1.A randomized study of peri-operative chemotherapy in patients with completed resected non-small-cell lung cancer

Purpose:To evaluate the effect of cisplatin-based peri-operation chemotherapy (CT) on survival after completed resection of non small cell lung cancer (NSCLC)Methods:A prospective, randomized, multicenter study was conducted by Shanghai Lung Cancer Team since Feb 1995 to Dec 2003 for stage Ⅰ~ⅢA NSCLC with completed resection. Patients were randomly assigned to receive pre-operative CT or no pre-operative CT (pre-op CT). Post-operative CT (post-op CT) were used for majority of the patients, except for partial stage I patients. Accumulated survival, log rank, MST, Cox uni-variance and multi-variance analyses, HR were used as statistics for evaluation Results:A total of 337 patients underwent randomization, 169 cases received pre-operative CT, and 168 cases didn't receive pre-operative CT. There was statistical survival difference between the group with no pre-op CT and with pre-op CT, 5-yr survival rate were of 47.85%∶ 36.52%, MST were 56.63∶39.14(P=0.03), respectively. Stage and post-op CT were the only two meaningful parameters with statistical survival difference calculated by multi-variance analyses (P0.05). There were 121 cases received more than 3 cycles post-op CT, 216 cases received less than 3 cycles post-op CT. The patients received more than 3 cycles had better yr-survival and MST than those received less cycles (P=0.04).Post-op CT was not benefit to the survival rate of stage I. Plantaris bend malformation of ankle joint,a serious epiphenomenon will occurs when patients lie on bed for a long time.A new type of ankle joint movement device is studied.Powered by electric motor with giant torque and super low velocity and controlled by timing controlling circuit,this device enable the ankle joint a 20?~ 30?back stretch and 40?~ 50?plantaris bend.Furthermore,it can set different moving angle and time according to individual conditions and can perform slow,continuous and reciprocating motion for different actual need.Patients whose ankle joints are unable or difficult to move actively can benefit from this device in preventing plantaris bend malformation of ankle joint,ankle joint stiffness and muscle atrophy of lower limb.
3.The Study of Rhesus Retinal Pigment Epithelium Cells Transplantation.

Objective To investigate the retinal pigment epithelium cells(RPE) and makeit the base of clinical application for transplantation.Methods The cultured RPE cells labeled by BrdU was injected into the host's subretinal space by external transcleral.The structure and ultrastructure of the transplanted RPE cells were observed by microscope and transmission electron microscope.Results The transplanted cells had the normal structure as the host's cells.They attached to Bruch's membrane with basal infoldings and forming microvill on the surface of the cells.Conclusion The external transcleral is a practical method for RPE transplatation.The transplanted cells can vitalize and form some normal ultrastrcture.
4.Current status of occupational hygiene habits and Helicobacter pylori infection of domestic service personnel in a Shanghai community

To explore the current status of occupational hygiene habits and Helicobacter pylori(Hp) infection of domestic service personnel in a Shanghai community and propose the community control measures.A total of 76 domestic service personnel and employers were selected into the investigation and control groups.And they answered questionnaire and received 14C-urea breath test.The career duration of 65% domestic service personnel were over 3 years.Home-based services accounted for 45%.The number of 49% part-time service accounted for two or more.And 84% of them dined together with service families.The difference of occupational hygiene habits and awareness of Hp infection had no statistical significance (P >0.05).The rate of Hp infection of domestic service personnel was 50% vs.38% for control group (x2 =2.162,P =1.141).Their consistent rate was 62% (Kappa =0.237).A high risk of Hp infection spread within or between the families is correlated with the professional behaviors of domestic service personnel.Eradication therapy for an existing patient should be started as early as possible and occupational practices standardized.
5.The diagnostic significance of CT tumor angioimaging on the peripheral lung cancer

2 cm).Immunohistochemistrical stain slice of operative specimen showed a lots of vascular endothelial cells and small vessels,which correlated with the tumor angioimaging area on CT images.The sensitivity,specificity,and positive predictive valve of tumor angioimaging in the lung cancers were 47.2%,91.3%,and 95.5%,respectively.Conclusion The “tumor angioimaging” sign can be regarded as another specific feature in peripheral lung cancer with contrast CT.
6.Multicenter randomized, double-blind, placebo-controlled trial of zonisamide as add-on therapy in patients with refractor partial seizures

Objective To evaluate the efficacy and tolerability of zonisamide (ZNS) as add-on therapy in patients with refractory partial seizures.Methods In this Chinese muiticenter, double-blind, randomized, placebo-contrclled trial, ZNS was compared with placebo add-on therapy in 217 patients (intent-to treat (ITT) population) with uncontrolled partial-onset seizures.All patients entered a 3-month baseline period followed by a 4-week titration interval and a 12-week maintenance period.The starting dose of ZNS group was 100 mg/d, increased by 100 mg/d every week and reached the goal of 400 mg/d.The main outcome was measured by the median of the percentage of decreased seizure frequency.The secondary ouwomes points included the percentage of patients who had seizure attacks decreased by more than 50%,and adverse events.Results The median of the percentage of decreased seizure frequency in ZNS group was 33.33%, and the placebo group was 0.Thirty-eight patients (34.23%) experienced more than 50% reduction in the seizure frequency in ZNS group, compared with 19.81% of patients (21 cases) in the placebo group (χ2 =5.7159,P =0.0168) ; Moreover, 13 (11.71%) patients in ZNS group and 5 patients in placebo group were seizure free, 25 patients in ZNS group and 16 patients in placebo group who had seizure attacks decreased by more than 50%.The availability rate in ZNS group was higher than placebo group (34.23% vs 19.81%, U=2.4701, P=0.0135).The most common adverse events in ZNS group were drowsiness, fatigue, decreased appetite, gastrointestinal complaints, insomnia and constipation.Conclusion Zonisamide treatment was generally well tolerated and was associated with significant reductions in seizure frequency as adjunctive treatment for partial-onset seizures.

Objective:
To explore the clinical application value of enhanced recovery after surgery (ERAS) in the laparoscopic surgery for cholecystolithiasis comorbid with choledocholithiasis.
Methods:
The prospective study was conducted. The clinicopathological data of 52 patients with cholecystolithiasis comorbid with choledocholithiasis who were admitted to the Third Affiliated Hospital of Zunyi Medical University from September 2016 to September 2018 were collected. Patients were divided into 2 groups by random number table: patients in observation group received laparoscopic cholecystectomy + choledocholithotomy + choledochoscopic exploration + T-tube drainage (or primary suture of common bile duct) and perioperative management guided by the concept of enhanced recovery after surgery (ERAS), and patients in control group received traditional perioperative management. Observation indicators: (1) surgical situations; (2) postoperative situations; (3) postoperative complications; (4) postoperative pain scores; (5) changes in hepatic function and blood routine during perioperative period. Follow-up using outpatient examination and telephone interview was performed to detect complications during the postoperative 6 months up to March 2019.
