Objective To evaluate the sufentanil-sparing effect of ketorolac tromethamine for postoperative analgesia in the elderly patients.Methods Sixty ASA Ⅱ or Ⅲ patients,aged ≥ 65 yr,with a body mass index of 18-24 kg/m2,undergoing elective gynecological operations,were randomly divided into 2 groups(n =30 each):sufentanil group(group S)and ketorolac tromethamine plus sufentanil group(group T).Both groups received combined intravenous-inhalational anesthesia and patient-controlled intravenous analgesia(PCIA)after operation.PCIA solution contained ketorolac tromethamine 180 mg and sufentanil 100 μg in 100 ml of normal saline in group T.After a loading dose of ketorolac tromethamine 30 mg was injected intravenously at 15 min before the end of operation,the PCA pump was set up with a 1.6 ml bolus dose,a 20 rain lockout interval and background infusion at a rate of 1.5 ml/h in group T.PCIA solution contained sufentanil 100 μg in 100 ml of normal saline in group S.After a loading dose of sufentanil 5 μg was injected intravenously at 15 min before the end of operation,the PCA pump was set up with a 1.6 ml bolus dose,a 20 min lockout interval and background infusion at a rate of 1.5 ml/h in group S.The effective analgesia(postoperative VAS scores at rest and during activity < 3)was maintained within 48 h after operation.The amount of sufentanil consumed within 48 h after operation and adverse effects were recorded.Results Compared with group S,the amount of sufentanil consumed within 48 h after operation was significantly reduced,and the incidence of nausea and vomiting,urinary retention and pruritus was significantly decreased in group T(P < 0.05).Conclusion Ketorolac tromethamine used with PCIA with sufenlanil has a significant sufentanil-sparing effecl for posloperative analgesia and improves the safety of analgesia in the elderly patients.
3.Effect of military performance stress on sympathetic adrenal medulla in submariners

Objective To evaluate the effect of military performance stress on the changes of concentrations in salivary chromogranin A ( CgA) ,β-endorphin (β-EP) and salivary IgA ( sIgA) of submariners.Methods Twenty-nine submarine soldiers were selected, and their saliva samples were collected separately at the end of a long dive trip and nine months after relaxation ashore.In addition,the saliva samples of twenty-eight graduate students were collected as the normal control.The method of ELISA was used to detect the level of salivary CgA,β-EP and sIgA.Results After long-term dive training, the submariners showed significantly decreased CgA,β-EP and sIgA.Conclusion After a long term dive trip, chronic military performance stress is associated with the decline of salivary CgA,β-EP and sIgA, indicating that the function of sympathet-ic adrenal medulla is suppressed.The biological significance of these changes needs to be assessed in the future.
4.Evaluation of performance stress among military submarine soldiers by using salivary cortisol,DHEA-S and cortisol/DHEA-S ratio assay

Objective To investigate the significance of salivary cortisol , dehydroepiandrosterone sulfate (DHEA-S) and cortisol/DHEA-S ratio changes for evaluation of military performance stress .Methods Forty submarine soldiers were selected, whose saliva samples were collected separately at the end of long-term dive training and after nine months of relaxation break.In addition, the saliva samples of thirty-four graduate students were collected the moment they finished a three-hour final examination and one week later .The method of ELISA was used to detect the levels of salivary cortisol and DHEA-S and to count their ratio .Results After long-term dive training , the submarine soldiers showed significantly decreased DHEA-S and an increased cortisol/DHEA-S ratio, but the cortisol level did not change very much .In contrast, the final examination stress did not change the level of cortisol , DHEA-S or the cortisol/DHEA-S ratio among these students.Conclusion This is the first study to show that long-term, chronic military performance stress is associated with the salivary DHEA-S and cortisol/DHEA-S ratio changes .The increase in the cortisol/DHEA-S ratio may be used as an important and useful biomarker to evaluate chronic stress .In addition , it is relatively simple and sensitive to detect stress biomarkers by using saliva samples .
5.Analysis of a case of systemic sclerosis complicated by noncirrhotic portal hypertension and literature review

Objective To improve the rheumatologists' understanding of noncirrhotic portal hypertension.MethodsA case of systemic sclerosis complicated by noncirrhotic portal hypertension was reported and the related literatures were reviewed.Results A 51-year-old female who had been diagnosed as systemic sclerosis presented clinically with an chronic onset of portal hypertension.She also had pancytopenia,splenomegaly,and significant esophageal varices.Liver function tests were normal.Hepatitis viral serology was negative.Ultrasound scan of liver revealed no focal lesion.ACT scan confirmed the absence of portal vein thrombosis.Taking into account the above evaluation we concluded that the patient had systemic sclerosis and noncirrhotic portal hypertension.The patient was on prednisolone and immunosuppressant and the condition was well.Conclusion Noncirrhotic portal hypertension complicated by autoimmune disease,especially SSC,is very poor,characteriged by significant portal hypertension as well as histological evidence that cirrhosis is absent.Rheumatilogist should pay attention to it.
6.Role of sodium oxalate in radiosensitivity of hypoxic NPC CNE2 cells

Objective To explore the influence of sodium oxamate on the radiaosensitivity of hypoxic nasopharyngeal carcinoma CNE2 cells. Methods The production of lactate and angiotensin Ⅱ in CNE2 with or without sodium oxamate treatment was detected by lactate assay kit and angiotensin Ⅱ ELISA kit. Cell growth of hypoxic CNE2 with or without sodium oxamate treatment was measured in vitro by MTT method. Radiosensitivity of hypoxic CNE2 with or without sodium oxamate treatment was tested by clone formation assay. Results Lactate and angiotensin Ⅱ production of CNE2 cells treated with sodium oxalate was inhibited. Sodium oxalate inhibited the proliferation of hypoxic CNE2 cells. The radiosensitivity of hypoxic CNE2 cells treated with sodium oxalate was largely improved. Conclusions Sodium oxalate inhibits the proliferation of CNE2 cells in hypoxic condition and significantly enhances the radiosensitivity of hypoxic CNE2 cells. The mechanism may be involved in its inhibiting of angiotensinⅡproduction.
7.Clinical characteristics analysis and literature review of 55 cases with panniculitis

Objective To improve the clinical understanding of the disease by retrospective analysing 55 cases of patients with panniculitis. Methods The hospitalized patients with panniculitis were collected from December 2011 to October 2018 in the Shanxi Dayi Hospital Affiliated to Shanxi Medical University. The demographics, clinical manifestations, laboratory examinations and treatments were analyzed and summarized, rate or composition ratio were applied for statistical description of the counting data. Results The proportion of males and females in the 55 patients was 1:2.23, with an average of (53±15) years (18-82 years). A total of 52 cases of nodular panniculitis (including 14 cases of mesenteric involvement) and 3 cases of special type of panniculitis were collected. Patients with nodular panniculitis were often presented with subcutaneous nodules or masses. According to the affected parts, they cowld be divided into skin sub-type and systemic sub-type. Among them, 32 cases were skin sub-type and 20 cases are systemic sub-type. The most frequently involved organs were mesenteric and kidney involvement. Nine of 55 patients was complicated with tumor. Patients with mesenteric panniculitis mainly presented with abdominal pain and bloating, which were mainly diagnosed according to CT findings. Patients with systemic panniculitis could be significantly relieved after treatment with coricosteroids and immunosuppressive agents. Conclusion Panniculitis is an inflammatory disease of unknown etiology. It is frequent in middle-aged and elderly people. It presents with various clinical manifestations and lacks specificity. The diagnosis is mainly based on pathological results. It is easy to be complicated with tumors. When subcutaneous nodules are found, pathological examination should be performed in order to avoid misdiagnosis.

We investigated the pathogen of an outbreak of lung infection with unknown causes.By epidemiological analysis,we used real-time PCR,ELISA,gold dipstick,VITEK2 and MALDI-TOF-MS to identify suspicious bacteria.We made use of serum plate agglutination test to confirm the suspicious bacteria and the patient serum.We isolated 2 strains of Cryptococcus albidus from environmental samples.There has been specific agglutination between suspicious bacteria and patient serum.This pneumonia may be related to the infection of Ccryptococcus albidus.
