1.Application of Dual-phase Multi-Slice Spiral CT Three-Dimensional Vascular Imaging Lesions in Liver Pathological Changes

Objective To evaluate the multi-slice spiral CT scan of liver dynamic dual-phase three-dimensional vascular imaging portal phase clinical value. Methods 80 cases in clinic, who were patients with liver function and imaging diagno- sis of liver and portal hypertension in liver cirrhosis, and 20 cases of healthy persons were carried out multi-slice spiral CT dual-phase scanning. The workstation used volume rendering techniques (VR) and maximum density multi-planar recon- struction technique for reconstruction. Results The hepatic arterial phase VR image and MIP MPR images can clearly show the celiac trunk, splenic artery, hepatic artery or artery and its branches, including 2-3 grade tumor blood supply variation of blood vessels and blood vessels, the portal venous phase, VR images and MIP MPR images clearly show the 1-6 level structure and the portal vein and hepatic vein branches of 1-3, with strong three-dimensional sense of space. Conclusion The multi-slice spiral CT three-dimensional reconstruction of portal vein imaging is a fast and effective non-invasive an- giography techniques, contributing to the clinical choice of reasonable efficacy of treatment programs and follow-up.
3.Sigma rectum pouch for urinary diversion(Report of 18 cases)

Objective To assess the continent diversion results of sigma rectum pouch after radical cystectomy. Methods The reconstruction of bladder with sigmoid was modified for treatment of 18 cases of bladder tumor.The intestine was incised over a length of 20~24 cm with the junction of sigmoid colon and rectum as the midpoint so as to create a low pressure reservoir for urine and side-to-side anastomosis was performed on the posterior borders of the rectosigmoid wall.Submucosal tunnel modified technique was em- ployed in antireflux urethral implantation,Urination has been controlled by anal sphincter.Results About 80 minutes was spent to finish a new low pressure pouch after radical cystectomy.Among 18 patients with this op- eration,the controlled emiction were good after pull out the anal duct and"J"stent in 1 week to 2 months.Af- ter 2 months,the times of urination is stable,4~5 times in daytime and 1~3 times during nighttime.Two pa- tients had nocturnal enuresis and the symptom vanished after 2 months. One patient had adhesive ileus, two patients had hyperchloremia acidosis and kaliopenia,one patient had urethral stump cancer.There is no com- plication as anastomotic block,renal function lesion and severe upper urinary tract infection. Conclusion This operative method was easy,emiction control was well,and with higher quality of life for patients.It is al- so a better alternative diversion procedure that would be easily accepted.
4.Concomitant Cisplatin and 5-Fluorouracil with Veinal or with Oral High-Dose Leucovorin in Treatment of Patients with Advanced Nasopharygeal Carcinoma

Objective: The aim of this study was to assess the efficacy and safety of veinal high dose Leucovorin + 5-FU and cisplatin regimen(PFL1) and oral high dose Leucovorin+ 5-FU and cisplatin regimen(PFL2) in the treatment of the patients with reccurent nasopharyngeal carcinoma. Methods: From January 1997 to December 1999,53 cases of histologically proven advanced nasopharyngeal carcinoma were unrandomly divided into 2 groups, of whom 28 cases were treated with PFL1 regimen, 25 cases with PFL2 regimen. Results: Complete response rate and respose rates of PFL1 regimen and PFL2 regimen were 17.9%, 64.3%,and 16%, 56%,respectively. All the patients had been followed-up till December 1999. Thirteen patients died and fifteen patients survived and median remission duration was 10.5±5.24 months(range,5-21 months) and median survival duration was 12.5±5.13 months(4-35 month) of PFL1 group. Fourteen patients died and eleven survived and median remission duration was (9.3±3.10) m (4-28 m) and median survival duration was(11.2±3.66) m (4-28 m) of PFL2 group. There was no significant difference in complete response rate, respose rate, median remission duration and median survival duration between the two groups (P>0.05). PFL1 and PFL2 chemotherapy regimens for nasopharyngeal mass, metastasis of neck or abdomen lymphnode, metastasis of lung were effective;for metastasis of bone, the efficacy of releasing pain was good, but the response rate was unsatistactory;for metastasis of liver had no effect. The major toxicity included neucopenia, nausea, vomiting, mucositis et al. (P>0.05). Vein inflammation had significant differences between two regimens (P<0.05). Alepecia and pigmentation had very significant differences between two regimens (P<0.01). Conclusions: Veinal or oral high dose Leucovorin + 5-Fu+cisplatin were effective combinnation in the chemotherapy of nasopharyngeal carcinoma and the survival duration were longer.
5.VARIATION OF TT VIRUSES IN MINNAN AREA AND EVIDENCE FOR TRANSFUSION TRANSMISSION

TT virus(TTV)DNA was tested by nested-PCR from sera of hepatitis patients and volunteer blood donors in Minnan area. The amplified segment was a 189 base pair region in TTV ORF2. A total of six sequences were obtained from three non-A to G hepatits patients and two from volunteer blood donors. The sequences were found to be with 82.9% to 99.3% homology to TTV Japanese strain and Chinese strain. The divergence of sequence in these six segments varied from 0.7% to 17.1%, which indicated that the TTV had been existing for a long time in this area. In the serum of a non-A to G hepatitis patient who was negative for TTV DNA in the 14th day of disease course turned to be positive in the 30th day, two TTV sequences were obtained which showed 92.1% nucleotide homology. It indicated that different TTV strains can co-exist in the same person. This patient's blood had been transfused ten times between the collection of his TTV negative sample and his positive serum sample. Seven of the blood donors were traced and sampled for sera, of which three were positive for TTV. For all 161 patients tested, the history of exposure to blood products was associated with an increased risk of TTV infection(relative risk, 3.0; 95% confidence intervals, 1.89~4.81).
6.Study on differential proteomic expression in human liver cells stimulated by trichloroethylene with proteomics.

OBJECTIVETo explore the differential proteomic expression in human liver cells L-02 induced by different dosages of trichloroethylene (TCE).
METHODSHuman liver cells L-02 were treated with different concentrations of TCE and the solvent control (dimethylsulfoxide). The total cellular proteins were separated using 2DE and visualized with silver staining after TCE treatment. The images were analyzed with Image Master 2D Platinum 5.0 analysis software. The differentially expressed protein spots were identified by matrix assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF-TOF-MS).
RESULTSFifteen protein spots with significant difference were found, and went upward or downward or disappeared after the stimulation of TCE with different dosages, which indicated that TCE induced the change of the proteomic expression in the liver cells. The mass spectrum identification and the IPI human database retrieval were used for identifying 9 proteins related to the L-02 Liver cells induced by TCE.
CONCLUSIONThe result provides an insight to TCE-related molecular mechanism and which might be useful for further study of the TCE-associated proteins and molecular markers.

7.Classification and repairment of "butterfly ear" deformity.

OBJECTIVETo classify and repair "Butterfly Ear" deformity which presents characters of dysplasia of inferior auricle of ear and congenital bat ear.
METHODThe repairment procedures include: type I: auricular cartilage flap inversion folding technique. type II: local ear skin flap. type III: soft tissue expander autogenous, rib cartilage framework.
RESULTSThe method was used in 19 cases from October 2001 to March 2005. Postoperative follow-up showed satisfactory results in all cases.
CONCLUSIONAccording to "Butterfly Ear" deformity classification, different technique could be applied.

8.Compare the effect of lipo- and normal prepared Sodium Morrhuate on ECV-304 cell line.

OBJECTIVETo compare the effect of Sodium Morrhuate on ECV-304 between its lipo- and normal preparation.
METHODSThe ECV-304 cell line was supplemented with Sodium Morrhuate and lipo-Sodium Morrhuate in order, and the result on morphology (microscope, Giemsa Staining and electron microscope), cell activity (MTT), and flow cytometer between the two preparation were compared.
RESULTSIn normal preparation group, cell's edema occurred. Chromatin was like catkins. Tumefaction and degeneration of mitochondrion and endoplasmic reticulum appeared. In lipo-Sodium Morrhuate group, the membrane was creased and processus appeared. Chromatin aggregates to the membrane of nucleus was like crescent, and then broken. The apoptotic body was formed. MTT changes showed that the curve of the normal preparation group was steep and the change time was short relatively, which cues the vital cells decreased sharply. The curve of lipo-Sodium Morrhuate group was gentle and the change time was long relatively, which cues the vital cells decreased slowly. The flow cytometer showed that typical apoptosis peak appeared.
CONCLUSIONThe normal preparation group shows an acute toxic effect on ECV-304 cell line, which result in a necrosis course, while lipo-Sodium Morrhuate shows a gradual releasing process, which may indicate a apoptosis course.

9.Sphingosine kinase regulates hepatocyte growth factor-induced migration of endothelial cells.

AIMTo elucidate the effect of sphingosine kinase (SPK) on the hepatocyte growth factor (HGF)-induced migration of endothelial cells.
METHODSWe constructed recombinant adenoviral vectors, which contain SPK gene and its mutant respectively. These adenoviral vectors were packaged and amplified in 293 cells. And intracellular SPK activity was assayed via measurement of [32]P radioisotope labeled S1P; the effect of SPK activation on HGF-induced migration of endothelial cell was observed by Transwell technique.
RESULTSAdenoviral mediated expression of SPK gene increased in ECV 304 cells intracellular SPK activity, which in turn enhanced the HGF-induced migration. Whereas these activities were blocked by the dominant negative SPK gene.
CONCLUSIONThese findings show that SPK activation plays important roles in the regulation of HGF-induced migration of endothelial cells.

10.Clinical significance of vascular distribution type of the expanded skin in the mastoid region in congenital microtia.

OBJECTIVETo classify the blood vessel distribution of the expanded skin in the mastoid region and its relevance to reasonable and reliable design of the expanded flap for auricular reconstruction in congenital microtia.
METHODSThe blood vessel distribution of the expanded skin in the mastoid region was observed by light permeation test. The expanded flaps with different ratio of length to width were designed according to their blood vessel distribution types.
RESULTSThe vascular distribution of the expanded skin in 403 cases was divided into five types. All the flaps survived completely.
CONCLUSIONSThe blood vessel distribution type of the expanded skin in mastoid region has great significance for the design of post-auricular expanded flap in auricular reconstruction.
