1.Effects of treadmill running at different intensities on three-dimensional structure of rat subchondral bones

Objective To explore the effect of treadmill running at different intensities on three-dimensional structure of rat subchondral bones.Methods A total of 24 female Wistar rats were randomly assigned into a sedentary (SED) group,a low-intensity running (LIR) group,a medium-intensity running (MIR) group,or a high-intensity running (HIR) group,each of 6.Rats in LIR,MIR and HIR groups underwent treadmill running exercises with respective protocol,while rats in SED group serve as controls.Eight weeks later,all animals were sacrificed and their proximal end of tibia was examined using Micro-CT.Results Compared with SED group,in HIR group the subchondral plate became thicker with higher BMD and lower porosity,while trabecular bone became more and denser with higher BMD and a more plate-like architecture.LIR and MIR failed to result in considerable changes in the microstructure.Conclusion Treadmill running has an intensity-dependent effect on the three-dimensional structures of subchondral bones.High-intensity running can evoke significant structural changes in subchondral bones.
2.The clinical significance of plasma LP (a),Hcy,hs-CRP and cTnI concentrations determination in patients with acute myocardial infraction

Objective To explore the clinical significance of plasma LP (a) ,Hcy ,hs-CRP and cTnI concentrations in patients with AMI .Methods Plasma LP(a) ,Hcy ,hs-CRP and cTnI concentrations of 162 patients with AMI and 165 healthy people recrui-ted as control group were determined .Immunofluorescent dry quantitative method and electro-chemiluminescence immunoassay were used in the determination ,and statistical analysis were used in data comparison .Results Before treatment ,the plasma LP(a) , Hcy ,hs-CRP and cTnI concentrations in 162 AMI patients were significantly higher than those in control group (P<0 .01) .After treatment ,the plasma hs-CRP concentrations in 97 improved patients returned to normal (t=1 .586 ,P>0 .05) ,but the plasma cTnI concentrations significantly decreased (t=3 .322 ,P<0 .01) .The plasma Hcy ,hs-CRP and cTnI concentrations in 36 died patients were significantly higher(P<0 .01) .Conclusion Plasma LP(a) ,Hcy ,hs-CRP and cTnI determination were valuable for the diagno-sis of AMI patients ,the most valuable indicators for patient prognosis and follow-up were plasma hs-CRP and cTnI .
3.Clinical analysis of extraperitoneal cesarean section plus forceps in repeated cesarean section

Objective To observe the clinical application of extraperitoneal cesarean section(ECS) plus forceps vs transperitoneal cesarean sections(TCS) in repeated cesarean section.Methods 98 multiparous women with scar uterus for elective repeated cesarean sections were recruited retrospectively,47 cases for ECS plus forceps(group A),and 51 cases for TCS(group B).The multiparous women with hyperglycemia not controlled,severe preeclampsia,heart disease,placenta previa,premature rupture of membrane,a history of > 1 cesarean section,myoma and/or ovarian neoplasm were excluded.Results Skin incision to baby delivery time and total operation time of group A were (7.7 ± 2.8) min and (42.8 ± 9.7) min,respectively,which were significantly shorter than (9.3 ± 3.2) min and (47.6 ± 9.4) min of group B,(t =2.700,2.497,P =0.008,0.014).There was significant difference in blood loss volume during the operation and postoperative 2 hours,which was (310.4 ± 106.3) mL,(365.3 ± 142.8) mL respectively(t =2.142,P =0.035).The Visual Analog Scale for pain (VAS pain) was (2.8 ± 1.8) in group A and (4.1 ± 1.9) in group B,respectively (t =3.252,P =0.002).The gastrointestinal function recovery mean time of group A was significantly shorter than that of group B (12.5 h versus 16.0h,t =2.771,P =0.007).And the postoperative febrile morbidity was significantly lower in group A than in group B(8.5% versus 25.5% ;x2 =4.918,P =0.033).The patients with chronic pelvic pain followed up after operation was 3 versus 12,and the difference was significant (x2 =5.143,P =0.026).There were no differences in neonatal Apgar score at 1 minute,birth asphyxia and wound healing rates.Conclusion ECS plus forceps can be safely used for repeated cesarean section,with the advantages of less operation time,less bleeding volume,lower postoperative morbidity,and fewer complications than TCS.
4.Correlation Analysis of the Detection Rate of Carbapenems-resistant Gram-negative Bacillus with Drug Consumption in Our Hospital

OBJECTIVE:To provide reference for standardizing the clinical use of carbapenem antibiotics and controlling drug-resistant bacteria infection. METHODS:The detection of 3 kinds of carbapenems-resistant Gram-negative bacillus in our hospi-tal during 2011-2016 were analyzed retrospectively. The consumption,target cure rate and treatment course of carbapenem antibiot-ics were analyzed statistically. The correlation between detection rate of drug-resistant bacteria with the consumption of carbapenem antibiotics was investigated by Pearson test. RESULTS:During 2011-2016,1222 strains of carbapenems-resistant Acinetobacter bauman (CRAB),655 strains of carbapenems-resistant Pseudomonas aeruginosa (CRPA) and 53 strains of carbapenem-resistant Escherichia coli (CRE) were detected in our hospital. The detection rates increased from 23.88%,8.92%,0.09% in 2011 to 80.34%,35.74%,0.97% in 2016. The types of carbapenem antibiotics in our hospital were mainly imipenem and meropenem. The consumption of them increased from 4222,145 g in 2011 to 7218,4387 g in 2016. The both target cure rates were all lower than 60%,and the proportion of the patients with treatment course >14 d was more than 65%. The detection rates of CRAB,CR-PA and CRE were positively correlated with the consumption of carbapenem antibiotics (r>0.9,P<0.05). CONCLUSIONS:The detection rate of carbapenems-resistant Gram-negative bacillus and drug consumption increase year by year in our hospital,and they have certain correlation. The target cure rate of carbapenem antibiotics in our hospital is in low level,and there is a long treatment course. They are should be standardized in the clinic. The selection of carbapenem antibiotics should be strictly followed clinical in-dications so as to reduce the generation of drug-resistant strains.
5.Effects of cyclic tensile stress on metabolism and apoptosis of chondrocytes inrat

Objective To investigate the effect of cyclic tensile stress (CTS) on the metabolism inand apoptosis of rat chondrocytes.Methods Primary rat chondrocytes were cultured on a Bioflex plate for one day and then stretched cyclically for 24 hours at a frequency of 0.5 Hz using a Flexcell-5000T apparatus.The cells were divided into 5 groups according to their stretching ratio:0% (the control group),2%,6%,10% and 14%.After the stretching,Col Ⅱ,Aggrecan,MMP-13 and ADAMTS-5 mRNA were measured using qPCRs,and the NO and PGE2 levels were measured using ELISA kits.Moreover,TUNEL staining and Annexin V-FITC/PI were used to analyze the apoptosis of chondrocytes.Results Compared with the control group,the average levels of Col Ⅱ and Aggrecan mRNAdecreasedin 10% and 14% groups [(0.738±0.11) and (0.58±0.13),(0.75±0.11) and (0.55±0.09)].In those groups,the MMP-13 [(2.47±0.47) and (2.88±0.36)] and ADAMTS-5 mRNA level [(2.39±0.33) and (2.75±0.49)],the NO [(6.96±0.96) and (8.28±0.82)] and PGE2 level [(6.83±0.66) and (7.15±0.71)] had increased significantly.In the 6% group the average levels of Col Ⅱ(1.76±0.30) and Aggrecan mRNA (1.93±0.14)of 6% group were significantly higher than the control group,but the NO level of the former (3.07±0.20) was significantly lower than the control group (3.89 ± 0.33).The apoptosis rate of chondrocytesin 2% and 6% groups were (0.065±0.013) and (0.063 ± 0.147),without significant differences to that of the control group (0.045 ± 0.008).However,compared with the control group,apoptosis in the 10% and 14% groups [(0.135 ±0.026) and (0.184±0.020)] increased significantly.Conclusion The effect of cyclic tensile stress on chondrocyte metabolism and apoptosis was magnitude-dependent.Ten percent and 14% CTS can increase the catabolism and apoptosis of chondrocytes.Ten percent and 14% strain can increase the catabolism and apoptosis of chondrocytes.Cyclic 6% strain can increase the anabolism of chondrocytes,but 2% strain has no impact on metabolism or apoptosis.

7.Study of long-term safety and efficacy of domestic Firebird rapamycin-eluting stent

Objective To evaluate the long-term safety and efficacy of domestic Firebird rapamycin-eluting stent in treatment of patients with coronary artery disease (CAD). Methods The 465 patients with CAD received percutaneous interventional therapy with domestic Firebird or imported Cypher rapamycin-eluting stent based on patients' will and they were followed up for more than 24 months. Results The 321 cases in Firebird group were followed up for (45.3 ±11.1)months, with 166 cases (51.7%) received reexamination by coronary angiography. The 144 patients in Cypher group were followed up for (46.1 ± 12. 1) months, with 82 patients (56.3%) received reexamination by coronary angiography. At the termination of follow-up, there were no significant differences in all-cause death (7.8% vs. 7.6 %, x2 = 1.32, P = 0. 250), cardiac death (5.9% vs.5.6%, x2 =0. 02, P=0. 877), acute myocardial infarction (2.8% vs. 3.4%, x2 =0. 15, P=0. 697),cardiovascular-cause rehospitalization (29.6% vs. 31.9 %, x2 =0.26, P=0. 610), acute and subacute thrombosis events (0.9% vs. 1.3%, x2 =0.19, P=0.661), late thrombotic events (1.2% vs.1.3%, x2 =0. 02, P=0. 900) and target lesion revascularization (3.4% vs. 4.2%, x2 =0. 15, P=0. 694) between two groups. Conclusions Domestic Firebird rapamycin-eluting stent has the same or similar long-term safety and efficacy with imported Cypher rapamycin-eluting stent.
8.Effect of Electroacupuncture on the Synaptic Number Density and Surface Density in the Cerebral Ischemic Areas of Rats with Focal Cerebral Ischemia at Different Time

Objective To investigate the effect of electroacupuncture(EA)in improving the reconstruction of synapse after cerebral ischemia.Methods Forty-five Wistar rats were divided into 3 groups:sham-operated group,ischemia group and EA group.The number density(Nv)of synapses and the surface density(Sv)of synaptic joint band were observed 1 hour,1 day,3 days,1 week and 3 weeks after ischemia and EA, respectively.Heat-coagulation-induced occlusion of the middle cerebral artery was performed to establish the model of focal cerebral ischemia.EA group received EA on the acupoints of Baihui and Dazhui.The changes of synaptie structure,Nv and Sv as well as the influence of EA on the above indexes were observed in different time.Results There existed significant decrease of Nv and Sv 1 hour,1 day and 3 days after establishment of models in the isehemia group(P0.05).Conclusion EA can reduce the cerebral ischemic injury by increasing the synaptic amount and area in the cortex of rat model of cerebral isehemia at early stage,promoting the cerebral recovery after ischemia,which will provide substantial morphological basis for the improvement of synaptic reconstruction and recovery from cerebral isehemia by EA at different time after ischemic cerebral injury.
9.Combined endoscopic-laparoscopic techniques for one-stage treatment of concomitant cholelithiasis and choledocholithiasis.

OBJECTIVETo assess the clinical effects of combined endoscopic-laparoscopic technique for one-stage treatment of cholelithiasis with concomitant choledocholithiasis.
METHODSA retrospective analysis was conducted of the clinical data of 30 patients (Group A) with cholelithiasis and choledocholithiasis receiving one-stage laparoscopic cholecystectomy (LC) combined with intraoperative encoscopic retrograde cholangio-pancreatography (ERCP) and 32 patients (Group B) receiving LC combined with 1aparoscopic common bile duct exploration. The operative time, blood loss, conversion to open surgery rate, time to postoperative ambulation, calculi residual rate, hospitalization cost and length of hospital stay were analyzed comparatively.
RESULTSThere were statistically differences between the two groups in hospitalization cost and length of hospital stay (P<0.05) but not in the other indices (P>0.05).
CONCLUSIONCombined endoscopic-laparoscopic techniques can be a safe and feasible option for one-stage treatment of concomitant cholelithiasis and choledocholithiasis to allow rapid postoperative recovery with a shortened hospital stay.

10.Comparison of laparoscope-assisted D2 radical total gastrectomy and open gastrectomy for gastric cancer.

OBJECTIVETo evaluate the clinical feasibility of laparoscope-assisted D2 radical total gastrectomy for gastric cancer.
METHODSFrom March 2011 to March 2013, 216 gastric cancer patients underwent D2 radical total gastrectomy in our hospital, including 118 laparoscopic and 98 open procedures. The intraoperative and postoperative outcomes between the two groups were compared.
RESULTSCompared with open surgery group, laparoscopic group had longer operation time [(253.9 ± 26.1) min vs. (206.2 ± 23.9) min, P<0.01], less intraoperative blood loss [(138.4 ± 34.0) ml vs. (266.3 ± 58.7) ml, P<0.01], shorter postoperative hospital stay [(10.3 ± 1.4) d vs. (13.9 ± 2.0) d, P<0.01], and lower morbidity of postoperative complication [16.1% (19/118) vs. 29.6% (29/98), P<0.05]. There were no significant differences in the number of lymph node removed [(26.2 ± 6.2 vs. 27.6 ± 5.6)], postoperative morbidity of complication [1.7% (2/118) vs. 2.0% (2/98)], and 2-year survival rate(99.0% vs. 98.6%) between the two groups (all P>0.05).
CONCLUSIONAs compared to open surgery, laparoscope-assisted D2 radical total gastrectomy is safe, feasible with the advantage of rapid postoperative recovery.
