Chinese Journal of Postgraduates of Medicine 2011;34(28):10-13
Objective To observe the effect of rosuvastatin on the expression of vascular cell adhesion molecule-1 (VCAM-1) of renal tissue in patients with the early-stage diabetic nephropathy. Methods Sixty patients with early-stage diabetic nephropathy were divided into two groups by random digits table:rosuvastatin group and regular group. The patients were given 10 mg/d rosuvastatin besides the conventional treatment in rosuvastatin group. The levels of blood glucose, blood fat, renal function, high sensitivity C-reactive protein(hs-CRP),urine albumin excretion rate(UAER) and the expression of VCAM-1 were observed. Results Compared with those before treatment, the levels of total cholesterol (TC),low density lipoprotein cholesterol (LDL-C),trtglyeride (TG) showed no decrease after treatment [(5.83 ± 1.02),(3.48 ±0.60),(7.12 ±0.99) mmol/L] in regular group (P >0.05),but significantly decreased in rosuvastatin group [ (4.51 ± 0.95 ),( 3.18 ± 0.47),(7.02 ± 0.87 ) mmol/L ] (P < 0.01 ).The levels of UAER and hs-CRP were significantly decreased in regular group and rosuvastatin group after treatment [ (78.3 ±31.7),(53.8 ± 24.9) μ g/min,and (2.15 ± 0.48),( 1.59 ± 0.55) mg/L] (P< 0.01 ),then rosuvastatin groupwas more conspicuous than regular group (P < 0.05 ).The expression of VCAM-1 was less in rosuvastatin group than that in regular group. Conclusion Rosuvastatin can down-regulate the expression of VCAM-1 of kidney tissues in patients with the early-stage diabetic nephropathy and these indicate that rosuvastatin can reduce urine protein by inhibiting the inflammatory process.

Journal of Environment and Health 1992;0(05):-
Haloacetic acids are the main disinfection by-products in chlorinated drinking water, which are known or suspected to carcinogenic. Many countries have promulgated haloacetic acids as the routine monitoring item for drinking water safety. In order to better control the formation of haloacetic acids in chlorinated drinking water, many techniques and analysis methods were exploited. This review summarized the development of techniques and analysis methods and the comparative analysis was conducted in technique cost, method detection limit, sensitivity, resistance to disturb and sample pretreatment. The electrospray mass spectrometry is the best choice, but the instrument and operation cost is much higher,so the standard method based on the gas chromatography will still be the popular method for drinking water quality analysis.

[Objective]To test and prove the mushroom shaped surface prosthesis of femoral head not only can be a weight-bearing device, but also could reduce stress-shelter and achieve long-term stability.[Method]A simplified three-dimensional model of femoral head was made. And the biomechanics of femoral head were compared by finite element analysis in two states:with and without the replacement.[Result]It showed that the maximum of strees in famoral head was less than 40Mpa,consequently the replacement had sufficient strength to meet its basic function.Furthermore, the prosthesis bear the force both at up part and down. Therefore it reduced stress-shelter.[Conclusion]This prosthesis can achieve immediate and long-term stability. And it provides new ideas for designing the femoral head resurfacing prosthesis.

Objective To study the expression of interleukin-17 (IL-17) in diabetic rat aorta and the effect of intervention with resveratrol, meanwhile,to explore the potential mechanisms of IL-17 induced diabetic vascular diseases and the protective role played by resveratrol in the epigenetic field. Methods The experiment was carried out in 4 groups:normal control group(NC),normal interventional group(NB),diabetic group(DM),and diabetic interventional group(DB),NB and DB groups were intervened with resveratrol.Immunohistochemistry was used to observe the histological localization of IL-17 and to measure the thickness of rat abdominal aorta.Western blotting,real-time PCR,and methylation-specific PCR were used respectively to compare the expression of IL-17 protein and mRNA,as well as DNA methylation in 4 groups. Results IL-17 mainly expressed in arterial intima of diabetic rats, the abdominal aorta in DM group was obviously thicker than that in NC and DB groups(P<0.05).IL-17 protein and mRNA expressions in DM group were significantly higher than NC group(P<0.05),and were reduced in NB and DB groups compared with NC and DM groups respectively. While DNA methylation levels of IL-17 in DM group were significantly lower than NC group(P<0.01),however,the levels in NB and DB groups were elevated accordingly as compared with corresponding groups. Conclusions The increased levels of IL-17 in aorta of diabetic rats suggests that IL-17 is involved in the process of inflammatory responses to diabetic macrovascular diseases, while resveratrol could inhibit the expression,it may play a role in protecting aorta, and the regulation of IL-17 gene promoter DNA methylation levels may be the potential mechanism underlying these two phenomena.

Objective To address the medical problems, in treatment of avascular necrosis of the femoral head, on loosening and subsidence of the prosthesis of femoral head. A set of Mushroom Shaped Surface Prosthesis of Femoral Head was designed. Methods Based on biomechanical of the femoral head and the principle of transferring the force through femoral head while reducing stress-sheltered, nickel-titanium alloy and other new medical alloy materials were used to develop a prosthesis, which not only can be a weight-bearing device, but also promotes bone-growth. Results The prosthesis is novel, compact and little-volume, and the manipulation of surgery is convenient and secure. Conclusion The design of this prosthesis can provide new ways or ideas for designing the femoral head replacement prosthesis to solve problems on loosening and subsidence.

Objective To explore the clinical application value of proton magnetic resonance spectroscopy (1H-MRS) in the metabolism of the hippocampus in non-demented Parkinson's disease(PD) patients. Methods 1H-MRS localized to both hippocampus, was carried out in 36 non-demented PD (14 patients with Hoehn-Yahr stage Ⅰ,8 patients with Hoehn-Yahr stage Ⅱ,8 patients with Hoehn-Yahr stage Ⅲ ,and 6 patients with Hoehn-Yahr stage Ⅳ) and 20 age-matched healthy control subjects. The metabolite ratios of N-acetylasparatate (NAA)/creatine (Cr),and choline-containing compounds (Cho)/Cr in PD patients were compared with those in the age-matched healthy control subjects. Results NAA/Cr ratios were reduced significantly and Cho/Cr ratios were increased sig-nificantly in both hippocampus of patients with Hoehn-Yahr stage Ⅳ compared to those in the age-matched healthy control subjects (P<0.05). However,no significant differences in NAA/Cr and Cho/Cr ratios were found between patients with Hoehn-Yahr stage Ⅰ-Ⅲ and the age-matched healthy control subjects (P>0.05). Conclusion 1H-MRS can provide the metabolic information ,and reflect the pathological changes of nerve cell in the hippocampus of non-demented PD patients with Hoehn-Yahr stage Ⅳ. It is helpful to predict the risk of dementia in PD.

Objective To make a preliminary investigation and summary of the technique and efficacy of the novel intracranial stent, Enterprise, together with hydro-detachable coils for the treatment of very small intracranial wide-necked aneurysms (diameter<3 nun and body-to-neck ratio<1.5). Methods Six cases with very small intracranial wide-necked aneurysms were treated with Enterprise stents and hydro-detachable coils. In 5 cases the Enterprise stent was implanted to cover the neck of the aneurysm, which was followed by the introduction of a microcatheter into the aneurysmal sac through the stent mesh to stuff hydro-detachable coils in order to fill the aneurysmal sac. In the remaining case, the microcatheter was placed into the aneurysmal sac before the Enterprise stent was inserted to embolize the aneurysm. Postoperative follow-up was conducted for 3-6 months. Results The operation was successfully completed in all 6 patients, with the implanted stents being in right place. The parent arteries remained patency in all patients. No complications occurred. Complete occlusion of aneurysmal cavity was obtained in four cases, and the occlusion degree of the aneurysmal cavity above 95% was seen in 2 cases. After the procedure, all the patients recovered well. Neither rebleeding nor symptoms related to thrombosis occurred during a clinic follow-up of 3-6 months. Conclusion Endovasculur embolization with Enterprise stent together with hydro-detachable coils is a safe and effective method for the treatment of very small intracranial wide-necked aneurysms. However, its long-term effect needs to be further observed.
