1.A Case of Expansion of a Right Internal Iliac Artery Aneurysm after an Exclusion Operation

This is a case report of an expansion of the right internal iliac artery aneurysm after an exclusion operation. A 72-year-old man, had undergone aneurysmectomy and graft replacement of a right external iliac artery aneurysm and a ruptured left iliac artery aneurysm, and exclusion of a right internal iliac artery aneurysm in 1995. Computed tomography showed an expansion of the right internal iliac artery aneurysm in 2003. We performed graft replacement and bypassing to the right external iliac artery. The patient had a satisfactory postoperative course.
2.A diagnosis of cerebral infarction was obscured by the symptoms of advanced skin cancer and dementia

The assessment of suffering in cancer patients affected by dementia can be challenging. We report a patient with advanced skin cancer and dementia who was admitted to our hospital without appropriate diagnosis as having cerebral infarction at previous hospitals. Case:An 85-year-old woman presented with existing cognitive dysfunction from 2009 and progression of a left facial skin tumor existing from 2013. She was admitted to hospital A for facial skin tumor with dysphagia, and diagnosed as having advanced skin cancer with dementia in June 2014. After palliative therapy for the skin tumor, she was transferred to hospital B. The physicians at both hospitals explained the presenting symptoms as complications of her advanced skin cancer and dementia. She was admitted to our hospital at 36 days after symptom onset, and we diagnosed subacute cerebral infarction based on head CT and MRI examinations. In this case, the patient’s advanced cancer and dementia might have interfered with the recognition of her symptoms of cerebral infarction. A detailed interview on admission might have contributed to our decision for further examination. Although the diagnosis of cerebral infarction could not palliate her physical symptoms, it might assist the family to recognize her suffering or to provide care perceiving her poor prognosis. Additionally, the diagnosis might have an effect for emotional satisfaction of the family.
3.The difference between effects of "power-up type" and "bulk-up type" strength training exercises. With special reference to muscle cross-sectional area, muscular strength, anaerobic power and anaerobic endurance.

In this study, the difference between the effects of “power-up type” and “bulk-up type” strength training exercise was investigated by analyzing parameters such as structural and functional adaptations in the neuromuscular system. Eleven subjects were divided into power-up and bulk-up groups. The power-up group comprised five male subjects who performed 5 sets at 90% of one repetition maximum (1 RM) with a 3-min rest between sets (repetition method) . The bulk-up group comprised six male subjects who performed 9sets at 80-60-50%, 70-50-40%, and 60-50-40% of 1 RM with rest intervals between sets of either 30 s or 3 min (interval method) . Both groups performed isotonic knee extension exercise twice a week for 8 weeks. The power-up group showed a lower rate of improvement than the bulk-up group in terms of cross-sectional area (CSA) of the quadriceps femoris at levels 30%, 50% and 70% from the top of the femur, and also in average isokinetic strength (Isok. ave. ; 180 deg/s, 50 consecutive repetitions) . However, the power-up group showed a greater rate of improvement in 1 RM, maximal isometric strength (Isom. max), and maximal isokinetic strength (Isok. max ; 60, 180, 300 deg/s) . Furthermore, the rate of reduction in strength over 50 consecutive isokinetic repetitions decreased in the bulk-up group. On the other hand, the power-up group showed no significant changes in the above throughout the entire training program. These results indicate that the characteristics of the two types of training exercise are as follows : (1) power-up exercise is effective mainly for improving muscular strength and anaerobic power, and (2) bulk-up exercise is effective mainly for improving hypertrophy and anaerobic endurance. These findings support the idea that “power-up type” and “bulk-up type” strength training exercises should be applied appropriately according to the training aim.
4.Effects of resistance training on CO2 excess and swimming performance in competitive swimmers.

The purpose of the present study was to determine the change in total excess volume of CO2output (CO2excess) due to bicarbonate buffering of lactic acid produced during exercise and change in swimming performance following resistance training for 8 weeks in competitive swimmers. Ten healthy university competitive swimmers were assigned to either a resistance training and swimming training group (COMBINE: N=5) or a swimming training only group (SWIM: N=5) . Muscle mass was measured using magnetic resonance imaging (MRI) . CO2excess and blood lactate concentration were measured during incremental exercise on a cycle ergometer and swimming performance was measured during competition. COMBINE showed a significantly higher percentage change in muscle mass (11.1±4.5%) than SWIM (3.5±2.5%) . The percentage change in CO2excess, CO2excess per body weight (CO2excess/BW) and CO2excess/BW per blood lactate accumulation (CO2excess/BW/ΔLa) during exercise was significantly higher in COMBINE (107.3±60.1, 102.6±56.8, 59.1±37.7%, respectively) than in SWIM (42.5±10.0, 42.9±10.4, 13.4±22.4%, respectively) . The percentage change in swimming performance was significantly higher in COMBINE (2.2±1.8%) than in SWIM (-2.0±3.6%) . A negative correlation between percentage change of muscle mass and percentage change of CO2excess/BW/ ΔLa (SWIM: r=-0.993, P<0.01, COMBINE: r=-0.744, P>0.05) was found. It was suggested that combined swim and resistance training resulted in greater increases in the bicarbonate buffering system (CO2excess/BW/ΔLa) . However, increases in muscle mass may have subsequently caused a relative decrease in the contribution of the bicarbonate buffering system.
5.Association between the Kihon Checklist and Bone Density in Elderly Women

The purpose of this study was to accumulate fundamental data for use in the future project for preventing or delaying institutionalization of the aged by clarifying the relationship between the Kihon Checklist and bone density. All the subjects were residents of the southern part of Akita Prefecture in the 65-74 age band. The Kihon Checklist was used. The women who marked the items on the list related to “intraoral conditions” tended to be low in bone density. Those who checked off the items related to “oral functions” -- the standard of judgment in determining the level of care needed - were inactive and depressed. Since bone density was an indicator of whether the elderly women were in need of nursing care, we thought it of paramount importance to encourage them to participate in the screening for osteoporosis on a regular basis and to take measures to maintain or improve bone density.
6.Effects of Drinking Habits on Results of Vital and Liver Functions Tests: A Survey of Old People in Rural Area

In view of the situation where an increasing number of old women as well as old men have health problems associated with drinking, some measures should be taken in terms of setting sensible limits of alcoholic intake for these people to keep to, the Report of Kenko (Health) Nippon 21 for 2011 warned. It also suggested that further studies be made to amass necessary data in order to introduce effective measures and give a right assessment to the situation.
The present study was carried out with the aim of providing fundamental pieces of information usable in primary prevention of alcohol-related health problems among the aged. For this purpose, we looked into the drinking habits as well as the numerical data of vital and liver functions tests of old people in a rural area in Yokote, Akita Prefecture. A total of 448 old people (206 men, 242 women) were involved in this study. Nineteen items out of 25 in the Kihon-Checklist, which was designed to reduce the need for nursing care, revealed notable decreases in vital functions in those men who took in over 21 grams of alcohol per day. Of those men specially categorized as individuals who are 65 and older and who are regarded as very likely to need nursing care in the near future, 88.9% took in 21 grams of alcohol. In women, there was no relation between drinking habits, BMI and liver functions. However, it was found that daily alcoholic consumption was greater in those specially categorized group of women at high risk for institutional care than in the other women.
Our study made it clear that drinking habits, which were found to bear on liver and vital functions, could serve as a yardstick for judging whether the old people are at high risk for institutional care. We concluded that it is necessary to spread the knowledge about the adverse effects of alcohol among the elderly and advise them to act their age and drink in moderation even before they enter advanced age.
7.Secondary Squamous Cell Carcinoma of the Oral Cavity in Young Adults after Hematopoietic Stem Cell Transplantation for Leukemia

Two cases of secondary oral squamous cell carcinoma (SCC), which developed in recipients of hematopoietic stem cell transplantation (HSCT) for leukemia, are reported. The first patient underwent allogeneic HSCT for chronic myelogenous leukemia at 32 years of age. He suffered from chronic graft-versus-host-disease (GVHD) of the oral mucosa after HSCT, and has subsequently received immunosuppressive therapy. He experienced metachronous multiple SCCs in the maxillary gingiva and the dorsum of the tongue at 36 years and 40 years of age, respectively. The second patient received autologous HSCT for acute myelogenous leukemia at 22 years of age, and she did not experience GVHD after transplantation. SCC developed in the lateral border of the tongue at 27 years of age. PCR analysis detected both HPV16 and HPV18 in the tongue tumor of the first patient, and only HPV18 in that of the second patient, suggesting that the infection of high-risk HPVs was possibly involved in the development of post-HSCT oral cancers in these patients. Since risk factors for post-HSCT oral SCC are not yet well recognized, long-term close follow-up is necessary for the early detection of secondary oral cancers in all transplant recipients.
8.Effect of Electro-Acupuncture Stimulation on Blood Flow in Trapezius Muscle-With A New Clearance Method-

[Purpose] We know of no reports on the effects of electro-acupuncture (EA) stimulation, on human skeletal muscle blood flow (MBF), examined quantitatively with a direct method. Therefore, we investigated changes in MBF before, during and after EA with a new clearance method. [Methods] Ten healthy adult volunteers (8 men and 2 women; median age 30.5 years) were given EA to the trapezius at the BL 10 and GB 21 acupoints, employing the left trapezius as the non-EA side to contrast with the right trapezius as the EA side, and using stainless needles of 50 mm length and 0.18 mm gauge. MBF data were surveyed for 2 minutes before EA, for 4 minutes during EA, and for 4 minutes after EA, and a total of10minutes were analyzed l(?). Blood pressure and heart rate were simultaneously measured. [Results] MBF on the EA side increased significantly during EA (p < 0.05). On the other hand, diastolic blood pressure and heart rate were shown to decrease during EA (p < 0.05). [Discussion] These results, showing that MBF increased on the EA side, while diastolic blood pressure and heart rate decreased, suggest a regional muscle pump action with EA. [Conclusion] In evaluating a new, direct means of quantitatively examining 99mTc04- clearance, an increase in MBF with right side EA stimulation was clearly shown. Because the present method is simpler and easier, and higher in accuracy than past methods, we believe it may be used more aggressively in future clinical studies of acupuncture/moxibustion.
9.Meningococcal disease outbreak related to the World Scout Jamboree in Japan, 2015

Problem: Six invasive meningococcal disease cases occurred among Scottish and Swedish nationals associated with the World Scout Jamboree (WSJ), an international mass gathering, held in Japan. The index case developed symptoms while returning home. The strains from all six cases were identical and seldom seen in Japan.
Context: Over 33 000 participants from 155 countries attended WSJ. At the Jamboree site, participants of the North of Scotland’s and Sweden’s units camped within the same subcamp and kept the same schedule of events. No information was available about the Swedish and Scottish cases’ close personal contact history.
Action: Health Protection Scotland investigated Scottish cases, conducted active case finding, provided chemoprophylaxis, vaccinated close contacts and advised Scottish WSJ participants and contacts to seek medical care if they developed symptoms. The Public Health Agency of Sweden recommended chemoprophylaxis to all participants in Sweden. In Japan, the Ministry of Health, Labour and Welfare (MHLW) requested the Scout Association of Japan advise all participants to seek medical attention if they developed symptoms. MHLW shared information about the event with local authorities, medical associations, and the Ministry of Education, Culture, Sports, Science and Technology.
Outcome: No additional case related to WSJ has been reported. This outbreak highlighted the risk for international spread of invasive meningococcal disease at international mass gatherings.
Discussion: Assessing risk, educating participants, enhancing surveillance and sharing timely information among related countries are significant for prevention and response against invasive meningococcal disease outbreaks at mass gatherings.
10.The effect of strength training in muscle and nerve is memorized and reinforced by retraining.

The purpose of this study was to prove the hypothesis that the effect of strength training is memorized and reinforced by retraining. Untrained university-age men participated in this training program. The retraining leg was subjected to 5 weeks of isometric training, 17 weeks of detraining and 5 weeks of retraining in knee extension. The contralateral training leg was subjected to 5 weeks of isometric training during the same period as the retraining phase of the retraining leg. Maximal isometric torque of knee extension increased after the 5-week training and remained at the trained level during the 17week detraining period. Torque gain by retraining of the retraining leg was 2.6 times greater than that of the contralateral training leg. These changes in isometric torque corres-ponded with changes in iEMG of the vastus lateralis. The cross-sectional area of the quadriceps femoris muscle did not change with training. Results support the hypothesis that the effect of strength training is memorized and reinforced by retraining. In addition, results show that these adaptations would be explained by recruitment and rate coding of motor units.
