5.Follow-up of laser peripheral iridotomy in fellow eyes with unilateral acute attack of primary angle closure glaucoma

Background Laser peripheral iridotomy(LPI) is used as the primary therapy for the eyes at risk of angle closure.But there are still 28% eyes with primary angle closure suspect occur angle closure within 2 years after LPI.It is necessary to explore the associated factors of the efficacy of LPI.Objective The aim of this study was to explore the changes of intraocular pressure(IOP) and anterior chamber angle in the fellow eyes of patients with unilateral acute attack of primary angle closure glaucoma (PACG) and analyze the relevant factors influencing the successful rate of LPI.Methods Eighty-seven fellow eyes received LPI from 87 patients with unilateral acute attack of PACG were included in this study and retrospectively analyzed.The IOP and gonioscopy were performed before and 1 week,3,6,9,12 months after LPI.Successful signs of LPI were defined as an IOP 6-21mmHg without any anti-glaucoma medication,none of glaucomatous neuropathy and the coincident visual field loss,and no need of additional glaucomatous medication or surgery.This study was approved by Ethic Committee of the First Hospital of Xinxiang Medical College,and written informed consent was obtained from each patient.Results Seventy-nine eyes of 79 cases finished the medical visit throughout the follow-up duration with the age 61.4±0.4 years and 33(41.8%) males and 46(58.2%) females.The mean IOP was lowed in various time points after LPI in comparison with before operation with the general difference among different time visiting groups(F=4.056,P<0.01).Shaffer grade was increased in superior,temporal and nasal quadrants and significant differences were found between 1 week group,3 months group or 6 months group and before operation group (P<0.05).LPI lessened the range of appositional angle closure (AAC) in postoperative 1 week group and 3 months group compared with pre-operative group (P<0.05),but no statistically significant reduce was seen in range of AAC from 6 months through 12 months after LPI in comparison with pre-operation (P>0.05).LPI was determined as successful in 61 cases (77.2%) during one-year fellow-up period.Several variables,such as preoperative IOP,Shaffer grade of each quadrant,mean Shaffer grade and the extent of AAC,were significantly different between LPI success and failure group(P<0.01).Cox stepwise regression analysis found that the success rate was significantly associated with the extent of AAC (Wald=48.150,RR=1.963,P<0.01) rather than pre-IOP,Shaffer grade of each quadrant,mean Shaffer grade,age and gender (P>0.05).Conclusion LPI can widen the anterior chamber angle and low IOP in the fellow eyes of PACG attack eyes.The success rate of LPI is associated with the extent of pre-AAC,suggesting that it is necessary for LPI in consideration of the extent of pre-AAC.
6.Whole course accelerated hyperfractionation radiotherapy for esophageal carcinoma

Objective To compare the treatment effects and toxicity of convention fractionation ra- diotherapy(CFR),late course accelerated hyperfractionafion radiotherapy(LCAFR) and whole course accel- erated hyperfractionation radiotherapy(WCAFR) for patients with middle or low thoracic esophageal moder- ately differentiated squamous cell carcinoma.Methods Ninety patients with middle or low thoratic esoph- ageal carcinoma were divided into three groups:1.CFR group,patients were irradiated 2.0 Gy/f,5 times a week,to a total dose of 70 Gy;2.LCAFR group,patients were first irradiated with CFR to 40 Gy,then fol- lowed by 1.5 Gy/f bid,at more than 6 hoursinterval,5 times a week,to a total dose of 70 Gy;3.WCAFR group,patients were irradiated 1.5 Gy/f bid,at more than 6 hoursinterval,5 times a week,to a total dose of 72 Gy.Results The 1-,2-,3-year local control rate was 47%,17% and 0% in CFR group,60%,20% and 20% in LCAFR group,60%,40% and 33% in WCAFR group,respectively.The 1-,2-,3-year survival rate was 47%,20% and 10% in CFR group,63%,43% and 17% in LCAFR group,and 63% ,50% and 33% in WCAFR group,respectively.There were better results in the latter two groups(P<0.01).The a- cute toxic effect was severer in the WCAHF and LCAF groups than in CF group.Highter grade 2-3 acute esophagitis rate was observed both in WCAHF and LCAF groups.There were no significant differences in late esophagitis rate between the three groups.The grade 1-2 acute radiation pneumonitis rates were higher in WCAHF group than in LCAF and CF groups.One patient died of radiation pneumonitis in WCAHF group. Conclusions Whole course accelerated hyperfractionation radiotherapy can significantly improve the local control rates and the 1-and 2-year survival rates of esophageal cancer,but can also increase the toxicity.
7.Effect of early goal directed therapy on tissue perfusion in patients with septic shock

BACKGROUND: This study aimed to observe the effect of early goal directed therapy (EGDT) on tissue perfusion, microcirculation and tissue oxygenation in patients with septic shock. METHODS: Patients with early septic shock (<24 hours) who had been admitted to the ICU of Zhongda Hospital Affiliated to Southeast University from September 2009 through May 2011 were enrolled (research time: 12 months), and they didn't meet the criteria of EGDT. Patients who had one of the following were excluded: stroke, brain injury, other types of shock, severe heart failure, acute myocardial infarction, age below 18 years, pregnancy, end-stage disease, cardiac arrest, extensive burns, oral bleeding, difficulty in opening the mouth, and the onset of septic shock beyond 24 hours. Patients treated with the standard protocol of EGDT were included. Transcutaneous pressure of oxygen and carbon dioxide (PtcO2, PtcCO2) were monitored and hemodynamic measurements were obtained. Side-stream dark field (SDF) imaging device was applied to obtain sublingual microcirculation. Hemodynamics, tissue oxygen, and sublingual microcirculation were compared before and after EGDT. If the variable meets the normal distribution, Student's t test was applied. Otherwise, Wilcoxon's rank-sum test was used. Correlation between variables was analyzed with Pearson's product-moment correlation coefficient method. RESULTS: Twenty patients were involved, but one patient wasn't analyzed because he didn't meet the EGDT criteria. PtcO2 and PtcCO2 were monitored in 19 patients, of whom sublingual microcirculation was obtained. After EGDT, PtcO2 increased from 62.7±24.0 mmHg to 78.0±30.9 mmHg (P<0.05) and tissue oxygenation index (PtcO2/FiO2) was 110.7±60.4 mmHg before EGDT and 141.6±78.2 mmHg after EGDT (P<0.05). The difference between PtcCO2 and PCO2 decreased significantly after EGDT (P<0.05). The density of perfused small vessels (PPV) and microcirculatory flow index of small vessels (MFI) tended to increase, but there were no significant differences between them (P>0.05). PtcO2, PtcO2/FiO2, and PtcCO2 were not linearly related to central venous saturation, lactate, oxygen delivery, and oxygen consumption (P>0.05). CONCLUSION: Peripheral perfusion was improved after EGDT in patients with septic shock, and it was not exactly reflected by the index of systemic perfusion.

9.Application prospect and expectation of fungistatic agents of plants in preservation of Chinese herbal medicines.

During the process of growth, harvesting, transportation, processing and storage, Chinese herbal medicines (CHMs) can be easily contaminated by fungi and their metabolites like mycotoxins, which not only express negative effects on the quality and safety of CHMs and their processed products, but also pose great threats to human health. Now, some chemical synthetic fungicides have been frequently used to control the growth of fungi and accumulation of mycotoxins in the preservation of CHMs. However, the concentration and type of chemical fungicides allowed for postharvest application are restricted due to the disadvantages of their high residual toxicity, long degradation period and pollution to the environment and so on. Therefore, it is critical to research and develop some highly effective, safe and non-toxic, natural, environment-friendly fungistatic agents from plants to prevent CHMs from being contaminated by fungi and mycotoxins. The paper reviews mycotoxins and their harmfulness, the effective compounds of fungistatic plants as well as the antifungal mechanism to provide scientific evidences for developing novel and effective fungistatic agents plants. Then, the application prospect of fungistatic agents from plants in the preservation of CHMs was discussed.

10.Relationship between Detective Time for Serum Cardiac Troponin I and Their Positive Rate in Children with Viral Myocarditis

Objective To explore the relationship between detective time for serum cardiac troponin I(cTnI) and their positive rate in diagnosis of viral myocarditis(VM).Methods Twenty-one cases of VM were designed as the test group.The serum cTnI were dynamically detected and compared with the normal control group.Results The serum cTnI were all negative in the normal control group,of 6 cases(28.6%) in the test group were positive when admission,of 7 cases(33.3%),8 cases(38.1%),9 cases(42.9%),13 cases(61.9%) and 4 cases(19%) were positive 6,12,18,24,48,72 h and 10 to 14 days laters respectively.There were statistic significances,compared the accumulative total positive rate of 48 h and 72 h after hospitalization with of emission and of 10 to 14 days after hospitalization,respectively.Conclusion Monitoring serum cTnI dynamically may increase the positive rate of cTnI for the suspected patients.
