Objective To investigate clinical and pathological characteristics and the expression of interleukin (IL)-6, IL-17 and tumor necrosis factor (TNF)-α in elderly patients with primary nephrotic syndrome.Methods A total of 75 elderly cases with primary nephrotic syndrome were chosen as the observation group, and 40 healthy elderly persons served as the control group.Fasting serum IL-6, IL-17 and TNF-α levels were measured by enzyme-linked immunosorbent assay (ELISA) for both groups.Results 67 of the 75 patients received pathological diagnoses, of whom 46 had membranous glomerulonephritis, 5 had focal segmental glomerulosclerosis, 5 had mesangiocapillary glomerulonephritis, 3 had mesangial proliferative glomerulonephritis, 3 had IgA nephropathy, and the rest included one case for each of the following disorders: glomerular minor lesion, sclerosing glomerulonephritis, diffuse proliferative glomerulonephritis, segmental proliferative glomerulonephritis, and minor glomerular lesions.The expression of IL-6, IL-17 and TNF-α had statistically significant differences between the two groups.There were no statistically significant differences in the expression of IL-6 and IL-17 between patients with different pathological characteristics, and the expression of TNF-α was higher in membranous glomerulonephritis patients than in patients with other pathological characteristics.Conclusions Membranous glomerulonephritis is the main pathological type in elderly patients with primary nephrotic syndrome.Serum IL-6, IL-17 and most notably TNF-α levels are elevated in patients with membranous glomerulonephritis.

Objective To investigate the efficacy of Ginkgo‐Leaf Capsule combined with tripterygium glycoside in the treat‐ment of diabetic nephropathy(DN) and its influence on CysC ,Hcy and VEGF .Methods Eighty patients with DN treated in our hospital from January 2013 to April 2015 were selected as research subjects and equally divided into the control group and the ob‐servation group according to the random number table ,40 cases in each group .Besides the conventional Western medicine treat‐ment ,the control group was given tripterygium glycoside ,while the observation group was given Kinkgo‐Leaf Capsule combined with tripterygium glycoside .The treatment course was 3 months .Then ,the curative efficacy ,indices of renal function ,levels of Cy‐sC ,Hcy and vascular endothelial growth factor(VEGF) ,and occurrence situation of adverse reactions were compared between the two groups .Results The total therapeutic efficacy rate in the observation group and the control group was 92 .50% and 80 .00%respectively without statistically significant difference (P>0 .05) .In the comparison with the control group after treatment ,the re‐nal function indices of serum creatinine ,urea nitrogen ,24 h urinary protein ,level of serum CysC ,Hcy and VEGF in the observation group were statistically decreased (P< 0 .05) .During the treatment process ,no severe adverse reactions occurred in the two groups .The incidence rate of adverse reactions in the observation group and the control group was 17 .5% and 12 .5% respectively without statistical difference (P>0 .05) .Conclusion Ginkgo‐Leaf Capsule combined with tripterygium glycoside is effective and reliable for treating DN ,which can significantly improve the renal function ,reduces the level of serum CysC ,Hcy and VEGF .

Objective　To study the changes of the four active substances endothelin(ET),calcitonin gene related peptide (CGRP),thromboxane B2(TXB2) and 6-Keto-prostaglandin F1α(6-Keto-PGF1α) in plasma when they acted on each other in the formation and development of diabetic retinopathy.Methods　Using the method of radio-immunity,the levels of ET,CGRP,TXB2,6-Keto-PGF1α in plasma were measured in four groups of patients with type 2 diabetes (group 1∶40 cases without retinopathy;group 2∶40 cases with primary retinopathy;group 3.40 cases with hyperplasia type of retinopath;group 4∶40 cases normal controls).Results　The results showed that ET increased progressively (P＜0.01) with the prolonging of the duration of DM and the worsening of retina damage、CGRP showed a decreasing tendency (P＜0.05,P＜0.01) in the advanced stage of DM retinopathy.Rectilinear regression analysis showed that the association of ET/CGRP with TXB2/6-Keto-PGF1α became closer with the worsening of deabetic retinopathy (r=0.44,P＜0.05;r=0.596,P＜0.01).Conclusion　The four active substances acted on each other in the formation and development of diabetic retinopathy.It had a significent meaning in the initial mechanism of DM,in the development of disease and in the direction of medication usage to understand their relationship.

Objective To explore the influencing factors of ALT abnormality among workers from food industry and related people from public places in Shenzhen and influencing factors.Methods 2,411 workers from food industry and people from public places who had physical examinations in our department from May to October in 2013 were involved in the investigation.ALT abnormality rate and the influencing factors were analyzed.Results The incidence of ALT abnormalities among workers from food industry and related people from public places in Shenzhen was 9.37%.The ALT abnormalities were correlated with censue register,gender,age,marital status,work duration in Shenzhen,education level and monthly income respectively(all P<0.05). Conclusion The ALT abnormality has a higher rate among people from cities,of male gender and old age,with longer work duration in Shenzhen and relatively high level of education and higher monthly income.Therefore,for the population,the health education should be strengthened and the healthy lifestyles should be advocated to effectively reduce the ALT abnormality rate.

Objectiye To investigate the expression of regulated on activation, normal T-cell expressed and secreted (RANTES) and occluding in brain, and investigate the role of expression of RANTES in the change of blood-brain barrier permeability of rats with acute pancreatitis. Methods 49 Sprague-Dawley rats were randomly divided into 7 groups according to random number table, including sham-operated group (SO) , acute edematous pancreatitis (AEP) 3, 6 h group and acute necrotizing pancreatitis (ANP) 3, 6, 12 and 24 h group. AEP and ANP were induced by retrograde injection of 0. 5% and 5% sodium taurocholate, respectively. RT-PCR, Western blot, and immunohistochemistry methods were performed to assess the expression of RANTES and occludin mRNA and protein in brain tissue. Results The RANTES mRNA expressions in SO, AEP 3, 6 h, ANP 3, 6, 12, 24 h group were 0, 0, 0, 0.36±0.05, 0.47±0.04, 0.65±0.05, 0.83±0.07, respectively; The RANTES protein expressions were 0, 0, 0, 0.42±0. 03, 0. 57±0.04,0.78±0.08, 1.05±0.08, respectively; the values in ANP group were significantly lower than those in SO group and AEP group (P<0.01). The occludin mRNA expressions were 1.21±0.07,1.17±0.07, 1.15±0.08,0.84±0.07,0.77±0.05,0.64±0.09,0.56±0.09, respectively, the occludin protein expressions were 1.18 ±0.08, 1. 16 ±0. 10, 1. 11 ±0. 10, 0. 90 ±0. 03, 0. 65 ±0."05, 0.57 ±0.05, 0.48 ±0.05, respectively, the values in ANP group were significantly lower than those in SO group and AEP group (P< 0.01). The expression of RANTES was positively related to the pancreatic pathologic score (r = 0. 936,P< 0. 001) ; the expression of occluding was negatively related to the pancreatic pathologic score (r = - 0. 943, P < 0.001). The expression of RANTES was negatively related to the expression of occluding (r = -0. 943, P <0. 001). Conclusions The expression of RANTES was progressively increased in the brain tissue in rats with ANP, while the expression of occluding was progressively decreased; RANTES may play an important role in the change of blood-brain barrier permeability via down-regulating the expression of occluding.

It has been shown that Spirulina platensis can regulate imminological functions.We report here that crude extract and purified components (phycocyanin and polysaccharide) from Spirulina platensis can induce secretion of IL 2 in splenocyte of BALB/C mice by means of MTT method.In the present study,we showed that all experimental components can't enhance proliferation of CTLL which was used in MTT method,but induce IL 2 secretion in splenocyte of BALB/C mice in three different concentration (0.01,0.1,1 g?L -1 ).Indeed the purified components especially phycocyanin part showed stronger IL 2 inducing activity than the crude one.IL 2 level was grow up when the incubation time of splenocyte and Spirulina platensis increased.In the concentration of 1 g?L -1 ,detected Spirulina platensis in our study assist IL 2 inducing of ConA (2mg?L -1 ) in splenocyte of BALB/C mice.

Objective To analyze driver genes status and its clinical characteristics of advanced lung adenocarcinoma, then evaluate the status of first-line treatment in a single centric real-world. Methods EGFR, ALK, ROS-1 gene in 204 advanced lung adenocarcinoma tissue were tested by ARMS-PCR method. And the relationship between driver genes status and clinical characteristics was analyzed as the first line treatment in real clinical practice. Results The positive rate of driver genes status in 204 advanced lung adenocarcinoma was 53.9％ (110/204) , including EGFR mutation 46.1％ (94/204) , ALK positive 6.4％ (13/204) and ROS1 positive 1.5％ (3/204). The driving genes status was significantly correlated with gender, smoking history, tumor staging and serosal invasion (P < 0.05). There were significantly differences among the proportion of first-line standard treatment in different subgroup (P = 0.000) , the first-line standard treatment rate of EGFR mutation, ALK/ROS1 positive and drive gene negative were 77.7％, 37.5％, and 46.8％ respectively. And the ratio of using 1 st generation EGFR-TKIs in all patients is 70.6％ (60/85). Conclusion More than half of advanced lung adenocarcinoma have driver genes changes, and EGFR-TKI first-line treatment has higher acceptability in real-word.

OBJECTIVE: To explore application of endoscope assisting in retrosigmoid approach vestibular schwannoma resection and its signification. METHOD: Through retrosigmoid approach with endoscope assisting, vestibular schwannoma in the cerebellopontine angle was removed in 12 cases. Assessment of function of facial nerve and auditory was made for all patients preoperative and postoperative. RESULT: Vestibular schwannoma in 12 patients were removed completely, then the facial nerve and the acoustic nerve were preserved after tumor removal. No complication was found in six months. We observed the hearing loss in postoperative patients, and there is no difference in facial nerve function between preoperative and postoperative. The alteration of auditory and facial nerves functions is no difference between two retrosigmoid approach groups with or without endoscope assisting. CONCLUSION Application of endoscope assisting in retrosigmoid approach can be helpful for totally resection of vestibular schwannoma in inner acoustic meatus without lesion of canalis semicircularis and glomus jugulare. Application of the endoscope assisting in retrosigmoid approach is helpful to provide the precise information of CPA anatomic structure and decrease the incidence rate of complication.

OBJECTIVE: By watching the effects of cochlear implants in the children with cochlear nerve defects, to evaluate the relationship and efficiency of HRCT AND MRI in diagnostic for cochlear nerve defects. METHOD: The imaging dates and the post-operative effects of the 10 cochlear nerve defect patients were retrospectively analyzed. RESULT: Three patients with bilateral IAC stenosis by HRCT and cochlear nerve defects by MRI but no other abnormal. Seven other patients showed normal IAC by HRCT including 1 showed type IP- I plus vestibular semi circular canal dysplasia. These 7 patients MRI showed 3 bilateral cochlear nerve absent and 4 bilateral cochlear nerve dysplasia. Ten cases of children after 12-32 month follow-up, 3 patients began speaking simple language, whose MAIS scale, CAP and SIR scores improved. One case just began speaking but pronunciation vague. Four cases had reactions to sounds, and can say monosyllabic words. Two cases without effects (1 CI abolished). Compared with other patients with normal cochlear nerve, the effects of the 10 cases were disappointed. CONCLUSION The effects of cochlear implant for the patients with cochlear nerve defects were poor. The preoperative MRI as sessment including oblique sagittal view for cochlear nerve is necessary. The preoperative communication with the parents telling ineffective result possibility is important.
