1.Comparison of the value of MRS and MRI in diagnosis of primary epilepsy with hippocampal lesions

Objective To discuss Magnetic Resonance Spectroscopy(MRS) on the localization diagnosis of primary epilepsy patients with hippocampal lesions before operation.Methods 50 patients with idiopathic epilepsy (epilepsy group) and 32 healthy volunteers (healthy control group) were underwent Magnetic Resonance Imaging ( MRI) and MRS examination.Results ( 1 ) MRI showed that results of examination in the healthy control group were all normal.Epilepsy group of 38 cases were found abnormal in hippocampal area,and all patients were unilateral. 27 cases of them reduced with hippocampal volume, while signal abnormality was in 7 cases, 4 cases occurred simultaneously reduced with hippocampal volume,and therer was no obvious abnormal signals in 12 cases of patients with epilepsy of hippocampus volume and signal.( 2 ) MRS examination results:the healthy control group MRS were normal.50 epileptic patients in group MRS showed that hippocampal abnormalities.The epilepsy group NAA/( Cho+Cr) ,NAA/Cho,NAA/Cr were significantly lower than those of the healthy control group;epilepsy group disease side hippocampus NAA/(Cho+Cr),NAA/Cho,NAA/Cr values have different degrees of reduction,compared with the contralateral side,the value of t were 12.716,9.155,17.476,all P<0.01.(3)50 cases of epilepsy patients in the epilepsy group disease side compared with the contralateral side,the side of hippocampus NAA/disease ( Cho+Cr) , NAA/Cho,NAA/Cr values were decreased,the test values of t were 7.295,6.303,2.761,all P<0.01.(4)50 cases of operation patients underwent postoperative pathological examination of resected tissue,resection of neurons,tissue showed different degrees of reduction,microglia proliferation and neuronophagia phenomenon.Comparison of the posi-tive rate of MRS and MRI examination,χ2 =13.636,P<0.01.Conclusion Magnetic resonance spectroscopy analy-sis can play the important role in the evaluation of epilepsy surgery preoperative epilepsy,MRS examination for preop-erative diagnosis positive rate is high,the localization diagnosis with high reliability.
2.EARLY DIAGNOSIS AND TREATMENT OF ACUTE TRANSFORMATION OF CHRONIC GRANULOCYTIC LEUKEMIA

61 cases of chronic granulocytic leukemia were reviewed in this paper. Based on the results, the following features are suggested as the diagnostic criteria for early acute transformation: (1) fever of unknown cause, (2) bone and joint pain, (3) a fall in hemoglobin, marked increase or decrease of the number of WBC's and platelets, (4) myeloblasts and promyelocytes exceed 5% in peripheral blood and 10% in bone marrow, and (5) poor or no response to previously effective drug therapy, It is also suggested that a rational therapeutic management in the chronic phase may help to prevent or delay acute transformation.
3.Treatment of 50 Acute Leukemia Patients with Autologous Hematopoietic Stem Cell Transplantation

We treated 50 acute leukemia patients with autologous hematopoietic stem cell transplantation. Among them, 10 patients were transplanted with unpurged autologous bone marrow (ABMT), 22 patients with purged autologous bone marrow(PABMT), 18 Patients with autologous blood hematopoietic stem cell (ABHSCT). The three years disease-free living incidences in the three groups were 75%,69.9% and 72.7%, respectively. The relapse rates were 36.6%, 27.2%, and 31.4%, respectively.A high three-year disease-free incidence and low relapse rate were found in the patients with less than two months between diagnosis and achieving complete remission (CR), transplantation after 6 months of achieving CR, conditioning treatment with chemotherapy and total body irradiation (TBI) and acute myeloid leukemia (AML). The results indicated that there are some correlations between the amount of infused mononuclear cells and the recovery of bone marrow hematopoiesis. No significant difference of therapeutic effiency was demonstrated among the three groups.
4.Effects of Trichinella spiralis infection on a murine model of OXZ-induced colitis

Objective To study the effects of Trichinella spiralis (T.spiralis) infection on mice with oxazolone (OXZ)-induced colitis and the possible immunologic mechanism.Methods Female BALB/c mice at age 6-8 weeks were randomly divided into four groups (A to D).Each mouse in groups B and D was infected with T.spiralis strains.Twenty-one days after T.spiralis infection,the mice in groups A and B were treated with 50% ethanol solution,while those in groups C and D were treated with OXZ to induce the murine model of colitis.Several parameters including survival rate,disease activity index (DAI),macroscopic damage and histological score,myeloperoxidase (MPO) activity and the expression of cytokines (IFN-γ,IL-4 and IL-10) in colonic and splenic tissues at mRNA and protein levels were measured 3 and 7 days after modeling.Results No significant differences in the survival rate,DAI score,macroscopic damage score,histological score,MPO activity were observed between mice from groups C and D (P>0.05).Pre-exposing the mice to T.spiralis strains neither alleviated the mucosal damages nor aggravated the condition of colitis.Compared with group C,the expression of IFN-γ on the third day and the expression of IFN-γ,IL-4 and IL-10 on the seventh day at mRNA and protein levels in colon and spleen tissues were significantly increased in mice treated with T.spiralis and OXZ (P<0.05).The expression of IL-10 at transcriptional level in spleen tissues on the third day was higher than that of group C (P<0.05).The expression of IL-4 and IL-10 at protein level in colon tissues on the third day were significantly up-regulated as compared with those of group C (P<0.05).Conclusion The severity of OXZ-induced colitis in the murine model was neither alleviated nor aggravated by pre-exposing the mice to T.spiralis strains.High doses of IL-10 not only weakened the regulatory effects on Th2 responses,but also induced the production of proinflammatory cytokines,resulting in a new drift of Th1/Th2 without aggravating Th2 responses.Further investigation on the mechanism of T.spiralis-induced over-expression of IL-10 should be conducted,which might increase the practicability of using T.spiralis strains against OXZ-induced colitis.
5.Prevention and treatment of fungous infection complications of hematopoietic stem cell transplantation

Objective To retrospectively evaluate the incidence and treatment of fungous infection compli-cations after hematopoietic stem cell transplantation. Methods The incidence, pathogenic microorganism, prophy-laxis,treatments of infectious complications in 150 patients, who accepted hematopoietic stem cell transplantation from September 1990 to Martch 2000 in our hospital were analyzed. Results The incidence of infectious complica-tions was 89.3% (134/150) in all 150 cases. Three patients (2%) died of the fungal infection. The incidence of the fungal infections was 32.5% (26/80) in patients who accepted treatment with impenem or/and ceftazidine, and 15.7% (11/70) in other patients without the above treatment (P<0.05). 12 fungal infection cases were treated with small-dosage of amphotericin B(10 mg/d) ,with the healing rate was 100%. Conclusion The strong antibac-terial prophylaxis can't reduce the incidence of infection ,but may increase the risk of fungal infection;small-dosage of amphotericin B is a new effective way to treat fungal infection.
6.Clinical Research on Infectious Complications after Hematopoietic Stem Cell Transplantation

OBJECTIVE To evaluate retrospectively the incidence and treatment of infectious complications within the first 60 days after hematopoietic stem cell transplantation,and to find more efficient anti-infective regimens. METHODS To study the incidence,pathogenic microorganism,prophylaxis,treatments of infectious complications in 150 patients accepted hematopoietic stem cell transplantation from April 1984 to March 1998 in our hospital.The results were analyzed statistically.RESULTS Incidence of infectious complications was 89.3% in all 150 cases.Three patients(2%) died of the fungal infection.The incidence of the infections was 32.5% in patients accepted treatment with imipenem or/and ceftazidine,and 15.7% in other patients without the treatment with imipenem or/and ceftazidine(P<0.02).CONCLUSIONS The strong antibacterial prophylaxis can′t reduce the incidence of infection,and may increase the chance of fungal infection.
7.An in-gel digestion method of chymotrypsin to improve sequence coverage of membrane protein by mass spectrometry.

In recent years, mass spectrometry has been widely used to study membrane protein structure and function. However, the application of mass spectrometry to study integral membrane protein is limited because there are many hydrophobic amino acids in the trans-membrane domain of integral membrane protein to cause low sequence coverage detected by LC-MS/MS. Therefore, we used vitamin K epoxide reductase (VKORC1), a human integral membrane protein, as a model to optimize the digestion conditions of chymotrypsin, and developed an in-gel digestion method of chymotrypsin to improve sequence coverage of membrane protein by mass spectrometry. By exploring the effects of calcium concentration, pH value and buffer system on the percentage of sequence coverage, number of total detected and types of unique peptide, and the size of unique peptide, sequence coverage and peptide diversity could be considered under condition of Tris-HCl buffer with 5-10 mmol/L calcium ion concentration and pH value 8.0-8.5. This method could make the sequence coverage of membrane protein to reach more than 80%. It could be widely used in the study of membrane protein structure and function, identification of interaction site between membrane proteins, and identification of binding site between membrane protein and small molecular drug.
