1.Treatment of complex posterior urethral strictures with urethroplasty via transpubic approach after pelvic fracture

Objective To evaluate the efficacy and safety of urethroplasty via transpubie ap-proach in treatment of complex posterior urethral strictures after pelyic fracture. Methods Urethroplas-ty via transpubic approach was done on 26 patients (21 males and 5 females, at mean age of 26 years) with complex posterior urethral strictures. Posterior urethral stricture was 2.5-4.0 cm long in 23 patiens and > 4.0 em in three. There were five patients with urethratresia. The perioporative complications and operative effect were evaluated after the broken ends of the urethra was thoroughly resected and treated with end-to-end anastomosis. Results A follow-up for 1-7 years ( mean 4 years) showed successful op-eration in 22 patients (85%), with normal urination and without complications like osteitis pubis, pelvic disassociation, pelvic instability or urinary incontinence. But obstructed urination was found in one (4%) and failed operation in three (11%). Conclusions Urethroplasty via transpubic approach takes advantages of precise and thorough scar excision, less complications and long term curative effect and is clinically feasible and safe for patients with complex posterior urethral stricture.
2.Effects of ginsenoside Rg-1 on the expressions of interleukin-6,bone gla protein in periodontal tissues in periodontitis rats

Objective:To determine the effects of ginsenoside Rg1 on the expression levels of bone gla protein(BGP) and cytokine interleukin-6(IL-6) in the periodontium of experimental periodontitis rats.Methods:One hundred 160-200 g Wistar rats were randomly divied into ten groups.Group A used as control were killed at the beginning of the experiment.Group B-J were experimental periodontitis models.Group B-E received the therapy of ginsenoside Rg1 were killed at 1st,2nd,3rd and 4th weeks.Group F-J received no treatment as periodontitis control were killed at 0,1st,2nd,3rd and 4th weeks.The expression levels of BGP and IL-6 in the periodontium were measured by immunohistochemistry.Results:The expression levels of BGP in the group F were significantly lower than that in the group A,the expression levels of IL-6 in the group F were much higher than that in the group A(P

OBJECTIVE To manage the surgical instruments borrowed from medical equipment companies effectively in order to prevent or decrease the incidence of hospital infection.METHODS We analyzed the hidden trouble inducing the hospital infection by data investigation.Effective management of surgical instruments was suggested.RESULTS The management method was accepted by most departments and the surgical site infection was well controlled.CONCLUSIONS Effective management of borrowed surgical instruments should be further emphasized to ensure the safety for operation.
4.Clinical significance of serum IL - 8, sICAM - 1 and sE - selectin in patients with chronic obstructive pulmonary disease

Objective To explore the roles of IL-8, sICAM-1 and sE- selectin in the airway inflammation of chronic obstructive pulmonary disease (COPD). Methods The levels of IL - 8, sICAM -1 and sE-selectin in the serum were measured by double-sandwich ELISA in 40 COPD patients and 20 healthy persons. 5.Research Progress in the Core Proteins of the Classical Swine Fever Virus.

The core protein (CP) of the classical swine fever virus (CSFV) is one of its structural proteins. Apart from forming the nucleocapsid to protect internal viral genomic RNA, this protein is involved in transcriptional regulation. Also, during viral infection, the CP is involved in interactions with many host proteins. In this review, we combine study of this protein with its disorders, structural/functional characteristics, as well as its interactions with the non-structural proteins NS3, NS5B and host proteins such as SUMO-1, UBC9, OS9 and IQGAP1. We also summarize the important part played by the CP in CSFV pathogenicity, virulence and replication of genomic RNA. We also provide guidelines for further studies in the CP of the CSFV.

6.Changes of caries activity and calcium, phosphorus concentrations in saliva of patients with nasopharyngeal carcinoma before and after radiotherapy.

OBJECTIVETo analyze caries activity of nasopharyngeal carcinoma (NPC) patients before and after radiation therapy. The study was also designed to analyze calcium and phosphorus concentrations in saliva of NPC patients before and after radiotherapy.
METHODSTwenty-eight NPC patients and 20 healthy controls were included in this study. Resazurin disc method was used to detect caries activity of NPC patients before and after 70 Gy radiotherapy. The atomic absorption spectrometry and molybdenum-stibium antispetrophotography method were used to analyze calcium and phosphorus concentrations in saliva of NPC patients before and after 70 Gy radiotherapy and the healthy controls.
RESULTSThe results of resazurin disc method showed caries activity of NPC patients was increased after radiotherapy (P < 0.01). Calcium concentration in saliva of NPC patients before radiotherapy was (63.19 +/- 3.27) mg x L(-1), phosphorus concentration was (132.96 +/- 5.13) mg x L(-1). After 70 Gy radiotherapy, calcium concentration was (33.38 +/- 0.32) mg x L(-1), phosphorus concentration was (49.18 +/- 2.66) mg x L(-1). The difference of calcium and phosphorus concentrations in saliva before and after radiotherapy was significant (P < 0.01).
CONCLUSIONCaries activities of NPC patients increase after radiotherapy. Calcium and phosphorus concentrations in saliva decrease after radiation therapy.

7.Effect of Cilazapril on cytokines and cardiac function in patients with congestive heart failure

Thirty seven patients with congestive heart failure (CHF) were divided into cilazapril group (n=19) and general treatment group (n=18). Serum levels of interleukin-6 (IL-6) ,interleukin-8 (IL-8) , interleukin-10(IL-10) and tumor necrosis factor-α(TNF-α) , left ventricular ejection fraction (LVEF) ,left ventricular end-diastolic diameter (LVEDD), cardiac output (CO) and fractional shortening (FS) were measured before and after treatment. Serum levels of cytokines were also measured in 40 healthy individuals (control group). Results: The serum levels of IL-6, IL-8, IL-10 and TNF-α in CHF patients were significantly higher than those in the control group ( all P<0.01 ) ; After treatment, the serum IL-6, IL-8 and TNF-α were significantly decreased (P<0.01 ,P<0.05 ) in the cilazapril group. The LVEF, FS, CO were significantly increased in the Cilazapril group ( P<0.01 ) ; And the serum levels of IL-6 were significantly decreased in the cilazapril group as compared with the general treatment group ( P<0.05 ), however, after treatment, the EF, LVEF, FS and CO had no statistical differences in the cilazapril group as compared with the general treatment group. In the control group only LVEF and FS improved(P<0.01) ; Cytokine levels showed no changes. It suggests that cilazapril can reduce the serum levels of pro-inflammatory cytokines, increased the serum levels of anti-inflammatory cytokine, protect and improved cardiac function in the patients with congestive heart failure.
8.Relationship between thrombospondin-1 gene GI678A polymorphism and acute coronary syndrome

Objective To investigate the possible association between the thrombospandin-1(TSP-1) gene GI678A (Ala523Thr)polymorphism and acute coronary syndrome (ACS) in a Chinese Han population.Method he ease cohort studied was compsed of 412 hospitalized patients with ACS recruited from four participating hospitals between November 2003 and May 2006.The diagnosis of ACS was based on the criteria of AHA/ACC set in 2002.The eontrul group was consisted of 319 age- and sex-matched subjects from partiei pating hospitals,and they were free from coronary artery disease judged by history,clinical examination,electrocardiography,exercise test and angiography.The TSP-1 GI678A polymorphism was determined by polymerase ehain reaction and restriction fragment length polymurphism analysis(PCR-RFLP).Results The prevalence OfAA genotype of the G1678A polymorphismin patients with ACS was significantly higher than that in the control subjects (49.5%% vs.40.4%,P=0.015).The frequencies of GA and GG genotypes were not significantly different between patients with ACS and controls (CA:39.3% vs.46.1%,P=0.070;CA;11.2% vs.13.5%,P=0.340).The frequencies of A allele in the ACS group and control group were 69.2% and 63.5%,respectively (P=0.022).Furthermore,multiple logistic regression analysis showed that the AA genotype was a significant risk factor for ACS (OR=1.52;95% CI:1.11~2.08;P=0.010).Conclusions The present findings suggest that the AA genotype in TSP-1 gene GI678A polymorphism may be associated with a risk factor for ACS in the Hart nationality of China.The AA genotype may be a genetic marker of the liability to the inheritance of AC,S.
9.Effects of different sputum suction methods on the incidences of pulmonary infections in patients with mechanical ventilation

Objective To investigate the effect of different sputum suction methods on the incidences of pulmonary infections in patients with mechanical ventilation.Methods Four hundred and sixty-six patients with mechanical ventilation were randomly divided into study group(n=219)and control group(n=247).The two groups were treated with continued balloon filling and airway humidification.Besides,the control group received conventional open sputum suction and the study group closed continuous negative pressure sputum suction.The two groups were compared in terms of incidence of pulmonary infections.Result The incidence of the study group(8.2%)was significantly lower than that of the control group(14.6%)(χ2=4.58,P<0.05).Conclusion Compared with the traditional open conventional sputum suction method,closed continuous negative pressure sputum suction may be more effective in reducing the incidence of pulmonary infection in patients with mechanical ventilation.
10.A new method for screening latent tuberculosis infection in Beijing army recruits

Objective To investigate the Mycobacterium tuberculosis infections in 2014 among Beijing army recruits, and evaluate a new method for screening latent tuberculosis infections.Methods A total of 194 army recruits were subjected to chest X-ray examination purified protein derivative(PPD) skin test, antibody detection, and interferon gamma release assay(IGRA) by ELISA combined with recombinant protein CFP10-ESAT6 and latent infection protein Rv2628.Results The positive rates of PPD skin test and antibody test were 49.7% and 15.5%, respectively.The latent infection rate of IGRA test was 22.2% in 194 cases after CFP10-ESAT6 stimulation.After stimulation of latent tuberculosis infection(LTBI) with Rv2628, IFN-γ level was significantly higher than that in healthy control group (P<0.05).The weak positive group of TST (5 mm≤diameter<15 mm) had a significantly higher level of IFN-γ than the strong positive group(diameter≥15 mm)(P<0.05),but after stimulation with CFP10-ESAT6,IFN-γ levels were not significantly different between the two groups(P>0.05).There was no significant difference between antibody negative and positive groups after stimulation by CFP10-ESAT6 and Rv2628 (P>0.05).The area under the ROC curve of Rv2628 diagnosis of tuberculosis infection was 0.84.When Youden index was 0.621,the specificity was 94.7% and sensitivity was 67.4%.ConclusionCombined detection of antigens Rv2628 and CFP10-ESAT6 specific IFN-γ values can be potentially used for differential diagnosis of active or latent tuberculosis infections.
