1.The ethical requirement of the placebo-controlled clinical study

Placebo-controlled clinical study is the ethical issue being debated all over the time.The ethics committee plays an important role in the assurance of the scientificalness of clinical research without violation of ethics.This article introduces the main points considered in the placebo-controlled clinical study in Medical Ethics Committee of Guang'anmen Hospital of China Academy of Traditional Chinese Medicine.These points are as follows,the benefit of the subject greater than the risk,considerations on the benefit / risk of the subjects,the disposal of compensation measures and adverse events in research program and fully informed consent as well as the model of final resolution.
2.Analysis on the Relevant Factors of Distribution of TCM Syndrome for Ankylosing Spondylitis

Objective To analyse the relevant factors of distribution of TCM syndrome for ankylosing spondylitis (AS). Methods 194 AS patients visiting the department of Rheumat Immunity, Guanganmen Hospital, were investigated in the way of horizontal study. And the relationship between TCM syndrome and age, sextuality, staging, grading of sacroiliac joint, laboratory index was studied. Results It was no significant difference of AS TCM syndrome with different age, sextuality, early、medium、advanced stage, and grading of sacroiliac joint. But it was different between the active stage and inactive stage. For the laboratory index, the ESR and CRP of each syndrome was different, while the comparison with the data of blood routine examination was no evident difference. Conclusion The distribution of AS TCM syndrome on active stage is mainly about the retention of damp-heat, and the inactive stage is mainly on the deficiency of kidney-yang syndrome. Compared with other sydromes, ERS and CRP in the syndrome of retention of damp-heat are significantly high. It is no significant changes of syndrome on aspects of different age, early、medium、advanced stages, and grading of sacroiliac joint.
3.System Improvement by Replacing Light-Conductor Camera with CCD Camera

Objective To improve the image intensifying X-TV system by replacing the light-conduct camera with CCD camera for future usage.Methods The low luminous and high sensitive CCD camera was used in the X-ray auto exposure dose control.Results Through the practical improvement,the CCD X-TV system met the clinical requirements.Conclusion It is an effective way to replace the old light-conduct camera with CCD camera,and it also helpful for the alternative solutions for X-TV.
4.Structure property of a new filtering material for making medical masks

Objective: To study the structure property of a superfine melt blow(MF) non woven material for making medical masks and its filtering functions. Methods: Five kinds of masks were tested and they were: 20 layer gauze mask, cotton spunlace mask, MF outpatient mask, MF medical protective mask Ⅰ and MF medical protective mask Ⅱ. Scanning electron microscope was used to observe the structure of the filtering materials. The aperture of the materials was measured and the filtering efficacy was tested. Results: The diameter of MF fibers was less than 3 ?m and the material made from it was randomly distributed with multi curve channels and complex structure. The filtering efficacy of the new material was 96.2% 99.5% for ambient aerosol particles with a diameter of 0.3 ?m, while the efficacy of 20 layer gauze mask was 50.9%. As for respiratory resistance the former was 26 43 Pa and the later was 48 Pa. Besides, MF mask had good humid proof function and the character of electronic charge, and the later could help capture the particles. Conclusion: The new masks made from MF material has good filtering function and low respiratory resistance, which can be used to prevent SARS virus.
5.The comparison of attentional control between earthquake anxious and normal children

Objective To investigate the characteristics of attentional control between Wenchuan earthquake anxious and normal children. Methods Using Screening Child Anxiety Related Emotional Disorders (SCARED), 18 earthquake anxious children were chosen as participants and 18 normal children were chosen as control group. They were asked to perform a visual search task. Results ( 1 ) The judgment accuracy of earthquake anxious children was lower than normal children's significantly ( ( 0.95 ± 0.01 ), ( 0. 98 ± 0. 01 ), P <0.01 ). ( 2 ) The reaction time of earthquake anxious children was significantly longer than normal children' s ((1664.5 ± 78.5 ) ms, ( 1110.7 ± 78.5 ) ms, P < 0. 01 ). ( 3 ) There was significantly faster performance in the valid condition than in the neutral( ( 1304.7 ± 61.3 ) ms, ( 1382. 3 ± 47.4 ) ms, P < 0. 05 ) and invalid condition ( ((1304.7 ± 61.3 )ms, (1475.8 ± 71.5 )ms; P < 0. 05 ), as well as faster performance in the neutral than in the invalid condition ((1382.3 ± 47.4) ms, ( 1475.8 ± 71.5 ) ms, P < 0.05 ) in all children. Conclusion The performance of earthquake anxious children on top-down attentional control task is less than normal children. It indicates that anxious emotion which is brought by traumatic incidents like earthquake influents earthquake anxious children' s cognitive processing ability.
6.Outcomes of open and minimally invasive esophagectomy for esophageal cancer

Objective To compare the safety,feasibility,and long-term outcomes between combined thoracoscopic and laparoscopic esophagectomy (TLE) and open three-field esophagectomy (OE) for esophageal cancer.Methods The data of 183 patients with esophageal cancer who accepted TLE (n =94) or OE (n =89) from February 2009 to December 2011 were retrospectively collected and compared.Results Demographics,pathologic data in each cohort were almost identical.No significant differences were observed in operative time,the number of lymph node harvested,rates of anastomotic leak,thoracic abscess,chyle chest,re-operation,vocal cord paralysis,gastric emptying dysfunction and morbidity.A significant difference was observed in blood loss (86.6 ±38.3 ml in TLE group vs.217.4 ±87.2 ml in OE group,P=0.000),rate of transfusion (1.1% vs.6.7%,P =0.045),hospital stay (13.9 ±7.5 d vs.17.1 ±10.2 d,P =0.017),rate of overall surgical complications (23.4% vs.38.2%,P =0.030),rate of pulmonary complications (9.6% vs.27.0%,P =0.002) and rate of arythmia (4.1% vs.12.4%,P =0.046).Median follow-up time was 28.0 ±2.0 months (95% CI:24.2-31.8).No difference between the two groups concerning survival was observed (P =0.954).Conclusion The TLE for esophageal cancer is safe and feasible.Although it cannot improve the survival of long-term,it is worth spreading in intraoperative and postoperative advantages compared with OE group.
7.Expression and significance of cytokeratin-19, vimentin, vascular endothelial growth factor-C, and cyclooxygenase-2 in Graves' disease and Hashimoto's thyroiditis

ObjectiveTo study the roles of cytokeratin-19 ( CK-19 ),vimentin,vascular endothelial growth factor-C( VEGF-C ),and cyclooxygenase-2 ( COX-2 ) played in the occurrence and development of Graves'disease(GD) and Hashimoto's thyroiditis(HT).Methods57 cases of GD and 58 cases of HT were enrolled in our study.Immunohistochemistry using SP method was carried out for assessment of the expression of CK-19,vimentin,VEGF-C,and COX-2 in the thyroid tissues.Results CK-19,VEGF-C,and COX-2 were expressed in the cytoplasm of thyroid follicular epithelial cells.Vimentin was expressed both in the mesenchyma and in the cytoplasm of thyroid follicular epithelial cells.The positive rates and expression intensities of CK-19 and VEGF-C in HT ( 86.2%,96.6% ) were significantly higher than those in GD ( 43.9%,56.1%,all P<0.05 ).The expression intensities of vimentin and COX-2 in GD ( 100.0%,93.1% ) were similar to those in HT ( 100.0%,91.2 % ),while the expression intensity of COX-2 in HT was significantly higher than that in GD( all P<0.05 ).The positive rates of CK-1 9 were much higher in type Ⅲ ( 81.3% ) of GD than in type Ⅰ ( 1 5.8% ) and type Ⅱ ( 40.9% ) of GD,and also higher in type P( 100% ) of HT than in type L(66.7% ) of HT.The positive rates of VEGF-C were much higher in type Ⅲ ( 87.5% ) of GD than in typeⅠ ( 36.8% ) and type Ⅱ ( 50.0%,all P < 0.05 ) of GD.ConclusionImmunohistochemical detection of the expression of CK-19,vimentin,VEGF-C,and COX-2 may carry clinical significance in revealing the occurrence and development as well as evaluating the prognosis of Graves'disease and Hashimoto's thyroiditis.
8.Effects of Gubenkeli capsule on matrix metallopeptidase 9 and tissue inhibitor of metalloproteinase1 levels in lung tissue of a rat model of chronic obstructive pulmonary disease*

BACKGROUND: The effectiveness and safety of traditional Chinese medicine treatment for chronic obstructive pulmonary disease (COPD) have been preliminarily approved by clinical practices.OBJECTIVE: To investigate the effects of Gubenkeli capsule on the protein expression of matrix metalloproteinase 9 (MMP-9) and tissue inhibitor of metalloproteinase1 (TIMP-1) in lung tissue of COPD model rats.METHODS: A total of 50 Wistar rats were randomly divided into five groups with 10 rats in each group: normal control, model, prednisone, Gubenkeli capsule-low dose, and Gubenkeli capsule-high dose. COPD rat models were established in all rats with the exception of the normal control rats by smoking and intratracheal instillation of LPS. At 29 days after COPD induction, rats from the prednisone, Gubenkeli capsule-low dose, Gubenkeli capsule-high dose groups were intragastrically administered prednisone (1.04 mg/kg per day), Gubenkeli capsule (0.4, 0.94 g/kg per day), once a day, to observer rat general conditions. Protein expression of MMP-9 and TIMP-1 in the lung tissue was detected by immunohistochemical methods. RESULTS AND CONCLUSION: Protein expression of MMP-9 and TIMP-1 in the lung tissue of COPD rats was significantly increased (P < 0.05). After drug intervention, the general conditions of COPD rats were greatly improved, and protein expression of MMP-9 and TIMP-1 in the lung tissue was decreased. Prednisone yields the strongest effects, followed by high-dose Gubenkeli capsule and low-dose Gubenkeli capsule. These findings demonstrate that Gubenkeli capsule alleviates the clinical manifestations of COPD model rats, improve airway remodeling, and correct the imbalance between prolease and antiprotease in a dose-response manner.
9.Design of New Ambulance Aid to Airport Reserve

Objective To develop a new ambulance aid on airport rescue of pilot and solve the difficult problems on medical staff, which carry the wounded from high aircraft cabin to the ground. Methods The device owned QG 0.85 trailer chassis. On the basis of it, self-made manual flexible extension-jib, flexible extension ladders and safety board were designed. The safety board was used to fix wounded pilots to protect secondary injury of backbone and C-spine. Results The maximal speed of equipment was 80km/h and spreading time is 3~7min, operating time is 4min and lifting height is 6 000mm. Conclusion The ambulance aid can be used in the location of flight accident quickly and rescued the pilots rapidly, adapted to any type fighters and met the needs of medical support in flight.
10.Simultaneous determination of four acids active compounds in Erodium stephanianum by RP-HPLC.

OBJECTIVETo establish a RP-HPLC method for the determination of four acids compounds including gallic acid, protocatechuic acid, corilagin and ellagic acid in Erodium Stephanianum.
METHODThe RP-HPLC separation was performed on an Agilent TC-C18 analytical column (4.6 mm x 250 mm, 5 microm). The mobile phase was methanol (A) -water containing 0.4% H3PO4 (B) with gradient elution mode at the flow rate of 0.8 mL x min(-1). The detection wavelength was set at 259 nm, and the column temperature was 30 degrees C.
RESULTThe liner ranges of gallic acid, protocatechuic acid, corilagin and ellagic acid were 0.059-2.360 g x L(-1) (r = 0.999 6), 0.017-0.672 g x L(-1) (r = 0.999 9), 0.351-14.040 g x L(-1) (r = 0.999 9), and 0.151-6.040 g x L(-1) (r = 0.999 8), respectively. The average recoveries (n = 3) were 99.45% (RSD 1.5%), 98.65% (RSD 1.7%), 100.3% (RSD 2.0%), and 98.90% (RSD 1.2%), respectively.
CONCLUSIONThe method is simple and accurate with a good reproducibility and can be used for quality control of Erodium stephanianum.
