1.Analysis of therapeutic effect of integrated traditional Chinese and Western medicine treatment in the elderly patients with vertebrobasilar insufficiency

Objective To observe the curative effect of integrated traditional Chinese and Western medicine treatment in the elderly patients with vertebrobasilar insufficiency.Methods Totally 120 elderly patients with vertebrobasilar insufficiency were randomly divided into two groups:treatment group and control group(n=60,each).All the patients received Vinpocetine injection by intravenous drip,besides,the patients in treatment group received compound recipe Xueshuantong capsule.Results The total effective rate in treatment group was 95.0%,which was significantly higher than that in control group(78.3%)(P<0.05).The plasm viscosity [(1.2±0.2) vs.(1.6±0.2) mPa/s]and fibrinogen [(3.1±0.6)vs.(4.4±0.5)g/L]were decreased after compound xueshuantong capsule treatment as compared with control group (P < 0.05 ) Conclusions Combination of traditional Chinese and Western medicine has definite therapeutic effect on the old patients with vertebrobasilar insufficiency and can be utilized widely.

3.The efficacy and safety of sirolimus in immunosuppression after liver transplantation

ObjectiveTo evaluate the efficacy and safety of Sirolimus (SRL) in immunosuppression following liver transplantation. Methods SRL was applied in 21 patients totally.Indication for adoption was Tac-related nephrotoxicity (4/21), suspiciously Tac-related hepatoxicity (8/21), Tac overdose (3/21), renal insufficiency pre-operation (2/21), or cancer (4/21). Median follow-up was 25. 4 months. Results SRL provided an adequate prophylaxis against rejection in all study patients, with one case of acute rejection. Sirolimus was Withdrawn in 2 cases due to its sideeffect. Tat-induced hepatoxicity in 6 cases and nephrotoxicity in 3 cases were relieved significantly.Conclusions SRL given alone appears to be an effective primary immunosuppressant regimen fororthotopic liver transplantation patients. Early conversion contributes to significant improvement of Tac-related hepatoxicity and nephrotoxicity.
4.Effect of Bie-Jia-Jian Pill on Blood Lipids, Endothelial Function and MMP-9 of Diabetes Rat Model with Atherosclerosis

This article was aimed to observe the preventive effect of diabetes with atherosclerosis (AS) treated with Bie-Jia-Jian Pill (BJJP) on level of blood lipids, endothelin (ET), nitric oxide (NO), and matrix metalloproteinase-9 (MMP-9). Male SD rats were randomly divided into 5 groups, which were the blank group, model group, low-dose BJJP group, high-dose BJJP group and Xue-zhi-kang (XZK) group, to observe changes on the level of blood lipids, ET-1, NO and MMP-9 within 5 groups after 8-week administration. The results showed that compared with the blank group, the level of the content of NO in the model group was significantly decreased (P< 0.05), the level of ET-1 was significantly increased (P < 0.05). Compared with the model group, the high-dose and low-dose BJJP group as well as XZK group can significantly decrease the level of ET-1 (P< 0.05, or P< 0.01), increase content of NO (P< 0.05, or P< 0.01), and decrease the expression of MMP-9 (P< 0.05). It was concluded that the BJJP can regulate blood lipids, protect endothelial cell of blood vessels, stabilize plaques for the anti-AS effect.
5.Multistage multivariate statistic quality control (MMSQC) for monitoring production process of traditional Chinese medicines.

OBJECTIVETo establish a method for monitoring the quality of intermediates generated in each working procedure during the production process of traditional Chinese medicine (TCM) , in order to ensure the batch-to-batch quality consistency of TCM products.
METHODThe multistage multivariate statistic quality control (MMSQC) was proposed to monitor production quality of TCMs based on multivariate data analysis technique. Hotelling T2 and SPE were adopted for monitoring the quality of intermediates generated in each working procedure. Danshen injection was taken as the example to introduce the application method of MMSQC.
RESULTMMSQC can monitor the quality of intermediates generated in multiple working procedures, which is simpler and more accurate compared with single-indicator monitoring method.
CONCLUSIONMMSQC can be popularized to monitor quality of multistage production of TCMs.

6.Application of fallopian tube embolization before in vitro fertilization and embryo transfer dealing with the hydrosalpinx

Objective To explore the effectiveness of the fallopian tubes embolization for the hydrosalpinx before in vitro fertilization and embryo transfer(IVF-ET).Methods The fallopian tubes embolization was performed on 46 hydrosalpinx patients-Fimt the hysterosalpingography was performed to show the positions and shape of the hydrosalpinx.Then the hydrops was drawn into the celiac cavity in order to be absorbed after performing fallopian tube recanalization. Finally,transv(o)ihal therapy was performed,putting the embolization microcoils into the fallopian tube through a micro-catheter.One month after the fallopian tubes embolization,hystemsalpingography was conducted to check for the effectiveness of the embolization.After 3 months.all the 46 patients received the treatment of IVF-ET,Results The interventional treatment of 82 fallopian tubes obtained one time success among 46 cases of fallopian tubes embolization.Among them,obvious results were achieved in 72 fallopian tubes,taking up 88%0f tIle total;effective results were seen in 10 fallopian tubes.accounting for 12%0f the total.No one was invalid.In the same period,compared with the 91 cases of non-hydrosalpinx as the control group.the embolization group of patients achieved a higher fertilization rate(69%),and clinical pregnancy rate(41%),compared with the control group(63%and 39%respectively),but without a significant difference(P>0.05).However,the ectopic pregnancy rate(O)and the abortion rate(8%)were significantly lower than the control group(8%,16%respectively;P<0.05).Conclusions Fallopian tubes embolization used in hydrosalpinx treatment before IVF-ET is an innovative approach,simple,safe,economical,with no negative impact on ovarian functiorr It can significantly increase the clinical pregnancy rate and prevent the occurrence of tubal pregnancy.It is a feasible and effective method.
7.Effect of Roux-en-Y gastric gastrointestinal bypass surgery on islet function of patients with type-2 diabetes mellitus pancreatic

Objective To investigate the effect of gastrointestinal Roux-en-Y gastric bypass surgery on blood sugar and insulin function of patients with type-2 diabetes mellitus.Methods Twenty-seven cases of gastric cancer patients with type-2 diabetes and undergone Roux-en-Y bypass the gastrointestinal treatment in the centre hospital of Cangzhou were selected as our subject.Body mass index (BMI),Glycosylated hemoglobin (HBA1c),Fasting and glucose (FPG),fasting insulin (FINS),Fasting C-peptide (FCP) levels were measured.Glucose (2 hPG),insulin (2 hINS) and C-peptide (2 hCP) levels were detected after 2 h for oral use 75 g glucose.Homeostasis model was applied to assess insulin resistance index (HOMA-IR).Results No significant change was seen in terms of BMI between before and after surgery.Compared to before surgery,the levels of FPG((7.58 ±0.84) mmol/L),2 hPG((10.43 ± 1.88) mmol/L),HbA1c((7.56 ± 1.15)%) and HOMA-IR(4.55 ±0.76) were lower in patients at 3 months after surgery ((9.93 ± 1.57) mtmol/L,(13.89± 2.13) mtmol/L,(9.88 ± 1.66) %,(4.55 ± 0.76),respectively,P < 0.05 or P < 0.01).FPG ((6.56± 0.80) mmol/L),2 hPG ((8.57 ± 1.32) mmol/L),HbA1 c ((6.37 ± 1.24) %),HOMA-IR (4.03 ± 0.45)of patients after 6 months were lower than that of before surgery and 3 months after surgery (P < 0.05 or P<0.01).However,the levels of FINS ((13.67 ± 1.96) mU/L),FCP((2.62 ±0.87) μg/L),2 hINS((49.91± 5.14) mU/L) and 2 hCP ((6.28 ± 1.65) μg/L) were higher in patients with 3 months after surgery compared to that of before surgery ((11.08 ± 1.69) mU/L,(1.78 ± 0.61) μg/L,(36.05 ± 4.03) mU/L,(4.28 ± 1.48) μg/L,P < 0.01).Meanwhile those indices after 6 months (FINS:(15.88 ± 2.05) mU/L,FCP:(3.30 ±0.68) μg/L,2 hINS:(67.40 ±5.68) mU/L,2 hCP:(9.39 ± 1.52) μg/L) were higher than that of before surgery and 3 months after surgery(P < 0.01).Conclusion Roux-en-Y gastrointestinal bypass can effectively reduce blood sugar level and improve the situation of Pancreatic Beta-cell function of gastric cancer patients with type-2 diabetes.
8.Establishment and evaluation of models for small-for-size graft injury without veno-venous bypass in miniature pigs

Objective To establish small-for-size (SFS) graft injury models in miniature pigs with high standardization, reproducibility and similarity to clinical situation. Methods Ba-Ma miniature pigs were introduced in this study and orthotopic liver transplantations (OLTs) were performed in 12 pigs with 30% liver volume allogeneil grafts (small portion of right paramedian lobe, right lateral lobe and caudate lobe) without veno-venous bypass. The profiles of intra-operational hemodynamics and metabolism were investigated. Animals were observed for 7 days with daily serum biochemistry and coagulation function exam. The survival rate related to operation itself and the SFS grafts were respectively calculated as well as the graft regenerative ratio at post-operational day (POD) 7. Results Graft weight as a percentage of the recipient's native liver weight (GW/RLW) and the total body weight (GW/BW) were (28. 63±4. 42)% and (0. 73±0.06)%. The mean operation time, anhepatic phase, and the time of blockage of infra-hepatic IVC were (191. 7±14. 2) min, (28. 3±3. 6) min, and (45. 0±5. 8) min. The survival rate related to the operation itself and the SFS graft were 83. 33% (10/12) and 40% (4/10), and the graft regenerative ratio at POD7 was (278. 06±42. 95) %. Contrast to the remarkable increase of heart rate and serum potassium during anhepatic phase, the mean arterial pressure, central venous pressure, rectal temperature, PH value and buffer excess had a significant decrease (P<0.01) with a gradual recovery after reperfusioa Serum ALT, AST, PT, Cr, and TB were significantly increased with a peak level at POD1 for the former 4 and POD2 for TB, and then began to decrease and favorably recovered at POD7, but TB, PT, and AST levels were still high when compared to those of prereperfusion (P<0. 05). Conclusion This model of OLT performed with 30% liver volume graft without veno-venous bypass was an ideal large animal model for series studies related to SFS graft injury.
9.Clinical effect of total hip arthroplasty in treatment of patients with ankylosing spondylitis and bilateral hip bony ankylosis

Objective To investigate the effect of total hip arthroplasty in treatment of patients with ankylosing spondylitis and bilateral hip bony ankylosis. Methods 43 patients with ankylosing spondylitis and bilateral hip bony ankylosis were randomly divided into observation group (23 cases) and control group (20 cases) in accordance with the principle of informed consent, individually treated by total hip arthroplasty and conservative treatment, the effect of two groups were compared and analyzed. Results In the observation group, the postoperative Harris hip score and the VAS pain score of patients were significantly better than the control group,the differences were all statistically significant between two groups( all P < 0.05 ). Excellent rate of the two groups were 91.3% and 60.0% respectively, and the difference was statistically significant ( x2 = 4.328, P < 0.05 ). Conclusion Total hip arthroplasty in treatment of patients with ankylosing spondylitis and bilateral hip bony ankylosis has better clinical effect, and could improve the quality of life of patients, so it was worthy of clinical application.
10.Metabolic characteristics and risk factors for newly diagnosed type 2 diabetes mellitus combined with non-alcoholic fatty liver disease

Objective To explore metabolic characteristics of and risk factors for newly diagnosed type 2 diabetes mellitus(T2DM) combined with non-alcoholic fatty liver disease (NAFLD).Methods One hundred and forty-two cases of newly diagnosed T2DM were divided into two groups according to whether they have comorbid NAFLD:group A (without NAFLD,n =79) and group B (combined with NAFLD,n =63).Data collected included body height,body weight,blood pressure,fasting plasma glucose (FPG),blood lipid,serum uric acid (UA),HbA1c and fasting insulin,body mass index and insulin resistance index with homeostasis model(HOMA-IR) were calculated to compare the clinical and biochemical parameters between groups A and B.Results (1) The difference of age and blood pressure between groups A and B were not statistical different (P > 0.05).Compared with group A,BMI ((26.79 ± 1.93) kg/m2 vs (24.61 ± 2.46) kg/m2,t =5.76),FINS((15.49±2.44) mU/L vs (13.20±2.17) mU/L),t =5.91),HOMA-IR((6.74± 1.32) vs (5.65 ±1.10),t =5.37),glycerin trimyristate (TG) ((2.94 ± 0.65) mmol/L vs (1.74 ± 0.46) mmol/L),t =12.86),low density lipoprotein cholesterin (LDL-C) ((3.46 ±0.73) mmol/L vs (2.78 ±0.86) mmol/L,t =5.07) and UA((342.41 ±71.49) mmol/L vs (312.98 ±66.24) mmol/L,t =2.54) were significantly higherand hight density lipoprotein cholesterin (HDL-C) ((0.99 ± 0.17) mmol/L vs (1.21 ± 0.29) mmol/L,t =5.33) was significantly lower in group B (P < 0.05).(2) Using whether to combined with NAFLD as dependent variable,and BMI,FINS,HOMA-IR,TG,LDL-C,HDL-C and UA as independent variable,logistics regression analysis showed that BMI,HOMA-IR and TG were risk factors for NAFLD(OR =2.838,19.241,and 2.019 respectively,P < 0.05).Conclusion Newly diagnosed type 2 diabetes mellitus combined with NAFLD have more obvious dyslipidemia and insulin resistance.Obesity,insulin resistance,hyper-triglyceridemia are risk factors for newly diagnosed type 2 diabetes mellitus combined with NAFLD.
