1.Construction of unified intelligence system and its application in hospitals

The basic design plan of unified intelligence system with a demonstration effect was worked out using the system integration method, and its technical implementation was elaborated in aspects of enterprise service bus, database integration, platform service, and key business application from the operational point. Its extended appli-cation in clinical practice, scientific research, decision-making and hospital management was studied in order to further improve the hospital management level.
2.Detection of subarachnoid haemorrhage with gradient echo T_2 ~*sequences

Objective To measure the sensitivity and specificity of gradient echo T*_2(GRE-T*_2) to subarachnoid haemorrhage (SAH). Methods 12 patients with SAH underwent MRI using T_1WI, T_2WI, FLAIR, GRE-T*_2 sequences and CT as well. Results (1)In the acute-stage of the patients, SAH was seen as an area of high signal intensity as compared with the surrounding cerebrospinal fluid in 66.6% of the cases on T_1-weighted images, and in 100% on FLAIR images; low signal intensities were seen in 50.0% of the cases on T_2-weighted images, and in 100% on GRE-T*_2-weighted images; (2) In the subacute of the patients, SAH was detected on T_1-weighted images (25.0% of cases), FLAIR (25.0%), T_2-weighted images (0), GRE-T*_2-weighted images (100.0%); (3) In the patients with atypical SAH, both CT and FLAIR sequence in MRI were negative for SAH, while lumber acupuncture and GRE-T*_2 had positive findings. Conclusion GRE-T*_2 is the most sensitive sequence of MRI for detecting acute and subacute SAH and has significant advantages over CT in the detection of subacute and atypical SAH.
3.Progress of T helper cell 17,regulatory T cell,podocyte injury and primary glomerular disease

In recent years,T helper cell 17(Th17),regulatory T cell(Treg) and podocyte injury attrac-ted widespread attention in the pathogenesis of primary glomerular disease. Th17 cells have the function of re-cruiting neutrophils and macrophages to the infected tissue through the secretion of cytokines such as IL-17. Treg cells have immune function, mediated immune tolerance, protecting the body against inflammatory injury. The imbalance of Th17 cells increase and Treg cells decrease could play an important role in the pathogenesis and progression of primary glomerular disease. As the important part of the glomerular filtration barrier,podocyte be-comes the focus in recent years. Study on relationship among Th17,Treg,podocyte injury and primary glomeru-lar disease will provide more theoretical basis for the prevention and treatment of primary glomerular disease.

Objective To explore the significance of hidden rheumatoid factor-IgM(HRF-IgM),anti-cyclic citullinated peptide antibodies(anti-CCP),antikeratin antibodies(AKA)combined test in the diagnosis of juvenile idiopathic arthritis(JIA).Methods The anti-CCP and rheumatoid factors(RF)were tested by enzyme linked immunosorbent assay(ELISA)in serum samples from 60 children with JIA,27 systemic lupus erythematosus(SLE),18 kawaski diseases and 50 healthy children who were enrolled to serve as normal controls.Indirect immumofluorescence method was used to test AKA.Using the QAE-Sephadex-A50 to separate RF then tested HRF-IgM by ELISA.Results Positive anti-CCP were found in sera of 13 patients with JIA(21.7%)and specificity 96.8%.RF were found lower test both with the JIA and other groups(?2=0.17 P=0.68).The results of AKA were similar to the anti-CCP,they were both lower sensitivity and higher specificity.There were 65% sensitivity and 85.3% specificity in the HRF-IgM,and great significant difference in positive rate in JIA and other group(?2=40.22 P=0.0001).Conclusions JIA children in the studied series has a higher detection rate and value of HRF-IgM as compared to the other control groups.HRF-IgM,anti-CCP,AKA combined test in JIA can prove to be a valuable reference index for diagnosing suspected JIA children.

8.Surgical therapy for malignancy patients complicated with hyperthyroidism

Objective To summarize the therapeutic experience on malignancy patients complicated with hyperthyroidism.Methods Clinical date of 10 cases of surgical malignancy complicated with hyperthyroidism admitted from May 2004 to May 2010 in Tianjin Medical University General Hospital were analyzed retrospectively.Results Four cases were treated by subtotal thyroidectomy before radical operation for cancer.Radical operation for cancer was performed on 6 patients after clinical symptoms of hyperthyroidism were controlled by perioperative antithyroid agents.Postoperatively 3 patients complicated with clinical manifestations similar to thyroid crisis.There was no postoperative mortality.Chemotherapy was given to 10 patients,and 1 patient was discontinued for chemotherapy caused leucopenia.Conclusions Hyperthyroidism should be controlled by surgery or antithyroid agents before patients of malignant diseases could proceed with radical surgery.Proper preoperative medication and effective postoperative management can reduce operation risk and help the patients get through the perioperative period safely.
9.Sphingosine kinase 1 and tumor.

Sphingolipids as an important regulator play a critical role in the cell biological functions. Among them, ceramide (Cer) and sphingosine (Sph) induce apoptosis and inhibit cell proliferation; on the contrary sphingosine 1-phosphate (S1P) promotes cell survival and proliferation. The balance between ceramide/sphingosine and S1P forms a so-called "sphingolipid-rheostat", which decides the cell fate. Sphingosine kinases, which catalyze the phosphorylation of sphingosine to S1P, are critical regulators of this balance. Here, we review the role of sphingosine kinase 1 (SphK1) in regulating fundamental biological processes and tumorigenesis and the potential of SphK1 as a new target for cancer therapeutics.
10.Potential targets for anti-liver fibrosis.

Liver fibrosis is a pathological process of the excessive accumulation of extracellular matrix, especially collagen al (I) in liver. Ultimately, hepatic fibrosis leads to cirrhosis or hepatic failure. Liver fibrosis and early cirrhosis can be reversed, thus control of the development of liver fibrosis is very important for preventive treatment of cirrhosis and hepatic failure. This is a review of potential targets for anti-hepatic fibrosis based on plenty of publications, including TGF-β1 and integrin α(v) and so on, aimed at providing novel therapeutic targets in liver fibrosis.
