Objective:To study the influence of thrombus aspiration combined tirofiban on patients with acute ST seg-ment elevation myocardial infarction (STEMI)after primary percutaneous coronary intervention (PCI).Methods:A total of 98 patients,who received primary PCI because of STEMI in our hospital from Jan 2012 to Mar 2013,were selected.They were divided into thrombus aspiration group (n=48,received pure thrombus aspiration)and com-bined treatment group (n = 50,received thrombus aspiration combined intracoronary tirofiban injection during PCI).Coronary angiography (CAG)instantly after PCI and follow-up condition during hospitalization and six months after discharge were compared between two groups.Results:(1)Compared with thrombus aspiration group after PCI,there were significant rise in TIMI blood flow grade [(2.3±0.6)grades vs.(2.7±0.3)grades],per-centage of TIMI flow grade 3 (72.9% vs.90.0%)and ST segment regression >50% rate within 90min after PCI (52.1% vs.74.0%),P < 0.05 or < 0.01,and significant reduction in percentage of postoperative no-reflow (18.8% vs.4.0%,P =0.038)in combined treatment group in hospital;(2)After six-month follow-up,left ven-tricular ejection fraction (LVEF)of combined treatment group was significantly higher than that of thrombus aspi-ration group [(58±6.3)% vs.(51±5.6)%,P <0.05].Conclusion:Thrombus aspiration combined tirofiban can effectively reduce coronary thrombus burden and improve cardiac function in STEMI patients during primary PCI.
3.Test of urine leukotriene FA in infants with bronchiolitis and its clinical value

Objective To study the prognosis of infants with bronchiolitis by testing urine leukotriene E4 (LTE4) level and investigating atopy's influences. Methods Urine LTE4 was tested in 38 eases with mild bronchiolitis (47 in acute stage, 17 in convalescent stage), 9 severe bronchiolitis cases, 15 atopic cases, 25 control cases. Peripheral blood was used to determine eosinophils count (EC) in acute bronchiolitis cases. Results (1) The level of urine LTE4 is obviously higher in cases of acute group (62.11 ± 12.23 pmol/L) than that of control group (22.19±1.50 pmol/L) , and the convalescent group (34.86 ±5.75 pmol/L) (F = 132.42, P < 0.01) ;Urine LTE4 level of convalescent group is higher than that of the control group (P < 0.01). (2) Urine LTE4 level is significantly higher in severe group (98.04 ± 8.04 pmol/L) than that of mild group (59.16 ± 12.25 pmol/L) (t = 9.92, P < 0.01). (3) Urine LTE4 level of atopy positive (88.75 ± 10.45 pmol/L) infants with bronchiolitis is significantly higher than atopy negative infants (55.28 ± 11.44 pmol/L)(t = 8.63, P < 0.01). (4) There is no significant correlation between the levels of urine LTE4 and EC for acute bronchiolitis. Condusions The level of urine LTE4 in acute bronehiolitis patients increases and remains high in convalescent stage;Higher urine LTE4 level in severe bronchiolitis cases indicates that urine LTE4 level is related to the severity of the disease;cysteinylleukotrenes is an important mediator of inflammation that may influence the prognosis of atopy positive infants with bronchiolitis;EC is not a good index to present the airway inflammation of infants with bronehiolitis.
4.An intensity window in the effects of pulsed electromagnetic fields on bone metabolism in ovariectomized rats

Objective To investigate the effects of pulsed electromagnetic fields (PEMFs) of different intensities on bone metabolism in ovariectomized rata. Methods Sixty 10-month-old female Sprague-Dawley rats were randomly divided into a normal group, an ovariectomized control group, a 0. 14 mT group, a 0. 16 mT group, a 0.18 mT group and an estrogen group. All rats except those in the normal group were subjected to bilateral ovariectomy. Beginning one week after the operation, the rats in the 0. 14 mT, 0. 16 mT and 0.18 mT groups were treated with PEMFs at 50 Hz, 60 min daily for 90 days, while those in the estrogen group received estrogen instead. During the experiment, the bone mineral density (BMD) of the whole body was observed dynamically, and local BMDs and biochemical indexes were measured after 3 months of treatment. Results Alkaline phosphatase (ALP) activity in the normal group was significantly lower than in the other groups. Tartrate-resistant acidic phosphatase 5b (TRAP5b)activity was elevated significantly, in the control and 0.14 mT groups compared with the normal group. After 2 months of treatment, whole body BMD was reduced significantly in the 0.14 mT group compared to the normal group. After 3 months of treatment, whole body BMD in the ovariectomized controls and the 0.14 mT group was reduced significantly compared to the normal group, but significantly elevated in the 0. 16 mT, 0.18 mT and estrogen groups when compared to the ovariectomized control group. At the end of 3 months of treatment, the trends in lumbar vertebral BMD were similar to those of the whole body BMD, with the femoral BMD in the ovariectomized group and the 0.14 mT group significantly lower than in the normal group, though the differences among the other groups were not statistically significant. Conclusions PEMF treatment at 50 Hz and 0.16 mT or 0.18 mT can promote bone formation. PEMFs at 50 Hz and 0.14 mT might stimulate bone resorption. An intensity-window effect exists in the action of PEMFs on bone metabolism in ovariectomized rats.
5.Effects of pretreatment with different doses of curcumin on expression of p-CREB and PGC-1α in hippocampus after global cerebral ischemia-reperfusion injury in rats

Objective To explore the effects of pretreatment with different doses of curcumm on the expression of p-CREB and PGC-1α in hippocampus after global cerebral ischemia-reperfusion (IR) injury in rats.Methods Three hundred male SD rats weighing 200-250 g were randomly divided 5 groups ( n = 60 each): sham operation group (group S), IR group, low, median and high dose curcumin group (group LC, MC, HC). Global cerebral ischemia was produced by occlusion of 4 vessels (cauterization of bilateral vertebral arteries and 15 min occlusion of bilateral common carotid arteries) according to the method described by Finkbeiner. Bilateral common carotid arteries were only exposed but not ligated in group S. Intraperitoneal curcumin 30, 100 and 300 mg/kg were injected at 1 h before ischemia in group LC, MC and HC respectively. Equal volume of dimethylsulfoxide (DMSO) was injected intraperitoneally in group S and IR. The rats were killed at 2, 6, 24 and 72 h and 7 d after reperfusion (12 at each time point). Brains were immediately removed and hippocampus was isolated. The number of apoptosis neurons was counted using TUNEL. The expression of p-CREB and PG C-1α protein in hippocampus was detected by Western blot. Results The number of apoptosis neurons, p-CREB and PG C-1α protein expression were significantly higher at each time point in the other 4 groups than in group S ( P < 0.05). The number of apoptosis neurons was significantly lower at T2-4 in group LC and MC, while p-CREB and PG C-Ⅰα protein expression wes significantly higher at T1-4 in group LC, MC and HC than in group IR (P < 0.05). The number of apoptosis neurons was significantly higher, while p-CREB and PGC-1α protein expression was significantly lower at T2-4 in group LC and HC than in group MC ( P < 0.05). Conclusion Curcumin can inhibit neuronal apoptosis in hippocampus and reduce global cerebral IR injury by up-regulating p-CREB and PG C-1α expression in rats and the effect was dose-related.

The present research was aimed to investigate the effects of sinusoidal electromagnetic fields (SEMFs) on the proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells in rats (rBMSCs) and to find out the intensity with the best therapeutic efficacy. Primary rat bone marrow mesenchymal stem cells were obtained from Wistar rats and screened by the adhesive method. The rBMSCs were exposed to sinusoidal electromagnetic fields with 50Hz frequency and intensities of 0 mT, 1.4 mT, 1.6 mT, 1.8 mT, 2.0 mT, and 2.2 mT respectively, 30 min per day. The proliferation of the rBMSCs was analyzed by MTT reduction assay. The osteogenic differentiation markers including ALP activity, calcium deposition, mineralized bone modulus and collagen I expression were compared between the rats in the exposed groups and those in the control group. The total cellular RNA was extracted after 6, 12, 24 and 48 hours, respectively. The gene expression of Osterix and IGF-1 was examined by quantitative reverse transcription-polymerase chain reaction (qRT-PCR). The absorbance of exposed groups was suppressed significantly in comparison with that in the control group. The exposure to the rBMSCs with intensity of 1.8 mT strongly enhances the osteogenic differentiation of rBMSCs, indicated by remarkably improved ALP activity, calcium deposition, collagen I expression and the number of mineralized bone nodules compared to that in the control group and other groups. Osterix and IGF-1 were also significantly improved (P < 0.05). The SEMFs with frequency and 50Hz and 1.4-2.2 mT intensities enhanced the osteogenic differentiation of rBMSCs, but inhibited their proliferation in the presence of 0.1% serum culture. Among the rBMSCs used in the tests, the one with 1.8 mT had the strongest activity, indicating that it could be the optimal intensity for the clinical application.

8.Time effect of sinusoidal electromagnetic field on enhancing the maturation and mineralization of osteoblasts in vitro.

The present research was to investigate the time effect of sinusoidal electromagnetic fields (SEMFs) at different exposure time on the proliferation and differentiation of osteoblasts (OB) in vitro. The newborn rat calvarial OB were isolated by enzyme digestion and divided randomly into 7 groups after one passage. The exposure times of the SEMFs were 0.5 h, 1.0 h, 1.5 h, 2.0 h, 2.5 h and 3.0 h, respectively, and the frequency was 50 Hz. The cells were exposed in the SEMFs of 1.8 mT. Those without SEMFs exposure were used as the control group. They were observed under the contrast phase microscope each day. After 48 h, cell proliferation was assayed by MTT method. The alkaline phosphatase (Alkaline Phosphatase, ALP) activities were measured after the exposure of SEMFs for 3 d, 6 d, 9 d and 12 d, respectively. The calcified nodules were stained by Alizarin Bordeaux after 10 d. The cells exposed in the SEMFs were arranged in Spiral appearance after 8 d. The SEMFs exposure time at 2.0 h, 2.5 h and 3.0 h significantly inhibited cell proliferation (P < 0.01) and 0.5 h, 1.0 h, 1.5 h groups more significantly than control groups (P < 0.05). When the 3 d, 6 d and 12 d the ALP activities of the 0.5 h, 1.0 h, 1.5 h and 2.0 h, times group were significantly higher than those in the control group (P < 0.05), and after 9 d the 1.0 h, 1.5 h and 2.0 h activity of ALP higher significantly than control and other groups (P < 0.01). Other groups had no effect on the ALP activity. Alizarin Bordeaux staining result showed the amounts of calcified nodules 1.0 h, 1.5 h and 2.0 h higher than control groups. The SEMFs at 50 Hz, 1.8 mT different time exposure groups inhibits the proliferation of OB, but they enhances the maturation and mineralization of the OB and SEMFs at 1.8 mT of the 1.5 h has the strongest activity.

9.China National Lung Cancer Screening Guideline with Low-dose Computed Tomography (2018 version).

BACKGROUND:
Lung cancer is the leading cause of cancer-related death in China. The results from a randomized controlled trial using annual low-dose computed tomography (LDCT) in specific high-risk groups demonstrated a 20% reduction in lung cancer mortality. The aim of tihs study is to establish the China National lung cancer screening guidelines for clinical practice.
METHODS:
The China lung cancer early detection and treatment expert group (CLCEDTEG) established the China National Lung Cancer Screening Guideline with multidisciplinary representation including 4 thoracic surgeons, 4 thoracic radiologists, 2 medical oncologists, 2 pulmonologists, 2 pathologist, and 2 epidemiologist. Members have engaged in interdisciplinary collaborations regarding lung cancer screening and clinical care of patients with at risk for lung cancer. The expert group reviewed the literature, including screening trials in the United States and Europe and China, and discussed local best clinical practices in the China. A consensus-based guidelines, China National Lung Cancer Screening Guideline (CNLCSG), was recommended by CLCEDTEG appointed by the National Health and Family Planning Commission, based on results of the National Lung Screening Trial, systematic review of evidence related to LDCT screening, and protocol of lung cancer screening program conducted in rural China.
RESULTS:
Annual lung cancer screening with LDCT is recommended for high risk individuals aged 50-74 years who have at least a 20 pack-year smoking history and who currently smoke or have quit within the past five years. Individualized decision making should be conducted before LDCT screening. LDCT screening also represents an opportunity to educate patients as to the health risks of smoking; thus, education should be integrated into the screening process in order to assist smoking cessation.
CONCLUSIONS
A lung cancer screening guideline is recommended for the high-risk population in China. Additional research , including LDCT combined with biomarkers, is needed to optimize the approach to low-dose CT screening in the future.
