1.Epidemiologic characteristics,risk factors and safety evaluation of traditional Chinese medicine induced liver injury

There have been increasing reports on liver injury induced by traditional Chinese medicine (TCM) and natural medicine in recent years. The risk of liver injury induced by TCMs has attracted more attention at home and abroad. In this paper,epidemiologic characteristics,risk factors,clinical features of TCMs induced liver injury were discussed. The myth about research and safety evaluation of TCMs induced liver injury was analyzed. Based on the property of TCMs and characteristics of their clinical use,this paper proposed the priorities of basic and clinical safety evaluation of TCMs induced liver toxicity. It is hoped that this study may provide reference for scientific evaluation of liver toxicity of TCMs.
2.Perioperative management of patients with heart transplantation: a report of 30 cases

Objective To sum up the experience of perioperative management of patients with heart transplantation.Methods In recent 3 years 24 cases of dilated cardiomyopathy,4 cases of(serious) valve disease,1 case of serious ischemic cardiomyopathy and 1 case of bi-ventricular assist(received) heart transplantation surgery.The mean by pass time was 75?24 min and the mean aortic clamp time was 72?8 min.Cyclosporine,MMF and prenisodone were adopted as anti-immune(response) strategy and the dose of cyclosporine was adjusted according to the serum concentration.(Intramyocardial) electrograms and tissue Doppler ultrasound technique were used to monitor the(immune) response situation.The myocardial biopsy was performed in case of the suspect of acute(rejection).Results In 5 postoperative deaths,3 cases died of low cardiac output,1 died of infection-(induced) multiple organ failure and 1 case died of serious bleeding.The postoperative complications(included) right heart failure in 6 cases,renal failure in 13 cases,and acute rejection in 4 cases.(Conclusion) The effective prevention and management of complications is the key point of success of heart transplantation.Application of comprehensive monitoring techniques is beneficial to the management of the immune rejection after transplantation.
3.Studies on immunogenicity of Schistosoma thioredoxin Ⅰ. Construction and identification of DNA vaccine of Schistosoma japonicum thioredoxin

ObjectiveTo construct and identify a DNA vaccine of Schistosoma japonicum (Chinese strain) thioredoxin. MethodsAccording to a cDNA sequence of S.japonicum (Philippine strain) thioredoxin, a couple of primers was designed with the BamH I restriction endonuclease site introduced in forward primer with ATG as start condon and EcoR I in reverse primer with TCA as termination codon. The gene encoding S.japonicum (Chinese strain) thioredoxin (SjcTrx) was amplified by RT-PCR using total RNA of schistosome as the template. The PCR products and the pcDNA3 plasmids were digested by restriction endonucleases BamH I and EcoR I. The target DNA fragment were purified and cloned properly into the eukaryotic expression vector of pcDNA3. The recombinant plasmids were then transformed into competent E.coli JM109 and identified by endonucleases digestion, PCR, agarose gel electrophoresis and sequencing. ResultsThe RT-PCR product was around 334 bp judged by agarose gel electrophoresis. The same fragments were obtained by restriction enzyme digestion from the recombinant plasmid and PCR with the plasmid DNA as a template. The recombinant plasmid, designated pcDNA3-SjcTrx, was sequenced and shown to be 97% and 43% identical in deduced amino acid sequences to that of S.japonicum (Philippine strain) and S.mansoni thioredoxin, respectively. ConclusionThe S.japonicum thioredoxin DNA vaccine had been constructed successfully, and further studies will be made in different animal models for its immunogenicity.
4.Downregulation of Raf kinase inhibitory protein in human lung cancer cell lines

Objective To detect the expression level of RKIP in human lung cancer cell lines and to investigate the relationships between RKIP and cell metastasis. Methods The expression level of RKIP in human lung cancer cell lines was analyzed by RT-PCR. To construct and express human RKIP fusion protein and to immunize New Zealand rabbit with purified fusion protein to prepare polyclonal antiserum.The antiserum was titered by ELISA. The specifcity of antiserum and expression level of RKIP in human lung carcinoma cell lines was analyzed by western blotting. Results The fusion protein was successfully expressed. The prepared polyclonal antibody reacted specifically with RKIP in human cells. Downregulation of RKIP was detected in three lung cancer cell lines with metastasis, and it is more significant in A549 with higher metastasis.Conclusion The RKIP antiserum with high titer and good specificity could meet the requirement for studying on RKIP. Downregulation of RKIP has correlation with human lung carcinoma cell metastasis.
5.Preliminary Study of TCM Syndromes of Acute Exacerbation of Chronic Obstructive Pulmonary Disease with Malnutrition

Objective To observe the TCM syndromes of acute exacerbation of chronic obstructive pulmonary disease (COPD) with malnutrition. Methods TCM Syndromes Questionnaire of Acute Exacerbation of COPD was formulated with TCM clinical epidemiology methods. Totally 220 cases of hospitalized patients met the inclusion criteria were divided into two groups, 120 cases of malnutrition and 100 cases of non-malnutrition. The clinical data were observed and syndrome elements of malnutrition group and non-malnutrition group were analyzed to discuss the rule of syndromes in acute exacerbation period. Results In addition to respiratory system, the frenquency of symptoms such as weight loss, shortness of breath, easy cold, aversion to wind and cold, poor appetite, abdominal distension, constipation, belching, dizziness and tinnitus, tooth loose and hair loss, soreness and weakness of waist and knees significantly increased in COPD with malnutrition, mainly involving in syndromes of spleen deficiency and incoordination between spleen and stomach. In patients with acute exacerbation of malnutrition COPD, the single syndromes were 36 cases (30%), the composite syndromes were 84 cases (70%). The simple excess syndromes were 38 cases (31.67%), the simple deficiency syndromes were 23 cases (19.17%), and the intermingled deficiency and excess syndromes were 59 cases (49.17%). Conclusion Patients with acute exacerbation of COPD with malnutrition are mainly composite syndromes and the intermingled deficiency and excess syndromes. Spleen deficiency and incoordination between spleen and stomach may be the important pathogenesis of COPD with malnutrition.
6.Early and mid-term efficacy of endoscopic vein harvesting undergoing coronary artery bypass grafting in patients over 70 years old

Objective To evaluate the early and mid-term efficacy of endoscopic vein harvesting(EVH) undergoing coronary artery bypass grafting(CABG) in patients over 70 years old.Methods Patients' data of CABG were analyzed retrospectively in General Hospital of Shenyang Military Region from June 2011 to January 2013.400 patients were enrolled in the study according to the inclusion criteria.All patients were divided into two groups by the method of harvesting great saphenous vein (GSV):EVH group (n =200) and open vein harvesting(OVH) group(n =200).Each group was divided into two subgroups:group A(age≥70 years old) and group B (age < 70 years old).The number of the group A was 85 in the EVH group and 79 in the OVH group.The number of the group B was 115 in the EVH group and 121 in the OVH group.The intraoperative findings,the quality of GSV and postoperative pain were evaluated,patients were followed up regularly after discharge.Results The postoperative complications occurred in the EVH group such as numbness and edema of lower limb in group A reduced greatly compared with group B(P < 0.05).However,in the OVH group,there were no great difference in group A and group B(P > 0.05) besides the postoperative pain (P < 0.05).Conclusion The early and mid-term efficacy of EVH undergoing CABG is satisfied,especially in the patients over 70 years old.
7.Meta-analysis of the Efficacy and Safety of Shenxian Shengmai Oral Liquid in the Treatment of Slow Arrhyth-mia

OBJECTIVE:To systematically review the efficacy and safety of Shenxian shengmai oral liquid in the treatment of slow arrhythmia,and provide evidence-based reference for clinical treatment. METHODS:Cochrane library,EMbase,PubMed, CJFD,CBM and VIP database were retrieved to collect the randomized controlled trails(RCT)of Shenxian shengmai oral liquid(tri-al group)vs.other Chinese medicines(control group)in the treatment of slow arrhythmia. After quality evaluation,the Meta-analysis was performed using Rev Man 5.14 software. RESULTS:A total of 13 RCTs were included,involving 1 023 patients. The results of Meta-analysis showed that the effective rate [RR=1.26,95%CI(1.18,1.35),P<0.001] and average heart rate [MD=5.80,95%CI (4.01,7.58),P<0.001] in test group were significantly higher than control group;there were no significant difference in the inci-dence of adverse reactions between test group and control group[OR=0.46,95%CI(0.17,1.22),P=0.12]. CONCLUSIONS:Shenx-ian shengmai oral liquid is effective with good safety in the treatment of slow arrhythmia. However,due to low quality of included studies,it remains to be further verified by high-quality and large-sample long-term RCT.
8.Identification of specific drugs for M_5 using m_5AChR-G_ (11?) fusion protein

Aim The m_5AChR-G_ 11? fusion protein was expressed by baculovirus-Sf9 cells system, then using it to identify the specific agonists and antagonists for m_5AChR via detecting the affinity of GDP and m_5AChR-G_ 11?. Methods The m_5AChR-G_ 11? fused cDNAs were generated via a two-step PCR protocol and inserted into pBacPAK9 virus vector. We expressed m_5AChR-G_ 11? fusion protein and m_5AChR protein using baculovirus-Sf9 cell system. [ 3H]QNB and [ 35S]GTP?S binding tests were performed to detect the expressional level of receptor proteins and determine the affinity of GDP and m_5AChR-G_ 11? fusion protein. Results The expression level of m_5AChR-G_ 11? was (47.6?3.2) nmol?g -1 protein. The affinity of GDP to G_ 11? partner changed in the presence of different muscarinic ligands. IC_ 50 values of GDP in the presence of ACh, YM796, Oxotremorine, Methixene, Dextimide and atropine were 128.0, 72.1, 68.5, 16.2, 14.9 and 9.7 ?mol?L -1 respectively, and that in the absence of muscarinic ligand was 20.8 ?mol?L -1. Conclusion The m_5AChR-G_ 11? fusion protein has the pharmacological specificity of M_5 receptor and the efficient coupling interaction of the two partner. Affinity of GDP to ligand bound m_5AChR-G_ 11? fusion protein represents the species of muscarinic ligands. ACh is a full agonist for m_5AChR-G_ 11? fusion protein, YM796 and oxotremorine are partial agonists, while methixene, dextimide and atropine are antagonists.
10.Research on the electrophysiological mapping and ganglianated plexi ablation techniques during the minimally invasive atrial fibrillation surgery

Objective To conclude the results of electrophysiological (EP) mapping and ganglianated plexis (GPs) ablation during the minimally invasive atrial fibrillation (AF) surgery.Methods During the period of Sep 2007 to May 2010 to-tally 185 paroxysmal atrial fibrillation cases were recruited receiving the minimally invasive AF surgery with video assistance.The EP mapping was used to test the dual direction block after ablation and the distribution of GPs was recorded.The sinus rhythm rate was followed up after surgery.Results All the minimally invasive surgery were performed successfully withoutdeath in the hospital.There was no permanent pacemaker implant and other serious complications.EP mapping results showed92% pulmonary vein (PV) potential to the left atrium (LA) could be isolated with the dual direction block for the paroxysmal atrial fibrillation cases.And there were 89% GPs positive,higher rate than the persistent AF.There were more GPs positive in the right PV area than the left and 86% GPs lied at the LA domain,not the PV orifice.At least 5 or 6 ablation lesions were needed to get the dual direction block and denervation of the GPs.The sinus rhythm rate after surgery after 3,6,12 and 24months is 83.7%,82.4%,85.4% and 83.9%.The 12 months follow up data showed the cases with more GPs positive (>6) had 81.3% sinus rate and the less GPs positive (≤2) group with 52.6%.Conclusion The EP mapping and GPsablation techniques during the minimally invasive AF ablation surgery could get good sinus rhythm rate during the long term follow up.
