1.Delayed onset diffuse lamellar keratitis following laser in situ keratomileusis

AIM: To report a case of late onset diffuse lamellar keratitis (DLK) after laser in situ keratomileusis(LASIK) in 11 months due to corneal trauma. ·METHODS: A 22-year-old female patient underwent uneventful bilateral LASIK using a laserSight SLX excimer laser and Moria II microkeratome. No complications were observed during the operation and the early postoperative period.·RESULTS: 11 months after LASIK, grade I DLK was diagnosed after the book paper hurt the right corneal epithelium. The DLK responded rapidly to topical corticosteroid therapy, and healed in 2 weeks without complication. ·CONCLUSION: Although DLK Wpically develops in the early postoperative period, it could occur months after surgery. Treatment should begin as soon as DLK is diagnosed.
2. Changes of MMP-2, MMP-9 contents in cerebral tissues and cerebral edema in cerebral ischemia-reperfusion rats

Objective To investigate the expression and activation of MMP-2, MMP-9 in cerebral tissues of cerebral ischemia-reperfusion rats and the effects of doxycycline on the cerebral edema. Methods Cerebral ischemia-reperfusion model was established by middle cerebral artery occlusion (MCAO). The rats were randomly divided into a sham-operated and 6 reperfusion groups according to different reperfusion periods (3 h, 6 h, 12 h, 24 h, 72 h, and 120 h groups). Dry weight method was used to evaluate hemispheric water content of the ischemic side after treatment with doxycycline. Western blotting analysis and Gelatin zymography were used to determine the expression and activity of MMP-2 and MMP-9 in the ischemic cerebral tissues. Results The protein expression and activity of MMP-2 were significantly increased in the ischemia tissue at 3 h and 120 h after reperfusion(P<0.01) ; the protein expression and activity of MMP-9 began to increase at 6 h after reperfusion (P<0. 01) , reached the peak at 24 h, and then returned to the basal levels at 120 h after reperfusion (P>0.05). The hemispheric water content of the ischemic side gradually increased with reperfusion periods compared with the sham-operated group. Dexycycline significantly reduced the hemispheric water contents compared with the normal saline at the same time points(P<0.05 or P<0.01). Conclusion MMP-2, MMP-9 can induce vasogenic brain edema by degrading basal membrane of capillary vessel. The alteration of MMP-2, MMP-9 expression and activities are likely associated with brain edema during cerebral ischemia-reperfusion.
3.Study on the safety of eye anterior tissues during plasmin and hyaluronidase inducing posterior vitreous detachment in pigs

· AIM: To evaluate the safety of eye anterior tissue when plasmin (Pm) and hyaluronidase (HS) are injected into pigs'vitreouses to induce posterior vitreous detachment(PVD).· METHODS: 15 pigs without ocular diseases were randomly assigned to groups A,B,C (5 in each group). For each pig, one eye was experimental, the other eye was control. The experimental eye received intravitreal injection with enzyme:group A: 50U(0.1mL) HS; group B: 0.5U(0.1mL) Pm; group C: 0.5U(0.05mL) Pm combined with 50U(0.05mL) HS; while the control eye received intravitreal injection with equivalent dose of balanced salt solution (BSS). Postoperative reactions in the eyes were carefully observed by clinical examinations such as slit-lamp microscopy and direct and indirect ophthalmoscopy and measurement of intraocular pressure (IOP). Eyeballs were extirpated 7 days after the operation and histological examination was carried out. The corneas and irises were observed under light microscope (LM). The epithelia of ciliary bodies and lens were studied with transmission electron microscope (TEM).· RESULTS: In all experimental eyes and control eyes of group A, B and C, histological examination showed: under LM ,the corneas and irises of experimental eyes had no histological abnormality in structure; Under TEM, epithelial cells of ciliary bodies and the lens were tightly arrayed with clear cellular boundaries, clear intracellular structure, intact cell membranes, and intact nuclei; There was no significant difference in the clinical examinations and in the change of IOP between preoperation and postoperation, between the experimental group and the control group.· CONCLUSION: With 0.5U Pm and HS 50U injected into pigs' vitreouses to induce PVD, alone or combined with two enzymes, evaluation revealed no evidence of toxicity on the eye anterior tissue. The dosage was proved to be safe for eye anterior tissue.

9.Key factors in design of case report form.

Case report form (CRF) is a key document for data collection in clinical trials. A well-designed CRF is required for database construction, data accuracy, data query/cleaning, CRF completion and statistical analysis. A well-defined process or SOP should be in place for CRF design. Data collection should fully meet the demand of study protocol. The layout of CRF should be clear with well-structured fields and standard coding for fields.

10.Shugan Jieyu Capsules combined with conventional therapy for type III B prostatitis complicated by sexual dysfunction.

OBJECTIVETo explore the clinical effect of Shugan Jieyu Capsules (SJC) on type III B prostatitis complicated by sexual dysfunction.
METHODSA total of 98 patients with type III B prostatitis complicated by sexual dysfunction were equally randomized to a control and a trial group, the former treated with the combination of biofeedback/electrical stimulation and the α-blocker Tamsulosin Hydrochloride, and the latter with oral SJC in addition, both for 8 weeks. Before and after treatment, the severity of the symptoms was determined with NIH-CPSI, the patients'sexual function evaluated with CIPE-5 and IIEF-5, and their anxiety, depression and other psychological problems assessed with Hamilton Anxiety Scale (HAMA) and Hamilton Depression Rating Scale ( HAMD). The results were subjected to statistical analysis and compared between the two groups.
RESULTSStatistically significant differences were found between the control and trial groups in the NIH-CPSI score (26.31 ± 7.91 vs 18.84 ± 6.63, P < 0.01), CIPE-5 premature ejaculation score (10. 41 ± 3.03 vs 14.37 ± 2.35, P < 0.05), IIEF-5 score (10.29 ± 3.97 vs 14.69 ± 4.19, P < 0.05), HAMA score (24.31 ± 1.78 vs 13.41 ± 4.21, P < 0.01), and HAMD score (25.24 ± 2.83 vs 14.49 ± 4.44, P < 0.01).
CONCLUSIONSJC can effectively relieve anxiety, depression and other psychological problems in type III B prostatitis patients with sexual dysfunction and improve their clinical symptoms as well.
