1.Long-term outcome of Legg-Perthes disease after Salter innominate osteotomy

[Objective]To observe radiographic morphology of femoral head at skeletal maturity of the Salter innominate osteotomy for treatment of patients with Legg-Perthes disease and evaluate sphericity of the femoral head and congruity of the hip.Based on system review of literatures,thes attempted to determine the indications and methods of treatment.[Method]Sixteen patients of Legg-Perthes disease were treated by Salter innominate osteotomy before 1994.The mean age of patients was six years and seven months at operation.Of them,only were 12 patients followed up 10 years and ten months avergely and mean ages of patients were 17 years and 2 months at last follow-up.Growth of plate of femoral head and Y cartilage of acetabulum was closed,indicating skeletal maturity.[Result]According to Stulberg classification of outcome,there were four cases in Stulberg class Ⅰ and six patient in class Ⅱ and one patient in class Ⅲ and one patients in class Ⅳ and no case in class Ⅴ.Lower limb of shortening was averagely 1.5 cm(ranged 0.8~2.0 cm)and the range of motion of hips was almost normal and no abnormal gait was observed.[Conclusion]The Salter innominate osteotomy could keep sphericity of the femoral head and congruity with acetabulum of the hip for children with Legg-Perthes disease and also could be in aid of preventing premature arthritis of hip.
2.Changes and clinical of serum 25-hydroxy vitamin D levels in rheumatoid arthritis

Objective To observe the changes of serum 25‐hydroxy vitamin D[25‐(OH)D] level of patients with rheumatoid ar‐thritis(RA) and study its clinical significance .Methods The method of chemiluminescence was adopted to detect peripheral blood serum 25‐(OH) D of 60 cases of RA patients(RA group) and 60 cases of healthy people(control group) .Results The serum 25‐(OH) D level of patients in RA group was(34 .37 ± 8 .87) nmol/L ,while that of the control group was(61 .33 ± 7 .74) nmol/L ;after comparison ,the differences were statistically significant (t= -6 .892 ,P< 0 .05);the serum 25‐(OH) D level of patients in RA group was in negative correlation with RA patient age ,body mass index(BMI) ,time of morning stiffness ,tender joint count(TJC) , swollen joint count(SJC) ,C‐reactive protein(CRP) ,erythrocyte sedimentation rate(ESR) ,visual analog scale(VAS) for disease o‐verall pain and health assessment questionnaire (HAQ) and disease activity score (DAS28)(r= -0 .201 ,-0 .255 ,-0 .362 ,-0 .296 ,-0 .273 ,-0 .198 ,-0 .323 ,-0 .356 ,-0 .379 ,-0 .418 ;P<0 .05 in all cases);but had no correlation with course of dis‐ease ,rheumatoid factor (RF) ,anti‐cyclic citrullinated peptide (CCP) antibody (anti‐CCP) and antibody concentration (P> 0 .05) . Conclusion The serum 25‐(OH) D level of RA patients is significantly low ,which might be associated with the RA disease activi‐ty .
3.The levels of serum anti-müllerian hormone in women with premature ovarian failure and its clinical significance

Objective To observe the changes of serum anti-mttllerian hormone (AMH) level in women with premature ovarian fail-ure (POF) and its clinical significance. Methods Fasting serum levels of AMH in 25 POF patients were measured with enzyme-linked im-mtmosorbent assay (ELISA). Twenty healthy women matched for age and body mass index (BMI) were chosen as controls. Results The serum AMH level in POF group was (1.1±0.1) pmol/L, which was significantly lower than that of control group(P < 0.01). The serum AMH levels in 20 patients of POF group were undetectable. The other 5 patients were treated with artificial cycle at first, and then ovulated with hMG/HCG.2 patients had ovulation and 1 patient was pregnant. Conclusion The serum AMH level in POF patients was significantly decreased, and the measure for serum AMH had some significance for POF diagnosis and treatment.
4.Dartos island skin flap in 1-stage urethroplasty for complex posterior urethral obliteration

Objective To report the experience in managing complex posterior urethral obliteration with dartos island skin flap. Methods 32 patients with complex posterior urethral obliteration were treated with dartos island skin flap substitution urethroplasty.Partial resection of the inferior pubic synphysis was needed to facilitate high proximal placement of the flap. Results The outcome was excellent in 31 (97%).Including subsequent procedures,the over all 3 months~7 years fllowup investigations were carried out in 27 patients.Urinary flow rates of more than 15 ml/s (for adults) and 8 ml (for children) could be achieved in 25 of 27 patients.Two patients with urinary flow decreased required further reconstruction.The hairy pouch formed within scrotal skin flap was noted in two patients. Conclusions Dartos island skin flap urethroplasty is a highly effective 1-stage method of reconstructing complex posterior urethral obliteration.
5.Evaluation of urinary NMP_(22) in the detection of transitional cell carcinoma of bladder

The sensitivity and specificity of NMP-,2 in diagnosing transitional cell carcinoma of bladder were 85. 7% and 60% when the cut-off was set 10 u/ml, In contrast, the sensitivity and specificity of urinec cytology were 32. 1% and 100%. Conclusions: Urinary NMP22 can be used to screen and follow up transitional cell carcinomas of bladder with high sensitivity and high specificity.

Objective To inquire into the diagnosis and management of renal angiomyolipoma.Methods The 22 cases of RAML were studied restrospectively.Results The diagnosis accuracy was 54.5% for B ultrasound and 81.8% for CT,18 were diagnosed as RAML and 4 were misdiagnosed as renal cell carcinoma preoperatively,of which 3 were diagnosed as RAML based on intraoperative frozen section study.11 had been performed enuoleation,7 partial nephrectomy,3 nephrectomy and 1 radical nephrectomy. Conclusions CT is the first line imaging method for RAML.the cow fatty component,intratumor hemorrhage and small tumor are the main canse of misdiagnosis in RAML.careful analysis to clinical feature and imaging data,intraoperative frozen sect,ion study can avoid misdiagnosis.nephron sparing surgery is the best operative management.

9.Effects of goal-oriented liquid therapy on postoperative complications in elderly patients undergoing radical gastrectomy for gastric cancer

Objective To analyze effects of target-oriented liquid therapy on postoperative complications in elderly hypertensive patients with radical gastrectomy.Methods 156 elderly patients with hypertension who underwent radical gastrectomy in our hospital from January 2014 to December 2015 were divided into the conventional group and the target group according to a random number table.Routine infusion was given to the patients in the conventional group,while the patients in the target group were treated with intraoperative target oriented liquid therapy.Intraoperative liquid intake and output volume,hospital stay,vasoactive drug use rate and the incidence of complications within 1 month after surgery were compared between the two groups.Results Target group versus conventional group showed that the colloid volume was(832.3 ± 216.6) ml vs.(1 031.3±245.8) ml,the amount of crystals was(1 157.6±149.2) ml vs.(1 559.8±201.4) ml,the total fluid replenishment was(1 940.2 ± 328.9) ml vs.(2 650.0 ± 276.6) ml,the hospitalization time was (9.8 ± 1.1) d and (12.5 ± 2.0) d (t =-5.607,-13.771,-18.021 and-10.447,respectively,all P=0.000).The use rate of vasoactive drugs was higher in the conventional group[69.2% (54/ 78)] than in the target group [29.5 % (23 / 78)] (x2 =24.645,P =0.000).The complications within 1 months after surgery was higher in the conventional group[12.8% (10/78)]than in the target group[3.8% (3/78)] (x2=4.112,P=0.043).Conclusions Target-oriented liquid therapy can improve the prognosis of elderly patients receiving radical gastrectomy,reduce the incidence of postoperative complications and promote the early rehabilitation.
10.Detection of Huanglongbing Pathogen in Citrus medica L. var. sarcodactylis (Noot.) Swingle by Polymerase Chain Reaction Analysis

[Objective] To establish an effective way for the detection of Huanglongbing pathogen in Citrus medica L. var. sarcodactylis (Noot. ) Swingle by polymerase chain reaction (PCR) analysis, and to provide evidence for the early diagnosis of the culture of pathogen-free plantlets and for the prevention and control of diseases. [ Methods ] The incidence of Huanglongbing disease was investigated in the main producing area of Citrus medica L. var. sarcodactylis (Noot.) Swingle. By using the special primers designed by the DNA sequence of Citrus Huanglongbing pathogen, PCR amplification of DNA from the samples was conducted. [Results] Symptoms similar to the Citrus Huanglongbing disease occurred in Citrus medica L. var. sarcodactylis (Noot.) Swingle growing in Zhaoqing, Guangdong. The results of PCR analysis showed that a special DNA fragment of 400bp had been detected in the affected plants of Citrus medica L. var. sarcodactylis (Noot. ) Swingle, but it was not found in the healthy plants, indicating that the symptoms were caused by the Huanglongbing pathogen. [ Conclusion ] PCR analysis is an effective way for the detection of Huanglongbing pathogen in Citrus medica L. var. sarcodactylis ( Noot. ) Swingle.
