1.Correlation between the expression of BAG-1, p53 and platinum-based chemotherapeutic sensitivity in NSCLC

Objective To detect the expression of BAG-1 and p53 in postoperative patients paraffin section of non-snmll cell lung cancer (NSCLC), and retrospectively analyze the relationships of clinical feature and platinum-based chemotherapeutic sensitivity in NSCLC, to further investigate the resistance mechanism of platinum. Methods The expression of BAG-1 and p53 was measured by immunohistochemistry in paraffin sections of 125 NSCLC postoperative patients. The existence and prognosis were analyzed after follow-up. Results BAG-1 and p53 showed high expression in NSCLC (positive rate 64.8 %, 46.7 %); BAG-1 positive or p53 negative expression had survival advantage(P<0.05); The group of BAG-1, p53 negative expression could benefit from platinum-based chemotherapy (P<0.05). Conclusion Expressions of BAG-1, p53 correlates with platinum-based chemotherapeutic sensitivity, and maybe become new types of clinical prognostic indicators and contribute to individualized treatment options for NSCLC.
2.Diagnostic values of quantitative dynamic contrast-enhanced MRI in evaluation of breast adenosis

Objective To investigate the value of quantitative dynamic contrast-enhanced MRI (DCE-MRI)in diagnosis of breast adenosis.Methods 54 patients with breast glandular lesions diagnosed as 4th or 5th grade in BI-RADS and proved by pathology un-derwent routine 3.0T quantitative DCE-MRI.Quantitative parameters including volume transfer constant (Ktrans ),plasma volume fraction (Vp )were measured.According to the pathological findings and MRI morphological characteristics,the lesions were divided into breast cancer group,adenosis group and control group.The contralateral healthy side of the adenosis group was regarded as control group.The quantitative parameters of the three groups were statistically analyzed.Results Pharmacokinetic parameters of Ktrans and Vp were (0.289±0.1 63)min-1 and 0.042 8±0.045 respectively in adenosis group (n=21),(0.959±0.45 1)min-1 and 0.057±0.079 in breast cancer group (n=33),and (0.048±0.022)min-1 and 0.01 6 ±0.01 9 in control group (n=21).Ktrans and Vp between adenosis and control group had statistical significance(Z =-5.733,-2.844,all P <0.05),Ktrans between breast cancer group and adenosis group had statistical difference (Z =-5.421,P =0.000 ).Vp between breast cancer and adenosis group had no statistical difference (Z=-0.009,P =0.993).Area under curve (AUC)of the receiver operator curve (ROC)to differentiate benign and malignant lesions of Ktrans were 0.941.When the cutoff were 0.304 min-1 ,the sensitivities and specificities were 93.9%,85.7%. Conclusion Quantitative DCE-MRI shows good diagnostic value in differentiation of breast glandular lesions and cancer.
3.Linear reference region model and Tofts model in dynamic contrast-enhanced MRI of discriminating benign and malignant breast lesions comparative study

Objective To investigate and eompare the diagnostic values of linear reference region model (LRRM) and Extended Tofts model in quantitative dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) analysis of benign and malignant breast lesions under different temporal resolutions.Methods Eight five suspicious breast cancer women underwent bilateral DCE-MRI exam, 37 patients were benigns and 48 patients were malignants.Among those, 31 patients(15 malignant, 16 benign) were scanned with 18 s per phase, and 54 patients(33 malignant, 21 benign) were scanned with 7 s per phase, and they were assigned into breast cancer group, benign lesion group and healthy gland group proven by surgery or biopsies.For the same model, Ktrans values of the three groups under different time resolution were first analyzed using Kruskal-Wallis H rank sum test.Receiver operator curve (ROC) was used to analyse the diagnostic efficiency of K values.Results Under high and low temporal resolutions, K values of the healthy group were (0.048 ±0.022) and (0.090±0.040)/min for extended Tofts model,(0.301 ±0.197) and (0.287±0.225)/min for LRRM model respectively.K values of the benign group were (0.289±0.163) and (0.211 ± 0.080)/min for extended Tofts,(0.624 ± 0.358) and (0.593 ± 0.165)/min for LRRM respectively.K values of the malignant group were (0.959±0.451) and (0.524±0.285)/min for extended Tofts,(1.576±0.935) and (0.956±0.180)/min for LRRM respectively.There were significant differences among the three different groups(P<0.05).Area under the ROC to differentiate benign and malignant breast lesions for Extended Tofts in high temporal, LRRM in high temporal, Tofts in low temporal and LRRM in low temporal were 0.941, 0.876, 0.850 and 0.933, with Ktrans cutoff values of 0.304, 0.917, 0.252 and 0.789/min,and sensitivity of 93.9%, 80.0%,80.0%, 80.0%;specificity of 85.7%, 90.5%, 81.2%, 87.5% respectively.Conclusion Under low temporal resolutions, Ktrans of LRRM model had better sensitivities and specificities in differentiation of benign and malignant breast lesions than Extended Tofts model, which was the opposite in high temporal resolutions.

The distributioa of Se in Hb and the effects of Se intake and its chemical forms oil the distribution were studied. The results showed that Se in human Hb was all located in globin and divided equally in a and ? chains. However, a part of Se could fall from Hb during the process of isolation when more Se as SeMet was taken and "excess" Se appeared in Hb. It was implicated that there were at least two kinds of linkage between Se and Hb --tight and loose.Regarding the components of amino acid in ? and ? chains, we assumed that a tight linkage between Se and Hb was in the form of SeMet, which was incorporated into Hb by dietary Se during the synthesis of Hb; the loose linkage was formed by a part of "excess" Se and CyS, especially No.93 CyS in ? chain, being unstable sclcnotrisulf ide or selenopersulfide, which broke easily by reduction during isolation. They might be utilized first by human body when Se was required.

Resident doctors participating in clinical teaching is a disputable topic.We analyze the current situation and difficulties in clinical teaching and propose the concept of RATS(residents as teachers) program in china aiming to strengthen the training of clinical teaching skills in residents and improving culture quality of resident doctors.And we also suggest the establishment of "clinical teaching data warehouses for resident doctors"to keep pace with requirements of our resident training program in new times.
7.Clinical application of G-lipopolysaccharides detection in patients with traumatic brain injury

Objective To study the application value of G-lipopolysaccharides(G-LPS) detection in patients with traumatic brain injury(TBI).Methods From Jan.2013 to Jun.2014,plasma G-LPS of TBI-patients and non-patients in this hospital were detected by using MB-80 microbiology analyzer and compared.Results of pathogenic microbiological detection of G-LPS positive TBI patients were analyzed,and G-LPS levels,detected before and after treatment,were also analyzed.Results G-LPS positive rate of TBI patients(33.33%) was higher than the 13.34% of non-TBI patients(P<0.05).After treatment,G-LPS level in G-LPS positive TBI patients significantly decreased(P<0.05).The common pathogenic bacteria,causing infection in TBI patients,included Acinetobacter bauman,Klebsiella pneumoniae and Escherichia coli,and the most common infection diseases included respiratory tract infections and urinary tract infections.Conclusion G-LPS detection could be used for the early detection of secondary infection in TIB patients,with significance for guiding clinical treatment.
8.Development of clinical laboratory vehicle for field rescue

Objective To develop a clinical laboratory vehicle with advantages in mobility,deployment,automation and informatization to meet the requirements of disaster relief.Methods Ford transit V348 ambulance was used as the base,which was equipped with routine blood analyzer,routine urine analyzer,biochemical analyzer and etc.The same instruments,agents and procedures were adopted as those in the clinical laboratory department of rear hospital,and LIS,air cleaning system and necessary auxiliary units were also involved in the modification.Results The clinical laboratory vehicle increased the working efficiency while decreased the errors.Conclusion The vehicle fulfills mobile medical support for military operations other than war such as disaster relief,and thus is worthy promoting practically.
9.Protective effect of polysaccharides from Lycium ruthenicum Murray against ultraviolet B radiation-induced photodamage in HaCaT cells

Objective To evaluate preventive effect of polysaccharides from Lycium ruthenicum Murray against photodamage in HaCaT cells,and to explore its possible mechanism.Methods Ultrasoundassisted extraction was used to extract polysaccharides from Lycium ruthenicum Murray in Qaidam Basin.In vitro cultured HaCaT cells were randomly divided into 3 groups:control group receiving no treatment,ultraviolet B (UVB) group irradiated with 30 nJ/cm2 UVB alone for 1 hour,experimental group pretreated with 2 g/L Lyeium ruthenicum polysaccharide solution followed 6 hours later by 30 mJ/cm2 UVB radiation for 1 hour.At twelve hours after UVB radiation,an inverted microscope was used to observe cell morphology.Then,MTS assay was performed to estimate cell proliferation,flow cytometry to detect cell apoptosis,an enzyme-labeled antigen method to detect levels of malondialdehyde (MDA) and glutathione peroxidase (GSH-Px),as well as to evaluate the activity of superoxide dismutase (SOD) and catalase (CAT),and enzyme-linked immunosorbent assay (ELISA) to measure levels of intedeukin-1 (IL-1) and tumor necrosis factor-alpha (TNF-α) in HaCaT cells and their culture supernatant.Results Compared with the control group,the UVB group showed obscure cell morphology,cell death and floating phenomenon,while cells became swollen but renained morphologically distinct in the experimental group.MTS assay revealed that the cell proliferative activity significantly differed among the above 3 groups (F =48.88,P < 0.01),and the cell proliferative activity was significantly lower in the UVB group (1.72 ± 0.12) than in the control group (2.34 ± 0.11,P < 0.05) and experimental group (2.11 ± 0.10,P < 0.05).Moreover,the apoptosis rate was significantly higher in the UVB group (82.41% ± 2.49%) than in the control group (3.98% ± 0.19%,P < 0.05) and experimental group (22.79% ± 0.97%,P < 0.05),as well as higher in the experimental group than in the control group (P < 0.05).Compared with the control group,the UVB group showed significantly higher levels of MDA,supernatant levels of IL-1 and TNF-α,and intracellular levels of TNF-α,but significantly lower GSH-Px levels and activity of SOl and CAT (all P < 0.05).However,there was no signifi-cant difference in the intracellular level of TNF-α between the UVB group and control group (P > 0.05).Conclusion Lycium ruthenicum polysaccharide has protective effects against photodamage in HaCaT cells,likely by reducing the synthesis and secretion of inflammatory substances as well as free radicals.
10.Effect of Dexamethasone on Bleomycin-induced Pulmonary Fibrosis in Rats

Objective To investigate the protective and cure effects of dexamethasone on bleomycin-induced pulmonary fibrosis.Methods32 rats were randomly divided into the control group (C-group, n=8), injury group (I-group, n=12) and dexamethasone group (D-group, n=12). The acute pulmonary model was established by intratracheal injection of bleomycin with rats of the I-group and D-group; while rats of the C-group injected with distilled water. After that, rats of the D-group were injected with dexamethasone sodium phosphate in intraperitoneal every day, those of the C-group and I-group were injected with saline. The animals were killed on the 3rd, 7th, 14th, and 27th days after treatment, and tests of bronchoalveolar lavage fluid (BALF), total lung collagen content and lung tissue processing were performed.ResultsPathological evidence of the I-group rats demonstrated that the alveolar compartment companied with massive inflammatory cell invasion and a number of myofibroblast proliferation became more thick. However, lung injury in the D-group rats got better than that in the I-group. Neutrophil percentage achieved peak in both I-group and D-group on the 7th day. But the neutrophil ratio in the D-group was significantly lower than that of the I-group on the 7th day ( P<0.05) and the 14th day ( P<0.01). Total lung collagen content achieved peak on the 14th day both in I-group and D-group, but that of the I-group was significantly higher than that of the C-group ( P<0.01) and the D-group was significantly lower than the I-group ( P<0.01).ConclusionDexamethasone plays a protective and cure role in lung fibrosis by efficiently inhibiting the gather and invasion of neutropils and restraining the increase of collagen secreted by proliferous fibroblasts.
