1.Significance of Urinary NT-proBNP Levels for Diagnosis of Chronic Heart Failure

Objective To investigate the value of the levels of urinary NT-proBNP whether is similar with the levels of blood NT-proBNP for the diagnosis of chronic heart failure.Methods 79 patients with chronic heart failure (NYHA Ⅰ~Ⅳ)as a research group,and 53 healthy people as a control group,the levels of urinary and blood NT-proBNP on two groups were measured respectively by Roche cobas h 232 cardiac markers detector and compared the difference of the levels of urinary NT-proBNP and blood NT-proBNP between the two groups.Results The levels of urinary and blood NT-proBNP in pa-tients with chronic heart failure were much higher than those in healthy subjects,respectively (P <0.01).The concentration of urinary NT-proBNP increased gradually with more severe symptoms.The level of urinary NT-proBNP was positively cor-related with the levels of blood NT-proBNP.Conclusion The levels of urinary NT-proBNP is a new candidate marker for the diagnosis of chronic heart failures and it provides a similar accuracy with the levels of blood.
2.Contrast of curative effects between extended full-thickness transanal local excision and traditional laparotomy for early stage low rectal cancer

Objective To investigate the advantages and prognosis of extended full-thickness transanal local excision in treating early stage low rectal cancer.Methods Clinical gastrointestinal surgery data of transanal local excision surgeries of 163 patients from January 2003 to December 2010 was reviewed.According to the different operation modes,the patients were divided into group A (transanal local expanded full-thickness resection group,78 cases),group B (traditional open operation group,85 cases),to observe the operation status and survival time.Results The operative time [(43.17±12.31) min vs (216.41±60.12) min,t =12.924,P =0.011],the volume of bleeding (30 ml vs 110 ml,Z =-2.522,P =0.012),the postoperative intestinal function recovery time [(1.61±0.76) d vs (3.72±1.46) d,t =2.681,P =0.037),the postoperative hospital stay [(5.39±1.21) d vs (22.79±8.31) d,t =6.964,P =0.023),the rate of complication [3.85 % (3/78)vs 24.71% (21/85),x2 =14.096,P < 0.001] of group A and group B showed statistical significance (P < 0.05),1 year,3 years,5 years survival rate of group A and group B were 100.0 %,93.5 %,89.5 % and 98.8 %,95.2 %,90.3 % respectively,the average survival time were respectively 57.08 months and 57.49 months,with no statistical difference (P > 0.05).Conclusions The extended full-thickness transanal local excision in treating early stage low rectal cancer can achieve the effect of laparotomy.Compared with laparotomy,it has more advantages such as shorter operative time,less bleeding,less invasive,faster postoperative recovery,and fewer occurrences of complications.
3.Analysis of T-cell receptor?gene rearrangement for cutaneous T-cell lymphomas

Objective To study the early diagnosis of cutaneous T-cell lymphoma(CTCL).Methods DNA from39formalin-fixed,paraffin-embedded tissues of24cases of CTCL,2cases of dubious lymphoma,and4cases with non-specific erythroderma,was amplified by polymerase chain reaction(PCR),with special primers of TCR V?1-8,V?9,and J?1/?2to analyze T-Cell receptor?gene clonal rearrangement(TCR-GR clonal).The specimens from30patients with non-specific dermatitis were taken as negative control.Results Clonal of TCR V?1-8was demonstrated in38of39paraffin-embedded specimens.V?9-GR was shown in37of39specimens TCR?-GR was shown especially in13patients with MF of early stage,2patients with dubious CTCL,and4patients with non-specific erythroderma,as well as in6of30patients with histiologically nonspecific dermatitis which suggested that these6cases were"clonal dermatitis".Conclusion Clonal TCR?-GR may be detected in patients with early CTCL,even when the histologic findings are not unequivocally diagnostic.
4.Ultrastructure and lanthanum element in BMSCs after treatment by Lanthanum Chloride

Objective:To investigate whether lanthanum(La 3+)can enter into ossification induced bone marrow stromal cells(BMSCs)cultured in vitro and its influences on the ultrastructure of BMSCs.Methods:Dog BMSCs were ossification induced and the cells of the third passage were cultuerd with the presence of lanthanum chloride(LaCl_3)at 5.564?102,5.564 and 5.564?10-2 ?g/ml respectively for 5 days,the same passage cells without LaCl_3 treatment were used as the controls.Transmission electron microscope(TEM),electron microprobe with wave spectrum(EMWS)and high resolution TEM with energy spectrum(HRTEM)were used to observe the change of BMSCs ultrastructure and the distribution of lanthanum ions in the cells.Results:Electron-dense granules in lysosome were observed in BMSCs treated with LaCl_3 but not in control BMSCs under TEM.Slight denaturation of some organelles was found in both LaCl_3 treated and control cells.EMWS examination showed that there was La element in BMSCs treated with 5.564?102 ?g/ml LaCl_3.La element was not found in all the treated cells under HRTEM.Conclusion:La 3+ at a certain concentration might enter into the ossification induced BMSCs cultured in vitro.
5.GANT61 as an inhibitor of Gli inhibits growth and invasion of esophageal adenocarcinoma

Objective To study the inhibitory effects of GANT61, as an inhibitor of Gli, on the growth of human esophageal adenocarcinoma cell lines OE19 and OE33, and their mechanisms thereof. Methods After treating with different concentrations of GANT61(30,20,13.333 3,8.888 8,5.925 9,3.950 6,2.633 7,1.755 8,1.170 5μmol/L),the cell viabilities of OE19 and OE33 were detected by MTS method, which expressed by IC50. The Gli1and Gli2 mRNA expressions treated with GANT61(10 μmol/L GANT61) or DMSO for 24 h were detected in OE19 and OE33 cell lines by real time fluorescence quantitative PCR. The protein expressions of Gli1, Gli2 and CyclinD1 treated with GANT61 or DMSO for 24 h were detected in OE19 and OE33 cell lines by Western blot assay. Transwell invasion assay was performed to evaluate the inhibiting effect on OE19 and OE33 cell invasion by the treatment of GANT61 or DMSO. Results The IC50 of GANT61 was 8.08μmol/L in OE19 and 9.65μmol/L in OE33 cells. Compared with DMSO group, Gli1 and Gli2 mRNA expressions and Gli1,Gli2 and CyclinD1 protein expressions were significantly decreased in OE19 and OE33 cells of GANT61 group (P<0.05). The number of penetrating cells was significantly reduced in OE19 and OE33 cells of GANT61 group compared with that of DMSO group (P<0.01). Conclusion GANT61 can inhibit the growth and invasion of esophageal neoplasms cells by down-regulating Gli1 and Gli2 mRNA expression,which indicates that Hedgehog signaling pathway may play an important role in carcinogenesis and progression of esophageal adenocarcinoma.
6.Simultaneous Determination of Catechol and Hydroquinone at Electrospun Palladium Nanoparticle/Carbon Nanofibers Modified Electrode

Palladium nanoparticle/carbon nanofibers(Pd/CNFs) were prepared by electrospinning technique with subsequent thermal treatments and were employed to modify glassy carbon electrode (Pd/CNF-GCE/ CME). Pd/CNF-GCE/CME exhibited excellent electrocatalytic activities towards catechol and hydroquinone,and the reversibility of catechol and hydroquinone were significantly improved. The effects of pH value on the oxidation peak current and potential of catechol and hydroquinone were studied by differential pulse voltamme-try(DPV),and 0. 1 mol/L PBS(pH 8.0) was selected as the supporting electrolyte. Under the optimized conditions,the oxidation peak current of catechol was linearly with the concentration of catechol in the range of 1-90μmol/L in the presence of 50 μmol/L hydroquinone with the detection limit of 0.3μmol/L(S/V =3) and the oxidation peak current of hydroquinone was linearly with the concentration of hydroquinone in the range of 2 - 100μmol/L in the presence of 50 μmol/L catechol with the detection limit of 1. 0μmol/L. Furthermore,the modified electrode exhibited good reproducibility and selectivity. The modified electrode has been used for the determination of catechol and hydroquinone in imitative water samples with satisfactory results.
7.Application of acellular dermal matrix and expandedflapin half auricular reconstructionwithrib cartilage grafts

BACKGROUND:Traumatic auricle defectsin upper 1/2 or lower 1/2,seriously involve theauricular cartilage and skin blood vessels. The autogenic rib cartilage graft and acelular dermal matrix have good histocompatibility, and expanded flapis a kind of thin and achromatic tissue for skin defect repair. OBJECTIVE:To explore theapplication ofacelular dermal matrix and expanded flap in half auricular reconstruction,and to find out the fine carving and anastomosis of autogenic rib cartilage graftas wel as its similarities with the ear and clinical significance. METHODS:Eight cases of half auricular defects were treated with expanded flap, autogenic rib cartilage graft, fine anastomosisofautogenic rib cartilage graft and residual earfor half auricular reconstruction,during which theacelular dermal matrixwas usedto promote residual ear docking and skul auricle angle formation. The reconstructionwasperformed in three stages:first,anexpander(volume, 80mL)wassubcutaneously implanted attheretro-auricular area;second, the auricular defects were reconstructed with fine rib cartilage graft, acelular dermal matrix and auriculoplasty;finaly, acelular dermal matrixwas usedto promote residual ear docking. Thenthehalf auricular reconstructionwas evaluatedby objective measurement and subjective rating. RESULTS AND CONCLUSION:Half auricular reconstruction was successful in al the eight caseswithout obvious complications, and the cartilage grafts were in good condition.During thefolow-up,thereconstructed auriclewasshapedwel andformed a good involution withtheresidualauricle. In addition, the flange was smooth withoutobviouscolor difference and edema, and its position, size and shape were consistent with those of thecontralateralone. Afterthefolow-up of 6 months, objective indicators showed that the affected side had no significant differencefromthe contralateralone(P> 0.05). In conclusion,theacelular dermal matrixcanobviously decrease the complications of the cartilage grafts andcontributeto agood shaped auricle;and half auricular reconstructionby expanded flap, autogenic rib cartilage graft,andfine anastomosis of autogenic rib cartilage graftcanachieve significant clinical effects.
8.Correlations of prognosis, outcomes with myocardial dysfunction of aneurysmal subarachnoid hemorrhage

Objective To investigate the correlation between aneurysmal subarachnoid hDepartment of Neurosurgery,Xi Dian Group Hospital,Xi'an 710077,Chinaemorrhage (aSAH) and myocardial dysfunction (MD).Methods Totally 96 cases of aSAH patients meeting the criterion were chosen,and the correlations of electrocardiograph (ECG),and ultrasonic cardiogram (UCG)with delayed cerebral ischemia (DCI),death and poor outcome and death were analyzed.Results Of the 93 cases of aSAH patients meeting the criterion,DCI rate was 59.2%,death rate was 19.4%,and poor outcome rate was 21.5%.With multivariable Poisson regression analysis,the results showed that wall motion score index (WMSI) > 1.1 was correlated with death and poor outcome,mid-ventricular wall motion abnormality (WMAs) was correlated with death,and apical WMAs was correlated with DCI.Conclusions Myocardial dysfunction is significantly related to outcome of aSAH.Thus,this study may provide a reference for prognosis assessment and treatment of aSAH.
9.Comprehensive procedures with biomaterial filling for secondary unilateral cleft lip nasal deformity

BACKGROUND:At present, the common filing materials used to correct secondary unilateral cleft lip nasal deformity include conchae cartilage, costal cartilage, Medpor implants, expanded polytetrafluoroethylene (ePTFE), alogenic acelular dermal matrix. OBJECTIVE:To analyze the therapeutic effects of comprehensive procedures with alogenic acelular dermal matrix or ePTFE for secondary unilateral cleft lip nasal deformity. METHODS: Thirty-six patients with secondary unilateral cleft lip nasal deformity were enroled, including 19 males and 17 females, aged 15-32 years. Alogenic acelular dermal matrix (n=22) or ePTFE (n=14) was used to correct nasal base colapse deformities. Anthropometry method was employed to make measurements. Fixed-point measurement was performed based on patient's pictures before and after correction. Long-term effects of these two kinds of filing materials were analyzed and assessed objectively and quantitatively. RESULTS AND CONCLUSION: After the folow-up of 6 months, al the patients were satisfied with their results, and no infection and no exposure occurred. The treatment effect of the alogenic acelular dermal matrix group was excelent in 16 cases and good in 6 cases; there were 10 cases of excelent and 4 cases of good in the ePTFE group. The objective indicators in the two groups were al improved at 6 months after correction (P < 0.05), but there was no difference between the two groups (P > 0.05). These findings indicate that alogenic acelular dermal matrix or ePTFE is useful to correct secondary unilateral cleft lip nasal deformity.
10.Study on effect of 17-β estradiol on cultured vaginal wall fibroblasts from patient with pelvic floor dysfunction after stretch

Objective To study the effect of 17-β estradiol on Collagen Ⅰ,lysyl oxidase like 1 (LOXL1),fibrinectin( FN )after stretch in the pelvic function dysfunction(PFD) patients vaginal wall fibroblasts cultured in vitro,and the effect in PFD pathogenesis.Methods Twelve patients with pelvic function dysfunction who under surgery in the second Hospital,Hebei Medical University from October 2009 to September 2010 were collected vaginal wall organization in this study.Use of tissue and collagenase digestion method for primary culture in the vaginal wall fibroblasts.After stretch with different concentrations of 17-β estradiol cultured fibroblasts.Total mRNA was extracted from fibroblasts and gene expression of Col Ⅰ,LOXL1,FN were measured.Results After stretch,the expression of Col Ⅰ ( 1.1872 ± 0.0733 vs 1.5035 ±0.0733,t =-4.815,P <0.01),LOXLI(0.7724 ±0.1873 vs 1.0855 ±0.0805,t =-5.111,P <0.01 ) were significant higher than before( P <0.01 ),the expression of FN (0.4290 ±0.1168 vs 0.4215±0.0830) was not significantly different ( P >0.05).The expression of all factor Col [ (3.0809 ±0.1862),LOXLI ( 1.5863 ±0.3241 ),FN( 1.1418 ± 1.0030) in high drug concentrations of 17-β estradiol were increased significant compared with controlled groups( P < 0.01 ).Conclusions 17-β estradiol increased synthesis of extracellular matrix components from cultured vaginal wall fibroblasts of patient with pelvic floor dysfunction after stretch.Stretch and estrogen may play an important role in PFD pathogenesis.
