1.Relationship of serum estradiol levels with age and lesion severity in women with knee osteoarthritis

BACKGROUND:The pathogenesis of knee osteoarthritis is stil not very clear, and there is a lack of early diagnosis methods. Because it is more common in middle-aged women, it is suggested that the pathogenesis of knee osteoarthritis may be closely related to serum estradiol levels in the human body. OBJECTIVE:To explore the correlation between the level of serum estradiol and the severity of osteoarthritis in female patients with knee osteoarthritis. METHODS:Seventy-six female patients with knee osteoarthritis admitted in the First Affiliated Hospital of Guangxi Medical University from February 2012 to December 2013 were enroled, and divided into premenopause group (n=42) and postmenopause group (n=34). According to Kelgren-Lawrence (K-L) grading, both two groups were subdivided into K-L 0, II and IV groups. The knee function was evaluated with Hospital for Special Surgery (HSS) Knee Score Standard in USA, the serum estradiol levels were detected with electrochemiluminescence immunoassay. Then, the correlation analysis was performed. RESULTS AND CONCLUSION: The serum estradiol levels of the premenopause group and postmenopause group were decreased significantly with the increase of K-L grade and age (P < 0.05). Under the same K-L grade, the serum estradiol levels in the postmenopausal patients were significantly lower than those in the premenopausal patients. Linear regression analysis showed that there was a negative relationship between age and serum estradiol level (R2=0.222,P < 0.05), and there was a positive correlation between HSS scores and serum estradiol levels (R2=0.415,P < 0.05). The severity of knee osteoarthritis in female patients shows a negative correlation with the level of serum estradiol but a positive correlation with age.
2.Immunohistochemical study of HLA-DR antigen in endometrial tissue of patients with endometriosis.

In order to evaluate the expression of HLA-DR antigen in glandular cells in eutopic and ectopic endometrium in patients with endometriosis, 19 infertile patients with endometriosis were analyzed immunohistochemically by labelled streptavidin biotin (LSAB) method. Nineteen infertile patients without endometriosis were studied as controls. The results showed that the expression of HLA-DR antigen in the glandular cells in both eutopic and ectopic endometrium was increased significantly as compared with that in the controls (P < 0.01). It is likely that aberrant expression of HLA-DR antigen in endometriotic tissue is involved in abnormal immunogenesis of endometriosis.

Objective To investigate the effect of continuous positive airway pressure therapy (CPAP) on heart rhythm and blood pressure variability in primary hypertensive patients with obstructive sleep apnea/hypopnea syndrome (OSAHS).Methods Ninety seven patients with OSAHS and primary hypertension were randomly divided into treatment group (n =49) and control group (n =48).Patients in control group receive antihypertensive medication only,while patients in treatment group received antihypertensive medication in combination with CPAP therapy for 4 weeks.The dynamic electrocardiogram (ESG) and ambulatory blood pressure for 24 h were measured before and after treatment in both groups.Results After CPAP treatment,the night index of standard deviation of the average NN intervals (SDANNniGHt) in treatment group was (89.0 ± 21.4) ms,while the day and night index of square root of the mean squared differences of successive NN intervals (rMSSDday and rMSSDnight) was (35.9 ± 17.2) ms and (45.7 ± 18.3) ms respectively.Compared with those before treatment,the index of SDANNnight was decreased and the indexes of rMSSDday and rMSSDnight were increased (P < 0.01).Compared with control group,the SDANNnight was decreased,and the rMSSDday and rMSSDnight were increased (P < 0.01,P < 0.05).After CPAP treatment,the night index of low-frequency power (LFnight) in treatment group was (15.3 ± 5.7) ms,which was lower than that before treatment,and the night index of high-frequency power (HFnigh) was (16.3-± 8.1)ms,which was higher than that before treatment (P < 0.01);the LFnight in treatment group was decreased and the HFnight was increased (P < 0.01).After CPAP treatment,the day and night indexes of systolic blood pressure (SBPday and SBPnight) and the night indexes of diastolic blood pressure (DBPnigh) t in treatment group were (136.1 ± 13.4) mmHg,(128.7 ± 9.8) mmHg (1 mmHg =0.133 kPa),and(83.7 ±7.2)mmHg respectivley,which were all lower than those in control group (P < 0.05 or P < 0.01).The incidences of morning peak blood pressure and non-dipper hypertension in control group were significantly higher than those in treatment group (P < 0.01).Conclusion CPAP therapy can effectively adjust the heart rate and blood pressure levels in patients with OSAHS and primary hypertension,especially in the night.
5.Effects of total hip replacement on knee joint motion

BACKGROUND: Human lower limb system under different motion statuses exhibits great difference in terms of kinetic characteristics and joint deformation magnitude. Total hip replacement has been shown to be the best method to solve the severe illness of hip joints today. A good understanding of the responses of human lower limb system, in particular the knee joint, following total hip replacement is significant for rehabilitation training of patients.OBJECTIVE: To analyze the effects of total hip replacement on knee joint deformation during motions by comparing the data collected from a patient who underwent total hip replacement 19 months ago and a healthy control subject under the same testing environment and method.METHODS: The patient was measured through the use of Optotrak Certus motion capture system (Northern Digital Inc.,California), and simultaneously measurements were performed in a healthy control subject presenting with similar body weight and body mass. Measurement indices included step length, step frequency, joint corner, and joint displacement curves. Speed and acceleration of each joint were calculated based on measured data.RESULTS AND CONCLUSION: Following total hip replacement, the structure of natural hip joint was destroyed. Although the gait did not change greatly, experimental data showed that knee joint deformed more greatly than normal gait, indicating the self-accommodation of human body, i.e., increasing the loading and deformation magnitude of knee joints can avoid the injuries caused by hip joint surgery.
6.Influence of repetitive ischemic preconditioning on metal elements during ischemic injury of spinal cord in rabbits

BACKGROUND: It has been reported that the ischemia preconditioning (IPC) had credible protective efficiency on ischemic injury of the spinal cord during aorta operation, but the mechanism of the protective efficiency of IPC had not been clarified.OBJECTIVE: To study the protective effects of repetitive IPC on ischemic injury of spinal cord and its mechanism in rabbits.DESIGN: A completely randomized controlled study based on the experimental animals.SETTING: Department of anesthesiology in a university hospital.MATERIALS: The experiment was completed in the Department of Anesthesiology, Renmin Hospital, Wuhan University during September and December 2002. Twenty-four Japanese rabbits were randomly and double-blindly divided into sham-operation group, ischemia-reperfusion group and IPC group with 8 rabbits in each group.INTERVENTIONS: In sham-operation group, abdominal aorta was not clamped. Spinal cord ischemia was induced by infra-renal aortic cross-clamp for 45 minutes in ischemia-reperfusion group. Before the 45 minutes ischemia, the rabbits in the IPC group underwent four cycles of ischemia preconditioning, i.e. clamping abdominal aorta for 5 minutes then reperfusion for 5 minutes.MAIN OUTCOME MEASURES: The concentrations of calcium, magnesium, copper and zincum in spinal cord were measured in the 7th day after operation. Postoperative neurological function, EMG of rear limb, and spinal cord histopathological changes were assessed in all groups after operation.RESULTS: The concentrations of calcium and copper in spinal cord in ischemia-reperfusion group were significantly higher than those in sham-operation group( P < 0. 05 or 0. 01 ), but magnesium and zincum significantly lower( P < 0. 05). Compared with IPC group, calcium in ischemia-reperfusion group was significantly higher( P < 0.01 ), but zincum significantly lower( P < 0.01 ) . The neurological function and histopatholohical changes in ischemia-reperfusion group were much lower than those in sham-operation group and IPC group ( P < 0.05 or P < 0.01) . And there was significantly worse change of EMG in ischemia-repeffusion group than that in IPC group(P < 0.01).CONCLUSION: Repetitive ischemic preconditioning can protect rabbit spinal cord from ischemia reperfusion injury quickly, and one possible reason for its protective effect is to maintain the balance of calcium, magnesium,copper and zincum in ischemic region.
7.Programming and rebuild of network system in hospital information construction

Objective To construct a safe,stable and credible system for the running of network system and improve its application effect.Method Starting with the information system construction in Xijing hospital,the existent network system was programmed and rebuilt by establishing a assistant switch core and constructing multi-center redundancy link.Conclusion The rebuilt information system of Xijing hospital attains the anticipated effect,and shows rationality and practicability in use.What's more,combined with actual experience,the rebuild of hospital network system was summarized and suggestions were given.
8.Expression of HMGB1 and the antagonistic effects of ethyl pyruvate on synovium in collagen-induced arthritis rats

Objective In this study,we elucidated the role of high mobility group box chromosomal protein 1 (HMGB1) in collagen-induced arthritis (CIA) rat and the antagonist role of ethyl pyruvate by using a rat model of CIA as the research object by comparing the expression of HMGB1 in normal control group,CIA model group and ethyl pyruvate group.Methods Thirty-six rats were randomly divided into 3 groups (n=12):normal control group,CIA group and ethyl pyruvate group.Then the 6 rats were dissected at the 6th,9th week respectively.Thc expression of HMGB1 was analyzed by immunohistochemistry and Pathology-image analysis software in the cytoplasma.The expression of HMGB1 mRNA with real time-polymerse chain reaction (PCR) was evaluate,and the HMGB1 expression of each group were compared with t-test.Results The immunohistochemical results of HMGB1 showed that the expression intensity in the normal control group,CIA model group and ethyl pyruvate group was 2.1±0.6,7.3±1.2,6.0±1.2 respectively at the 6th week; and 2.2±0.7,12.4±4.5,5.5±1.0 at the 9th week respectively.The HMGB1 mRNA real time-PCR results had shown that the relative quantification of the normal control group and CIA model group were 1,2.865,2.602respectively at the 6th week and 1.005,4.694,1.729 at the 9th week.At those two points, the HMGB1 expressions of HMGB1 antagonist group were significantly higher than those of the normal controls (P<0.05).In addition,there was statistical significant difference(P<0.05) in the HMGB1 expression when compared with the placebo group.Furthermore, when the degree of HMGB1 expression among the three groups was compared,the HMGB1 antagonist group was decreased significantly (P<0.05).Conclusion The results has demonstrated that HMGB1 could induce inflammation in the synovial tissue of CIA rats,and has provided the rationale that HMBG 1 could be the target of treating rheumatoid arthritis (RA).The results of this study have shown that ethyl pyruvate could antagonize the effect of HMGB1.This finding may provide a new therapeutic target for the treatment of RA.
9.Characterization of the epitopes recognized by monoclonal antibody 3A8 specific to broad-spectrum Gram-negative bacteria

Objective:To screen and characterize the common epitopes recognized by monoclonal antibody 3A8,which binds several kinds of Gram negative bacteria,which were recognized by monoclonol antibody 3A8 agaisnt different strains of LPS.Methods:Phage clones binding to 3A8 were screened from C7C phage displayed peptide library,the attached phage clones were eluted by LPS2630(O111:B4).After 3 rounds of panning,the positive phage clones were identified by ELISA,and the amino acid sequences of these positive clones were deduced from DNA sequences.In order to predict how these peptide mimics interaction with 3A8,peptides(A2 and A5) based on conserved sequences of positive clones were synthesized and conjugated with KLH for further study.The binding of A2-KLH and A5-KLH to 3A8 were identified by ELISA.Results:15 of 33 phage clones were identified as positive clones and shown specific binding with 3A8.LPS2630 potently inhibited the binding of phage clone to 3A8.In analysis of the amino acid sequence,there were seven kinds of sequences containing highly hydrophilic residues,and Ser Pro Pro/Pro X Pro was the conserved sequence.The peptides A2 and A5 could bind to 3A8 specially.Conclusion:The conserved sequences containing Ser Pro Pro/Pro X Pro are obtained,which are recognized by mAb 3A8 against broad spectrum Gram negative bacteria and several strain LPS.The synthetic peptides based on the conserved sequence can bind to 3A8,indicating that the peptides can mimic a common epitope of Gram negative bacteria,which be expected as candidate epitope for vaccinization.
10.Clinical analysis of 32 cases with pulmonary embolism following major surgery of gastrointestinal malignant tumors

Objective: To study the etiology, clinical manifestation, diagnosis, treatment and precaution of pulmonary embolism following major surgery of gastrointestinal malignant tumors. Methods: Thirty-two cases of pulmonary embolism following major surgery of gastrointestinal malignant tumors were analyzed retrospectively. Results: The most common clinical manifestations were dyspnea, thoracodynia and palpitation. The D-dimer of all cases was positive. The examination of pulmonary arteriography by spiral computed tomography might assist the diagnosis of the disease. Twenty-two patients were discharged through the treatment of oxygen therapy and anticoagulation. Six patients recovered through the treatment of assisted respiration by respirator, thrombolysis and anticoagulation. Four patients died of respiratory failure. Conclusion: The operation of gastrointestinal malignant tumors can increase the incidence of pulmonary embolism. Arteriography by spiral computed tomography and D-dimer can assist the diagnosis and thrombolysis and anticoagulation were effectivel for the treatment.
