1.Endoscopic diagnosis and treatment of primary gastrointestinal malignant lymphoma

Objective To summarize the endoscopic characteristics of Primary Gastrointestinal Malignant Lymphoma (PGIML) and increase the diagnostic accuracy of this method to aid the clinical treatment.Methods The clinical data of 67cases with PGIML from 2004 to 2009 were studied retrospectively.ResultsPGIML lacked specific clinical symptoms.The main clinical symptoms were abdominal pain, melena and abdominal mass.In our study, most PGIML(37/67,55.8%)were detected in stomach.Nodular appearance of mucosal surface was the most common endoscopic finding.The positive rate of endoscopic biopsy for the diagnosis of PGIML was 68.7 % (46/67 biopsy cases).All cases had Non-Hogkin Lymphoma (NHL).The most common immunophenotype was B-cell majority (50/67).Thirty cases had MALT.ConclusionIt is hard to diagnose PGIML at early stage,combination of various diagnosis tests should be considered to solve this problem.The effectiveness of surgery on PGIML should be further confirmed by prospective randomized clinical trials.
2.Efficacy of Zegui Longshuang capsules combined with clarithromycin sustained release tablets for elderly prostatitis and impact on inflammatory cytokines in prostatic fluid

Objective To investigate the effects of IL-2, IL-6 and IL-1βlevels in prostatic fluid by Zegui Longshuang capsules combined with clarithromythin sustained release tablets in elderly patients with prostatitis.Methods A total of 116 elderly patients with chronic prostatitis from our hospital were collected, patients or family members signed an agreement, and randomly divided into experimental group and control group with 58 cases in each group.Patients in control group were treated by clarithromycin sustained-release tablets, patients in the experiment group were treated by Zegui Longshuang capsules combined with clarithromycin sustained release tablets, treated for 30 d, compared the prostatic fluid leucocyte, lecithin bodies and IL-1β, IL-6, IL-2 changed of the two groups patients before and after treatment.At the same time, compared clinical efficacy and complications. Results The clinical effective rate of the control group 74.14% was lower than the experiment group 89.66%, with statistical significance (P<0.05);the prostatic fluid leucocyte and lecithin bodies of the experiment group improved situations were better than the control group after treatment, the IL-1β, IL-6, IL-2 and TNF -a levels in prostatic fluid of the experiment group were lower than the control group, with statistical significance (P<0.05), and the total adverse reactions rate of the two groups with no difference.Conclusion Zegui Longshuang capsules combine with clarithromycin sustain release tablets can significantly reduce the prostatic fluid of IL-2, IL-6 and IL-1βlevels in elderly prostatitis, the effect is obvious.
3.Attaching importance to protection and reconstruction of spinal stability and performing spinal surgery vigorously and assuredly

Spine surgery is a branch of science bridging neurosurgery and orthopedics.In our country,spinal neurosurgery started relatively late and its development is unbalanced regionally despite rapid development in recent years.In this paper, we make an objective evaluation of the development and present situation of spinal neurosurgery in China and its existing problems.We also introduce some theoretical knowledge about stability of the spine as well as some methods commonly used in clinic to protect and reconstruct spinal stability. We urge neurosurgeons to pay more attention to the learning of and research on spinal stability and biomechanics,and perform spinal surgery vigorously and assuredly.
4.The surgical therapy of recurrent retroperitoneal malignant fibrous histiocytoma:an analysis of 29 cases

Objective To sum up the clinical experience in the diagnosis and treatment of recurrent retroperitoneal malignant fibrous histiocytoma (RMFH). Methods[WT5”BZ] Clinical data of 29 recurrent RMFH from 1990 to 2002 were retrospectively analyzed. ResultsTwelve (41.4%) cases underwent total tumor resection after the first recurrence. Seven cases underwent multiple viscera resection,of which two did after the first recurrence,three after second and the other two after third recurrence. Twenty-five patients were followed up for 6 months to 8 years,The 1,3 and 5 year survival rate was 73.2%,46.7% and 19.8% respectively. [WT5”HZ]ConclusionTotal tumor resection is an effective way to treat RMFH,and multiple viscera combined resection increases the success rate of total tumor resection.

A L-arginine-producing mutant UN100-12(SG~(r),AE~(r)) was derived from Corynebacterium glutamicum ATCC138761 by combination treatment with ultraviolet(UV) and N-methyl-N-nitro-N-nitrosoguanidine(NTG). It could accumulate 16.6g/L L-arginine in the medium containing glucose as carbon source and ammonium sulfate as nitrogen source, when it was culrured at 30℃ for 4 days. And also this mutant has good stability of descendiblity of L-arginine.
7.Effects of different gastrointestinal reconstruction methods after distal subtotal gastrectomy on nutritional status and blood glucose in patients with type 2 diabetes and gastric cancer

Objective To observe the effects of different gastrointestinal reconstruction methods after dis -tal subtotal gastrectomy on nutritional condition and blood glucose in patients with type 2 diabetes ( T2DM) and gastric cancer.Methods 88 patients with T2DM and gastric cancer undergoing radial distal gastrectomy were studied and among them 43 patients had Billroth I gastrointestinal reconstruction , 22 patients had Billroth II gas-trointestinal reconstruction and 23 patients had Roux-en-Y gastrointestinal reconstruction .Body mass index ( BMI) , serum albumin and prealbumin of the 3 groups were measured preoperatively and 1 year after surgery . The patients were followed up .Results There was no significant difference among the 3 groups in preoperative values(P>0.05).One year after surgery, BMI, serum albumin and peralbumin in the 3 groups had different de-grees of reduction , but showed no statistical difference ( P>0.05 ) .The effective rate of diabetes control was 18.60%in Billroth I group , 72.73%in Billroth II group , and 73.91% in Roux-en-Y group and the difference had statistical significance(χ2 =17.390 3,P<0.05).The effective rates of diabetes control in Billroth II group and Roux-en-Y group were higher than that in Billroth I group (χ2 =18.340 9,P<0.05;χ2 =19.480 4,P<0.05), and there was no evident difference between Billroth II group and Roux-en-Y group(χ2 =0.008 1,P=0.928 4).Conclusion Billroth II and Roux-en-Y gastrointestinal reconstruction can improve glycemic metabo-lism of patients with T2DM and gastric cancer without significantly reducing the nutrition status .

9.Oxidation and isomerous conversion of ?-asarone and borneol in Shenchang Injection

Objective To analyze what will create after oxidation in Shenchang Injection, the oxidated mechanism, and isomerous conversion of ?-asarone and borneol in it. Methods To analyze the sample of Shenchang Injection before and after oxidation by GC-MS and to compare the composition of ?-asarone and borneol before and after sterilization. Results Some of ?-asarone was oxidized and changed to asarylaldehyde (about 0.88%—2.82% of total composition ) and saishinone (about 0.24%—0.46% of total composition), some of the borneol and isoborneol was oxidized and changed to camphor when three samples of Shenchang Injection are oxidized; some of ?-asarone (cis-) change to ?-asarone (trans-, about 1%—2%), (7.85?7.27)% of the isoborneol (cis-) changed to borneol (trans-) when sterilized at 100 ℃ for 30 min. Conclusion ?-Asarone and borneol in Shenchang Injection are involved in the oxidated reaction and have isomerous conversion after high pressure sterilization.

The principal constituents of Radix Astragali include Radix Astragali soap glucoside,Radix Astragali polysaeeharide,amino-butyric acid,calcium and trace elements (selenium,manganese,iron,zinc,copper).Radix astragali has the function of protecting heart and kidney,two-ways regulating blood sugar and the blood pressure,resisting tumor,hypoxia,senile and oxidation,and enhancing immunity.This article reviewed pharmaceutical effects of Radix Astragali and its preparation of recent studies.
