2.Clinical Study on the Treatment of Humeral Supracondylar Fracture in Children with Manual Reduction Combined with Oral Medicinal Herbs and Fumigation

Objective To observe the clinical effect of treating humeral supracondylar fracture in children with manual reduction combined with oral medicinal herbs and fumigation.Methods 80 children with humeral supracondylar fracture were involved in clinical observation.Patients with Gartland Type Ⅰ and Type Ⅱ were selected and treated with manual reduction and plaster external fixation.Gartland Type Ⅲ cases were selected and treated with Kirschner-wire transfixion.All of patients were treated with oral medicinal herbs and fumigation.Results All children with humeral supracondylar fracture recovered.Based on the clinical function test,39(48.75%)cases had excellent effects,39(48.75%)cases had good effects,and 2(2.50%)cases had fair effects.No Volkmann contracture or cubitus varus deformity occurred.Conclusion The treatment of humeral supracondylar fracture in children with manual reduction combined with oral medicinal herbs and fumigation has a good effect.
3.Measurement of intraocular stray light and its clinical significance

Stray light is a physical phenomenon that degrades visual quality when light scattering occurs in the human eye.It is a main cause of the disabling glare.Stray light originates in cornea,iris,sclera,lens and retina,and these ocular tissues contribute to the total amount of stray light in a normal eye.The light scattering intensity increases when the transparency of any optical element mentioned above reduces.Stray light can be measured by several different methods,but the measurements still lacks a standardized procedure and outcome.Further clinical study is necessary to quantify and standardize the measuring method of stray light.This article aims to introduce the concept,source and clinical significance of stray light,and to provide its advanced measurement techniques and methods.

6. Renal cell carcinoma complicatedwith sarcomatoid differentiation: Treatment, prognosis and literature review

Objective To retrospectively analyze the clinical data of 16 patients with renal cell carcinoma complicatedwith sarcomatoid differentiation, so as to discuss the treatment and prognosis of the condition. Methods A total of 547 patients with renal cell carcinoma were admitted in our department during 2005-2009, and 16(2. 9%) ofthem also had sarcomatoid differentiation, including 7 males and 9 females, aged (52 ± 10) years old, ranging 36-70 years old. Gross hematuria was found in 5 patients, backache in 5, fatigue and weight loss in 2, cough and hemoptysis in 1; and 3 patients were accidentally found with renal occupation during physical examination. All tumors were solitary, with 7 in the left and 9 in right. The maximum tumor diameter was (7. 8±4. 1) cm, ranging 2-15 cm. Full examination was done to confirm the tumor stage preoperatively. Preoperative examination showed that 2 patients had retroperitoneal lymph node enlargement, 1 had right pulmonary hila lymphatic metastasis, 1 had livermetastasis, 2 had renal vein and (or) the inferior vena cava tumor thrombi, 3 had bone metastases, 1 had invasion of ipsilateral adrenal gland, and 1 had invasion of the upper ureter. All the 16 patients underwent surgical treatment,and their specimens were subjected to H-E staining and immunohistochemical examination. Five patients were treated with INF-α after operation and were followed up. Results All patients received radical nephrectomy. Pathological examination showedthat 9 patients had clear cell renal cell carcinoma with sarcomatoid differentiation, 3 had chromophobe renal cell carcinoma with sarcomatoid differentiation, and the other 4 had unclassified renal cell carcinoma with sarcomatoid differentiation. No recurrence or metastasis was seen in the 4 pT1N0M0 patients. All the pT2-T4 patients died, with the progression-free survival time being (5. 6±4. 5) months, ranging 2-16 months; the overall survival time was (8. 3± 5. 6) months, ranging 3-20 months. Conclusion Preoperation staging of patients with renal cell carcinoma complicated with sarcomatoid differentiation has a great impact on patient survival. Operation is the most effective treatment for patients with early stage, and routine follow? up examination is recommended post operatively. Targeted therapy may be an effective way for patients of advanced stag.

8. Finite element model analysis of lumbar biomechanical characteristics after nucleus discectomy

Objective To Use three-dimensional Sinite element model analysis for studying the lijmbar biomechanical characterislics following mjcleijs discectomy in a palierrt with lijmbar iritervertebral disc protrvion. Methods A novel CAD method was adopted to acciirately create the Smite teemerrt modtes of L4/5 segment, mdiidmg the modCls for normal disc, degenerated disc, denucleated disc immediatcly after operaSion (denucleated modCl), and denuCleated disc after a longer lime period (scar growth model) Furthermore, the biomechanical parameters of the foUr modtes were examined under SiVe different loads, riamtey, compresSion, fleXion, extemion, forward and backward shear loads. Resutts (1) Under the iiVe loads, the siiffttess of degerterated modCl was ncreased compared with that of the normal disc under all 5 different loads. (2)The sfiffiiess of dermcleated modCl was decreased than that of degerterated disc and ncreased than that of the normal disc. (3)The siiffness of scar growth modte was enhanced and eVen siripassed that of the degerterated modte due to the fact that the scar tissite filled the irrtradisc caVity. (4) The facet contact force was decreased under CleXion, exteniion, and backward shear loads in dermcleated modCl, and were increased under exteniion, forward and backward shear loads in scar growth modCL (5) The backward budge in the degerterated modte wasobViously siighter than that in the rormal modte. The backward bulge of denucleated modte increased to some ejrterrt, and its inner annulus fibrous had inward bUlging. Conclusion NuCleus discectomy has differerrt effects on the biomecharics of liimbar mofion segment in pafierrts at different iime points after operafion. The impacts on liimbar stabliity and postefiior structure are small immediatery after nuCleus discectomy, and the liimbar moiion segmerT may sfiffen and the stress of ariicular process may ncrease after a longer iime period following nuCleus discectomy.
