Objective To observe the clinical characteristics and prognosis of primary nephrotic syndrome (PNS) in the elderly. Methods The clinical and pathological features of 52 elderly patients with PNS and 64 non-elderly patients with PNS were retrospectively analyzed and compared.The elderly patients with PNS received routine treatment:oral corticosteroids prednisone 1mg·kg-1 ·d-1.After 8 weeks maintenance treatment,if symptoms were alleviated,the prednisone dose was gradually reduced to a maintenance dose, then was stopped gradually. If symptoms were not alleviated, the cyclophosphamide or cyclosporine, mycophenolate mofetil was added. Results There were significant differences in the numbers of patients complicated with hypertension, infection,chronic renal insufficiency and hematuria between the elderly group and non-elderly group (P＜0. 05＝.But there were no significant differences in the level of blood albumin, quantitative measurement of 24 hours urinary protein and incidence rate of acute renal insufficiency between the two groups (P＞0.05). The major pathological types of PNS in the elderly were membranous nephropathy (46.2 %),IgA nephropathy (23. 1 %) and focal segmental glomerulosclerosis ( 11.5 %), respectively. And the major pathological types of PNS in non-elderly group were mesangial proliferative glomerulonephritis (32.8%), IgA nephropathy ( 25.0% ) and minimal change nephropathy ( 20. 3 %), respectively.Complete remission after treatment was found in 31 patients(59.6%), partial remission in 18 cases and inefficacy in 3 cases. Conclusions The major clinicopathological type of PNS in the elderly is membranous nephropathy and should be treated using corticosteroids and immunosuppressive agents,with different effects in different pathological types.

Objective To investigate the correlation between asymmetric dimethylarginine (ADMA) and endothelial dysfunction in patients with type 2 diabetic nephropathy.Methods The 81 patients with type 2 diabetic nephropathy were selected.According to the urinary albumin excretion rate (UAER),the patients were divided into micro-albuminuria group (DN1 group,UAER 21-199 mg/24 h,38 cases) and macro-albuminuria group (DN2 group,UAER ≥ 200 mg/24 h,43 cases).The 20 healthy people were defined as control group.Intimal-media thickness and endothelial dysfunction of the radial artery of right forearm were detected by color Doppler ultrasound.The serum level of ADMA was determined by enzyme-linked immunosorbent assay.Results There was no significant difference in radial artery inner diameter intimal-media thickness among the 3 groups (P ＞ 0.05).The Endothelial dependent diastolic function (EDD) and endothelial independent diastolic function (EID) in DN1 group and DN2 group were significantly lower than those in control group [(10.45 ± 2.58)％ and (7.56 ± 2.17)％ vs.(15.72 ± 3.05)％,(15.42 ± 2.71)％ and (15.37 ± 2.92)％ vs.(19.31 ± 3.76)％,P ＜ 0.05],and the EDD in DN2 group was significantly lower than that in DN1 group (P＜ 0.05).The serum ADMA in DN1 group and DN2 group was significantly higher than that in control group [(0.63 ± 0.08) and (0.92 ± 0.12) μ mol/L vs.(0.39 ± 0.05)μmol/L,P ＜0.05],and in DN2 group it was significantly higher than that in DN1 group (P ＜0.05).In patients with type 2 diabetic nephropathy,the serum ADMA and EDD had negative correlation (r =-0.81,P =0.020),but the serum ADMA and C reactive protein had positive correlation (r =0.75,P =0.034).Conclusions The serum level of ADMA is significantly increased in patients with type 2 diabetic nephropathy.There is a close correlation between ADMA and endothelial dysfunction of artery.

Objective:To observe the effect of dexmedetomidine used in ophthalmic surgery anesthesia on the recovery quality in children. Methods:Totally 126 children with ophthalmic surgery were randomly divided into the control group and the observation group with the same induction program. The control group was given propofol with target controlled infusion during the operation, while the observation group was given dexmedetomidine 0. 6μg·kg-1 before the induction, and infused up in 10min, propofol with target controlled infusion was used during the operation, and stopped infusion 10min before the end of the operation. The cough number and degree, the occurrence of agitation and extubation time of the two groups were observed and compared during the anesthesia recovery period. Results:The cough incidence and agitation incidence was 36. 51% and 4. 76% in the observation group, respectively, which were significantly lower than those in the control group (74. 60% and 33. 33%, P<0. 05). The cough degree between the two groups showed significant difference (P<0. 05). The first cough time and the extubation time between the two groups showed no significant difference (P>0. 05). Conclusion:Dexmedetomidine used in ophthalmic surgery anesthesia of children can effectively control cough frequency and degree during the recovery period and reduce the agitation to improve the quality of clinical anesthesia recovery.

To measure the relative affinity and type Specificity of neutralizing monoclonal antibodies against the hexon antigen of adenovirus type 7,a simple ELISA double antibody binding system has been developed.By this method,ralative affinity of nine MAbs was estimated from the antibody concentrations at approximately 50% of plateau binding and were ranked.The theoretical basis for this method was discussed. Hybridomas secreting antibodies of desired affinity can be selected at an early stage after fussion by measuring relative affinity of hybridomas supernatants.The type specificity was judged by the difference between end-point concentration against adeno- virus type 7 and type 3.And the relation between affinity and specificity of MAbs was discussed.

Objective To investigate the effects of different analgesia protocols on immune function in patients undergoing surgery for ovarian neoplasms after neoadjuvant chemotherapy.Methods Forty ovarian neoplasms patients ageing from 31 to 62 years undergoing neoadjuvant chemotherapy were randomly divided into 2groups:epidural analgesia group (group E,n =20) and intravenous analgesia group (group Ⅰ,n =20).All patients underwent surgery under total intravenous general anesthesia.Patients in group E were given 2 mg morphine in epidural space at half an hour before abdomen was closed,then epidural analgesia pump was installed,with ropivacaine composite morphine in the pump.Analgesia time was 48 h.Patients in group Ⅰ were given sufentanil 0.1 μg/kg intravenously at half an hour before abdomen was closed,then analgesia pump was installed,with sufentanil compound flurbiprofen ester in the pump.Analgesia time was 48 h.The cervical venous blood samples were obtained from the patients at 30 min before surgery(T0),immediately(T1),12 h(T2),24 h(T3) and 48 h (T4) after the end of operation for determination of the expression of CD3 +,CD4 +,CD8 + on T cells and natural killer cell.Visual analogue scales (VAS)and adverse reaction at T2-4 were recorded.Results No statistical difference was found between group E and group Ⅰ in VAS and adverse reaction.Compared with T0,CD3 +,CD4 + T-lymphocytes,CD4 +/CD8 +,and NK cell decreased.Compared with group E,CD8 + increased at T1-2in both groups and at T3-4 in group Ⅰ.CD3 +,CD4+ T-lymphocytes,CD4+/CD8 + and NK cell decreased while CD8 + increased at T2-4 in group Ⅰ.Conclusion Epidural analgesia may be better to promote the immunologic function of ovarian neoplasms patients undergoing surgery after neoadjuvant chemotherapy.

Microarray technology are being performed more widely than ever before on many areas in lifescience,although the technology is still evolving,the challenge of performing a microarray experiment is no longer in the data generation,but in extracting useful information and utilizing it to get the results with biological meanings.Some methods and tools used for expressional microarray data mining based on previous work were summarized.These methods include gene clustering,GO analysis,regulating pathway analysis,and related algorithm.We hope this can be helpful for those researchers who are implementing expressional microarray for biological analysis.
