Introduction
In this review, we present: (1) trends of environmental lead exposure of global dimension including Japan, (2) environmental lead exposure issue in the field of international health, (3) effects of environmental lead exposure in mothers and children, (4) introduction of our research in Kunming, China, and (5) further consideration. Lead poisoning is one of the oldest diseases to humankind because human has been used lead for long time to fabricate various products. Industrial uses of lead have decreased sharply in many industrially developed nations including Japan in the past several decades. However, environmental lead exposure still remains an important problem because of rapid industrialization in less developed nations and the persistence of lead in the environment. Since elevated blood lead levels of immigrating children and people who are exposed to lead from imported consumer products have been reported, health-care providers should be aware of the information for appropriate follow-up care. In addition, lead is a well-known human reproductive toxin, and moreover, several studies have demonstrated that lead can be mobilized from the maternal skeleton during pregnancy and lactation. It is very important to prevent lead exposure for not only mothers and children but also women in pre-pregnancy period. Health burden of environmental lead exposure are only a part of issues of environmental lead pollution. To solve this problem, health-care providers should state expert opinion and collaborate with other field of specialists.
2.EFFECTS OF PHYSICAL EXERCISE ON FEMORAL BONE MINERAL DENSITY AND OSTEOCYTE MICROMORPHOLOGY IN YOUNG OVARIECTOMIZED (OVX) RATS

To investigate the effects of exercise on femoral regional bone mineral density and osteocyte micromorphology in young ovariectomized (OVX) rats, animals were divided into an ovariectomized group (OVX group), an ovariectomized + exercise group (OVX + exercise group), and a sham surgery group (SHAM group). Femoral bone mineral density was significantly lower in the OVX and OVX + exercise groups than in the SHAM group. A large difference in bone mineral density in the distal femoral epiphysis was observed between the OVX and OVX + exercise groups. In the OVX group, bone lacunae showed less immunostaining for DMP-1(dentin matrix protein-1) and osteocyte processes were fewer than in the SHAM group. In the OVX + exercise group, osteocyte processes were thicker (0.44 to 0.66 μm) than in the OVX group (0.22 to 0.26 μm). These results indicate that exercise induced structural changes in the femur, including formation of osteocyte processes and strengthening of the communication network between osteocytes.
3.A Case of Off-Pump Coronary Artery Bypass for Acute Myocardial Infarction with Cardiogenic Shock.

A 67-year-old man was considered a candidtate for CABG because coronary angiogram showed obstruction segment 6 and stenoses of segments 9 and 12. He underwent emergency CABG due to acute myocardial infarction (AMI) with cardiogenic shock caused by hemorrhage from a gastric ulcer. Because of hypoxia due to pulmonary edema and acute renal failure an intraaortic baloon was inserted. He had a history of cerebrovascular stroke. Although coronary angiogram revealed multiple vessel disease, we performed off-pump coronary artery bypass (saphenous vein graft-left anterior descending artery) for salvage, because cardiopulmonary bypass was considered very risky and further systemic heparinization might be fatal. He has returned to his job, and is now free from angina. As AMI with cardiogenic shock is often caused by a lesion in the LAD, CABG without cardiopulmonary bypass may be an effective technique in certain selected patients.
4.A Third CABG Procedure (Axillo-Coronary Bypass) Using the MIDCAB Technique.

A 77-year-old man had undergone CABG (coronary artery bypass grafting) (SVGs (saphenous vein grafts) to LAD (left anterior descending coronary artery), OM (obtuse marginal) and RCA (right coronary artery)) 15 years previously. Three years previously, he underwent CABG again (LITA (left internal thoracic artery)-OM, RGEA (right gastroepiploic artery)-RCA) due to recurrence of angina pectoris, but there was no evidence of graft disease in the SVG to the LAD. Six months before the present procedure, graft disease developed in the SVG to the LAD and caused unstable angina pectoris. Therefore, the left axillary artery was bypass grafted to the coronary artery (LAD) using SVG without cardiopulmonary bypass by means of the MIDCAB (minimally invasive direct coronary artery bypass) technique. The patient has had no angina pectoris subsequently. Postoperative angiography revealed that the graft was patent. The axillo-coronary (LAD) bypass appears to be a useful procedure for re-revascularization to the LAD in patients with no available arterial graft, such as ITA (internal thoracic artery) or RGEA.
5.Noninvasive Positive Pressure Ventilation (NIPPV) for Patients with Acute Respiratory Failure after Cardiovascular Surgery.

NIPPV provides positive pressure ventilation through a face mask without intubation. We performed NIPPV for 2 patients with acute respiratory failure following cardiovascular surgery. (Case 1) A 63-year-old man, who had had COPD (Hugh-Jones class III), underwent replacement of the aortic arch. He was extubated after 5 days. However, he was re-intubated under controlled ventilation because of deterioration of his respiratory condition. The patient had NIPPV after extubation on postoperative day 14 because he was alert and had no cardiovascular compromise. On the 18th postoperative day he was weaned from NIPPV. (Case 2) A 67-year-old man underwent coronary artery bypass surgery. On the next day he was extubated, but he suffered from hypoxemia due to impaired respiratory condition on postoperative day 3. The patient underwent NIPPV instead of conventional mechanical ventilation because his condition was stable except for respiration. Respiratory condition improved quickly and he was weaned from NIPPV on the 7th postoperative day. NIPPV is an effective method for managing patients with acute respiratory failure after cardiovascular surgery.
6.A case of successful recovery and long term survival from postcardiotomy cardiogenic shock by means of left ventricular support using a centrifugal pump with IABP.

A 49-year-old man who had had severe tripple vessel coronary disease and low left ventricular function (EF=29%) underwent coronary artery bypass grafting. Following the procedure he could not be weaned from cardiopulmonary bypass (CPB) even with an intra-aortic balloon pumping (IABP). Left ventricular assist using a centrifugal pump (CFP) together with IABP was then utilized with a dramatic recovery from profound postcardiotomy cardiogenic shock, and the CPB was successfully terminated. The left ventricular function recovered gradually from intraoperative myocardial damage. The CFP was successfully removed at 86 LHB hour and the IABP at 9th postoperative day. At present 12 months after LHB, he reveals no angina. Settting up the left ventricular support using a CFP is simple and not-time-consuming, so this system with IABP is of practical use in the case of unpredicted postcardiotomy cardiogenic shock.
7.Prognosis of biopsy-confirmed metabolic dysfunction- associated steatotic liver disease: A sub-analysis of the CLIONE study

Background/Aims:
Metabolic dysfunction-associated steatotic liver disease (MASLD) was recently proposed as an alternative disease concept to nonalcoholic fatty liver disease (NAFLD). We aimed to investigate the prognosis of patients with biopsy-confirmed MASLD using data from a multicenter study.
Methods:
This was a sub-analysis of the Clinical Outcome Nonalcoholic Fatty Liver Disease (CLIONE) study that included 1,398 patients with NAFLD. Liver biopsy specimens were pathologically diagnosed and histologically scored using the NASH Clinical Research Network system, the FLIP algorithm, and the SAF score. Patients who met at least one cardiometabolic criterion were diagnosed with MASLD.
Results:
Approximately 99% of cases (n=1,381) were classified as MASLD. Patients with no cardiometabolic risk (n=17) had a significantly lower BMI than patients with MASLD (20.9 kg/m2 vs. 28.0 kg/m2, P<0.001), in addition to significantly lower levels of inflammation, ballooning, NAFLD activity score, and fibrosis stage based on liver histology. These 17 patients had a median follow-up of 5.9 years, equivalent to 115 person-years, with no deaths, liver-related events, cardiovascular events, or extrahepatic cancers. The results showed that the prognosis for pure MASLD was similar to that for the original CLIONE cohort, with 47 deaths and one patient who underwent orthotopic liver transplantation. The leading cause of death was extrahepatic cancer (n=10), while the leading causes of liver-related death were liver failure (n=9), hepatocellular carcinoma (n=8), and cholangiocarcinoma (n=4).
Conclusions
Approximately 99% of NAFLD cases were considered MASLD based on the 2023 liver disease nomenclature. The NAFLD-only group, which is not encompassed by MASLD, had a relatively mild histopathologic severity and a favorable prognosis. Consequently, the prognosis of MASLD is similar to that previously reported for NAFLD.
