4.Effect of pterygium excision combined with limbal stem cell on visual acuity and refractive status

Abstract? AlM: To study the effect of pterygium excision combined with limbal stem cell on visual acuity and refractive status.?METHODS:A total of 120 cases ( 178 eyes ) pterygium cases were enrolled in our hospital from March 2012 to May 2014 and divided into observation group ( 60 cases, 90 eyes) and control group (60 cases, 88 eyes) according to different treatment methods. Patients in observation group received pterygium excision combined with limbal stem cell transplantation, those in control group received pterygium excision. Vision levels, astigmatism degrees were compared. The effects of age, working environment and medial rectus muscle insertion distance from corneal limbus on postoperative visual acuity were analyzed.? RESULTS: Before operation, acuity levels and astigmatism degrees of two groups had no difference ( P>0. 05 ); 1, 2 , 3, 4wk after operation, visual acuity in observation group were higher than that in control group (0.49±0.09 vs 0.34±0.07, 0.76±0.11 vs 0.42±0.04, 0.95± 0. 14 vs 0. 59±0. 08, 1. 09±0. 18 vs 0. 74±0. 10), astigmatism degrees were lower than that in control group (0. 96±0. 12 vs 1. 85±0. 28, 0. 68±0. 08 vs 1. 51±0. 23, 0. 54±0. 08 vs 1. 23± 0.19,0.42±0.07 vs 0.91±0.14); visual acuity of patients older than 45 years old, indoor work, medial rectus muscle insertion distance from corneal limbus 3-5 mm were higher, astigmatism degrees were lower.?CONCLUSlON:Pterygium excision combined with limbal stem cell transplantation is helpful to improve visual acuity and astigmatism degree; recovery of visual acuity is affected by age, working environment and medial rectus muscle insertion distance from corneal limbus.
5.Etiology analysis of male pseudohermaphroditism

Objective To discuss the etiology of male pseudohermaphroditism. Methods The clinical data of 42 male pseudohermaphroditisms were retrospectively analyzed.Among the 42 cases (mean age of 10 years,range from 8 to 19 years),39 caseg social gender were female sex and 3 were male.Physical examination indicated abnormal external genitalia,cryptorchidism,hypospadias and mass of pelvis or inguinal.Chromosome examination showed that 39 cases were 46,XY,2 cases of 45,X/46.XY and 1 case of 45,X/47,XXY.Etiology analyses were established in 42 cases by eytogeneties,endocrinology,B-ultrasound or CT examination,celiac exploration and pathology examination.Results All the 42 cases received operation.All cases were treated by hormone replacement according to the social gender which they selected.Thirty-three cases were followed up for 7 to 10 years. Conclmion The etiology of male pseudohermaphroditism includes anti-androgen syndrome,testosterone synthetase deficiency,resistant LH/hCG by testis,simple gonadal dysgenesis,mixed gonadal dysgenesis and resistant mullerian duct syndrome.
6.Deguelin down-regulates the expression of cyclin D1 gene in zebrafish embroys through the whole mount in situ hybridization

BACKGROUND:In the early development of zebrafish embryos, cels divide and proliferate rapidly, but low concentration of deguelin can delay the development of zebrafish embryos.
OBJECTIVE:To observe the effect of different concentrations of deguelin on cyclin D1 gene expression in zebrafish embryos.
METHODS:Though normal fertilization, zebrafish embryos that incubated to the 2-cel stage (about 0.75 hour after fertilization) and shield stage (6 hours after fertilization) were colected and put into 12-wel plates treated with 100, 200, 400 nmol/L deguelin at 28.5℃in an incubator til the shield period and 24 hours after fertilization, respectively. Simultaneously embroys treated with 1% dimethyl sulfoxide solution were as a control group, cultured in the same conditions. Cyclin D1 RNA probes were prepared for the whole mountin situhybridization, observing staining by an upright fluorescent microscope camera to detect the effect of deguelin on cyclin D1 expression in zebrafish embryos.
RESULTS AND CONCLUSION:Deguelin showed significant negative regulation on the expression of cyclin D1 gene in zebrafish embryos. Cyclin D1 expressed in outsourcing cels in embryos of shield stage, and a significant reduction in the expression of cyclin D1 came up with the increasing concentrations of deguelin. In the 400 nmol/L deguelin treatment group, there was nearly no expression of cyclin D1. Cyclin D1 expressed in the brain, central nervous system, immature eye, somites, trunk, and tail of embryos at 24 hours after fertilization, and reduced significantly in the 100 nmol/L deguelin treatment group, especialy in the proliferative area. In the 200 and 400 nmol/L treatment groups, the embryonic development slowed down signficantly, and cyclin D1 gene mainly expressed in the dorsal ectoderm cels.
7.Experience of surgical treatment for autoimmune hepatitis complicated with cirrhosis and hyper-splenism

Objective To investigate the effect of splenectomy on autoimmune hepatitis complicated with cirrhosis and hypersplenism.Methods Nine cases of autoimmune hepatitis complicated withcirrhosis and hypersplenism were treated with splenectomy and periesophagogastric devascularization.Results All patients recovered well.Their liver functions recovered to the preoperative level within twoweeks after operation.No significant differences were found between preoperative and postoperative levelsof transaminase and bilirubin(P >0.05).The count of leukocyte and platelet rose to the normal level andthere were significant differences between the preoperative and postoperative levels(P <0.05).Patients'symptoms of hypersplenism were corrected and their coagulation function improved significantly (P <0.05).Conclusion In patients with autoimmune hepatitis complicated with cirrhosis and hypersplenism,splenectomy with periesophagogastric devascularization is an effective procedure for improving hypersplenism,enhancing blood coagulation,and reducing bleeding.
8.Posture and the Bedrest Time after Percutaneous Coronary Intervention (review)

The patients must lie down on the sickbed for a period after percutaneous coronary intervention (PCI). Most patients will feel uncomfortable such as lumbago, backache, abdominal distension, insomnia, dysuria and etc. Therefore measures including adjusting posture, starting exercise as early as possible and preventing bleeding should be taken.
