1.Effects of acupuncture combined with Buyang Huanwu Decoction on the cerebral blood flow hypo perfusion model rats with carotid artery stenosis based on high-throughput sequencing

Objective:To explore the effects of acupuncture combined with Buyang Huanwu Decoction on intestinal flora in cerebral blood flow hypo perfusion model rats with carotid artery stenosis.Methods:Totally 40 rats were randomly divided into sham-operation group, model group, TCM treatment group and acupuncture and drug combination treatment group, with 10 rats in each group. Except the sham-operation group, the other groups were prepared cerebral ischemia model by needle control and thread embolism method. TCM treatment group received Buyang Huanwu Decoction 100 mg/kg for gavage, once a day, and the intervention lasted for 2 weeks. In the acupuncture and drug combination group, based on the TCM treatment group, Baihui and its left and right sides of 2 mm were selected for acupuncture, once a day, and continuous intervention was performed for 2 weeks. Neurological function evaluation and behavioral function score were performed 7 and 14 days after administration, respectively. 16S rRNA sequencing was used to comprehensively characterize the structure and composition of fecal microflora of rats in each group. Linear discriminant analysis Effect Size (LEfSe) was used to analyze the difference of intestinal bacteria among groups.Result:On the 7th and 14th day after administration, compared with the model group, the neurological function score in the TCM treatment group and the acupuncture and drug combination group decreased ( P<0.05), and the behavioral function score increased ( P<0.05). Compared with model group, the Shannon index of TCM treatment group and acupuncture and drug combination group increased ( P<0.05). The abundance of Firmicutes increased ( P<0.05), and the abundance of Bacteroidetes and Proteobacteria decreased ( P<0.05); the abundance of Clostridia increased ( P<0.05), and the abundance of Gammaproteobacteria decreased ( P<0.05). The abundance of Escherichia-Shigella and Bacteroides decreased ( P<0.05); the abundance of lactobacillus significantly increased ( P<0.05). Conclusion:Acupuncture combined with Buyang Huanwu Decoction can improve the symptoms of cerebral hypoperfusion model rats with carotid artery stenosis, and the mechanism may be to increase the abundance of probiotics.
2.Analysis on the characteristics and patterns of work-related musculoskeletal disorders among sonographers in Guangdong Province

Objective To analyze the characteristics of work-related musculoskeletal disorders (WMSDs) among sonographers in Guangdong Province, and to explore the disease pattern of the cases. Methods A total of 512 sonographers from 31 hospitals in Guangdong Province were selected as the research subjects using stratified cluster sampling method. The prevalence of WMSDs in the past year was investigated using the Musculoskeletal Disorders Questionnaire, and the characteristics of WMSDs were analyzed. Latent class analysis was used to identify the disease pattern of WMSDs. Results The overall prevalence of WMSDs was 94.3%. The top five affected body parts were right shoulder, neck, right hand/wrist, lower back and right forearm/elbow, with the prevalence of 80.3%, 75.4%, 61.1%, 55.5% and 45.3%, respectively. The prevalence of WMSDs was higher on the right side for the shoulder, hand/wrist, forearm/elbow, hip/leg and knee compared with the left side (80.3% vs 31.3%, 61.1% vs 13.9%, 45.3% vs 10.0%, 17.4% vs 8.6%, 13.1% vs 8.4%, all P<0.05). The prevalence of WMSDs increased with work years, as well the prevalence of WMSDs in the top five affected body parts among the sonographers (all P<0.05). However, there were no statistical differences in prevalence of WMSDs between general hospitals and maternal and child health hospitals, tertiary hospitals and non-tertiary hospitals, Pearl River Delta hospitals and non-Pearl River Delta hospitals; there was also no statistical difference between different genders and age groups of the sonographers (all P>0.05). The best-fit latent disease pattern for sonographers WMSDs comprised three categories: symptom of neck-right shoulder, symptom of neck-lower back-right shoulder-right elbow-right hand/wrist, and symptom of multi-parts above the knees, with the latent probabilities of 0.438, 0.427 and 0.135, respectively. Conclusion The prevalence of WMSDs in sonographers is extremely high, with a dose-effect relationship with work years. The most common affected parts are neck, lower back and right shoulder, right hand/wrist, and right forearm/elbow. The prevalence of WMSDs in the right side of limb was higher than that in the left. WMSDs primarily occur in multiple parts simultaneously. The most common symptoms occur in the neck-right shoulder and neck-lower back-right shoulder-right elbow-right hand/wrist.
3.Preparation and In Vitro/In Vivo Evaluation of Celecoxib Self-microemulsifying Drug Delivery Systems

Objective To investigate celecoxib self-microemulsifying drug delivery system (CXB-SMEDDS) that was developed to increase the dissolution rate and oral bioavailability of celecoxib.Methods The formulation of CXB-SMEDDS was optimized by pseudo-ternary phase diagrams analysis.The appearance, morphology, particle size distribution and in vitro drug release behavior of CXB-SMEDDS were investigated after diluted by water.The bioavailability of CXB-SMEDDS was determined by oral administration to rats compared with CXB suspension.Results An optimized formulation was selected: Medium chain triglycerides as oil phase, Tween 20 as surfactant, Transcutol HP as cosurfactant.The ratio of oil phase, surfactant and cosurfactant was 2∶9∶9.Upon mixing with water, CXB-SMEDDS formed a clear and transparent microemulsion solution with homogeneous small spherical under transmission electron microscopy.For particle size of CXB-SMEDDS was found to be (57.6±14.2) nm.The in vitro dissolution test indicated a significant improvement in release characteristics of CXB.The AUC of CXB-SMEDDS and CXB suspension were (5.54±0.94) and (3.32±0.59) mg·L-1·h, respectively.The relative bioavailability was 166.9%.Conclusion The SMEDDS can significantly increase celecoxib dissolution in vitro and bioavailability in vivo.
4.Postoperative brain tissue recovery in patients with bilateral chronic subdural hematomas

Objective To observe the brain tissue recovery in patients with bilateral chronic subdural hematomas after drilling and drainage treatments. Methods The clinical data of 143 patients with bilateral chronic subdural hematoma performed drilling and drainage in our hospital from January 2011 to December 2016 were collected, including gender, age, the longest and widest diameters of the skull and the longest diameter of the initial hematoma, the widest diameter of the initial hematoma, the widest diameter of the postoperative hematoma, and other clinical information. The postoperative absorption of hematoma and recovery of brain tissues were analyzed. Results In these 143 BCSDH patients, 119 were male and 24 were male; the mean age of these patients was (67.7 ±1.1) years (21-92 years old), and the mean age of male patients was (67.2 ±1.2) years and the mean age of female patients was (70.2±2.5) years. The longest diameters of the skull was 134.8 mm-177.8 mm (averaged 155.5 mm± 0.7 mm) and the widest diameters of the skull was 113.5 mm-147.0 mm (averaged 131.7 mm±0.5 mm). The longest diameters of the left and right initial hematomas were (113.4±2.5) mm and (117.6±2.1) mm,and the widest diameters of the left and right initial hematomas were (15.8±0.5) mm and (17.2±0.8) mm. There was no significant difference in the recovery rate of brain tissues between patients with different gender, age or lesion sides (P<0.05). The recovery rate of brain tissues was positively correlated with the longest and widest diameters of initial hematomas (r=0.143, P=0.018; r=0.181, P=0.002). A total of 58 patients (40.6%) presented a single-side distance<10 mm from the brain tissue to the inner edge of the skull within one week after surgery, and a total of 48 patients (33.6%) showed both sides brain recovery to<10 mm. Conclusions Drilling and drainage are the first line treatment for patients with bilateral chronic subdural hematoma and contribute to the re-location of brain tissues. The rate of brain tissue recovery is positively correlated with the size of primary hematoma.
5.Correlation analysis on clinical learning environment and stress among nursing students during initial period of clinical practice

Objective To explore the status of clinical learning environment and stress among nursing students during initial period of clinical practice and analyze the correlation between them.Methods With purpose sampling method,219 nursing students,who did rotating practice in eight departments of Beijing Electric Power Hospital of State Grid from July 2015 to February 2016 were selected and surveyed by clinical learning environment assessment scale and practice stress scale.Results The nursing students' assessment on clinical learning environment was (172.99±19.36) points,and the average score of entries was (4.12±0.51). Scores of various dimensions with a descending order were:interpersonal relationship (4.31±0.50),working environment and teamwork spirit (4.25±0.50),task orientation (4.23±0.47),innovation (4.11±0.52),students' involvement (4.06±0.51),and individuality (3.51±0.86). Scores of the nursing students' perceived stress during initial period of practice were (31.89±13.94) points,with average score of (1.10±0.48). Scores of stress sources with a descending order were:lack of professional knowledge and skills (1.44±0.61) points,taking care of patients (1.33±0.55),assignments and workload (1.19±0.60),clinical environment (0.93±0.52),clinical teachers (0.88±0.53),and companions and daily life (0.75±0.55). Perceived stress had a negative correlation with learning environment (r=-0.160,P<0.05).Conclusions The more satisfied to the clinical learning environment,the less stress the students perceive;it is recommended that students' learning environment should be improved,so as to relieve their perceived stress during clinical practice and improve quality of nursing education.
6.Laser Scanning Confocal Microscopical Study of the Effect of Acupuncture and Medicine on NSC Differentiation in Rats with Stroke of qi Deficiency and Blood Stasis Type

Objective To investigate the effect of cluster needling at scalp points plus gavage of modified Buyang Huanwu decoction on NSC differentiation in rats with ischemic stroke of qi deficiency and blood stasis type.Methods A model of qi deficiency and blood stasis syndrome was made using healthy rats. Dil dye was given for pre-labelling after the success of model making. The rats were randomized into groups A, B, C, D, E and F. Group F consisted of 4 rats and each of the other groups, of 12 rats. A rat model of ischemic stroke of qi deficiency and blood stasis type was made by MCAO at 48 hrs after pre-labelling. After the success of model making, group A was the model group, group B was treated by cluster needling at scalp points, group C was treated by an oral gavage of Buyang Huanwu decoction, group D was treated by an oral gavage of modified Buyang Huanwu decoction, group E was treated by cluster needling at scalp points plus an oral gavage of modified Buyang Huanwu decoction and group F was the sham operation group. The rats were sacrificed at various time points respectively and the materials were taken. By nerve function assessment, double immunofluorescence staining and laser scanning confocal microscopy, a comparative study was conducted to investigate the effects of different treatments on NSC differentiation.Results There was a statistically significant difference in the nerve function score between group B, C, D, E or F and group A after one week of treatment (P<0.01), between group B or E and group A after two weeks of treatment (P<0.05,P<0.01) and between group E and group C or D after one and two weeks of treatment (P<0.01,P<0.05). After one and two weeks of treatment, there were statistically significant differences in Brdu﹢/NeuN﹢ cell count, Brdu﹢/GFAP﹢ cell count, Dil﹢/Brdu﹢NeuN﹢ cell count and Dil﹢/Brdu﹢/GFAP﹢ cell count between group B, C, D or E and group A (P<0.01), between group D or E and group B or C (P<0.01) and between groups E and D (P<0.01).Conclusions Various treatments have a promoting effect on nerve stem cell differentiation in the rats. Of various treatments, cluster needling at scalp points plus gavage of modified Buyang Huanwu decoction has the best therapeutic effect.
7."Effect of ""in-and out-hospital"" continued nursing care mode on emotional state and health behavior of breast cancer patients undergoing chemotherapy after radical mastectomy"

Objective To explore the effect of in-and out-hospital continued nursing care on emotional state and health behavior of breast cancer patients undergoing chemotherapy after radical mastectomy.Methods Forty breast cancer patients undergoing radical chemotherapy in the oncology department of our hospital during March 2012 to February 2014 were put into the study group and another forty patients from the same department during March 2014 to February 2016 into the control group.The control group received traditional nursing care,while the study group in-and out-hospital continued nursing care mode.The health promotion lifestyle assessment scale (HPLP-Ⅱ),self rating anxiety scale (SAS) and self rating depression scale (SDS) were used for the evaluation before intervention and 2 months after the intervention.Results After intervention,there were no significant differences in the total scores on health behaviors and the dimensions,anxiety and depression (P>0.05).Two moths after intervention,the scores on health behaviors and the dimensions in the study were all significantly higher than those in the control group and the scores on anxiety and depression were significantly lower (P<0.05).Conclusion The in-and out-hospital continued nursing mode is beneficial for the breast cancer patients undergoing chemotherapy after radical mastectomy to adjust their emotions and improve their health behaviors.
8.The antithrombotic therapy for patients with atrial fibrillation undergoing percutaneous coronary intervention

Objective To compare the efficacy and safety of singular double antithrombotie therapy (DT) using warfarin plus clopidogrel and the combined antithrombotie therapy of 3-month triple antithrombotie therapy (TT) using warfarin, aspirinand clopidogrel and 9-month double antithrombotie therapy (DT) for the patients with atrial fibrillation undergoing PCI. Methods Ninety patients with atrial fibrillation undergoing PCI were randomly divided into two groups evenly: one group was treated with dual antithrombotic therapy group (DT) and the other group with the combined therapy, e. g. 3-month triple antithrombotie therapy (TT) and 9-month double antithrombotie therapy (DT + TT for short). All patients were followed-up by 12 months. The two groups were compared in terms of incidences of death , myocardial infarction , stroke , target-vessel revascularisation , stent thrombosis and bleeding adverse events. Results The incidences of myocardial infarction, stroke, target-vessel revascularisation , stent thrombosis and bleeding adverse events in the TT + DT group were all significantly lower than the DT group (P < 0.05). The follow-up on the safety indicated that the rate of bleeding in the TT +DT group was insignificantly higher than the DT group (P > 0.05). Conclusion There is no significant difference in safety between the two groups. However, the therapy of TT + DT is more effective.
9.The ultrasound imageing analysis of fetal hemivertebrae

Objective To summarize the characteristics of prenatal and postnatal ultrasonic image in hemivertebra fetal malformation. Methods The present study enrolled 12 ultrasound diagnosed hemivertebra fetal malformation and confirmed by either magnetic resonance imaging (MRI) or X-ray in Guangdong Provincial Maternal and Children's Hospital from January 2012 to May 2013. All the ultrasonic image were collected to summarize their characteristics. Results In the twelve cases of prenatal ultrasonic image of hemivertebrae: (1) Vertebral anomaly patterns: the irregular-shape of bones are smaller than normal vertebral bony hyperechoic which inserted into the normal intervertebral in wedge-shape, scoliosis or angular deformity. (2) Diseased parts:in the 12 fetuses, 5 had single vertebral deformity while the other 7 had multiple vertebral deformities. For the location of the lesion, there were 6 dysmorphia appeared in thoracic, 3 appearedin lumbar, 1 appeared in sacrococcygeal, 1 appeared in both cervical and thoracic, and 1 appeared in both thoracic and lumbar. (3) Associated with other malformations: 4 term delivery fetuses confirmed diagnosis of hemivertebra malformation by MRI (the diagnosis of hemivertebra was confirmed by both the fetal MRI and postnatal X-ray in 2 of them within 3 days);8 fetuses had additional anomalies, including heart malformations. Three of them were induced abortion, the X-ray images of the fetal sample showed hemivertebradysmorphia. Five women chosed induced laborin local hospital, and the X-ray images of the fetal were diagnosed as hemivertebradysmorphia. One of them with rib deletion in thoracic was missed diagnosed. Conclusions Three-dimensional ultrasonicimages can clearly show the degree of scoliosis, locate vertebra. It provides important imaging evidence for the treatment of fetal dysmorphiaafter labor.
10.Comparison of the effects of different analgesic methods after UPPP.

OBJECTIVE:
To explore the effects of different analgesia methods after UPPP.
METHOD:
Ninety cases of patients uvulopalatopharyngoplasty were divided into 3 groups randomly, and 30 cases in each group. The group A was the blank control group without any analgesia measures. The cases in group B were treated with intramuscular injection of parecoxib sodium 40 mg after surgery immediately, and continued injecting 40 mg after 12 hours, 24 hours and 36 hours respectively. 100 mg tramadol replaced 40 mg parecoxib sodium in group C. The VAS scoring was performed after surgery 12, 24, 36, 48, 72, 96 hours in 3 groups, and we observed adverse reaction such as lethargy, nausea, vomiting, dizziness, skin rash and so on.
RESULT:
The group B and C reduced the pain significantly compared with blank control group. The pain scores in group B were significantly decreased than that in group C (P<. 05).
CONCLUSION
The analgesic effect of parecoxib sodium after UPPP is significant and better than tramadol. It is worthy to use widely in clinical due to its better effect and less side effect.
