1.Thoughts of Establishing Stroke Unit with Characteristics of Traditional Chinese Medicine

This paper reports the situation of the treatment for stroke and introduces stroke unit (SU) in overseas hospitals : its important role in the emergency treatment and rehabilitation, the characteristics, advantages and managing modes. Moreover,it discusses the possibility and necessity of establishing SU with characteristics of traditional Chinese medicine.
2.New operative risk assessment for orthopaedic surgery(ORAOS)

[Objective]To establish a scoring standard of operative risk assessment for the orthopaedic surgery and evaluate the reliability of it.[Method]Anivariable analysis was performed for the preoperative and intraoperative clinical data of 768 patients undergoing orthopaedic surgery from Jan.2001 to Dec.2008,the risk factors associated with orthopaedic surgery were selected.Then multivariable analysis with logistic regression was performed for the selected risk factors.The scoring standard was made through these data.Reliability of the standard was evaluated by predicting of 160 patients in spinal surgery from 2006 to 2008,comparing with the real situation of each operation.[Result]Anivariable analysis showed age,heart function,blood pressure,electrocardiogram,sternum,liver function,renal function,albumin,blood sugar,number of operative variables within four weeks,magnitude,blood loss preoperation,timing of operation were risk factors.[Conclusion]The ORAOS could appropriately predict the occurrence of patients' postoperative complication before operation.
3.The influence of preconditioning on the activity of NF-?B and its myocardial protective effects

Objective: To investigate the influence of ischemic and pinacidil preconditioning on the activity of NF-?B and its myocardial protective effects.Methods: Eighty isolated rabbit hearts were randomized into five groups(Group C?K?I?P?G).Hearts were removed and mounted in a Langendorff apparatus perfused with 37℃ oxygenated Krebs-Henselet buffer.All groups were subjected to gobal myocardial ischemia for 40 min and reperfusion for 60 min or 120 min.Cardiac functions were monitored at 10 min equilibration ?15 min?30 min?60 min and 120 min reperfusion;Coronary flow(CF) was measured at 10 min equilibration and 30 min reperfusion;Expression of NF-?B P65 was determined at 10 min equilibration?60 min and 120min reperfusion.Results:(1)After ischemic reperfusion,the recovery of cardiac function and CF in Group I and Group P was significantly superior to that in Group C and Group K(P
4.lnfluence on long - term changes of refractive status in children after congenital cataract intraocular lens implantation

To discuss long-term changes of refractive status in children after congenital cataract intraocular lens ( lOL) implantation and analyze its related influence factors.METHODS: Seventy cases ( 110 eyes ) with congenital cataract from January 2013 to January 2014 in our hospital were selected, according to the age of patients, they were divided into group A (<2 years old), group B (2~6 years old) , group C ( 7 ~ 12 years old ) . They were all given cataract extraction combined with lOL implantation in congenital cataract. Corneal curvature, axial length, refractive status of three groups after operation were compared, the influence factors ocular axis changes were analyzed. RESULTS: There were no statistically significant differences in corneal curvature of groups A, B, and C after operation ( P>0. 05 );The differences had statistical significances of the corneal curvature of group A after surgery and 3mo after operation (P<0. 05), there were no significant differences of the corneal curvature of group B, group C after surgery and 3mo after operation (P>0. 05);the corneal curvature 3mo after operation of group A was significantly higher than that of groups B and C (P<0. 05), there were no significant differences in the corneal curvature between group B, and group C after 3mo ( P>0. 05 ); There were no statistical significances on axial length after operation in groups A, B, and C (P>0. 05);Ocular axial length difference of group A 3mo after surgery was significantly higher than that of groups B and C ( P < 0. 05 ), ocular axial length had no statistical difference of group B, group c 3mo after surgery ( P>0. 05). Refractive status of three groups after operation showed myopia moving trend, refractive change rate and refractive diopter of group A after operation was significantly higher than that of group B, group C, the difference was statistically significant (P<0. 05). CONCLUSlON: After congenital cataract extraction combined with lOL implantation, patients present myopia refractive state trend. Performance of children below 2 years old is significant. The main factor of myopia is axial growth.
5.Effect of nursing intervention on rehabilitation of stroke patients with hemiplegia

Objective To explore the effect of nursing intervention on rehabilitation of stroke patients with hemiplegia.Methods 60 cases of hemiplegic patients after stroke were divided into the control group and the experimental group with 30 cases in each group randomly.The rehabilitation status was evaluated and compared using self rating depression scale (SDS) and activities of daily living (ADL) scale before and after intervention.Results After nursing intervention,ADL score and SDS score were better,treatment effect was also better,but SDS total score showed no significant difference.Conclusions Nursing intervention can effectively reduce or eliminate depression of patients after stroke,strengthen subjective initiative of patients,so that patients can actively cooperate with the treatment,and promote disease rehabilitation.
6.Evaluation on rationality of curriculum of AIDS and drug abuse and its teaching quality

Objective To evaluate the teaching quality of AIDS and drug abuse and to provide evidences for teaching improvement.Methods Quantitative research and qualitative research was combined.Questionnaire including evaluation of curriculum,teachers and teaching quality was conducted among 273 students majored in anesthesiology and clinical medicine (2006 grade) as well as in forensic medicine,laboratory and stomatology (2008 grade) in quantitative research.Observation and indepth interview were used in qualitative research to observe the teaching process and receive students'feedback.Information about students' evaluation criterions and attitude toward teachers were collected by qualitative research.Chi-square test and descriptive statistic were used with SPSS 13.0.Results 90.8% students thought this lesson was necessary and important and the contents were accepted by them.24.2% students expected increasing study hours and having practice classes.Evaluation of course time,contents and importance were different by students of different majors.Average scores of students' evaluation on teachers' quality,teaching contents and teaching methods were 25.2,22.4,11.0,respectively.Three aspects of teaching methods got the lowest scores:vivid (2.71 points out of 3.0 points),interaction (2.71 points out of 3.0 points) and flexibility (2.72 out of 3.0 points).Interview results demonstrated that students have diversified demand for teaching methods and expect to increase practice lessons and multimedia teaching as well as to effective communicate between teaching and learning.Conclusions Course of AIDS and drug abuse is accepted by students.As a whole,students are basically satisfied with teachers,however,teachers and related sectors should pay more attention to teaching method improvement and teaching skill enhancement.
7.Atorvastatin reduces atherosclerosis in rabbits with hypercholesterolemia by inhibiting inflammatory response

Objective To investigate the roles of interleuldn (IL)-17,IL-10,and matrix metalloproteinase12 (MMP-12) in the pathogenesis of atherosclerosis and further to understand the intervention mechanism of atorvastatin for atherosclerosis.Methods Eighteen Japanese white rabbits were randomly divided into 3 groups:control,model and intervention groups (n =6 in each group).The control group,model group and intervention group were fed basic diet,high-fat diet,and high-fat diet plus atorvastatin (5 mg/(kg·d),respectively for 12 weeks.The levels of serum total cholesterol (TC) and triglyceride (TG) were determined by enzymatic colorimetric method; the levels of low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) were determined by selective precipitation method; the levels of IL-17,IL-10,and MMP-12 were measured by double antibody sandwich enzyme-linked immunosorbent assay (ELISA); the pathological morphology changes of atherosclerotic plaque was observe by HE staining; and the thickness of the intima,intima/media ratio,plaque area,and plaque proportion were calculated by image analysis software.Results The expression levels of serum TC,TG,LDL-C,and IL-17,as well as those of IL-17 and MMP-12 of the intervention group were significantly lower than those of the model group (all P <0.05),but they were still higher than the control group (all P <0.05),while there was no significant difference in the level of HDL-C.The level of serum IL-10 of the intervention group was higher than that of the model group and the control group (all P<0.05).The thickness of intima,intima/media ratio,plaque area,and plaque proportion of the intervention group were all significantly lower than those of the model group (all P <0.05).Correlation analysis showed that the plaque proportion was significantly positively correlated with IL-17 (r =0.847,P =0.001) and MMP-12 (r =0.839,P =0.001),and was negatively correlated with IL-10 (r =0.794,P =0.002); IL-17 was significantly positively correlated with MMP-12 (r =0.709,P=0.001),and was significantly negatively correlated with IL-10 (r =0.738,P<0.001).MMP-12 was significantly negatively correlated with IL-10 (r =0.563,P =0.015).Conclusion The increased levels of IL-17 and MMP-12,as well as the decreased level of IL-10 illustrate that inflammatory factors play an important pathological role in the pathogenesis of atherosclerosis.In addition to lipid-lowering effect,atorvastatin may decrease the degree of chronic arterial inflammatory response and play a role in anti-atherosclerosis by decreasing the expression of IL-17 and MMP-12,as well as increasing the expression of IL-10.
8.Relationship between urinary tract infection and age of heminephrectomy in duplex kidney infants

Objective To analyze the relationship between urinary tract infection and age of heminephreetorny in duplex kidney infants.Methods The records of infants diagnosed as a duplex kidney and received heminephrectomy were reviewed,who were admitted in the Third Affiliated Hospital of Zhengzhou University between Aug.2009 to Jul.2013.Results There were 26 duplex kidney infants altogether.Fifteen infants (57.7%) were in urinary tract infection group,age of heminephrectomy was from 2 months to 36 months (3 years),the median age of 7 months.Eleven infants (42.3%) were without urinary tract infection group,and the age of heminephrectomy was from 0.2 months (6 days) to 10.0 months,the median age was 4 months.The age of heminephrectomy was older in urinary tract infection group than that in non-infection group (P =0.045).In urinary tract infection group,all of the 15 infants had presence of lots of white blood cells in the urine before surgery and pyuria was found in the upper pelvis and the ureters when surgery,the result of urine culture was Escherichia coli (4 cases),Klebsiella pneumoriae (1 case),Pseudomonas aeruginosa (1 case),and culture negative 9 cases.In urinary tract infection group,infections could not be healed only by medical management,9 patients had fever and 2 patients had hypoalbuminemia,but all the symptoms were healed after surgery.In the group without urinary tract infection,all of the 11 infants had normal findings by urinalysis and urine culture,and fever and hypoalbuminemia were not present.There was no surgery complication related to lower ages of heminephrectomy and heminephrectomy for duplex kidneys was safe in small infants.Conclusions The rate of urinary tract infection is higher when the age of heminephrectomy is older in duplex kidney infants.Diagnosis and heminephrectomy should be performed early for infants with the poor kidney function since there is on sense to retain the damaged kidneys.

10.Clinical effect of alendronate sodium on bone metabolism and bone mineral density for treating the elderly female with osteoporotic distal radius fractures

Objective To investigate the clinical effect of alendronate sodium on bone metabolism and bone mineral density (BMD) for treating the elderly female with osteoporotic distal radius fractures.Methods Ninety elderly female with osteoporotic distal radius fractures patients were divided into treatment group and control group with 45 cases each according to the method of random number list.The two groups were given the medication treatment after 3 d of external fixation stenting.The control group was given calcium dimension D and the treatment group was given alendronate sodium and calcium dimension D.The course of treatment was 6 months.Before and after treatment,the BMD and bone metabolism [bone alkaline pbosphatase(BALP),tartrate-resistant acid phosphatase-5b(TRACP-5b)] were detected in both groups.The wrist function of two groups was evaluated.Results Before treatment,the level of BMD of treatment group and control group were (0.472 ± 0.157),(0.519 ± 0.173) g/cm2,there was no significant difference (P >0.05).After treatment,the level of BMD of treatment group and control group were (0.893 ±0.298),(0.571 ± 0.184) g/cm2,the BMD of treatment group after treatment was significantly higher than that before treatment and control group after treatment(P < 0.05).The level of BMD of control group after treatment had no significant difference compared with before treatment (P > 0.05).Before treatment,the level of BALP,TRACP-5b of treatment group and control group were (53.76 ± 17.92),(5.61 ± 1.87) U/L and (51.86 ± 17.29),(5.54 ± 1.85) U/L,the level of BALP,TRACP-5b after treatment had significant difference compared with before treatment and control group after treatment (P <0.05).There was no significant difference in control group before treatment and after treatment (P > 0.05).The good rate of rehabilitation of wrist function in treatment group was 91.1% (41/45),significantly higher than that in control group [51.1% (23/45)] (x2 =17.524,P <0.01).Conclusions The alendronate sodium has a role to suppress bone resorption and increase BMD.It can promote the recovery of wrist function.
