in early childhood through multiplicative and additive interaction analyses. Results:The prevalence rates of overweight and obesity before pregnancy were 21.2% (260 cases) and 6.6% (81 cases) respectively. The prevalence of excessive gestational weight gain and GDM was 57.7% (708 cases) and 30.9% (379 cases). The early childhood BMI trajectories were named low, medium, and high trajectories, accounting for 30.5%, 45.4% and 24.1%, respectively. After controlling potential confounding factors, it was found that pre-pregnancy overweight ( OR=1.54, 95% CI: 1.12-2.12), obesity ( OR=2.33, 95% CI: 1.41-3.85), and excessive gestational weight gain ( OR=1.47, 95% CI: 1.10-1.97) were risk factors for being in the high BMI trajectory in early childhood. GDM was not significantly associated with early childhood BMI trajectories ( P>0.05). Compared with the independent effects of pre-pregnancy overweight/obesity ( OR=1.90, 95% CI: 1.17-3.09) and excessive gestational weight gain ( OR=1.45, 95% CI: 1.03-2.04), the risk of being in the high BMI trajectory in early childhood was greater when the two factors coexisted ( OR=2.38, 95% CI: 1.60-3.54). However, both the multiplicative and additive models showed no interaction effect between pre-pregnancy overweight/obesity and excessive gestational weight gain. Conclusions:Maternal pre-pregnancy overweight/obesity and excessive gestational weight gain are independent risk factors for children being in the high BMI trajectory in early childhood, providing scientific evidence for obesity prevention.
9.A prospective cohort study of association between early childhood body mass index trajectories and the risk of overweight

Objective:To compare the association between body mass index(BMI)trajectories deter-mined by different methods and the risk of overweight in early childhood in a prospective cohort study,and to identify children with higher risk of obesity during critical growth windows of early childhood.Methods:A total of 1 330 children from Peking University Birth Cohort in Tongzhou(PKUBC-T)were included in this study.The children were followed up at birth,1,3,6,9,12,18,and 24 months and 3 years of age to obtain their height/length and weight data,and calculate BMI Z-score.Latent class growth mixture modeling(GMM)and longitudinal data-based k-means clustering algorithm(KML)were used to determine the grouping of early childhood BMI trajectories from birth to 24 mouths.Linear regres-sion was used to compare the association between early childhood BMI trajectories determined by different methods and BMI Z-score at 3 years of age.The predictive performance of early childhood BMI trajecto-ries determined by different methods in predicting the risk of overweight(BMI Z-score>1)at 3 years was compared using the average area under the curve(AUC)of 5-fold cross-validation in Logistic regres-sion models.Results:In the study population included in this research,the three-category trajectories determined using GMM were classified as low,medium,and high,accounting for 39.7%,54.1%,and 6.2%of the participants,respectively.The two-category trajectories determined using the KML method were classified as low and high,representing 50.3%and 49.7%of the participants,respectively.The three-category trajectories determined using the KML method were classified as low,medium,and high,accounting for 31.1%,47.4%,and 21.5%of the participants,respectively.There were certain differ-ences in the growth patterns reflected by the early childhood BMI trajectories determined using different methods.Linear regression analysis found that after adjusting for maternal ethnicity,educational level,delivery mode,parity,maternal age at delivery,gestational week at delivery,children's gender,and breastfeeding at 1 month of age,the association between the high trajectory group in the three-category trajectories determined by the KML method(manifested by a slightly higher BMI at birth,followed by rapid growth during infancy and a stable-high BMI until 24 months)and BMI Z-scores at 3 years was the strongest.Logistic regression analysis revealed that the three-category trajectory grouping determined by the KML method had the best predictive performance for the risk of overweight at 3 years.The results were basically consistent after additional adjustment for the high bound score of the child's diet balanced index,average daily physical activity time,and screen time.Conclusion:This study used different methods to identify early childhood BMI trajectories with varying characteristics,and found that the high trajectory group determined by the KML method was better able to identify children with a higher risk of overweight in early childhood.This provides scientific evidence for selecting appropriate methods to de-fine early childhood BMI trajectories.
10.Effect of Acupuncture at “Zhibian (BL 54)-to-Shuidao (ST 28)” on Key Factors of Endogenous Exogenous Apoptotic Pathways in Asthenospermia Model Mice

ObjectiveTo observe the effect of acupuncture “Zhibian (BL 54)-to-Shuidao (ST 28)” on the reproductive function of asthenospermia model mice and to explore the possible mechanism. MethodsForty-two male C57BL/6 mice were randomly divided into blank group, model group, and acupuncture group, with 14 mice in each group. Cyclophosphamide 30 mg/(kg·d) was injected intraperitoneally for 7 days to establish model of asthenospermia for the model group and the acupuncture group, while 0.9% sodium chloride solution 10 ml/(kg·d) was injected intraperitoneally for 7 days in the blank group. After successful modeling, mice in the acupuncture group received acupuncture at “Zhibian (BL 54)-to-Shuidao (ST 28)” once a day for 20 minutes, 6 times a week for 3 weeks; the remaining two groups were fixed without acupuncture. Daily observations were made on the general conditions of mice in all groups, and changes in body weight after intervention were recorded. On the next day after completing the treatment, 6 male mice selected randomly from each group to test the sperm quality as well as testicular and epididymal weights, and calculate the corresponding indices; ELISA was used to test the levels of testosterone (T), follicle-stimulating hormone (FSH), and luteinizing hormone (LH) in serum; HE staining and TUNEL staining were performed to observe the pathological morphology and apoptosis of testicular and epididymal tissues; fluorescence quantitative PCR and western blot were used to detect changes in the expression of apoptosis-related factors (Fas), Fas-associated death domain protein (FADD), B-cell lymphoma 2 (Bcl-2), Bcl-2-associated X protein (Bax), and cysteine-aspartic acid protease (caspase)-3 mRNA and protein in testicular tissue. The remaining 8 male mice in each group were housed with estrus-cycling mice of the same strain at 1∶1 ratio, and the pregnancy rate and number of embryos per litter in each group were determined after mating. ResultsIn comparison with the blank group, mice in the model group exhibited reduced body weight, decreased testicular mass, testicular index, epididymal mass, and epididymal index. Additionally, there was a decrease in total sperm count, sperm motility, and sperm viability. Serum levels of T, FSH, and LH were reduced. The apoptosis rate increased, and the expression levels of Fas, FADD, Bax, and Caspase-3 mRNA and proteins were elevated, while Bcl-2 mRNA and protein expression levels decreased (P<0.01). Pathological abnormalities were observed in testicular and epididymal tissues, with disrupted arrangement of seminiferous tubules and a decreased number of spermatogenic cells within the tubular lumen. Furthermore, the pregnancy rate and the number of embryos per litter decreased significantly after mating (P<0.01). In comparison with the model group, mice in the acupuncture group showed improvements in testicular mass, testicular index, epididymal mass, epididymal index, total sperm count, sperm motility, and sperm viability. Serum levels of T, FSH, and LH increased. The apoptosis rate decreased, and the expression levels of Fas, FADD, Bax, and Caspase-3 mRNA and proteins decreased, while Bcl-2 mRNA and protein expression levels increased (P<0.05 or P<0.01). Morphological improvements were observed in testicular and epididymal tissues, with a compact arrangement of seminiferous tubules and an increased number of spermatogenic cells within the tubular lumen. Furthermore, the pregnancy rate and the number of embryos per litter increased significantly after mating (P<0.01). ConclusionAcupuncture “Zhibian (BL 54)-to-Shuidao (ST 28)” can improve testicular tissue apoptosis and enhance reproductive function in a mouse model of asthenospermia. Its mechanism may be associated with the modulation of key factors in the extrinsic membrane receptor pathway (Fas-mediated pathway) and the intrinsic mitochondrial pathway (Bcl-2/Bax-regulated pathway) in testicular tissue.
