1.Evaluation of molluscicidal effect of screening snails in soil layer

Both the snail number and the detection rate of living snails by means of snail screening in soil layer were higher than those of snail survey on soil surface. It is indicated that snail screening in soil layer can make up the shortage of survey on soil surface and improve the efficiency of snail control.
2.Clinical Observation of Spectral Entropy Monitoring as A Measure of the Conscious State during Neurosurgical Operation

Objective:To evaluate the clinical effectiveness of conscious state-monitoring device based on time-frequency-balanced spectral entropy of electroencephalogram monitoring during neumsurgical operation.Methods:Sixty-six patients scheduled for elective neumsurgical operation were randomly divided into two groups:control group(Ⅰ group,n=30)and observation group(Ⅱ group,n=36).Patients in control group were titrated to maintain adequate conscious state based on the monitoring of stable intraoperative hemodynamics.In the observation group,anesthetic was given to adjust to maintain the spectral entropy value to keep the adequate conscious state during operation.The values of mean arterial blood pressure (MAP),heart rate(HR),state entropy(SE)and response entropy(RE)were recorded before anesthesia induction,skin incision,at 3 min after intubation,at brain tissue dissection and during the last 10 min.And the parameters before recovery from anesthesia,the total amount of narcotic given were also recorded.Results:The values of SE and RE were significantly lower in the observation group than those in control group at 3 rain after intubation,skin incision,at brain tissue dissection and during the last 10 min,whereas MAP and HR were higher in control group compared with those of the observation group(P < 0.05 or P <0.01).The recovery index including recovery time of spontaneous breathing,time to eye opening and extubation was significantly decreased in observation group than that in the control group(P < 0.05 or P < 0.01).The total amount of narcotic consumption was significantly higher in the control group than that of the observation group(P < 0.01).Conclusion:During neumsurgical operation,the monitoring of conscious state by spectral entropy is more effective and better than hemodynamic standard practice.

4.Clinical study of five-pointed star lattice sutures in treatment of patellar fractures

Objective To explore a simple and effective operative procedure for treatment of pa-tellar fractures.Methods The clinical data of 108 patients (including 76 males and 32 females) with patellar fractures were analyzed.The age of the patients ranged from 18 years to 82 years.There were 67 patellar fractures on the right extremities and 41 on the left.Fracture types included transfractures in 43 patients, comminuted fractures in 54, torn fractures in eight and longitudinal fractures in three.Period from injury to operation ranged from 3 hours to 10 days.During operation, the broken patella was exposed for reduction and temporary pliers fixation;then, a five-pointed star woven with two absorbable sutures was placed on the broken patella, two semi-circular sutures around the patellar edge were made with su-tures which were through five points of the five-pointed star.When two sutures were pulled and knotted,the five-pointed star was also stretched to fix the patellar fractures firmly.Models of transverse patellar fractures were made in 20 knee joints of catties, which were divided into two groups randomly.Patellar fractures in Group A were fixed with five-pointed star lattice sutures and those in Group B with AO inten-sion bands.Loading test was performed on quadriceps femoris with materials test system for measuring the width of each fractured patella after the test.Results All patients were followed up for 6-60 months (mean 20 months) , which showed that all patellar fractures were healed.According to Bostman scoring system, the efficacy was excellent in 76 patients and good in 32.The experiment showed no statistical difference in the fracture disjunction distance between two methods (P > 0.05).Conclusion For treatment of patellar fractures, five-pointed star lattice sutures have the advantages of simple operation,reliable fixation, early postoperative exercise, fast recovery, satisfactory outcome and free need of reoper-ation for removing internal fixation.
5.Analysis of antibiotic resistance and eradicatation effects of Helicobacter pylori in digestive diseases in Huzhou district

Objective To investigate the antibiotic resistance trend of commonly used antibiotics of Helicobacter pylori (H.pylori) in Huzhou district,and to summarize the efficacy of eradication in related digestive diseases.Methods In year 2009,2013,2014 and 2015,8 139 gastric mucosa samples of patients undergoing gastroendoscopy examination were collected and H.pylori strains were isolated and cultured.The situation of resistance to levofloxacin,clarithromycin,metronidazole,amoxicillin,tetracycline and furazolidone was analyzed.The infection and antibiotics resistance of H.pylori were analyzed in 11 digestive diseases including functional dyspepsia,chronic gastritis,acute gastritis,duodenitis,gastric ulcer,duodenal ulcer,gastrointestinal dysfunction,gastric cancer,residual gastritis,reflux esophagitis and gastric lymphoma.The eradication schemes and eradication rate of H.pylori was reviewed in six digestive diseases including functional dyspepsia,chronic gastritis,duodenitis,gastric ulcer,duodenal ulcer and reflux esophagitis.Fisher's exact test was performed to compare the differences among the groups.Results A total of 3 263 H.pylori strains were obtained and the infection rate of H.pylori was 40.09% (3 263/8 139).The H.pylori infection rates of 11 digestive diseases were from 0 to 57.89%,and which was high in duodenal ulcer,gastric ulcer,duodenitis,chronic gastritis and functional dyspepsia (57.89%,726/1 254;49.83%,301/604;42.91%,118/275;37.45%,1 518/4 053 and 36.78%,146/397;respectively).The results of single antibiotic resistance analysis in six digestive diseases needed or planed for H.pylori eradication indicated that resistance rate of H.pylori to levofloxacin and clarithromycin reached 23.09% (663/2 871) and 17.87% (513/2 871),respectively.The resistance rate tolevofloxacin dramatically increased from 5.03% (8/159) in 2009 to 28.24% (586/2 075) in 2015;the resistance rate to clarithromycin increased from 12.58% (20/159) in 2009 to 21.78% (452/2 075) in 2015;meanwhile,the resistance rate of H.pylori to metronidazole was nearly 100.00%.However,the resistance rates to amoxicillin,tetracycline and tetracycline were all zero.The results of double antibiotic resistance analysis in six digestive diseases needed or planed for H.pylori eradication indicated that the rate of both levofloxacin and clarithromycin resistance was 7.73 % (222/2 871).The double antibiotic resistance rate of levofloxacin and clarithromycin in different diseases fluctuated between 4.82 % and 10.96 %.Totally 1 479patients of six digestive diseases were treated with quadruple therapy,and 1 363 patients were followed up after eradication therapy,with the eradication rate of 85.99% (1 172/1 363).Conclusions In Huzhou district,for six common digestive diseases needed or planed for H.pylori eradication,any combination of two drugs in all three drugs including amoxicillin,tetracycline and furazolidone is the first choice for treatment.Only when patients are allergic to penicillin or furazolidone and tetracycline can not be obtained,will levofloxacin and clarithromycin be chosen.A high eradication rate can be achieved by choosing eradication schemes according to the results of H.pylori drug sensitivity test in local region.
6.Evolution and development of potent monobactam sulfonate candidate IMBZ18g as a dual inhibitor against MDR Gram-negative bacteria producing ESBLs.

A series of new monobactam sulfonates is continuously synthesized and evaluated for their antimicrobial efficacies against Gram-negative bacteria. Compound 33a (IMBZ18G) is highly effective in vitro and in vivo against clinically intractable multi-drug-resistant (MDR) Gram-negative strains, with a highly druglike nature. The checkerboard assay reveals its significant synergistic effect with β-lactamase inhibitor avibactam, and the MIC values against MDR enterobacteria were reduced up to 4-512 folds. X-ray co-crystal and chemoproteomic assays indicate that the anti-MDR bacteria effect of 33a results from the dual inhibition of the common PBP3 and some class A and C β-lactamases. Accordingly, preclinical studies of 33a alone and 33a‒avibactam combination as potential innovative candidates are actively going on, in the treatment of β-lactamase-producing MDR Gram-negative bacterial infections.
