1.Iron metabolism and neonatal hypoxic-ischemic encephalopathy
International Journal of Pediatrics 2010;37(6):604-605,609
Iron homeostasis is essential for the maintenance of the body. Iron transports oxygen and electron, and participates in the synthesis of neurotransmitters and myelin. However, iron overload in the body is harmful. In recent years studies have found iron deposition would be poisoned significantly, particularly in the central nervous system tissues and nerve cells, which will cause pathological changes of central nervous system. This paper describes the iron and cycled iron changes in hypoxic-ischemic brain, abnormal iron metabolism and mechanisms of HIE.

Objective: To study protective effect of taurine on gut barrier after hemorrhagic shock-reperfusion. Method: Twenty-four white rabbits were divided randomly into 3 groups: taurine group (A),shock group (B), control group (C). Result: Malondialdehyde and Ca~(2+) contents of intestinal mucosa remained unchanged in group A, but increased significantly in group B, Incidence of bacterial translocation in B group was markedly higher than that in C group at 30 min following reperfusion. Under light and electronic microscope,in comparsion with A and C groups, intestinl mucosa damage in B group became more severe. Conclusion: Taurine can protect gut barrier from intestinal ischemia reperfusion injury induced by hemorrhagic shock through reducing oxide free radical and preventing calcium overload.

Objective To determine the changes of blood cytokines values and inductions of cytokines expressions after CPB and cardiac valve replacement. Methods Thirty patients undergoing cardiac valve replacement were randomly selected, The plasma cytokines levels or activities and inductions of cytokine expressions were measured before CPB, 1st, 3rd, 7th, 14th d after operation. Results Compared with the baselines, IL 1 activity and souluble interleukin 2 receptor(SIL 2R) level increased on the 1st, 3rd d , then decreased significantly on the 7th d, plasma IL 8 level increased in 14 d after CPB, but IL 2 activity decreased during 7 d postoperatively(P0.05); induction of IL 1 expression increased significantly on the 1st d and decreased on the 3rd d (P

To solve the problems of induction of anesthesia in patients with extreme trachea occulusion, femoral-femoral cardiopulmonary bypass (FFCPB) was used in 9 patients with nearly total tracheal obstruction prior to induction of anesthesia. After FFCPB was set up under local anesthesia, general anesthesia and surgery began soon. The results showed the patients were quiet and co-operative 3 to 5 mins following FFCPB with the disappearance of tissue hypoxia syndrome and attenuation of hypertension and taehycardia. During FFCPB PaO_2 increased from 7.6~9.8 to 16~40kPa, PaCO_2 decreased from 7.4~8.2 to 4.5~5.9 kPa, SaO_2 increased from 71.6~79.9 to 98.0~99.9%, urine volume, blood pressure and heart rate were maintained at satisfactory levels. The results indicate that FFCPB prior to induction of anesthesia is a rapid, safe and effective method in rescuing patients with nearly complete obstruction of the main trachea.

Objective: To observe the effects of salvia miltiorrhizae compound (SMC) on serum endothelin (ET), TXB_2 and PGI_2 levels in patients undergoing CPB. Method: Twenty patients with congenital heart disease were randomly divided into control group (group Ⅰ) and treated group (group Ⅱ). SMC (200mg/kg) were given intravenously in group Ⅱ before operation and during rewarming, in group Ⅰ eqivalent volumes of normal saline were administered at the same time point. The serum ET,TXB_2 and PGI_2 levels were measured at preoperation (T_0) 5min (T_1), and 30min (T_2) following CPB, 10min (T_3), 30min (T_4) and 60min (T_5) following reperfusion, 24h (T_6) after operation. Result: In both groups, the serum ET level were increased significantly during reperfusion, but much less in group Ⅱ than that in group Ⅰ at T_4, T_5 and T_6(P

Objective Crossclamping of aorta causes a sudden increase in systemic vascular resistance. Spinal cord may suffer ischemic injury during aortic aneurysm surgery. This study was designed to evaluate the hemodynamic and and spinal cord protective effects of captopril during suprarenal aortic crossclamping in rabbits. Methods Twenty-four albino rabbits were randomly divided into three groups :sham operation group (A),control group (B) and captopril group (C). The descending aorta was crossclamped above the level of renal artery for 30 min and then declamped. In group C a bolus of captopril 2 mg?kg -1 was given intravenously 10 min before crossclamping followed by a 0.5mg?kg -1?min -1 infusion for 30 min. The same volume of normal saline was given in other 2 groups. The carotid artery pressure(BP 1) and femoral artery pressure(BP 2) were recorded before injection, 5, 20,30min after crossclamping and 5,60,180min after declamping. Spinal cord was examined at the level of T 10 and L 3 for morphological changes and MDA content 180 min after declamping. Results 1. BP 1 increased significantly after crossclamping in group B. In group C there was no significant change in BP 1 after crossclamping. In group C there was no significant change in BP 1 after crossclamping. In group B BP after declamping was significantly lower than that before crossclamping. In group C BP at 60 and 180 min after declamping was significantly higher than that at the same intervals in group B(P

Secreted protein acidic and rich in cysteine (SPARC) is expressed in esophageal cancer abnormally. On the one hand, recent studies show that high SPARC expression is correlated with progression and metastasis of esophageal cancer. On the other hand, high SPARC expression increases chemosensitivity and improves short-term efficacy in patients.

International Journal of Surgery 2012;(12):832-836,封3
Anastomotic leak belongs to the serious complications of low anterior resection with high morbidity and fatality. In recent decades, many strategies aimed at lowering the incidence of anastomotic leakage have been developed. This review focused on the methods for preventing anastomotic leakage through searching PudMed and Wanfang data for all related papers.Strategies were categorised as defunctioning stoma, transcecal catheter ileostomy, indwelling rectal tube,valtrac-secured intracolonic bypass technique, free take-back ileostomy.Every strategy has its own advantages and disadvantages. But to date, except defunctioning stoma, none of the methods has been widely accepted due to the lack of high level evidences. However, free take-back ileostomy can avoid stoma related complication and readmission for closure and its initial effect is good,so deserve to further research.

Acute respiratory distress syndrome (ARDS) is a common clinical critical condition of the respiratory system, with a high fatality rate and lack of effective drug treatment, hence it is a global medical problem to be solved urgently. Recently, with the in-depth research on extracellular vesicles (EVs) at home and abroad, more and more evidences suggest that EVs play a critical role in the initiation, development and progression of ARDS and have potential clinical application value. The role of different cells derived EVs in ARDS will be reviewed in this paper.
