1.Development of acetabulum and the content of bone morphogenetic protein 7 after local lesion of cartilago acetabularis in a rabbit model

BACKGROUND:In a primate model of ulna defect, exogenous recombinant human bone morphogenetic protein-7 can promote the healing of autologous bone defect.
OBJECTIVE:To probe into the variation of the development of acetabulum and the content of bone morphogenetic protein 7 after local lesion of cartilage acetabularis in rabbits.
METHODS:Thirty-six rabbits (4-week-old) were anesthetized to expose the left hip joint by the anterolateral approach. A 1.5 mm×2.0 mm×0.5 mm sample of the cartilago acetabularis was scraped from each rabbit as experimental group;and the right hip joint was also exposed as control group. To evaluate the hip development, X-rays were used at 4, 6, 8 weeks after the operation, 12 rabbits were kil ed randomly at 4, 6, 8 weeks after operation for general observation, histological examination and immunohistochemical assay.
RESULTS AND CONCLUSION:By the radiographic examination, the acetabulums became shal ow and irregular in morphology. The shape of these femoral heads was applanate. Semiluxation and dislocation could be observed in some cases. Hematoxylin-eosin staining showed the regenerated fibrous tissue in the lesion region gradual y migrated from the meager layer to the deep layer to form fibrous cartilage, cartilage cells from proliferative and hypertrophic layers lost their normal columnar structure and arranged disorderly. Immunohistochemistry detection showed that over time, the dyeing of bone morphogenetic protein 7 became light in color. In the control group, a typical structure of articular hyaline cartilage was observed. The results suggest that damaging the cartilago acetabularis led to developmental hip dysplasia, and the content of bone morphogenetic protein 7 from al regions decreased by the extension of time.

4.Saddle prostheses in the treatment of resecting pelvis sarcoma around the acetabulum to salvage limbs of 12 cases

Twelve cases of pelvis sarcoma around acetabulum were selected from Department of Orthopaedics, Wuxi Second Hospital, Nanjing Medical University between January 2001 and December 2007 including 5 males and 7 females, aged 51-72 years. According to pathological diagnosis, there were 3 cases of sarcoenchondroma, 2 cases of osteosarcoma, 3 cases of giant cell tumor of bone, 2 cases of adenocarcinoma, and 2 cases of osteoblastoma. According to tumor position by Enneking dividatur sectors, there were 5 cases of Ⅰsector, 4 cases of Ⅱsector, and 3 cases of Ⅲ sector. The Harris score of pre-replacement was 88.5?8.8. All cases were treated by saddle prosthetic replacement successfully, and no accidents happened. The operation time was 2.5-3.5 hours; blood volume was (1 250?407) mL. Twelve cases were followed up for 18 months (range 5-36 months). One case died of tumor metastasis and body exhaustion 5 months postoperation. Infection of incision took place in 1 case, and cured by operation with antibiotic combined with bone cement chain pearls. The Harris scores 4 months post-replacement: 6 cases were between 40-49, 3 cases between 50-59, and 3 cases between 60-69. Twelve cases could walk with double orthosis 3 months postoperation, and 3 cases could walk with single orthosis 6 months postoperation; 1 case walked with near normal walking, but major walking was bad. No artificial joint loosening or dislocation occurred. Although the joint function of major cases treated by saddle prosthetic replacement was not fairly satisfactory, it is a better treating method for malignant tumor whose limbs can be reserved and live their daily life by orthosis.
5.Clinical and electroencephalographic manifestations of Hashimoto's encephalopathy

Clinically rare with an acute or subacute onset,Hashimoto's encephalopathy (HE) is characterized as autoimmune encephalopathy with elevated anti-thyroid autoantibodies.If timely diagnosed and treated,its prognosis is often fair.Four HE cases admitted from January 2012 to June 2014 were analyzed with a literature review.HE 4 cases had a significantly higher level of thyroperoxidase (TPO) while the lowest increase over four folds.There were cognitive decline,memory loss and even coma.And 50% had abnormal electroencephalogram (EEG) consistent with radiographic lesions.In short,EEG may aid an early diagnosis of HE.

Objective To survey gene expression profiles in nonlesional refractory temporal lobe epilepsy(TLE)and to further verify the difference of gene expression.thus to evaluate the possible molecular pathogenesis of this kind of epilepsy that can help to supply a new way for the diagnosis and treatment.Methods The TLE samples and control cases were studied by means of cDNA microarray consisting of 1 8 000 genes.Reverse transcription polymerase chain reaction(RT-PCR)Was performed to measure the expression alterations of SH3GL2.BTNN2A2 and KCNJ4 mRNA in temporal cortex samples from patients who had undergone temporal lobectomy surgery for intractable epilepsy.Tissue from 10 subjects who did not have epilepsy served as controls.Results The known genes differently expressed in those TLE samples involved immunity correlation factor genes,signal conduction genes,ion channel transportation genes;mitochondria function genes and SO on were identified.Among which.the expression of SH3GL2 mRNA Was significantly increased in epileptic brain(1.022±0.547)compared with the controls(0.446±0.171,t=-3.181).In TLE group(0.481±0.196),the expression of BTN2A2 mRNA was also significantly higher than that of control subjects(0.243±0.111,t=3.351).Compared with control group(O.795±0.112),the expression of KCNJ4 mRNA Was significantly decreased in TLE patients(0.438±0.178).Conclusions cDNA microarray is an efficient and high.throughout method to survey gene expression profiles in intractable temporal lobe epilepsy.The variation of those gene expressions might be a potential etiological agent for TLE that may offer a novel target for anticonvulsant therapy.

Plantaris bend malformation of ankle joint,a serious epiphenomenon will occurs when patients lie on bed for a long time.A new type of ankle joint movement device is studied.Powered by electric motor with giant torque and super low velocity and controlled by timing controlling circuit,this device enable the ankle joint a 20?~ 30?back stretch and 40?~ 50?plantaris bend.Furthermore,it can set different moving angle and time according to individual conditions and can perform slow,continuous and reciprocating motion for different actual need.Patients whose ankle joints are unable or difficult to move actively can benefit from this device in preventing plantaris bend malformation of ankle joint,ankle joint stiffness and muscle atrophy of lower limb.
10.Dynamic hip screw plates assisted with blocking screws for treatment of intertrochanteric fracture

Objective To investigate effects of dynamic hip screw (DHS) fixation assisted with blocking screws in treatment of intertrochanteric fracture.Methods A total of 150 patients with intertrochanteric fracture were selected as the combined treatment group,of which the patients managed by DHS intemal fixation were followed by the parallel placement of a blocking screw closely beneath the main screw for stability with its end rested on the plate.Another 87 patients managed by DHS fixation alone were determined as the control group.The surgical outcomes of the two groups were analyzed and compared.Results Good fixation without external displacement of femoral neck and cut-out of the main screw in femoral head was achieved in the combined treatment group.Also,fracture nonunion or death did not exist in the combined group.The patients in combined treatment group could take care of themselves after operation in the absence of severe pain and functional disturbance,with higher excellence rate (98.7%)than that in control group (P < 0.01).Whereas,the operation failure rate was significantly lower in combined treatment group than in control group (P < 0.05).Conclusion DHS fixation assisted with blocking screws is an ideal treatment of incomplete intertrochanteric fracture in medial wall of calcar femorale for it can achieve reliable fixation and definite curative effect,favor fracture healing,early functional exercise and low incidence of postoperative complications.
