Chinese Circulation Journal 2004;0(01):-
Objective:To summarize the experience of surgical treatment of valvular heart disease with enlarged or small left ventricles. Methods:Valvular operations were performed in 473 patients, including 38 with enlarged left ventricles(group Ⅰ)and 40 with small left ventricles(group Ⅱ),from April 1998 to June 2005.Postoperative complications between two groups were com- pared and analyzed statistically. Results:The postoperative eomplications were ventricular arrhythmia, low output syndrome, lung infection, renal failure and brain embolism.Low output syndrome tended to occur in group Ⅱ(20%),whereas there was a higher incidence of ventricular ar- rhythmia in group Ⅰ(65.79%).The difference between groups was significant(P<0.05). Conclusion:To improve the successful operative rate in patients with enlarged or small left ventricles,we should take effec- tive measures to manage the post-procedural complications, such as ventricular arrhythmia(mainly in group Ⅰ)and low output syndrome(mainly in group Ⅱ).

Objective To explore the influence of XTP4 on genomic expression profile of hepatocyte through screening and cloning of genes trans-regulated by XTP4-expressing plasmid. Methods The expressive vector pcDNA3.1(-)-XTP4 was constructed by routine molecular biological methods, and suppression subtractive hybridization (SSH) method was employed to detect the mRNA differentially expressed by the HepG2 cells transfected with pcDNA3.1(-)-XTP4 and pcDNA3.1(-), respectively, using lipofectamine. The twice enriched PCR products were subcloned into T/A vectors to set up the subtractive library. Amplification of the library was carried out in E. coli strain JM109. The screened cDNA were sequenced and analyzed in GenBank with Blast search after PCR. Results The amplified subtractive library containd 21 positive clones. Colony PCR analysis showed that there were 16 clones containing 200-1000bp inserts. Sequence analysis was performed and 9 kinds of encoding sequences were achieved. These genes trans-regulated by XTP4 protein involved in hepatic fibrogenesis, tumorgenesis, mitochondrial function, and cell growth regulation. Conclusions The findings obtained by SSH provide significant data for a preliminary understanding of the biological function of a new identified gene-XTP4. These results will pave the way for the study of the molecular mechanism of the transactivating effects of HBxAg and the development of new therapy for chronic hepatitis B.

Objective To investigate the short-term clinical efficacy and complication of esophageal cancer patients with two-field lymph node dissection by thoracolaparoscopic esophagectomy surgery and open surgery. Methods One hundred and fifty esophageal cancer patients with two-field lymph node dissection were selected, and they were divided into control group (using open surgery, 75 cases) and observation group (using thoracolaparoscopic esophagectomy surgery, 75 cases) by random digits table method. The operation time, bleeding amount, hospital staying time, number of lymph node dissection, reoperation rate, intensive care unit (ICU) transferring rate and postoperative complication were compared. Results There was no statistical difference in operation time between 2 groups ( P>0.05). The bleeding amount and hospital staying time in observation group were significantly lower than those in control group: (210.33 ± 30.71) ml vs. (254.59±35.28) ml and (8.45±1.52) d vs. (11.61±2.08) d, there were statistical differences (P<0.05). The number of chest lymph node dissection in observation group were significantly higher than those in control group:(17.20±4.06) pieces vs. (10.44±2.65) pieces, and there was statistical difference (P<0.05). There were no statistical differences in reoperation rate and ICU transferring rate between 2 groups ( P>0.05). There were no statistical differences in incidences of hoarseness and anastomotic stenosis between 2 groups ( P>0.05). The incidences of pulmonary infection and arrhythmia in observation group were significantly lower than those in control group:17.33%(13/75) vs. 30.67%(23/75) and 2.67%(2/75) vs. 14.67%(11/75), and there were statistical differences (P<0.05). Conclusion Compared with open surgery, thoracolaparoscopic esophagectomy surgery with two-field lymph node dissection for esophageal cancer patients can effectively reduce the degree of operative trauma, accelerate postoperative rehabilitation process, improve the effects of lymph node dissection, and reduce postoperative complication risk.
