Background:For patients with liver cirrhosis and acute decompensation(AD),it is of great clinical importance to predict short-term mortality at admission. It has been reported that CLIF-C OF,MELD and MELD-Na score can accurately predict the short-term mortality, but all these scoring systems are complicated and have limits in their application. Aims:To define a simple and objective scoring system -- simplified MELD score for short-term mortality prediction in HBV-related cirrhotic patients with AD. Methods:A total of 890 consecutive HBV-related cirrhotic patients with AD hospitalized during Jan. 2005 to Dec. 2010 at Shanghai Ren Ji Hospital were enrolled retrospectively. Clinical data and patients’outcome were collected, and simplified MELD score was calculated by using total bilirubin, international normalized ratio and creatinine values at admission. Patients were classified into different prognostic groups according to their 28-day mortalities and simplified MELD score. Kaplan-Meier survival curve was used to analyze the 1-year accumulate survival rate, and ROC curve was used to evaluate the performance of different scoring systems in predicting 28-day mortality. Results:Simplified MELD score at admission could classify HBV-related cirrhotic patients with AD into low, moderate and high 28-day mortality groups and different long-term prognostic groups;the score of low, moderate and high 28-day mortality group was 0-2,3 and 4-6,respectively,and the corresponding mortality was 5. 5% ,19. 8% and 48. 6% ,respectively. Simplified MELD score had the same good performance as compared with the CLIF-C OF,MELD and MELD-Na scores in predicting 28-day mortality, the area under ROC curve was 0. 828,0. 831,0. 828 and 0. 830,respectively. Conclusions:Simplified MELD score can accurately classify HBV-related cirrhotic patients with AD into low, moderate and high 28-day mortality groups at admission. It is convenient for using in clinical practice.

AIM To investigate the anti-atherosclerosis mechanisms of arecoline. METHODS Fifty-eight male rats were assigned to control, model, arecoline 1 mg?kg -1 and arecoline 5 mg? [FQ(12。46,X-WZ]-kg -1 groups randomly. The rats in model and arecoline-supplied groups were fed with hypercholesterol diet. The following variables were measured: HE staining of rat aorta; serum level of TC, LDL-CHO, HDL-CHO, IL-8, ET-1 and NO; expression of eNOS protein on rat aorta; expression of MCP-1, ICAM-1, CXCR-2 and eNOS mRNA on rat aorta. RESULTS Arecoline increased plasma level of NO and the expression of eNOS protein and mRNA, decreased plasma level of IL-8 and the expression of ICAM-1, MCP-1 and CXCR-2 mRNA on rat aorta. CONCLUSION The anti-atherogenic effects of arecoline seems to be closely involved in increasing plasma level of NO and eNOS protein and mRNA expression, decrease in plasma IL-8 level and down-regulation of the expression of ICAM-1, MCP-1 and CXCR-2 genes.-

AIMTo investigate the anti-atherosclerosis mechanisms of the novel compound PPVP. METHODSThirty-six male rats were a ssigned to control, model and PPVP 5 mg?kg -1 groups randomly. The rats in model and PPVP 5 mg?kg -1 groups were fed with hypercholesterol diet for inducing atherolcerosis model. The following variables were measured: HE stainin g of rat aorta; serum level of TC, LDL-CHO, HDL-CHO, IL-8, ET-1, PGI 2, TXA 2 and NO. RESULTSPPVP increased serum level of NO, decreased plasma level of TXA 2 and inhibited the over expression of IL-8. CONC LUSIONThe anti-atherogenic effect of PPVP seems to be closely involved in increasing serum NO level, decreasing plasma level of TXA 2, and inhibiting over expression of IL-8. The result also suggests that the anti-atherogenic ef fect of PPVP in high cholesterol-fed rat may not due to the regulation of plasm a lipid profile, plasma level of ET-1 and PGI 2.

Ribosome inactivating protein(RIP) is a kind of toxin plant pr ot ein in which extensively lives in the body of higher plants and controls ribosom e's function. Beside it can control protein's combination,it has lots of biolog ical reactivity as resisting giving birth and tumor and controlling HIV. At fir st, RIP is isolated from seeds of bitter melon. The result of SDS-PAGE indicate s that there are lots of RIP in the abstraction liquid. Then we study the antivi ral action of RIP through the cell of CEF and SPF chicken embryos. The results s how that RIP can resist NDVF_48E_8,MDVCVI_988 and FPV-SD4 to so me extent.

OBJECTIVETo observe effects of mild moxibustion and lovastatin on immunologic function in rabbits with chronic hyperlipidaemia and atherosclerosis (AS) to initially explain regulating rules of mild moxibustion on immunologic function.
METHODSAmong thirty-two Japanese male big-ear rabbits, 8 rabbits were randomly selec ted as a blank group, the rest 24 rabbits were fed with method of endothelial injury and high-fat diet to establish AS model. The blank group was raised with normal diet and free water. After ten weeks of model establishment, the rest 24 rabbits were randomly divided into a model group, a moxibustion group and a medicine group, eight rabbits in each one. Moxibustion was applied at "Shenque" (CV 8) and "Zusanli" (ST 36) for 10 min per acupoint per day in the moxibustion group, while intragastric administration of 3.6 mg/kg lovastatin capsule was applied in the medicine group. After treatment, serum was acquired. Spectrophotometry method was adapted to measure cholesterol (TC) and high-density lipoprotein (HDL-C) and evaluated atherosclerosis index (AI), while enzyme-linked immunosorbent assay method was used to measure interferon-gamma (IFN-gamma) and interleukin-4 (IL-4).
RESULTS(1) The serum TC and HDL-C in the model group were significantly higher than those in the blank group, moxibustion group and medicine group (all P < 0.01). The mean value of AI was 1.683 +/- 0.486 in the moxibustion group, which was obviously lower than 20.301 +/- 4.022 in the model group (P < 0.01). (2) The ratio of Th1/Th2 was 0.569 +/- 0.143 in the moxibustion group and 0.445 +/- 0.079 in the medicine group, which were significantly lower than 0.917 +/- 0.255 in the model group (both P < 0.01), but there was no statistically significant difference between the moxibustion group and the medicine group (P > 0.05).
CONCLUSIONThe moxibustion for AS could reduce atherosclerosis index, influence drift and bias of helper T cell and regulate balance between humoral immunity and cellular immunity. As a result, status of relative balance of immunity is acquired, which could slow down the development of atherosclerosis and process of thrombus burst.

Chinese Journal of Epidemiology 2010;31(5):513-516
Objective To study the daily physical activity patterns of pupils in Beijing, and to identify the association between the socioeconomic status (SES) and physical activities. Methods 1502 pupils were recruited from 2-5 grades of eight primary schools in Beijing. Information on physical activity, sedentary activity, and transportation styles inside and outside schools were collected using a "7-day physical activity questionnaire". Results Gymnastics, running, playing games, walking in queues etc. Were the most common activities at school, with the proportion as 94.13%, 85.55%, 77.26%, and 71.32% respectively. Physical labor, playing games, running and walking were the common activities outside schools, with the proportion as 72.14%, 69.70%,65.05%, and 64.64% respectively. However, pupils seldom participated in the ball games. Significant differences between SES groups were observed on the patterns of both physical and sedentary activities. Compared to low-SES pupils, high-SES pupils more frequently participated in dancing, walking upstairs or downstairs, swimming, skating, but less frequently (P<0.05) in walking, rope skipping, kicking shuttlecock or rubber band skipping. Conclusion Data from our study showed that both the quantity and the distribution of different activities did not meet the need of the Chinese children. Physical activity in association with SES was also demonstrated.
