1.Exploration of Operation Pattern at the First Time of Spontaneous Perforation of Congenital Choledochal Cyst in Children

ObjectiveTo explore the diagnostic methods and surgery pattern at the first time of spontaneous perforation of congenital choledochal cyst.MethodsEleven cases(4 male,7 female) with spontaneous perforation of congenital choledochal cyst were 6 months to 5 years old,and their average course of disease were 4 days.Gustily abdominal distension,abdominal pain,crying and fever were present in all cases.Jaundice(7 cases) and emesis(5 cases) appeared.All cases were detected with physical sign of peritonitis by physical examination.Choledochal cysts were confirmed by CT or B ultrasound in 8 cases.All cases accepted abdominal paracentesis and biliary ascites was drawn.Three different operative procedures were performed:choledochocyst excision & Roux-Y choledocho-jejunostomy(2 cases),choledochotomy with T-tube drainage(3 cases),and cholecystostomy(6 cases).Nine children receiving external drainage operation accepted a second operation to rebuild biliary tract(choledochocyst excision & Roux-Y choledochoje-junostomy) after 3 to 6 months.ResultsAll cases had got satisfactory therapeutic efficacy without any grave complication such as fistula of anastomotic stoma,infection of biliary tract or obstruction of biliary tract.During operation,perforations were located in the juncture of choledochus and cystic duct in 5 children and were not found in the other 6 children.In the second operation,the cases receiving cholecystostomy had less peritoneal adhesion,anatomic structure changes,haemorrhage[(30-50) mL vs(100-200) mL] and operation time[(2.5-3.0) h vs(3.5-5.0) h] than those receiving choledochotomy with T-tube drainage,and did not appear inadequate drainage for cystic duct obstruction.ConclusionsFor children with more organ inflammatory edema and adherence and in a bad overall condition,the first-time operation of cholecystostomy is more reasonable.
2.Short-term clinical observation of modified resin-bonded fixed partial denture utilizing tooth undercuts.

OBJECTIVETo observe a short-term clinical result of modified resin-bonded fixed partial denture(MRBFPD) utilizing tooth undercuts.
METHODSOne hundred and twenty-nine patients received restoration and totally 196 missing teeth were restored with 148 MRBFPD. According to the positions of abutments in the dental arch, all the MRBFPD were divided into three groups, namely anterior prostheses (40), posterior prostheses (95) and prostheses spanning anterior and posterior (13). The prostheses were examined at 2 weeks, 1 month, 3 months, 6 months, and every year after denture delivery. Retention and marginal integrity of MRBFPD, sensibility, secondary caries and gingival bleeding index were clinically evaluated. The evaluation results were graded as good, acceptable, and bad according to modified United States Public Health service (USPHS) criteria. Survival in this study was defined at two levels: "complete" survival (survival without any debonding), and "functional" survival (survival including loss of retention on one occasion and successful rebonding of the original MRBFPD without further debonding).
RESULTSThe average follow-up period was 26 months. Fifteen prostheses failed and were rated as bad with USPHS criteria, and ten prostheses were evaluated as "acceptable" because the gingival bleeding index has been rated as 3 or more at one appointment but reduced to 1 or 0 after treatment. The results of the other 123 prostheses were evaluated as "good". The "complete" survival rates were 80% (32/40) for anterior prostheses, 11/13 for prostheses spanning anterior and posterior, and 95% (90/95) for posterior prostheses, while the "functional" survival rates were 80% (32/40), 11/13 and 99% (93/95) respectively. The "complete" survival rate of posterior prostheses was statistically higher than that of anterior prostheses (χ(2) = 5.433, P < 0.05). The "functional" survival rate of posterior prostheses was not only higher than that of anterior prostheses (χ(2) = 13.179, P < 0.01)but also higher than that of prostheses spanning anterior and posterior (χ(2) = 4.143, P < 0.05) statistically.
CONCLUSIONSThe results of this preliminary study suggest that short term survival rate of MRBFPD in posterior area was statistically better than that in anterior area. The outstanding advantage of MRBFPD is the minimum need of dental tissue reduction. However, their long-term success needs further investigation.

Dexamethasone can upregulate the expression of HLA-G on placenta.The 14 bp deletion polymorphism in exon 8 of HLA-G gene might not have a significant influence on the development of ICP.
4.Significance of detection of Tau protein and β amyloid protein levels in cerebrospinal fluid in patients with mild cognitive impairment

ObjectiveTo study the levels of Tau protein and β amyloid protein(Aβ) in cerebrospinal fluid(CSF) in patients with mild cognitive impairment(MCI) and their significance of diagnosis. MethodsSensitive enzyme linked immunosorbent assay were performed to detect the CSF levels of Tau protein and Aβ in 32 patients with MCI and 15 healthy controls. ResultsSignificant elevation of the level of Tau protein and decrease of the level of the Aβ were observed in patients with MCI compared with healthy controls.ConclusionThe assays of the CSF Tau and Aβprotein levels can provide valuable biological marker for the diagnoses and treatment of MCI.

6.Association between gut-enriched Kruppel-like factor and prognosis of patients with gastric cancer.

OBJECTIVETo explore the expression of gut-enriched Kruppel-like factor 4(KLF4) in gastric cancer, and its association with prognosis.
METHODSSurgical specimens were collected from 264 patients undergoing radical surgery between 2004 and 2009 in the Affiliated Qianfoshan Hospital, Shandong University. KLF4 mRNA level of specimens was detected by real-time PCR. KLF4 protein expression was measured by immunohistochemistry on tissue microarray, which contained primary gastric cancer, corresponding para-cancerous tissue, and paired lymph node metastases.
RESULTSReal-time PCR revealed that mRNA level of KLF4 was down-regulated in gastric cancer compared with paired normal gastric mucosa. Immunohistochemistry on tissue microarray showed gastric cancer tissues had significantly lower KLF4 levels compared with paired normal gastric tissues. By univariate and multivariate analysis, KLF4 was a significant predictor of survival and recurrence.
CONCLUSIONKLF4 expression is significantly down-regulated in gastric cancer, and is an independent predictor of survival and recurrence.

7.Apoptosis of Lewis Lung Carcinoma Cells Induced by Microwave via p53 and Proapoptotic Proteins In vivo

Background:Microwave therapy is a minimal invasive procedure and has been employed in clinical practice for the treatment of various types of cancers.However,its therapeutic application in non-small-cell lung cancer and the underlying mechanism remains to be investigated.This study aimed to investigate its effect on Lewis lung carcinoma (LLC) tumor in vivo.Methods:Fifty LLC tumor-bearing C57BL/6 mice were adopted to assess the effect of microwave radiation on the growth and apoptosis of LLC tumor in vivo.These mice were randomly assigned to 10 groups with 5 mice in each group.Five groups were treated by single pulse microwave at different doses for different time,and the other five groups were radiated by multiple-pulse treatment of a single dose.Apoptosis of cancer cells was determined by terminal deoxynucleotidyl transferase dUTP nick-end labeling assay.Western blotting was applied to detect the expression of proteins.Results:Single pulse of microwave radiation for 5 min had little effect on the mice.Only 15-min microwave radiation at 30 mW/cm2 significantly increased the mice body temperature (2.20 ± 0.82)℃ as compared with the other groups (0.78 ± 0.29 ℃,1.24 ± 0.52 ℃,0.78 ± 0.42 ℃,respectively),but it did not affect the apoptosis of LLC tumor cells significantly.Continous microwave radiation exposure,single dose microwave radiation once per day for up to seven days,inhibited cell division and induced apoptosis of LLC tumor cells in a dose-and duration-dependent manner.It upregulated the protein levels of p53,Caspase 3,Bax and downregulated Bcl-2 protein.Conclusions:Multiple exposures of LLC-bearing mice to microwave radiation effectively induced tumor cell apoptosis at least partly by upregulating proapoptotic proteins and downregulating antiapoptotic proteins.Continuous radiation at low microwave intensity for a short time per day is promising in treating non-small-cell lung cancer.
8.Assessment of comparative genomic hybridization experiment by an in situ synthesized CombiMatrix microarray with Yersinia pestis vaccine strain EV76 DNA.

OBJECTIVEThe quality of microarray data influences the accuracy of comparative genomic analyses to a large extent. To ensure that the results obtained by using an in situ synthesized microarray are accurate, data quality is to be assessed by evaluating the melting temperature (Tm) of probes, probability of false synthesis rates, and fragmentation of labeled targets.
METHODSDNA from the Yersinia pestis vaccine strain EV76 was used for microarray analyses. Microarray results were confirmed by PCR. Statistical and bioinformatics methods were employed to perform microarray data analyses and evaluation.
RESULTSCorrelation coefficients of the three datasets were above 0.95 after two-time stripping and hybridization with a labeled DNA with the size of fragmentation being 200 bp - 2 kb, which showed that the hybridization results were highly reproducible. Correlation coefficients were lower with the values ranging from 0.87 to 0.92 between the datasets generated from hybridization with different sizes of the labeled DNA fragment. For the relationship between Tm and signal intensity, there was a different distribution of Tm in the lowest 300 or 3,000 probes with a range of 70 °C-72 °C and the highest 300 or 3,000 probes with a range of 72 °C-74 °C.
CONCLUSIONThe results of this study suggest that the initial microarray design may affect the accuracy of final analyses and that the probe Tm and the size of the labeled fragment may be the two factors of the greatest importance.

9.Adenovirus-mediated delivery of bcl-2 gene attenuates cisplatin-induced degeneration of cultured spiral ganglion cells

Objective To assess the protection against cisplatin-induced ototoxicity by adenovirus-mediated overexpression of the bcl-2 gene in cultured spiral ganglion cells(SGC).Methods SGC from P3 rats were cultured in vitro and exposed to adenovirus vector carrying green fluorescent protein gene(Ad-GFP),followed by immunocytochemical analysis for expression of the neuron-specific marker Neurofilament200(NF200)and detection under laser scanning confocal fluorescence microscope. Then, SGC were transduced by Ad-bel-2 and the expression of human bcl-2 protein was evaluated by Western Blot. Finally, the cultures of SGC were divided into 4 groups: the group of Ad-bcl-2 transfection followed by cisplatin treatment, the group of Ad-GFP transfection followed by cisplatin treatment, the group of cisplatin treatmentonly and the untreated group. Cisplatin worked for 48 hours at a concentration of 2μg/ml. Outcome measures included survival humber of SGC and longest neurite length by using ImageJ software. Results SGC were cultured successfully in vitro and transfected by adenovirus vector safely and efficiently. By Western Blot, human bcl-2 protein was expressed in the group after exposure to Ad-bcl-2,but not in the Ad-GFP transfected SGC. Cisplatin exposure resulted in shrinking of neuritis and pyknosis of cell body, even cell death. Expression of bel-2 in the SGC provided a significant level of protection against cisplatin-induced SGC degeneration. Conclusions Our results suggest that SGC can be transduced by adenovirus vector safely and efficiently in vitro. Adenovirus-mediated delivery of the bcl-2 gene attenuates cisplatin-induced SGC degeneration.
10.Repairing tibial post-traumatic osteomyelitis with bone and skin defect by Ilizarov technique at stage I.

OBJECTIVETo explore clinical effects of Ilizarov technique at stage I for repairing tibial post-traumatic osteomyelitis with bone and skin defect.
METHODSFrom June 2010 to December 2013,44 patients with tibial post-traumatic osteomyelitis with bone and skin defect were treated with Ilizarov technique at stage I . Among them, there were 35 males and 9 females aged from 18 to 70 years old with an average of 42.5 years old. Bone defect ranged from 4 to 16 cm, skin defect ranged from 3 cm x 4 cm to 5 cm x 16 cm. The operation was performed debridement thoroughly, removed inflammatory bone section, osteotomy invasively, install circular external fixator by Ilizarow technique; screw nut were rotated at 1 week after operation, and prolonged 0.5 to 1.0 mm everyday. Wound surface, new born callus and bone healing were observed to evaluate clinical effects.
RESULTSAll patients were followed up from 11 to 36 months with an average of 18.5 months. Bone defect after osteotomy was from 6 to 22 cm with an average of 11.5 cm; the time of wound healing time ranged from 21 to 79 d with an average of 38 d; bone defect healing time was from 8 to 15 months with an average of 12.5 months. All patients were cured, no recurrent infection, refracture and shorten of calf deformity were occurred.
CONCLUSIONRepairing tibial post-traumatic osteomyelitis with bone and skin defect by llizarov technique at stage I has advantages of less trauma, low inflammatory recurrence rate, could avoid multiple complex operation, and receive definite curative effect.
